Unmasking of sinus node dysfunction by ajmaline.

A 75-year-old male with bifascicular block presented with recurrent giddiness and presyncope. Prolonged monitoring of the cardiac rhythm did not reveal any arrhythmia. Electrophysiological evaluation, including an ajmaline stress test, was performed. Ajmaline had an unusual effect. It reproducibly induced sinus arrest and thus unmasked a latent sick sinus syndrome.

Intraprocedural Cardiac Arrest and Multivessel Spasm During Transcatheter Aortic Valve Replacement.

BACKGROUND: Cardiac arrest during transcatheter aortic valve replacement (TAVR) is an infrequent complication often attributed to coronary obstruction. Causes include sinus sequestration, ostial obstruction by native leaflets or transcatheter valve skirt, and calcium embolization. CASE SUMMARY: An elderly patient with severe aortic stenosis sustained an intraprocedural cardiac arrest due to left main coronary artery spasm with concomitant iliofemoral spasm during TAVR. He was successfully treated with a drug-eluting stent and underwent TAVR 2 days later. DISCUSSION: This case highlights an u

Sinus Arrest and Bradycardia Induced by Carotid Baroreceptor Reflex Activation during Rotational Angiography: A Case Report.

OBJECTIVE: We describe a rare patient with a cavernous sinus dural arteriovenous fistula (CS DAVF) in whom diagnostic rotational angiography (RA) caused sinus arrest and bradycardia. CASE PRESENTATION: A 79-year-old woman with no previous history of cardiovascular diseases presented with left oculomotor nerve paresis. Conventional angiography confirmed a bilateral CS DAVF. During a three-dimensional RA (3DRA) examination of the left internal carotid artery, sinus arrest occurred. Subsequently, the use of 3DRA to image the left external carotid artery and the use of cone beam computed tomograp

Cardiac sodium channel blockade after an intentional ingestion of lacosamide, cyclobenzaprine, and levetiracetam: Case report.

CONTEXT: Lacosamide treats partial seizures by enhancing slow inactivation of voltage-gated sodium channels. The described cardiac toxicity of lacosamide in the literature to date includes atrioventricular blockade (PR prolongation), atrial flutter, atrial fibrillation, sinus pauses, ventricular tachycardia and a single cardiac arrest. We report a second case of cardiac arrest following an intentional lacosamide overdose. CASE DETAILS: A 16 year-old female with a seizure disorder was found unresponsive in pulseless ventricular tachycardia after intentionally ingesting 4.5 g (76 mg/kg) lacosam

[Heart arrest during dipyridamole scintigraphy: report of 3 cases].

Potentially life threatening rythm disturbances, such as third degree A-V block and sinus arrest without "escape rythm", represent infrequent and unexpected events during dipyridamole thallium scintigraphy. We report three cases of cardiac arrest requiring resuscitation maneuvers after dipyridamole infusion during myocardial thallium scan. On the basis of these clinical observations we suggest that a trained cardiologist and resuscitation kit should be available in nuclear lab. Moreover elderly patients may require careful screening for preexisting conduction defects and sick sinus syndrome. W

Profound bradycardia with lidocaine during anesthesia induction in a silent sick sinus syndrome patient.

Sick sinus syndrome is caused by sinus node dysfunction that renders it unable to function as a pacemaker. Patients with sick sinus syndrome are often asymptomatic or have symptoms that are mild and nonspecific. Lidocaine (0.5 mg/kg) injection is used for reduction of pain associated with intravenous injection of propofol. Episodes of marked bradycardia with sinus arrest after prophylactic lidocaine injection are reported in a 69-y-old man with no apparent preoperative cardiac disease or electrocardiographic abnormality. Surgery was postponed, and he was later diagnosed with sick sinus syndrom

A tale of two brothers: anomalous coronary arteries in two siblings.

