Chest drain and thoracotomy for chest trauma.

Traumas are the leading cause of death in the first four decades of life. Nevertheless, thoracic traumas only seldom require invasive procedures. In particular, chest drain placement is required in case of pleural disruption causing haemothorax, pneumothorax or haemopneumothorax. Although large-bore chest drains have been traditionally used in case of haemothorax, recent evidences seem to question this routine, showing good performances of small-bore and pig tail drains. Although it is a common procedures, experience and training is needed to avoid complications which might be even lethal. Sur

Patient attitudes to sternotomy and thoracotomy scars.

BACKGROUND: There are no data comparing patient attitudes to sternotomy and thoracotomy scars following surgery for congenital heart disease (CHD). METHODS: Two hundred and one patients with a scar from CHD surgery (105 sternotomy, 36 thoracotomy, and 60 both scars) had a structured interview to explore attitudes to their scar. RESULTS: Comparable proportions of each group reported that they did not like or hated their scar (23/105 [22 %] sternotomy, 9/36 [25 %] thoracotomy, 17/60 [28 %] both scars). Significantly more patients stated that they where embarrassed by and/or their choice of clo

[Explorative thoracotomy in patients with bronchogenic carcinoma].

The authors analyse in a retrospective study a group of 31 patients with explorative thoracotomy, which was performed on account of inoperable bronchogenic carcinoma. The causes of inoperability were assessed and the period of survival after explorative thoracotomy was evaluated. From the total number of 496 patients indicated in 1985-1990 for surgery on account of bronchogenic carcinoma 465 pulmonary resections were made and 31 (6.25%) explorative thoracotomies. In all these patients the c TNM and p TNM classification was assessed. Disagreement in the evaluation of these parameters was found

Esophagectomy with or without thoracotomy. Is there any difference?

Two operative approaches for esophageal carcinoma were compared with respect to operative morbidity and mortality by means of multivariable analysis. From 1980 to 1986, 152 patients underwent resection by laparotomy and right-sided anterolateral thoracotomy with an intrathoracic anastomosis. From 1986 to 1989, 141 patients underwent resection by transhiatal blunt dissection with a cervical anastomosis. The stomach was the preferred organ for reconstruction. Paresis of the recurrent laryngeal nerve and leakage of the cervical anastomosis occurred significantly more often in the transhiatal grou

Resuscitative thoracotomy performed in the operating room.

The efficacy of resuscitative thoracotomy in the trauma patient has been questioned. Survival rates are variable, but a review of resuscitative thoracotomy in the emergency department of our institution documented an overall survival rate of only 1.8%. Higher survival rates may be anticipated in patients initially presenting with signs of life who can be transported directly to the operating room prior to the need for resuscitative thoracotomy. To test this hypothesis, the clinical course of all injured patients undergoing urgent or exigent thoracotomy in the operating room between July 1983 a

[Exploratory thoracotomy in surgery of lung cancer].

An account is given in this paper of results obtained from radiotherapy and combined therapy in cases of regionally spreading carcinoma of the lung. Exploratory thoracotomy was definitely found to be accompanied by a minimum of complications (4.8 +/- 1.2 per cent) and low lethality (1.7 +/- 0.7 per cent). Complete remission, lasting more than 36 months, was recorded from 15.4 +/- 7.2 per cent of exploratively thoracotomised patients, following radically programmed radiotherapy. Extensive surgery, combined with preoperative and/or postoperative radiotherapy, was found to be practicable in cases

Cryoanalgesia in the management of pain after thoracotomy.

The demand for postoperative analgesia was compared between 29 patients treated with cryoanalgesia to the relevant intercostal nerves during thoracotomy and a control group who did not have cryoanalgesia. The cryoanalgesia group required significantly less (p < 0.005) postoperative analgesia than did the control group.

The role of emergency room thoracotomy in trauma.

The charts of 175 patients who underwent emergency thoracotomy (ET) in the emergency room (ER) between 1972 and 1978 were reviewed to determine the efficacy of this procedure. Seven cases of nontraumatic cardiac arrest were excluded from analysis. Although 150 patients were transported to the ER within 1 hour of injury, 60% either had no vital signs (91 cases) or were agonal (20 cases) on admission to the ER. The trauma was blunt in 60 cases and penetrating in 108. The major sites of injury were heart, major vessels, head, liver, and lung. Thirty-six patients died in the ER, 83 died in the ope

Resuscitative thoracotomy in France: For whom? By whom?

