The role of fibrinolytic agents in the management of thrombotic complications associated with vascular access devices.

VADs have made a significant contribution to improving quality of life for patients requiring long-term therapies. Clearly, this means of vascular access has offered relief from repeated venipuncture for IV access or blood sampling. Reports of the incidence of associated noninfectious complications vary between 14% and 25%. These rates are significant and warrant the development of strategies to manage such events. Fibrinolytic therapy offers a viable option for patients who have developed complications of a thrombotic nature. Although direct IV push administration may be relatively well accep

Early experience with CELT ACD vascular access closure device for neurointerventional procedures.

INTRODUCTION: The CELT ACD (Vasorum Ltd, Dublin, Ireland) is an access closure device achieving hemostasis with a steel radiopaque structure deployed into the arterial wall without sheath exchange. There is limited data regarding its use in neurointerventional procedures, particularly with larger arteriotomies. METHODS: A prospective registry of neurointerventional procedures at a single center between July 2024 and February 2025 was retrospectively reviewed. Consecutive transfemoral cases with 6, 7, and 8F sheaths undergoing closure with a newly-deployed 6 or 7F CELT were included. The prima

Silver iontophoretic catheter: a prototype of a long-term antiinfective vascular access device.

A silver iontophoretic catheter (SIC) was developed consisting of two electrically charged parallel silver wires helically wrapped around the proximal segment of a vascular catheter. In vitro and in vivo activities of this catheter were compared with those of an aseptic catheter coated with chlorhexidine and silver sulfadiazine (CH/SS). The SIC demonstrated broad-spectrum in vitro inhibitory activity against bacteria and Candida albicans comparable to that of the CH/SS-coated catheter. The durability of activity was determined by incubating catheters in serum at 37 degrees C for various time i

Implications for maintaining vascular access device patency and performance: Application of science to practice.

INTRODUCTION: Vascular access devices are commonly inserted devices that facilitate the administration of fluids and drugs, as well as blood sampling. Despite their common use in clinical settings, these devices are prone to occlusion and failure, requiring replacement and exposing the patient to ongoing discomfort/pain, local vessel inflammation and risk of infection. A range of insertion and maintenance strategies are employed to optimize device performance; however, the evidence base for many of these mechanisms is limited and the mechanisms contributing to the failure of these devices are

Evaluating Normal Saline Locking With Totally Implanted Vascular Access Devices for Patients on 90-Day Maintenance Flushing.

BACKGROUND: Heparin has been the preferred locking solution for many organizations for totally implanted vascular access devices (TIVADs). Published data are limited on the use of normal saline alone as a locking solution for TIVAD 90-day maintenance flushing. OBJECTIVES: The purpose of this evidence-based practice initiative was to determine whether locking TIVADs with normal saline alone during a 90-day maintenance flushing protocol resulted in an increase in partial occlusions. METHODS: A clinical team monitored alteplase administration to measure partial occlusions. Prior to heparin deim

Skin Necrosis Distal to a Rapid Infusion Catheter: Understanding Possible Complications of Large-bore Vascular Access Devices.

Rapid infusion catheters (RICs) allow expedient conversion of peripheral intravenous (PIV) catheters to peripheral sheaths; however, little is known about potential complications. In this case, a 64-year-old male polytrauma patient had a 20-gauge PIV catheter in the right cephalic vein upsized to an 8.5 French (Fr) RIC without incident during an arrest with pulseless electrical activity (PEA). On RIC post-placement day two, the patient developed edema and ecchymosis extending from the right dorsal mid-hand to the antecubital fossa, just distal to the RIC insertion point. Compartments were soft

[Arterial Access Site Complications after Percutaneous Interventions with Special Regard to the Application of Vascular Closure Devices].

Due to an increase in the number of performed endovascular procedures, the number of local access site complications is rising too. Used mainly for treatment of peripheral arterial disease and coronary heart disease, endovascular procedures are gaining importance. Access site complications include bleeding, haematoma, pseudoaneurysm, arteriovenous fistula and arterial thrombosis. Aiming to reduce immobilisation, length of hospital stay, costs and access site complications, vascular closure devices (VCD) were introduced in the mid 1990s, but current trials failed to demonstrate the superiority

Increased vascular access complications in patients with renal dysfunction undergoing percutaneous coronary procedures using arteriotomy closure devices.

BACKGROUND: Arteriotomy closure device (ACD) use has increased following percutaneous transfemoral coronary procedures (PTCP). However, their safety in patients with chronic kidney disease (CKD) is not known. Therefore, we evaluated the complication rates of ACD among patients with CKD. METHODS: Six-hundred ten consecutive patients who underwent PTCP and ACD were retrospectively studied. Patients were grouped according to their creatinine clearance (CrCl in ml/min/1.73 m2) calculated by the Cockcroft-Gault formula using the National Kidney Foundation classification system; Stage I (CrCl > or

First-in-human evaluation of a novel subcutaneous intraosseous vascular access device for outpatient infusions.

