Intrathecal morphine for spinal fusion in children.

Intrathecal morphine (0.025 mg/kg) was administered preoperatively to 20 children undergoing major surgery of the rachis, ie, either posterior (16 cases) or anterior (4 cases) spinal fusions. During the surgical procedure, hemodynamic control was easily maintained. Blood loss was slightly less than 50% of blood volume and the mean infusion rate of blood was 6.3 +/- 4.5 ml/kg/hour for the duration of surgery. The technique did not significantly interfere with wake-up tests, and memory of awakening was only observed in one case. The postoperative course was also positively affected. The children

Spinal fusions to the sacrum in adults with scoliosis.

Forty-five skeletally mature patients averaging 44.3 years of age had spinal fusions which extended to the sacrum for pain and/or progression of their scoliosis. The primary diagnosis was idiopathic scoliosis in 35 patients, congenital scoliosis in two patients, and paralytic scoliosis in eight patients. A single curve pattern was present in 41 patients and the remainder had double primary curves. Thirty-eight patients had single stage procedures and 35 of these were done posteriorly. There was an evolution in the type of posterior instrumentation used over the 12 years. Thirty-five patients (

Results of spinal fusion for radiation scoliosis.

There were 150 children with neuroblastoma and 183 children with Wilms' tumor who received radiation therapy, orthovoltage up to 1967, and cobalt therapy thereafter. In all except two cases, radiation therapy crossed the midline. Of all of the children treated, 13 required spinal fusion. There were six with Wilms' tumor; three had kyphosis; and three had scoliosis. Nine fusions were done with three pseudoarthroses, one broken rod, and two hook pullouts. There were seven with neuroblastoma, of whom five had kyphosis and two had scoliosis. Nineteen spinal fusions were done on these seven patient

Abdominal aortic calcification is associated with impaired fusion after elective spinal fusion.

BACKGROUND CONTEXT: The interplay between vascular compromise and spinal pathology has been underexplored in the context of spinal fusion outcomes. Abdominal aortic calcification (AAC) is associated with various degenerative spinal conditions, potentially due to impaired perfusion. However, the relationship between AAC and the success of osseous fusion mass formation following spinal fusion remains unexplored. OBJECTIVES: To investigate the association between AAC severity and fusion success following posterior lumbar spine fusion surgery. STUDY DESIGN: Retrospective cohort study. PATIENT P

Opioids delay healing of spinal fusion: a rabbit posterolateral lumbar fusion model.

BACKGROUND CONTEXT: Opioid use is prevalent in the management of pre- and postoperative pain in patients undergoing spinal fusion. There is evidence that opioids downregulate osteoblasts in vitro, and a previous study found that morphine delays the maturation and remodeling of callus in a rat femur fracture model. However, the effect of opioids on healing of spinal fusion has not been investigated before. Isolating the effect of opioid exposure in humans would be limited by the numerous confounding factors that affect fusion healing. Therefore, we have used a well-established rabbit model to s

Robot-assisted, minimally invasive spinal tumor resection and posterior spinal fusion.

Integrating robotics with neuronavigation and tubular dilation enables minimally invasive posterior stabilization and tumor resection. A 40-year-old woman presented with diffuse spinal metastases with spinal cord compression. This surgical video demonstrates the authors' entire minimally invasive workflow for robot-assisted percutaneous pedicle screw placement, arthrodesis, and navigated tubular resection. The authors review their institutional experience with 14 cases using this workflow. The advantages of this approach include the high accuracy of pedicle screw placement, minimal blood loss,

The effect of zoledronic acid on the volume of the fusion-mass in lumbar spinal fusion.

BACKGROUND: Few studies have explored the effects of bisphosphonates on bony healing in patients undergoing spinal fusion surgery. Most previous studies used animal models and found that bisphosphonate shows negative effects on spinal fusion consolidation. We intended to evaluate the effect of a single-dose of zoledronic acid on the volume of the fusion-mass in lumbar spinal fusion. METHODS: A retrospective review was carried out on 44 patients with symptomatic degenerative lumbar spinal stenosis who underwent one or two-level posterolateral fusion from January 2008 and January 2011. They wer

Efficacy of electrical stimulation for spinal fusion: a meta-analysis of fusion rate.

