Multisystem inflammatory syndrome following mild COVID-19 in an unvaccinated Tongan adult.

Multisystem inflammatory syndrome in adults (MIS-A), is a rare post-infectious complication of COVID-19. We describe an illustrative case of MIS-A in an otherwise well, SARS-CoV-2 unvaccinated 25-year-old Tongan man who presented to hospital 30 days after mild COVID-19 illness. We highlight the progression of his illness, including treatment in the Intensive Care Unit (ICU) for cardiogenic shock, and detail temporal evolution of clinical, laboratory and radiographic features of his illness. Clinicians should be alert for possible MIS-A in the weeks after a surge in COVID-19 cases.

Rapid, long-distance dispersal by pumice rafting.

Pumice is an extremely effective rafting agent that can dramatically increase the dispersal range of a variety of marine organisms and connect isolated shallow marine and coastal ecosystems. Here we report on a significant recent pumice rafting and long-distance dispersal event that occurred across the southwest Pacific following the 2006 explosive eruption of Home Reef Volcano in Tonga. We have constrained the trajectory, and rate, biomass and biodiversity of transfer, discovering more than 80 species and a substantial biomass underwent a >5000 km journey in 7-8 months. Lifestyle interventions for obesity prevention among adolescents in Pacific Islands Countries and Territories: a narrative review of outcomes and sociocultural, spatial and temporal dimensions.

This narrative review synthesizes peer-reviewed studies of diet and/or physical activity lifestyle interventions for obesity prevention among adolescents in Pacific Island Countries and Territories. Nine databases were searched from inception to June 2024 for English and French publications. Of 2068 records identified, nine studies from Fiji, Tonga, New Caledonia, and French Polynesia met inclusion criteria. Outcomes extracted included anthropometric indicators of body fatness, physical activity, and diet, alongside information on how interventions addressed sociocultural (communities and cult

The Prevalence of Scabies, Skin Infection and Rheumatic Heart Disease in a Cross-Sectional Study of Tongan Primary School Children.

BACKGROUND: Scabies is a common health issue in the Pacific and may contribute to rheumatic heart disease (RHD). This study assessed the prevalence of scabies and other skin conditions among Tongan schoolchildren and examined the association between these conditions and RHD. METHODS: Children from four primary schools on Tongatapu were screened for scabies, other skin conditions and RHD. Health workers were trained to recognise scabies and related skin conditions accurately. All children also underwent echocardiographic screening to detect heart valve abnormalities consistent with RHD. Statis

Global, regional, and national prevalence of overweight and obesity in children and adults during 1980-2013: a systematic analysis for the Global Burden of Disease Study 2013.

BACKGROUND: In 2010, overweight and obesity were estimated to cause 3·4 million deaths, 3·9% of years of life lost, and 3·8% of disability-adjusted life-years (DALYs) worldwide. The rise in obesity has led to widespread calls for regular monitoring of changes in overweight and obesity prevalence in all populations. Comparable, up-to-date information about levels and trends is essential to quantify population health effects and to prompt decision makers to prioritise action. We estimate the global, regional, and national prevalence of overweight and obesity in children and adults during 1980-20

Ancient genomes reveal distinct human dispersals and social stratification in the settlement history of western Remote Oceania.

The settlement of western Remote Oceania involved interactions among deeply divergent populations. Initial settlement ∼3,000 years ago by Lapita-associated, Southeast Asian-related groups was followed by Papuan-related expansions and later sea voyages from Polynesia. We present genome-wide data spanning the past three millennia from the eastern Solomon Islands, Vanuatu, Fiji, and Tonga. We find that the Papuan-related expansion proceeded at a slower pace than the earlier Lapita-associated expansion and reached Fiji through at least two distinct human dispersals, centuries earlier than previous

Lymphatic filariasis in soldiers exposed in INDOPACOM.

Some military organizations in the U.S. Indo-Pacific Command (INDOPACOM) give returning soldiers presumptive treatment for filariasis. As there have been few clinical cases in recent decades, the historical basis for this chemotherapy was reviewed. During the Second World War, U.S. Marines stationed on Polynesian islands such as Tonga, Samoa, and Fiji experienced clinical lymphatic filariasis. Although thousands of both U.S. and Australian soldiers served in New Guinea, few, if any, cases of lymphatic filariasis were ascribed to Melanesia. While the French Army reported dozens of cases of fila

The Role of Heterotypic DENV-specific CD8+T Lymphocytes in an Immunocompetent Mouse Model of Secondary Dengue Virus Infection.

Dengue is the most prevalent arthropod-borne viral disease worldwide and is caused by the four dengue virus serotypes (DENV-1-4). Sequential heterologous DENV infections can be associated with severe disease manifestations. Here, we present an immunocompetent mouse model of secondary DENV infection using non mouse-adapted DENV strains to investigate the pathogenesis of severe dengue disease. C57BL/6 mice infected sequentially with DENV-1 (strain Puerto Rico/94) and DENV-2 (strain Tonga/74) developed low platelet counts, internal hemorrhages, and increase of liver enzymes. Cross-reactive CD8+ T

Case report: an area postrema syndrome revealing a neuromyelitis optica spectrum disorder associated with central nervous system tuberculosis in a young Togolese (black African) woman.

