Failures and complications of transurethral ureteroscopy in 297 cases: conventional rigid instruments vs. small caliber semirigid ureteroscopes.

To evaluate the safety and effectiveness of ureteroscopic stone manipulation, we retrospectively reviewed the failure and complication rates of two series of retrograde ureteroscopies (URS) according to the instrument used: the conventional rigid ureteroscopes and the new fiberoptic small size ureteroscopes. From July 1985 to December 1992, we performed 248 URS in 238 patients using a conventional rigid ureteroscope. From January 1992 to December 1992, we performed 49 URS in 47 patients using a semirigid ureteroscope. The failure rate was 14.5% in the first group of patients. The early and lat

Multimodal comparison of 19 single-use flexible ureteroscopes: bench testing, operative video assessment, and surgeon ratings.

PURPOSE: Single-use flexible ureteroscopes (fURSs) differ substantially in optical and mechanical properties, yet comparative evidence remains limited. We performed a multimodal in vitro and clinical assessment of 19 commercially available single-use fURSs to quantify performance variability and examine which device characteristics are associated with surgeon-reported usability. METHODS: Each of 19 models was bench-tested for field of view, laser-fiber safety distance, irrigation flow, deflection, image resolution, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and color discrimi

The 'suction concept': everything a flexible ureteroscope should have.

OBJECTIVE: To develop an accurate 'suction concept' that should be part of every flexible ureteroscope (FU) and review recent literature on endourology's new technologies to determine if they incorporate all the features of our new concept, as several milestone developments in retrograde intrarenal surgery have led to a re-framing of innovation goals with recent focus on novel laser technology and suction techniques. METHODS: A literature review on the main suction (suctioning ureteric access sheath [SUAS] or flexible and navigable SUAS [FANS], steerable ureteroscopic renal evacuation [SURE],

AVA: Automated Viewability Analysis for Ureteroscopic Intrarenal Surgery.

Ureteroscopic intrarenal surgery comprises the passage of a flexible ureteroscope through the ureter into the kidney and is commonly used for the treatment of kidney stones or upper tract urothelial carcinoma (UTUC). Flexible ureteroscopes (fURS) are limited by their visualization ability and fragility, which can cause missed regions during the procedure in hard-to-visualize locations and/or due to scope breakage. This contributes to a high recurrence rate for both kidney stone and UTUC patients. We introduce an automated patient-specific analysis for determining viewability in the renal colle

The "no-touch" technique in the flexible ureteroscopic approach of renal stones.

Large meta-analyses demonstrated that ureteral access sheaths (UAS) have specific complications during and after flexible ureteroscopy (fURS). The present study focused on the technical aspects, advantages, drawbacks, and limitations of the latest "no-touch" technique (NTT) in the flexible ureteroscopic therapeutic approach of renal stones. A total of 288 patients with a single pyelocaliceal stone (largest diameter between 11 and 29 mm) underwent fURS: 144 using the 12/14 Fr UAS (group 1) and 144 without UAS (group 2). For NTT, we used four types of ureteroscopes: Olympus URF-V2 (8.5 Fr) - 33

Prevention strategies for ureteral stricture following ureteroscopic lithotripsy.

Ureteral stricture formation after ureteroscopic lithotripsy is a late complication that can lead to hydronephrosis and a subsequent risk of renal deterioration. The specific incidence is unknown, and the mechanism of stricture formation has not been completely explained. In this review, we summarize the current evidence regarding the incidence of this condition and discuss its pathogenesis. We then list preventive strategies to reduce the morbidity of ureteral strictures.

Ureteroscopic resection of upper-tract transitional-cell carcinoma.

Technological advances have increased the applicability of endoscopic treatment for upper-tract transitional-cell carcinoma (TCC). Percutaneous and ureteroscopic tumor resection have become reasonable treatment options for patients with anatomically or functionally solitary kidneys, bilateral upper-tract tumors, significant renal insufficiency, or comorbid disease that would preclude standard open surgery. This approach also is being used increasingly on those with a normal contralateral kidney in whom nephroureterectomy and en bloc removal of the ipsilateral ureteral orifice and surrounding b

Laparoscopic pyeloplasty with simultaneous pyelolithotomy using a flexible ureteroscope.

