Direct tracheobronchopexy via left lateral thoracotomy for severe tracheobronchomalacia.

An infant with pulmonary atresia/ventricular septal defect/major aortopulmonary collateral arteries underwent unifocalization, ventriculoseptal defect closure and placement of a right ventricle to pulmonary artery conduit via median sternotomy. Aortopexy and pulmonary arteriopexy via redo sternotomy were insufficient to allow weaning of continuous positive airway pressure and he required direct tracheobronchopexy via left lateral thoracotomy to alleviate posterior trachealis intrusion along the length of the trachea and left main bronchus. He also underwent laryngotracheoplasty with placement

Tracheal stenting improves airway mechanics in infants with tracheobronchomalacia.

The compliance and expiratory resistance of the tracheobronchial tree is increased in infants with tracheobronchomalacia because of a weakness in cartilaginous support of the airway. Life threatening episodes may occur in these patients due to airway collapse. The goals of this study were to compare the effects on respiratory system mechanics of stenting the airway with either continuous positive airway pressure (CPAP) or a long tracheostomy tube. Five infants were studied: two had bronchopulmonary dysplasia (BPD), 2 had associated congenital anomalies, and one had tracheomalacia and polydacty

Case Report: Bronchial splinting for the treatment of compressive tracheobronchomalacia.

Compressive tracheomalacia is a rare subtype of pediatric airway malacia. We report a case of compressive tracheomalacia treated with bronchial splint implantation. No significant complications occurred during the 2-year follow-up, and the child maintained normal growth and development. Bronchial splint implantation is a safe and effective therapeutic option for such cases and can markedly improve respiratory function in children with favorable long-term tolerance.

Clinical survey and predictors for the development of tracheobronchomalacia in preterm infants.

BACKGROUND: Tracheobronchomalacia (TBM) contributes to the increased morbidity and mortality observed in preterm infants. Effective strategies for the prevention of TBM are necessary to achieve better outcomes. We sought to identify risk factors associated with the development of TBM in preterm infants. Optimal cut-off values for each risk factor were also determined. METHODS: A total of 80 infants who were born at 36 week's gestation or earlier and underwent flexible bronchoscopy were included in our study sample. A comparison of demographic and clinical risk factors between those with TBM (

An Adult Case of Pulmonary Artery Sling Accompanied by Tracheobronchomalacia.

Pulmonary artery (PA) sling is a congenital disease in which the left PA abnormally arises from the right PA and is usually diagnosed during the infantile period. We present an adult case of PA sling accompanied by tracheobronchomalacia found in a 49-year-old woman with a history of recurrent pneumonia. Computed tomography of the chest showed that the left lung was nourished by two aberrant PAs. Bronchoscopy demonstrated achondroplasia of the trachea and the right bronchus, which we speculate to have resulted in their stenosis. The recurrent pneumonia was attributable to these tracheobronchial

[Characteristics of tidal breathing pulmonary function in children with tracheobronchomalacia].

OBJECTIVE: To investigate the characteristics of tidal breathing pulmonary function in children with tracheobronchomalacia (TBM). METHODS: In this study, 30 children who were diagnosed with TBM using electronic bronchoscopy were enrolled in the observation group; 30 healthy children were recruited in the normal control group. For individuals in each group, the assessment of tidal breath pulmonary function was performed at diagnosis and 3, 6, 9, and 12 months after diagnosis. RESULTS: There were no significant differences in tidal volume, inspiratory time, expiratory time, and inspiratory to

Is silicone stent insertion a clinically useful option for tracheobronchomalacia?

BACKGROUND/AIM: Tracheobronchomalacia (TBM) leads to the obstruction of expiratory airflow and interference with secretion clearance. Stabilization of the airway wall using a silicon stent or laser coagulation of the posterior wall may be treatment options. This study aimed to retrospectively analyze which interventional bronchoscopic method could be used to provide airway stabilization and gain control of symptoms and for whom this method could be used. MATERIALS AND METHODS: Fifteen patients who had received treatment in our interventional pulmonology unit were analyzed. We analyzed the tec

Evaluation of tracheobronchomalacia by dynamic flexible bronchoscopy. A pilot study.

