[An anatomical variation: the aberrant termination of the thoracic duct].

In a male cadaver dissected at the Department of Morphology, University del Valle, Call (Colombia), a rarely described anatomical variation was found. It consisted of an aberrant termination or drainage of the thoracic lymph duct. Normally, this duct ascends in the thorax behind the esophagus, gradually diverges towards the left side of the neck and ends in the left jugulo-subclavian confluent--either in the internal jugular vein or in the subclavian vein. In the case of this cadaver, the thoracic duct diverged towards the right side of the neck to end in the right internal jugular vein. The p

Thoracic duct cyst of the neck: a case report.

A thoracic duct cyst was excised from the left side of the neck of a 63-year-old woman. It was 6.5cm in length and 3.5cm in width, and was located behind the common carotid artery and jugular vein. The proximal portion of the thoracic duct with a narrowed lumen was in direct contact with the lateral surface of the cyst and the venous angle. The lower end of the cyst extended into the mediastinum to immediately above the aortic arch; the inferior portion of the thoracic duct descended from the bottom of the cyst into the mediastinum behind the aorta. The cyst was unilocular with a fluid content

[Adrenergic innervation of lymph nodes and the thoracic duct].

By means of incubation of slices in 2% solution of glyoxylic acid distribution of adrenergic fibers in the rabbit lymph nodes and in the thoracic lymphatic duct has been studied. Adrenergic fibers get into parenchyma of the lymph nodes via two ways. The first--the perivascular, when the nervous fibers make a plexus and get into the node along the blood vessels, the second--diffuse nervous fibers get together with trabecules in between the lymphoid nodules. The distribution density of the adrenergic fibers is not the same in different groups of the lymph nodes. In the lumbar nodes it is the hig

Thoracic duct cyst of the mediastinum. A case report.

There are only a few reports in the literature on cysts of the thoracic duct. This case report concerns a 56-year-old female patient, who was operated on because of a mediastinal mass that later proved to be a cyst of the thoracic duct. The operation and immediate postoperative course were uneventful. Six months later, the patient was doing well and no further investigations were recommended.

Anatomical observations on the ending of the human thoracic duct.

Fifty-one cadavers of human adults of both sexes were examined for the ramifications and site of opening of the cervical segment of the thoracic duct. It was found that the duct opened into veins of the neck through either one (84.31%) or two branches (15.69%). The most frequent site of opening was in the junction of the left internal jugular vein with the subclavian vein (18 individuals), but another frequent site (15 individuals) was in the internal jugular vein. The duct opened at the angle formed by the internal jugular vein and the internal posterior jugular vein in five individuals, and

Selective Lymphatic Duct Embolization in Patients With Chylothorax and Thoracic Duct Side-Branch Injury.

Chylothorax, the accumulation of chyle in the pleural space, is a potentially fatal condition that necessitates intervention. Options range from conservative treatment, such as dietary modifications and pharmacological therapy, to percutaneous and surgical interventions. This case describes a 56-year-old male status post-lung wedge resection who developed a high-output chylothorax. Initial management with octreotide and a low-fat diet was unsuccessful. A percutaneous lymphangiogram identified extravasation of contrast from a thoracic duct side-branch. Instead of traditional thoracic duct embol

Thoracic Duct Embolization via Retrograde Cannulation of a Leaking Stump through the Thoracic Cavity.

Thoracic duct embolization has emerged as an alternative treatment for refractory chylothorax, in addition to thoracic duct ligation. Thoracic duct embolization is typically performed via direct puncture of the cisterna chyli and cannulation of the thoracic duct, or via a retrograde approach to the thoracic duct through the venous angle. The former requires a long puncture through abdominal organs; the latter has anatomical limitations depending on the case. The reported success rates of both approaches are insufficient to provide a complete solution. We report a case of refractory chylothorax

Factors Contributing to Successful Transvenous Retrograde Thoracic Duct Cannulation.