Laparoscopic pyeloplasty is rapidly becoming the gold standard in the repair of adults with uretero-pelvic junction obstruction. Renal calculi are a recognized complication of uretero-pelvic junction obstruction and their presence has often been used to justify percutaneous nephrolithotomy and endopyelotomy as primary treatment. Stones developing secondary to uretero-pelvic junction obstruction have been removed through simultaneous laparoscopic pyeloplasty and pyelolithotomy, but with rigid laparoscopic instruments access is limited to the renal pelvis. As exemplified within this report, endo

Ureteroscopic laser treatment of upper urinary tract tumors.

OBJECTIVE: To summarize the present status of ureteroscopic laser treatment of upper urinary tract tumors. SUMMARY AND BACKGROUND DATA: Small diameter rigid and flexible ureteroscopes can provide convenient access to the upper urinary tract. The small diameter of the instruments and the working channels are ideally suited for the placement of laser fibers as an intraluminal ablative technique. METHODS: The authors reviewed the literature and their own experience with laser treatment of upper tract tumors for the description of instruments, techniques, and the results achieved. RESULTS: At l

Electrohydraulic lithotripsy of upper ureteral calculi with semirigid ureteroscope.

Forty-three patients with single ureteral calculi located above the pelvic brim were treated by electrohydraulic lithotripsy (EHL) using a semirigid mini-ureteroscope. Of the 43 calculi, 36 (84%) were fragmented in one procedure. Six calculi (14%) were pushed back to the renal pelvis and treated successfully by complementary extracorporeal shockwave lithotripsy (SWL). The total success rate of ureteroscopic manipulation thus was 98%. The operation time (mean 26.4 minutes) and postoperative hospital stay (mean 2.53 days) were relatively short. Complications were limited to four (9%) minor perfo

[Diagnosis of the upper urothelial tumor using a rigid ureteroscope].

We preoperatively diagnosed four cases suspected as malignant urothelial tumor of upper urinary tract using a rigid ureteroscope. In three cases of ureter tumor, biopsy was successful and showed low grade malignancy. Nephroureterectomy was performed in two cases with upper ureteral tumor. The tumor was fulgurated ureteroscopically in a case of lower ureteral tumor. In one case of renal pelvic tumor, biopsy was unsuccessful though the tumor was observed as a papillary configuration. Successively, biopsy using a flexible ureteroscope was performed and the tumor was treated by fulguration. Ureter

Ureteroscopic manipulation of stones in the ureter: four years experience.

Our experience of 223 ureteroscopies and stone manipulations is reported. Stone removal was successful in 82% of the cases whereof the stones were pushed back into the renal pelvis and subsequently managed with percutaneous nephrolithotomy or extracorporeal shock wave lithotripsy in 24%. For distal stones the success rate was 88%. Reasons for failure were acute obstruction of the ureter and previous surgery or irradiation to the pelvis. In six cases (4%) strictures requiring treatment developed secondary to ureteroscopy. Ureteroscopy can be a tedious and difficult procedure with a considerable

Ureteroscopic diagnosis and treatment of urinary calculi during pregnancy.

Flexible ureteroscopy has been utilized in two pregnant patients. In one, the procedure was for diagnosis of symptoms simulating ureteral colic, and in the other for the removal of a distal ureteral calculus. The diagnostic procedure was completed with local anesthesia alone, and both procedures were performed without radiation to the mother or fetus. The potentially great benefit must be weighed against the risks of minimal endoscopic intervention in each patient. We advocate the early but judicious use of flexible ureteroscopy in pregnant patients with urinary calculi.

Characteristics of current digital single-use flexible ureteroscopes versus their reusable counterparts: an in-vitro comparative analysis.

BACKGROUND: Single-use flexible ureterorenoscopes (fURSs) have been recently introduced aiming to offer solutions to the sterilization, fragility and cost issues of the reusable fURSs. In order to be a viable alternative, the single-use scopes must prove similar capabilities when compared to their reusable counterparts. The goal of our in-vitro study was to compare the current reusable and single-use digital fURSs regarding their deflection, irrigation and vision characteristics. METHODS: We compared in-vitro 4 single-use fURSs-LithoVue™ (Boston Scientific, Marlborough, Massachusetts, USA), U