We report a case of a 10-year-old boy with aberrant left coronary artery from the right sinus of Valsalva, whose presenting sign was cardiac arrest. The patient's asymptomatic younger brother was found to have aberrant right coronary artery from the left sinus of Valsalva. This is the first report of a familial clustering of coronary artery anomalies in which the initial presentation of one subject was sudden cardiac death and a sibling harboring a similar lesion was identified while asymptomatic.

Cardiac arrhythmias the first month after acute traumatic spinal cord injury.

OBJECTIVE: Cardiovascular complications including cardiac arrest and arrhythmias remain a clinical challenge in the management of acute traumatic spinal cord injury (SCI). Still, there is a lack of knowledge regarding the characteristics of arrhythmias in patients with acute traumatic SCI. The aim of this prospective observational study was to investigate the occurrence of cardiac arrhythmias and cardiac arrests in patients with acute traumatic SCI. METHODS: As early as possible after SCI 24-hour Holter monitoring was performed. Additional Holter recordings were performed 1, 2, 3, and 4 weeks

Prolonged asystole induced by trigeminocardiac reflex accompanied with abnormal heart rate variability during percutaneous balloon compression: a case report.

Trigeminocardiac reflex (TCR) can result in bradycardia and even cardiac arrest, and is reversible with elimination of the stimulus. Here, we report the case of a 68-year-old man who experienced cardiac arrest during percutaneous balloon compression for the treatment of trigeminal neuralgia. In this patient, sinus rhythm did not recover after stimulation removal, causing us to successfully perform cardiopulmonary resuscitation (CPR). The patient regained a sinus rhythm and was pretreated with atropine 0.5 mg, allowing the operation to be started again. The operation was completed successfully

Anomalous Right Coronary Artery Arising from the Left Sinus of Valsalva in a Young Athlete.

Cardiac events associated with congenital coronary abnormalities are rare but potentially life-threatening in a young population. Most of these patients are not diagnosed before their initial cardiac event. Amongst such coronary artery anomalies, sudden death is frequently seen in an anomalous origination of a coronary artery from the opposite sinus. We herein present the case of a patient who presented with sudden cardiac arrest associated with an anomalous right coronary artery originating from the left sinus of Valsalva. Surgical treatment was selected because there was evidence of reversib

Sinus arrest during radiofrequency ablation of the atrioventricular-node slow pathway: implications and possible mechanisms.

Radiofrequency ablation has been associated with changes in autonomic function. In this case, a 52-year-old woman was referred for electrophysiologic study of recurrent supraventricular tachycardia. Typical slow/fast atrioventricular node re-entry tachycardia was induced and confirmed during the study. Radiofrequency ablation of the slow pathway of the atrioventricular node led to sinus arrest, which resolved once ablation was stopped. Given the distance of the ablation site from the sinus node, we inferred that mechanisms other than direct injury to the sinus node were involved. To our knowle

Effects of cardiac lymphostasis on sinus nodes. A histological study in canine experiment.

The mechanism of sinus node dysfunction caused by cardiac lymphostasis was studied in dogs with histological and electrophysiological methods. A total of seventeen mongrel dogs were used in this study. Out of them 12 animals were operated to induce cardiac lymphostasis and sacrificed 3, 6, and 10 days after operation. In 3 of the dogs, in addition to cardiac lymphostasis, local cardiac hypertension was induced by constricting the descending aorta (artificial coarctation of the aorta). The other five dogs were sacrificed as normal controls. The signs of deteriorated pacemaker function of the si

[Alleviation of carotid sinus syncope and removal of cardiac pacing after regression of cervical malignant lymphoma].

A 68-year-old man developed a rapidly-growing right cervical tumor, a biopsy of which allowed for the diagnosis of diffuse large B-cell lymphoma, not otherwise specified. Magnetic resonance imaging demonstrated a right cervical mass lesion of 80 mm in diameter that extended from the medial region of the parotid gland to the posterior region of the neck. While undergoing a chest X-ray in an upright position, he lost consciousness and briefly fell. A transient loss of consciousness recurred while changing his position on the bed, and an electrocardiogram at that time revealed sinus arrest of a s

Cardiac Arrest during Temporary Clipping for Ruptured Internal Carotid Artery Aneurysm: A Case Report.

