Vehicle-assisted suicidal strangulation: A case report and literature review.

Vehicle-assisted suicidal strangulation is an unusual method of suicide where the victim completes suicide by pulling a ligature tied around their neck using the acceleration of the vehicle they are driving. This report describes a case of a 48-year-old man who completed suicide by tying a rope to his neck and to a parked tractor, then using his car to pull the rope. The on-scene investigation found the body a few meters from his car, which was still running, and a snapped rope tied to a tractor nearby. The autopsy revealed a bruised, encircling ligature mark with underlying injuries to the ne

Assisted suicide for prisoners: An ethical and legal analysis from the Swiss context.

Should prisoners be allowed to access assisted suicide? Whereas the ethical and legal issues regarding assisted suicide have now been extensively discussed in the literature, surprisingly scarce attention has been given to the pressing issue of inmates requesting assistance in dying. Through an analysis supported by the Swiss legal framework, I first argue that the principle of equivalence in prison medicine, which states that prisoners ought to receive the same level of health care as the general population does not prove a solid basis in arguing for prisoners' right to assisted suicide. The impact of forensic investigations following assisted suicide on post-traumatic stress disorder.

In Switzerland, all deaths through assisted suicide are reported as unnatural deaths and investigated by a forensic team (police, medical examiner, and state attorney). However, there is limited knowledge concerning the impact these forensic investigations have on the development of post-traumatic stress disorder, complicated grief, or depression in those who have lost a loved one. A cross-sectional survey of 85 family members or close friends who were present at an assisted suicide was conducted in December 2007. The Impact of Event Scale, Inventory of Complicated Grief, and Brief Symptom Inv

Communicating suffering in the context of medically assisted suicide: Psychological challenges and ethical discussion.

Typically, one of the necessary conditions for accessing medically assisted suicide (MAS) is the presence of suffering that the patient deems intolerable, which is then communicated to the physician. During the consultation, the physician has the responsibility to explore this suffering, if the patient so wishes, to identify appropriate palliative care (PC) options to propose. However, the patient might desire to end life; thus, the physician is in the difficult position of offering forms of care directed at symptom relief and the preservation of life while the patient approaches the physician

The Case Against Legalizing Medically Assisted Suicide or Voluntary Active Euthanasia in Islamic Regions.

This essay presents arguments against legalizing medically assisted suicide (MAS) or voluntary active euthanasia (VAE) in Islamic regions. MAS and VAE should be determined by particular regions based on their respective mainstream moral cultures. Taking the Islamic regions of Bangladesh as examples, the essay argues that MAS or VAE should not be legalized in such regions because it would contradict the Islamic classical moral convictions, authentic interpretations, and relevant bioethical codes that prioritize saving human lives and providing quality palliative care rather than MAS or VAE for

[Assisted suicide: assessment and evaluation of the ability to make a freely responsible decision].

This article addresses the question of how the capacity to make a freely responsible decision in assisted suicide can be assessed and evaluated. Based on medical ethics and legal principles as well as international comparisons, it is argued that the decision to commit suicide places special demands on autonomy, decision-making and inner strength. The concept of "free responsibility" is analysed in contrast to legal capacity and capacity to consent and is located in its proximity to these concepts. An operationalisation is proposed that includes understanding, autonomous evaluation, sustainable

Assisted and unassisted suicide in men and women: longitudinal study of the Swiss population.

BACKGROUND: In Switzerland assisted suicide is legal if no self-interest is involved. AIMS: To compare the strength and direction of associations with sociodemographic factors between assisted and unassisted suicides. METHOD: We calculated rates and used Cox and logistic regression models in a longitudinal study of the Swiss population. RESULTS: Analyses were based on 5 004 403 people, 1301 assisted and 5708 unassisted suicides from 2003 to 2008. The rate of unassisted suicides was higher in men than in women, rates of assisted suicides were similar in men and women. Higher education was po

Physician-Assisted Suicide: Considering the Evidence, Existential Distress, and an Emerging Role for Psychiatry.

Physician-assisted suicide (PAS) is one of the most provocative topics facing society today. Given the great responsibility conferred on physicians by recent laws allowing PAS, a careful examination of this subject is warranted by psychiatrists and other specialists who may be consulted during a patient's request for PAS. In this article, recent evidence regarding the implementation of PAS in the United States and The Netherlands is reviewed. Support is found for some concerns about PAS, such as the possibility that mental illness occurs at higher rates in patients requesting PAS, but not for

Psychological and ethical considerations concerning the debreather and assisted suicide: a commentary on Ogden.