Resuscitative thoracotomy is preferentially addressed to patients with penetrating thoracic injury and suffering from severe treatment-resistant hemodynamic instability, without pulse or in cardiopulmonary arrest for at most 15minutes. It is practicable in an emergency room, or ideally, in an operating theater. The procedure always begins with left anterolateral thoracotomy and can be prolonged through transversal bi-thoracotomy or, more rarely and according to the presumed origin of the hemorrhage, through median sternotomy. In most cases the procedures to be carried out are relatively simple

An unusual cause of stridor after thoracotomy.

Acute airway obstruction after endotracheal intubation is uncommon. Treatment focuses on protecting the patient's airway and preventing long-term complication of tracheal stenosis. This article describes the successful treatment of a patient with acute subglottic stenosis via mechanical debridement of granulation tissue.

Emergency thoracotomy in thoracic trauma-a review.

Thoracic trauma is one of the leading causes of death in all age groups and accounts for 25-50% of all traumatic injuries. While the majority of patients with thoracic trauma can be managed conservatively, a small but significant number requires emergency thoracotomy as part of their initial resuscitation. The procedure has been advocated for evacuation of pericardial tamponade, direct control of intrathoracic haemorrhage, control of massive air-embolism, open cardiac massage and cross-clamping of the descending aorta. Emergency thoracotomy can be defined as thoracotomy "occurring either immed

[Minor thoracotomy for the treatment of spontaneous pneumothorax].

We have recently established the following therapeutic principles for spontaneous pneumothorax, and have obtained favorable results. 1. All patients, including those with a first episode, are subjected to thoracotomy. 2. The standard technique is minor thoracotomy through a skin incision of 5 cm which is aesthetically superior, except complicated cases with chronic obstructive lung disease in elderly patients or giant bullae. 3. Bilateral simultaneous axillary minor thoracotomy is applied to bilateral pneumothorax, whether synchronous or metachronous. 4. Thoracotomy is attempted by other appro

[Transaxillary thoracotomy in surgery for spontaneous pneumothorax].

Spontaneous pneumothorax in young individuals is due to rupture of apical bullae in the majority of cases. At operative treatment for spontaneous pneumothorax in young individuals in whom radiography of the thorax has not revealed evidence of bullous emphysema or other pathological changes in the lungs, it is therefore rational to employ a transaxillary incision which renders possible resection of bullae and mechanical pleurodesis. Compared with operation via a postero-lateral thoracotomy, which is the standard operative technique in Denmark, the morbidity is lower in transaxillary thoracotomy

Urgent thoracotomy for pulmonary or tracheobronchial injury.

Three hundred eighty-eight of 7,283 (5.3%) admitted trauma patients underwent urgent thoracotomy. In 61 patients (15.7%), pulmonary or tracheobronchial injury prompted thoracotomy (11, blunt; 50, penetrating). Pulmonary hemorrhage necessitated thoracotomy in 54 patients (88.5%); tracheobronchial injury in five patients (8.2%). The mortality was 27.9%. Nine patients (14.8%) underwent pneumonectomy: eight died of intractable hemorrhagic shock during thoracotomy despite rapid control of pulmonary hemorrhage: one died of sepsis. Esophagectomy without thoracotomy for carcinoma of the esophagus.

During the past 2.5 years, 13 patients underwent esophagectomy for carcinoma of the esophagus without the use of a thoracotomy. During the same period, 81 operations on the esophagus or cardia were performed, 73 of which were esophagogastrectomies. Two patients died, for a hospital mortality rate of 2.7 percent. Of the 13 patients, there were 7 women and 6 men with an average age of 59.7 years. The lesion was located in the cervical esophagus in two, the upper thoracic esophagus in eight and the lower esophagus in three. One patient died on the 12th postoperative day, for a hospital mortality

Indication for thoracotomy and chest wall stabilization.

In multiply injured patients with major blunt thoracic injuries, the continued high mortality is mainly caused by the additive effect of the unstable flail chest associated with pleural and lung injuries upon the pulmonary gas exchange disturbance caused by haemorrhagic shock. A more active approach with early thoracotomy and chest wall stabilization is recommended. Three groups of injuries provide the appropriate indication: 1. Penetrating injuries, ruptures of organs and great vessels, when associated with serial rib fractures, which should be stabilized 'on retreat'. 2. Anterior or lateral
