Long-Term Indwelling Tandem Polymeric Ureteral Stents for Benign Ureteral Obstruction.

Objective: To assess the outcome of tandem polymeric internal stents (TIS) for benign ureteral obstruction (BUO). Material and Methods: We conducted a retrospective study that included all consecutive patients treated for BUO by means of TIS in a single tertiary center. Stents were replaced routinely every 12 months or earlier, when indicated. The primary outcome was permanent stent failure, and the secondary outcomes included temporary failure, adverse events, and renal function status. Kaplan-Meier and regression analyses were used to estimate outcomes, and logistic regression was used to as

10-Year Experience with Reinforced Ureteral Stents for Malignant Ureteral Obstruction.

INTRODUCTION: Malignant ureteral obstruction (MUO) hinders the management of malignant diseases. Indwelling stent is a common method to release renal obstruction, but stent failure with obstruction is frequent. The studies conclude that stent obstruction divides survival by 2 or even 4. We carried out the present study in order to evaluate drainage efficiency and overall survival by using stents with distinctive degrees of stiffness. MATERIALS AND METHODS: We performed an analysis of 156 patients with MUO drainage at a single institution from June 2009 to June 2019. RESULTS: Of the 156 patie

Ureteral metal stents: 10-year experience with malignant ureteral obstruction treatment.

PURPOSE: Ureteral patency in malignant ureteral obstruction cases is a therapeutic challenge. We report our long-term experience with palliative treatment for extrinsic malignant ureteral obstruction with percutaneous placement of metal mesh stents. MATERIALS AND METHODS: From January 1996 to December 2005, 90 patients with a mean age of 59 years (range 35 to 80) with ureteral obstruction due to extrinsic ureteral compression and/or encasement by primary or metastatic tumors, or retroperitoneal lymphadenopathy underwent implantation of self-expandable metal mesh stents. A total of 119 ureters

Ureteral ischemia model: an explanation of ureteral dysfunction after chronic obstruction.

BACKGROUND AND PURPOSE: Many models of smooth muscle ischemia have been developed to explain organ insufficiency or failure. Ureteral decompensation may also be described in these terms. We anticipate that ureteral ischemia will result from overdistention brought about by obstruction. A preliminary model to create an ischemic ureter is described herein. MATERIALS AND METHODS: Six white New Zealand female rabbits were used for this study. All had their left ureters surgically ligated at the level of the urinary bladder. The right ureters served as controls. In the acute-phase group, the ureter

Comparison between Antegrade versus Retrograde Ureteral Stent Placement for Malignant Ureteral Obstruction.

PURPOSE: To compare the technical success of antegrade uteral stent (AUS) and retrograde ureteral stent (RUS) placements in patients with malignant ureteral obstruction (MUO) and to determine the predictors of technical failure of RUS. MATERIALS AND METHODS: This study retrospectively included 61 AUS placements (44 patients) performed under fluoroscopic guidance and 76 RUS placements (55 patients) performed under cystoscopic guidance in patients with MUO from January 2019 to December 2020. Technical success rates of the 2 techniques were compared using inverse probability of treatment weighti

Paradoxical tubular obstruction after release of ureteral obstruction in rat kidney.

The healing process of hydronephrosis was studied using rat kidney. Following release of 2 weeks' unilateral hydronephrosis, renal papilla and cortex were examined by scanning electron microscopy. Shortly after the release of obstruction, massive loss of brush border and ballooning of epithelial cells were noted in proximal tubules. Distal tubules and collecting tubules also showed severe ballooning of epithelial cells. Actually most of them were completely or nearly obliterated in spite of the release of ureteral obstruction. Two weeks later, on the other hand, the many tubular components bec

Factors Associated with Ureteral Stent Failure in Patients with Malignant Ureteral Obstruction.

Introduction: Management of malignant ureteral obstruction (MUO) with ureteral stents remains a clinical challenge, often involving frequent stent exchanges attributable to stent failure or other urological complications. We report our institutional experience with ureteral stents for management of MUO, including analysis of clinical factors associated with stent failure. Methods: We performed a retrospective review of patients treated with indwelling ureteral stents for MUO in nonurothelial malignancies at our tertiary-care institution between 2008 and 2019. Univariate Cox proportional hazard

Failure of ureteral stents subject to extrinsic ureteral obstruction and stent occlusions.