Mechanical stimulation of the trigeminal nerve during craniofacial, skull base, or dental surgeries may cause bradycardia, hypotension, or cardiac arrest. This phenomenon is called trigeminal cardiac reflex (TCR). We encountered a rare case of a patient who experienced sinus arrest due to temporary clipping of the intracranial carotid artery during the clipping of a ruptured aneurysm. We discuss possible reasons for the occurrence of TCR in this case. A man in his 30s with no medical history presented with a sudden-onset headache. Computed tomography revealed a subarachnoid hemorrhage in the b

Cardioneuroablation for Refractory Vasovagal Storm Secondary to a Right Hilar Tumor.

BACKGROUND: Cardiac syncope due to sinus arrest may result from either a vasovagal cardioinhibitory mechanism or an intrinsic sinus node dysfunction. CASE SUMMARY: A 67-year-old woman with a recently diagnosed right hilar mass presented with new-onset episodes of syncope associated with sinus arrest. Resting electrocardiogram was normal, and no electrolyte abnormalities were identified. Detailed analysis of telemetry recordings revealed a typical vasovagal cardioinhibitory response. The patient underwent a cardioneuroablation procedure. Since the procedure, there has been no recurrence of syn

[Advances in the development of ocular cardiac reflex in extraocular muscle surgery].

Ocular cardiac reflex (OCR) usually occurs in ophthalmic surgery, especially in extraocular muscle(s) surgery. OCR generally can cause sinus bradycardia, arrhythmia, reduced atrial pressure, ventricular tachycardia, ventricular fibrillation, ventricular doublet, dizziness, nausea and other symptoms. Severe cases will appear cardiac arrest, respiratory arrest, etc. Recent studies on the mechanism of OCR and the relationship between surgical operation and anaesthesia in extraocular muscle surgery and the occurrence of OCR are reviewed in this paper in order to reduce the occurrence of OCR and tr

Frequency of arrhythmias and other cardiac abnormalities in fulminant hepatic failure.

In a series of 106 patients with fulminant hepatic failure and grade 4 encephalopathy, cardiac arrhythmias and other abnormalities occurred in 92 per cent. The most common was sinus tachycardia (75%) and this was the only abnormality in 22 per cent of the patients. Sudden cardiac arrest occurred in 25 per cent, various ectopic beats in 20 per cent, and heart block or bradycardia in 18 per cent. Other electrocardiographic abnormalities, mostly of the T wave and ST segment, were found in 31 per cent. Cardiac and respiratory arrests were usually unrelated to each other and both frequently occurre

Fatal cardiac tamponade due to coronary sinus thrombosis in acute lymphoblastic leukaemia: a case report.

We report a rare case of fatal cardiac tamponade attributed to coronary sinus thrombosis. An 83-year-old man was admitted to the hospital complaining of general fatigue. Laboratory examination revealed marked increase of atypical lymphoblastic cells in peripheral blood. CHOP therapy was started under the diagnosis of acute lymphoblastic leukemia. The patient died, however, of sudden cardiac arrest in the initial course of the chemotherapy. Autopsy revealed cardiac tamponade with markedly dilated and congested coronary vein induced by coronary sinus thrombosis. A condition similar to leukemia-r

Beyond a Single Etiology: Tachy-Brady Syndrome and Possible Coexisting Focal Epilepsy Presenting as Transient Loss of Consciousness.

Transient loss of consciousness has overlapping cardiac and neurologic causes, and distinguishing syncope from seizure can be difficult. The co-occurrence of a primary cardiac rhythm disorder with a possible coexisting epileptic process is infrequently reported, as most cases instead describe one condition mimicking or triggering the other. We present a case of a 77-year-old woman with hypertension, hyperlipidemia, and paroxysmal atrial fibrillation who awoke with an abnormal chest sensation, a dream-like experience, and urinary incontinence and was admitted for evaluation of a possible seizur