BACKGROUND: Area postrema syndrome (APS) is considered to be one of the most specific clinical presentations of neuromyelitis optica spectrum disorders (NMOSDs). In sub-Saharan Africa, NMOSDs and even more so those revealed by an APS, are rarely reported. However, studies among mixed populations have shown that NMOSDs disproportionately affect black people with relatively more frequent encephalic involvement. We report a case of APS revealing an NMOSD associated with central nervous system (CNS) tuberculosis in a young Togolese woman residing in Togo (West Africa). CASE PRESENTATION: A 28-yea

Illicit drug use and responses in six Pacific Island countries.

We report here on the illicit drug situation in six Pacific nations: Fiji, Papua New Guinea, Samoa, Solomon Islands, Tonga and Vanuatu. The report is based on the 'Situational analysis of illicit drug issues and responses in Asia and the Pacific', commissioned by the Australian National Council on Drugs Asia Pacific Drug Issues Committee. The situational analysis was a comprehensive desk-based review; data sources included published and unpublished literature and information from key informants. A range of psychoactive substances have been used traditionally across the Pacific region. Cannabis

Rapid prehistoric extinction of iguanas and birds in Polynesia.

The Tongoleleka archaeological site on Lifuka Island, Kingdom of Tonga, is a rich accumulation of pottery, marine mollusks, and nonhuman bones that represents first human contact on a small island in Remote Oceania approximately 2,850 years ago. The lower strata contain decorated Lapita-style pottery and bones of an extinct iguana (Brachylophus undescribed sp.) and numerous species of extinct birds. The upper strata instead feature Polynesian Plainware pottery and bones of extant species of vertebrates. A stratigraphic series of 20 accelerator-mass spectrometer radiocarbon dates on individual

Molecular Identification of Gambierdiscus and Fukuyoa (Dinophyceae) from Environmental Samples.

Ciguatera Fish Poisoning (CFP) is increasing across the Pacific and the distribution of the causative dinoflagellates appears to be expanding. Subtle differences in thecal plate morphology are used to distinguish dinoflagellate species, which are difficult to determine using light microscopy. For these reasons we sought to develop a Quantitative PCR assay that would detect all species from both Gambierdiscus and Fukuyoa genera in order to rapidly screen environmental samples for potentially toxic species. Additionally, a specific assay for F. paulensis was developed as this species is of conce

Country of birth and hospital treatment for psychosis in New South Wales.

BACKGROUND: Migration has been found to be a risk factor for schizophrenia in several high-income countries. AIM: To examine whether overseas migrants to New South Wales (NSW) have higher rates of admission to psychiatric hospitals for psychotic disorders, including schizophrenia and mania, compared to people born in Australia. METHODS: The country of birth of people admitted to public mental health units for the treatment of psychotic illness and for non-psychotic disorders between 2001 and 2010 was compared to the country of birth for the NSW population in the 2006 census. The innate immune response following multivalent dengue vaccination and implications for protection against dengue challenge.

Understanding the immune response to dengue virus (DENV) is essential for developing a dengue vaccine that is protective against all 4 DENV serotypes. We evaluated the immune response after vaccination (live attenuated tetravalent dengue vaccine TV005 or trivalent admixture) and after challenge with DEN2Δ30 (Tonga/74) to better understand the importance of homotypic immunity in vaccine protection. Significant increases in IP-10 expression were observed following receipt of either the trivalent or tetravalent vaccine. After challenge, a large increase in IP-10 expression was observed in the pla

A new point cloud processing method unveiled hidden coastal boulders from deep vegetation.

Huge coastal boulders are useful to reconstruct the size of past extreme waves such as those associated with tsunamis and storms using inverse-type or forward-type boulder transport models. These models fundamentally require the precise shape of boulders. Traditionally, they have often been assumed to be rectangular or ellipsoidal with three axes measured in the field. However, if the boulder's shape is complex, this method is unable to represent the actual shape accurately. Therefore, it prevents estimation of the tsunami or storm size reasonably using models. For this reason, boulders have r

Exploring structural violence in the context of disability and poverty in Zimbabwe.

BACKGROUND: While it is widely assumed that disability, poverty and health are closely linked, research falls short of fully understanding the link. One approach to analysing the links between disability and poverty is through the concept of structural violence, referring to social structures that contribute to the impoverishment of individuals or communities. These structures can be political, ecological, legal and economic, among others. OBJECTIVE: To explore structural violence and how it affects families of children with cerebral palsy among the Tonga ethnic group living in poor rural com

Shearing instabilities accompanying high-pressure phase transformations and the mechanics of deep earthquakes.

Deep earthquakes have been a paradox since their discovery in the 1920s. The combined increase of pressure and temperature with depth precludes brittle failure or frictional sliding beyond a few tens of kilometers, yet earthquakes occur continually in subduction zones to approximately 700 km. The expected healing effects of pressure and temperature and growing amounts of seismic and experimental data suggest that earthquakes at depth probably represent self-organized failure analogous to, but different from, brittle failure. The only high-pressure shearing instabilities identified by experimen