PURPOSE: To quantify the occurrence of stent failure and the dynamic behavior of urine flow in ureter-stent systems, including the relative flow in the ureter and stent lumina, subject to various degrees of ureter and stent blockage. METHODS: Numerical simulations based on computational fluid dynamics (CFD) were used to quantify urine flow behavior in stented ureters, in the presence of extrinsic ureteral obstruction (EUO) and stent occlusions. Two stented ureter configurations were considered, one with circumferential occlusion of the ureter and the second with pressure on one side of the ur

Metallic ureteral stents in malignant ureteral obstruction: clinical factors predicting stent failure.

PURPOSE: To provide clinical outcomes of the Resonance metallic ureteral stent in patients with malignant ureteral obstruction, as well as clinical factors predicting stent failure. METHODS: Cancer patients who have received Resonance stents from July 2009 to March 2012 for ureteral obstruction were included for chart review. Stent failure was detected by clinical symptoms, image studies, and renal function tests. Survival analysis for stent duration was used to estimate patency rate and factors predicting stent failure. RESULTS: A total of 117 stents were inserted successfully into 94 urete

Determinants of ureteral obstruction after percutaneous nephrolithotomy.

BACKGROUND: Ureteral obstruction after percutaneous nephrolithotomy (PCNL) may require prolonged drainage with a nephrostomy tube (NT) or ureteral stent, but it is not well understood how and why this occurs. The goal of this study was to identify risk factors associated with postoperative ureteral obstruction to help guide drainage tube selection. METHODS: Prospective data from adult patients enrolled in the Registry for Stones of the Kidney and Ureter (ReSKU) who underwent PCNL from 2016 to 2020 were used. Patients who had postoperative NTs with antegrade imaging-based flow assessment on po

Fibroepithelial ureteral polyps presenting as ureteropelvic obstruction.

A 57-year-old woman presented with bilateral abdominal pain and flank discomfort. Imaging studies, consisting of CT scan, diethylene triamine pentaacetic acid renal scan with Lasix and a retrograde pyelogram, indicated an obstruction at the uteropelvic junction (UPJ), possibly due to fibroepithelial polyps within the ureter. A robotic pyeloplasty revealed a ureteral diverticulum and a thin, still-attached fibroepithelial polyp of approximately 2 cm in length. The patient tolerated the procedure well and was discharged one day postpyeloplasty with no reported complications. This rare clinical s

Bilateral ureteral obstruction after appendectomy in children.

Five cases of bilateral ureteral obstruction after appendicectomy are presented. All five patients were boys (age range, 9 to 15 years). All of them had had severe appendicitis. Based on the urethrocystoscopy findings, edema of the posterior bladder wall appeared to be the cause of obstruction of both distal ureters. This is confirmed by the immediate recovery of renal function after installation of bilateral uretercatheters. It is known that contamination of the peritoneal cavity can occur by organisms leaking from a gangrenous or perforated appendix. This can cause localized inflammatory ede

Effects of ureteral obstruction on renal growth.

Renal insufficiency due to congenital obstructive nephropathy is a consequence of arrested or abnormal renal growth. A number of experimental studies have shown that the younger the age at the time of unilateral ureteral obstruction (UUO), the more severe the growth impairment of the ipsilateral kidney, and the greater the compensatory growth of the opposite kidney ("counterbalance"). Urinary obstruction in early fetal life results in renal dysplasia and a decrease in the number of functioning nephrons. The renin-angiotensin system is highly activated in early development, and UUO further incr

Retroperitoneal polyarteritis nodosa presenting as ureteral obstruction.

Polyarteritis nodosa (PAN) may be systemic or isolated in distribution and may involve virtually any organ or tissue in the body. Retroperitoneal PAN with ureteral obstruction as the first manifestation of the disease, to our knowledge, has not been previously reported. Two such unusual cases are described.

Ureteropelvic junction obstruction associated with ureteral hypoplasia.

We present 3 cases in which a neonate had unilateral hydronephrosis associated with a ureter hypoplastic throughout its length. In all 3 infants the affected kidney failed to visualize on excretory urography and function of the kidney proved to be disappointing despite its relatively normal appearance on external examination. In each case significant renal dysplasia was present microscopically and ultimately the kidney had to be removed. A retrograde pyelogram should be considered whenever a hydronephrotic kidney in an infant fails to visualize, and if this shows a long hypoplastic ureter that