PURPOSE: To identify key factors for successful transvenous retrograde cannulation (TVRC) of the thoracic duct. MATERIALS AND METHODS: A total of 47 consecutive patients (62.1 ± 13.2 years; 32 men) who underwent attempted TVRC between July 2016 and July 2021 were included. Reasons for interventions were chylous leakage from the chest (n = 36), abdomen (n = 6), and other sites (n = 5). Patient age, sex, access vein (femoral vs brachial), anatomic classification (presence of dominant channel vs plexiform) of the terminal thoracic duct, and engagement of a diagnostic catheter into the juguloveno

Immunologic competence of thoracic duct cells. I. Delayed hypersensitivity.

Delayed hypersensitivity was produced in donor Lewis rats by sensitization with soluble protein antigens emulsified in complete Freund's adjuvant. Cells of their thoracic duct lymph were collected for varying periods of time and transferred intravenously to isogenic Lewis recipients. With this model the following conclusions were reached: 1. Delayed hypersensitivity was transferred by thoracic duct cells. 2. The longer the drainage of the thoracic duct, the fewer cells were needed to achieve a successful transfer. With continuing drainage the proportion of small lymphocytes decreased and large

Microsurgical Thoracic Duct Lymphovenous Bypass in the Adult Population.

Thoracic duct occlusion can lead to devastating complications, resulting in recalcitrant chylothoraces, ascites, generalized lymphedema, metabolic derangement, and death. Lymphatic extravasation has traditionally been managed conservatively and, in recent years, using minimally invasive techniques, such as thoracic duct ligation and embolization. However, these measures are often limited in application and therapeutic success, resulting in chronically difficult conditions with few modalities available for definitive management. Advances in microsurgery have allowed for surgical treatment and r

Development of intractable ascites due to thoracic duct hypertension.

We describe a 69-year-old dialysis-dependent patient who developed intractable ascites after zone 2 aortic reconstruction for a type IA thoracic endovascular aneurysm repair endoleak. Investigation as to the cause of ascites revealed a unique set of clinical circumstances leading to intractable bloody ascites. Investigation included imaging and invasive testing to diagnose the culprit mechanism. Ultimately, interventional catherization of the left subclavian vein illustrated an abnormally elevated pressure in the left subclavian vein. It was thus determined that, owing to the combination of a

Interventional Therapies for Thoracic Duct Injury and Intractable Chylothorax.

Injury to the thoracic duct with resultant chylothorax can cause significant patient morbidity and mortality. Conservative treatment strategies often fail to address the problem. Open surgical and percutaneous approaches are often required to manage patients with refractory chylothorax. This review describes in detail the major role of minimally invasive interventional therapies for thoracic duct (TD) injury. The review emphasizes strategies for identifying the TD on preprocedural imaging and describes various techniques for percutaneous access to the TD. The advantages and disadvantages of se

Thoracoscopic Thoracic Duct Ligation for Congenital and Acquired Disease.

PURPOSE: Congenital and acquired chylothorax presents a unique management challenge in neonates and infants. A failure of conservative therapy requires surgical ligation to prevent continued fluid and protein losses. This article examines a 15-year experience with thoracoscopic ligation of the thoracic duct. PATIENTS AND METHODS: From June 1999 to December 2013, 21 patients presented with chylothorax refractory to conservative management. Sixteen patients presented following cardiac procedures, 1 after tracheoesophageal fistula repair, 1 after extracorporeal membrane oxygenation cannulation,

A unique approach to thoracic duct leak after esophagectomy.

Chylothorax is a severe complication of esophagectomy. Those who do not respond to conservative measures require reoperation. We have described a minimally invasive technique to control a late postoperative chyle leak. A 41-year-old patient underwent an Ivor-Lewis esophagectomy. Day 4 after surgery he was found to have an esophageal leak. He underwent thoracotomy and esophageal stent insertion. On day 20, a radiologic drain was placed to control a small supradiaphragmatic collection. The collection was found to be chyle, and 2.5 L was drained per day. As this was 3 weeks after thoracotomy, a t
