Radiographic spectrum of pleuropulmonary tularemia.

Pleuropulmonary disease was seen in 50 of 62 patients (81%) with proven tularemia. Radiographic findings included patchy subsegmental air space opacities (74%), hilar lymphadenopathy (32%), and pleural effusion (30%). Less common manifestations were air space opacification of an entire lobe or segment, cavitation, oval opacities, pericardial effusion, linear opacities and septal lines, apical and miliary disease resembling tuberculosis, a mediastinal mass, empyema with bronchopleural fistula, and residual cystic changes, calcification, and fibrosis. Pleuropulmonary tularemia may be easily misd

Pulmonary tularemia: A diagnosis not to overlook.

Tularemia is a zoonotic disease caused by Francisella tularensis, a highly virulent bacterium, with an increasing incidence worldwide. Pulmonary involvement is uncommon and often presents misleading clinical and radiological features. This case series aims to describe the clinical characteristics, imaging features, and management of patients with pulmonary tularemia. All cases diagnosed in our department between 2016 and 2023 were included. Seven patients were identified. Clinical presentation was nonspecific, most frequently including fever, respiratory symptoms, fatigue, and headaches. Chest

Tularemia as a waterborne disease: a review.

Francisella tularensis is a Gram-negative, intracellular bacterium causing the zoonosis tularemia. This highly infectious microorganism is considered a potential biological threat agent. Humans are usually infected through direct contact with the animal reservoir and tick bites. However, tularemia cases also occur after contact with a contaminated hydro-telluric environment. Water-borne tularemia outbreaks and sporadic cases have occurred worldwide in the last decades, with specific clinical and epidemiological traits. These infections represent a major public health and military challenge. Hu

Tularemia and the oculoglandular syndrome of Parinaud.

An 18-year-old female came to our clinic with complaints of a tender lump just under her jaw on the left side and another lump in front of her left ear, ecchymosis around the eye and some redness in the eye at the same side. After administering antibiotic therapy for two days we suspected of tularemia and referred the patient to the Infectious Diseases Department. A serum sample was taken and a fine needle aspiration biopsy was performed. The patient was diagnosed with tularemia, the oculoglandular syndrome of Parinaud. Tularemia should also be kept in mind for differential diagnosis in patien

[Outbreak of tularemia in the Republic of Dagestan].

In Daghestan in the focus of the flood-plain swamp type 64 persons fell ill with tularemia in 1999. During the epizootological survey 8 Francisella tularensis strains were isolated from vectors and carriers and in 7 samples taken from open water reservoirs, as well as in 1 sample obtained from Ixodes ticks, tularemia antigen was detected. Humans were infected mainly by the transmissive route, as found in 71.8% of patients. 28% of patients were infected through contacts. The disease took mainly a mild course, registered in 95.6% of patients. The disease affected those persons who had not been i

[The glycolipids of the causative agent of tularemia].

Up to 10 glycolipids were detected in F. tularensis with the use of thin-layer chromatographic techniques. These glycolipids were slime antigens of F. tularensis membrane. Attenuated F. tularensis strains were found to have defects in their glycolipid composition: in the vaccine strain glycolipid 8 was replaced by more polar lipid 8-a; the avirulent strain had only two glycolipids, and one of them was not typical for virulent strains. Considering that glycolipids differed from entero-bacterial Vi-antigen in their physical-chemical and biological properties, the suggestion was made that the use

A case of tularemia meningitis in Sweden.

Tularemia meningitis occurred in a previously healthy 64-year-old man from Northern Sweden. The patient was immunocompetent and recovered without sequelae. The bacterial isolate conformed by most criteria with the Euro-Asian biovar Francisella tularensis var. palaearctica. Tularemia meningitis may mimic meningitis caused by Mycobacterium tuberculosis.

Airborne transmission of tularemia in farmers.