This article offers a commentary on the report by Russel Ogden (2010/this issue) on the use of the "debreather" for suicide assistance by the NuTech (or New Technologies for Self-Deliverance) program. The emergence of NuTech is set within its historical and political context. Nu Tech is criticized for its anarchic and extreme advocacy of self-determination to the neglect of other psychological and ethical considerations (e.g., the individual's emotional state, the mutability of his or her quality of life, and his or her decision-making capacity). Some of the risks of covert, unregulated, assis

Assisted suicide by oxygen deprivation with helium at a Swiss right-to-die organisation.

BACKGROUND: In Switzerland, right-to-die organisations assist their members with suicide by lethal drugs, usually barbiturates. One organisation, Dignitas, has experimented with oxygen deprivation as an alternative to sodium pentobarbital. OBJECTIVE: To analyse the process of assisted suicide by oxygen deprivation with helium and a common face mask and reservoir bag. METHOD: This study examined four cases of assisted suicide by oxygen deprivation using helium delivered via a face mask. Videos of the deaths were provided by the Zurich police. Dignitas provided protocol and consent information

Reasons why people in Switzerland seek assisted suicide: the view of patients and physicians.

BACKGROUND: Assisted suicide is permitted in Switzerland provided that assistance is not motivated by selfish reasons. Suicides are commonly performed with the assistance of right-to-die organisations and the use of a lethal dose of barbiturates prescribed by a participating physician. We examined the reasons physicians provided for writing the prescription and the reasons patients gave for requesting assistance in dying. METHODS: We analysed all reported cases of assisted suicide that were facilitated by right-to-die organisations between 2001 and 2004 in the city of Zurich, and for which bo

Regulating Risk and Autonomy in Assisted Suicide: Conway V Secretary of State for Justice.

Noel Conway appealed against the dismissal of his claim in the High Court, that section 2(1) Suicide Act 1961 is incompatible with his Article 8(1) ECHR right to respect for private and family life. Mr Conway's appeal concerned three main issues: first, the correct way the court should respond to Parliament's decision not to amend section 2(1) Suicide Act; secondly, the way the Divisional Court addressed Mr Conway's alternative scheme; and thirdly, the weight to be placed upon personal autonomy. The Court of Appeal held that Parliament is better placed for determining the purported legal right

Euthanasia and assisted suicide of persons with psychiatric disorders: the challenge of personality disorders.

BACKGROUND: Euthanasia or assisted suicide (EAS) for psychiatric disorders, legal in some countries, remains controversial. Personality disorders are common in psychiatric EAS. They often cause a sense of irremediable suffering and engender complex patient-clinician interactions, both of which could complicate EAS evaluations. METHODS: We conducted a directed-content analysis of all psychiatric EAS cases involving personality and related disorders published by the Dutch regional euthanasia review committees (N = 74, from 2011 to October 2017). RESULTS: Most patients were women (76%, n = 52),

The debreather: a report on euthanasia and suicide assistance using adapted scuba technology.

In response to the general prohibition of euthanasia and assisted suicide, some right-to-die activists have developed non-medical methods to covertly hasten death. One such method is a "debreather," a closed system breathing device that laypersons can use to induce hypoxia for persons seeking euthanasia or assisted suicide. This article presents data from nine cases where the debreather was used on humans, resulting in eight deaths. The covert properties of the debreather make it almost impossible for medical examiners and law enforcers to detect its use. Clandestine behavior circumvents legal

Psychedelic therapy at the end of life: unexplored alternative to assisted suicide in the UK.

In this editorial, I examine the parallels between conventional and assisted suicide. Psychedelic therapy has been shown to alleviate end-of-life distress and reduce suicidality. I advocate for rescheduling psychedelics to enable research into their effectiveness as an adjunct to palliative care, prior to institutionalising assisted suicide in the UK.

Euthanasia and Assisted Suicide of Patients With Psychiatric Disorders in the Netherlands 2011 to 2014.

IMPORTANCE: Euthanasia or assisted suicide (EAS) of psychiatric patients is increasing in some jurisdictions such as Belgium and the Netherlands. However, little is known about the practice, and it remains controversial. OBJECTIVES: To describe the characteristics of patients receiving EAS for psychiatric conditions and how the practice is regulated in the Netherlands. DESIGN, SETTING, AND PARTICIPANTS: This investigation reviewed psychiatric EAS case summaries made available online by the Dutch regional euthanasia review committees as of June 1, 2015. Two senior psychiatrists used directed

Patient requests for euthanasia and assisted suicide in terminal illness. The role of the psychiatrist.

Psychosocial assessment and treatment are critical elements of care for terminally ill patients who desire hastened death. Most patients, in saying that they want to die, are asking for assistance in living--for help in dealing with depression, anxiety about the future, grief, lack of control, dependence, physical suffering, and spiritual despair. In this article, the authors review current understandings of the psychiatric aspects of requests by terminally ill patients for assisted suicide and euthanasia; describe an approach to the common problems of physical, psychological, social, and spir