OBJECTIVES: Dynamic flexible bronchoscopy is the "gold standard" for assessing changes in airway luminal size associated with tracheobronchomalacia, but the procedure has not been adequately validated. The present study was designed to test the validity of diagnosing tracheobronchomalacia by dynamic flexible bronchoscopy through assessing inter- and intraobserver agreements in estimating degree of central airway collapse associated with tracheobronchomalacia. METHODS: This prospective observational pilot study enrolled consecutive patients with suspected tracheobronchomalacia scheduled for dy

Single-Center Experience of Tracheobronchoplasty for Tracheobronchomalacia: Perioperative Outcomes.

BACKGROUND: We present trends in practice as our experience has grown and report the postoperative morbidity and its associated factors after tracheobronchoplasty. METHODS: A retrospective cohort study was conducted of 161 patients who underwent tracheobronchoplasty from October 2002 to September 2016. The main outcome was development of a postoperative complication within 30 days of the operation. Postoperative complication events were graded using the Clavien-Dindo system. The study patients were divided into two consecutive cohorts to examine trends in systems of care. Postoperative morbid

[Analysis of 53 cases with bronchoscopically confirmed pediatric tracheobronchomalacia].

OBJECTIVE: Tracheobronchomalacia is one of the common respiratory tract dysplasia in children. Its symptoms are nonspecific, and routine methods are unreliable in the assessment of tracheobronchomalacia in children. In addition, many physicians are confused about its clinical characteristics, so tracheobronchomalacia is often underdiagnosed. The purpose of this study was to explore the clinical features of tracheobronchomalacia in children and to investigate the diagnostic value of flexible bronchoscopy for children with tracheobronchomalacia. METHOD: For diagnosis and treatment, 229 children

Great vessel anomalies and their impact on the surgical treatment of tracheobronchomalacia.

BACKGROUND: Tracheobronchial compression (TBC) from great vessel anomalies (GVA) can contribute to tracheobronchomalacia (TBM) symptoms. The frequency, impact on symptoms and optimal management of GVA in these patients, with or without a history of esophageal atresia (EA), are still unclear. STUDY DESIGN: Patients who underwent surgery for TBM/ TBC between 2001 and 2017 were reviewed. Demographics, type of GVA, and operative interventions were collected. The frequency and treatment modalities of GVA between EA and non-EA patients were compared. RESULTS: Overall, 209 patients met criteria; 12

Awareness, Competencies, and Practice Patterns in Tracheobronchomalacia: A Survey of Pulmonologists.

BACKGROUND: Tracheobronchomalacia (TBM) is a disorder of expiratory central airway collapse. TBM is separate from excessive dynamic airway collapse. Historically TBM has lacked a universally accepted definition. No consensus recommendations on evaluation and management exist. We suspect these unresolved issues contribute to deficits in pulmonologists' awareness and management of TBM. METHODS: We created a 20-question survey obtaining information about overall awareness, knowledge base, competencies, and practice patterns in managing TBM. The survey was disseminated via email by American Colle

Laser tracheobronchoplasty: a novel technique for the treatment of symptomatic tracheobronchomalacia.

The management of tracheobronchomalacia is a very challenging problem with few treatment options. This study aims to evaluate the outcomes of a novel surgical treatment for membranous tracheobronchomalacia. A consecutive series of patients with tracheobronchomalacia were treated with two to three holmium laser scarring surgeries of the hyperdynamic tracheal and bronchial walls for the purpose of stiffening them through fibrosis. Patients filled out a Dyspnea Index questionnaire before and after treatment. Ten patients were treated for their tracheobronchomalacia with a mean age of 54 years. Sy

Tracheobronchial Membrane Tapering Resection for Tracheobronchomalacia With Wide Posterior Membrane.

BACKGROUND: Tracheobronchomalacia is characterized by excessive dynamic airway collapse, often associated with a wide posterior membrane. This report describes outcomes of an innovative tapering membrane reduction tracheobronchoplasty (TMRT) designed to narrow the posterior membrane and improve functional airway stability. METHODS: This was a retrospective study of patients who underwent TMRT for severe tracheobronchomalacia with a wide posterior membrane from 2022 to 2024 at 2 institutions. Under bronchoscopic guidance, a longitudinal strip of posterior membrane was resected, with concurrent