In a tularemia epidemic during 1982 in northern Finland, 53 patients showed no peripheral portal of entry for infection or associated lymphadenopathy. Respiratory symptoms were observed in 72% of the patients. 26/38 cases had abnormal chest films. Hilar adenopathy was the most common finding (36%). Four patients did not receive antibiotics; 43 received tetracyclines, 5 streptomycin and 1 cefuroxime and amoxycillin. All patients recovered. 50 patients acquired the infection during common farming activities, such as making fresh hay with a hay-cutter, handling dry hay, threshing, etc. Thus, airb

A case report of ulceroglandular tularemia.

Tularemia, caused by the bacterium Francisella tularensis, is a zoonotic disease that has been reported in many countries across the globe. It may manifest in many forms, like pneumonic, oropharyngeal, glandular, ulceroglandular, or oculoglandular. In Poland, one of the most common types of tularemia is ulceroglandular. Typical symptoms of ulceroglandular tularemia include painful ulcers at the site of the insect or animal bite, along with systemic flu-like symptoms. Tularemia is generally effectively treated with appropriate antibiotics, however, frequent misdiagnosis and subsequent improper

[Pulmonary tularemia : a diagnosis not to be missed].

Tularemia (caused by the facultative intracellular Gram-negative bacillus Francisella tularensis) is an endemic zoonotic disease in Europe, which exhibits different clinical patterns. Following the glandular form, pneumonia is the second most frequent manifestation in Switzerland. Pulmonary tularemia often has a subacute course and fails to respond to beta-lactam antibiotics. It can also mimic tuberculosis, because of the presence of systemic symptoms, such as fever, sweats and weight loss. History of animal exposure is not always reported. Clinicians should be aware of pulmonary tularemia. Th

An outbreak of tularemia in southwestern Turkey.

INTRODUCTION: Tularemia has reemerged and spread throughout Turkey, and the number of cases has increased. In this study, we report on a waterborne outbreak of tularemia in the spring of 2013 in a region which was previously disease-free, and we investigated the reasons for the outbreak. METHODOLOGY: The index case, a 17-year-old male, was diagnosed with oropharyngeal tularemia. An outbreak investigation was initiated after receiving information from other patients with similar symptoms from the same village along with Balkıca, Tavas, and Denizli. An epidemiological and environmental investig

Tularemia in a Prosthetic Joint Infection.

Periprosthetic infections occur in approximately 0.8% to 1.9% of all total knee arthroplasties (TKAs). Even with these low rates, it is rare to find a zoonotic bacterium causing a periprosthetic infection. In this case report, the authors identify the second documented case of a total joint infection with Francisella tularensis in the world and the first in the United States. A 58-year-old man underwent a left TKA in 1994 and a right TKA in 1997 for severe primary bilateral knee osteoarthrosis. In 2015, he underwent polyethylene exchange for polyethylene wear. Subsequently, he developed repeat

Serendipitous Treatment of Tularemia in Pregnancy.

We present a young pregnant woman who developed ulceroglandular tularaemia following a bite wound from a kitten. She grew Francisella tularensis from the ulcer. While awaiting bacterial culture results and serology for Bartonella, she was treated with azithromycin, with resolution of fever and axillary tenderness. Treatment recommendations for tularemia are either gentamicin or doxycycline, both of which can be perilous to the fetus. A Centers for Disease Control and Prevention report on the macrolide susceptibility of North American isolates of this organism has been underappreciated. The una

An overview: tularemia and travel medicine.

Tularemia is a bacterial zoonotic infection. The disease is endemic in most parts of the world, has been reported through the northern hemisphere between 30 and 71° N latitude. Francisella tularensis causes infection in a wide range of vertebrates (rodents, lagomorphs) and invertebrates (ticks, mosquitoes and other arthropods). Humans can acquire this infection through several routes including; a bite from an infected tick, deerfly or mosquito, contact with an infected animal or its dead body. It can also be spread to human by drinking contaminated water or breathing contaminated dirt or aeros