BACKGROUND CONTEXT: Many clinical studies have demonstrated the effectiveness of electrical stimulation as an adjunct to spinal arthrodesis. However, there is a paucity of comparative data among different electrical stimulation techniques. PURPOSE: To compare the efficacy of three electrical stimulation methods for spinal fusion based on the literature review. SAMPLE: Twenty-one articles, meeting all the inclusion criteria, were selected. A total of 1,381 patients were evaluated. STUDY DESIGN: Systematic literature review and meta-analysis. OUTCOME MEASURES: Fusion rates were determined us

Distal Junctional Failure Following Pediatric Spinal Fusion.

BACKGROUND: Adjacent segment pathology is a known complication after spinal fusion, but little has been reported on junctional failure. A series of adolescent patients presented with acute distal junctional failure (DJF). We sought to determine any common features of these patients to develop a prevention strategy. METHODS: A retrospective review was conducted of pediatric patients who developed DJF after instrumented spinal fusion performed at 2 institutions from 1999 to 2013. Patients with proximal junctional failure or junctional kyphosis without failure were excluded. RESULTS: Fifteen su

Total Hip Arthroplasty After Spinal Fusion Surgery.

Given the close functional connection between the hip and the spine, their simultaneous degenerative pathology is a common problem seen in orthopedic patients and has been termed the "hip spine syndrome". Demographic change and an increasing number of operations have led to more patients who have undergone both total hip arthroplasty and spondylodesis especially of the lumbar spine. Studies have shown significant dislocation rates in this cohort of as high as almost 30%. Biomechanical studies and considerations show that spinal surgery directly influences the functional acetabular position and

Unexplained visual loss after lumbar spinal fusion.

Two cases of visual loss after spinal fusion surgery are described. In both cases, surgery was lengthy, the patient's head was placed in a dependent position, and hemodilution and deliberate hypotension were combined. One patient was achondroplastic, the other obese. Possible risk factors associated with ischemic optic neuropathy after anesthesia and surgery are discussed.

Posterior spinal fusion: allograft versus autograft bone.

The effectiveness of allograft bone for posterior spinal fusion in neuromuscular scoliosis is controversial. Thirty patients with cerebral palsy, treated with posterior spinal fusion, were divided into two groups. Group 1 consisted of 18 patients treated by posterior spinal fusion using autogenous bone graft. Group 2 consisted of 12 patients treated by posterior spinal fusion using freeze-dried allograft bone. The average preoperative curve of 70 degrees in Group 1 was corrected to 35 degrees (50% correction). At 3.2 years average follow-up, the curves averaged 51 degrees (46% loss of correcti

Minimum acceptable outcomes after lumbar spinal fusion.

BACKGROUND CONTEXT: Defining success after spinal surgery remains problematic. The minimal clinically important difference (MCID) in pain or functional outcomes is a common metric often calculated independent of perceived risk and morbidity, which is an important consideration in large procedures such as spinal fusion and instrumentation. PURPOSE: The purpose of this study was to describe a method of assessing treatment success based on prospective, patient-reported "minimum acceptable" outcome for which they would undergo a procedure. These goals can then be compared at follow-up to gauge ho

Treatment of symptomatic flatback after spinal fusion.

Fifty-five patients who had loss of lumbar lordosis after spinal fusion and subsequently had corrective osteotomies were studied. When they were first seen, fifty-two patients (95 per cent) were unable to stand erect and forty-nine (89 per cent) had back pain. The previous use of distraction instrumentation with a hook placed at the level of the lower lumbar spine or the sacrum was the factor that was most frequently identified as leading to the development of the flatback syndrome. Sixty-six extension osteotomies were performed in these fifty-five patients. Nineteen patients (35 per cent) had

Lumbosacral spinal fusion with Knodt distraction rods.

Knodt distraction rods have been used in conjunction with lumbar spinal decompression and/or fusion to open the intervertebral foramen. A review was undertaken to evaluate the efficacy of the Knodt Rods in 24 patients with multiple level foramenal canal stenosis. All 24 patients were treated by wide decompressive laminectomy, foramenotomies and bilateral-lateral fusion. One half of those 24 patients were treated with supplemental Knodt Rods and the other half was treated without Knodt Rods. Pre- and post-operative radiologic and clinical results were compared between the two groups. Although t

Lunato-triquetral fusion and associated spinal abnormality.

Lunato-triquetral fusion is a rare congenital anomaly. Its association with skeletal dysplasias, tarsal coalitions and synostotic states in the upper limbs and other parts of the body is well established. There is not mention in the English literature about its association with congenital spinal abnormalities and secondary osteoarthrosis in the wrist and two such cases are now described. Both cases presented with symptoms in the dominant hand and had symptoms of backache in the past. Their clinical course and X-ray findings are described.