BACKGROUND: Conventional intraosseous (IO) access is typically reserved for emergency situations and limited to short-term use (⩽48 h). We introduce a novel, fully subcutaneous IO device designed to provide stable, longer-term vascular access in non-emergent settings. This device may benefit patients requiring intravenous therapies with challenging venous access or chronic kidney disease (CKD) who need to preserve peripheral and central veins for future arteriovenous fistula creation for hemodialysis. AIM AND METHODS: This prospective, single-arm, first-in-human study evaluated the feasibilit

Nursing and midwifery practice for maintenance of vascular access device patency. A cross-sectional survey.

BACKGROUND: Up to 85% of hospital in-patients will require some form of vascular access device to deliver essential fluids, drug therapy, nutrition and blood products, or facilitate sampling. The failure rate of these devices is unacceptably high, with 20-69% of peripheral intravenous catheters and 15-66% of central venous catheters failing due to occlusion, depending on the device, setting and population. A range of strategies have been developed to maintain device patency, including intermittent flushing. However, there is limited evidence informing flushing practice and little is known abou

Evaluation of Skin Colonisation And Placement of vascular access device Exit sites (ESCAPE Study).

BACKGROUND: Skin microorganisms may contribute to the development of vascular access device (VAD) infections. Baseline skin microorganism type and quantity vary between body sites, yet there is little evidence to inform choice of VAD site selection. OBJECTIVE: To compare microorganisms present at different body sites used for VAD insertions and understand the effect of transparent dressings on skin microflora. METHODS: The ESCAPE observational study consisted of three phases: (1) skin swabs of four sites (mid-neck, base neck, chest, upper arm) from 48 hospital patients; (2) skin swabs of fiv

Plug- Versus Sutured-Based Vascular Closure Devices for Large Bore Arterial Access: A Systematic Review and Meta-Analysis.

Vascular complications after cardiovascular interventions requiring large-bore arterial access remain frequent and devastating. Whether plug-based vascular closure devices (P-VCD) or sutured-based vascular closure device (S-VCD) are superior in terms of safety and efficacy remains unclear. Aim of our systematic review and meta-analysis was to compare P-VCD with S-VCD. We systematically screened 3 major databases for 2-arm studies comparing P-VCD with S-VCD. Primary endpoint was device success. Secondary endpoints included in-hospital all-cause mortality, major and minor bleeding, major and min

Arteriovenous fistula creation in a patient without a pulse: Vascular access in patients with left ventricular assist devices.

Left ventricular assist devices are used in heart failure patients as bridge to transplantation or increasingly as a destination therapy. These patients frequently have renal dysfunction and many reach end-stage renal failure. If haemodialysis is required, minimization of infection risk is essential. Arteriovenous grafts have been recommended for these patients due to hypothetical concerns regarding fistula maturation due to continuous flow. A case is described where a brachiocephalic arteriovenous fistula was successfully formed and used for dialysis without issue. This is one case of a small

Mynx vascular closure device achieves reliable closure and hemostasis of percutaneous transfemoral venous access in a porcine vascular model.

AIMS: Vascular closure device (VCD)-based venous closure has been anecdotally reported, but systematic evaluation of the reparative response of the vessel wall to venous closure is lacking. The need to control groin complications, and minimize risks associated with postponed sheath removal under conditions of persistent anticoagulation, has generated interest in the role of VCDs for venous access closure. We sought to characterize the vessel wall response to venous closure, both acutely and in delayed fashion at 30 days using angiography, ultrasound, and histology. METHODS: Ten venous 7 Fr sh

Vascular Closure Devices for Large Bore Femoral Access After Transfemoral Transcatheter Aortic Valve Replacement: A Network Meta-Analysis.

BACKGROUND AND AIMS: Large-bore femoral access is commonly required in transfemoral transcatheter aortic valve replacement (TF-TAVR), but vascular access complications remain prevalent, impacting patient outcomes. Various vascular closure devices (VCDs) are used for hemostasis, including the suture-based Prostar XL, Perclose ProGlide, and ProStyle, and the plug-based MANTA device. However, comprehensive comparative evidence on these VCDs, particularly single Perclose strategies, is lacking. METHODS: A systematic review and network meta-analysis were conducted according to PRISMA guidelines. W

Access site complications following transfemoral coronary procedures: comparison between traditional compression and angioseal vascular closure devices for haemostasis.

BACKGROUND: Vascular closure devices such as angioseal are used as alternatives to traditional compression haemostasis. Although the safety and efficacy of angioseal are confirmed, their use remains controversial because of the potential complications of these devices compared with those of traditional compression haemostasis. The aim of this study was to compare the access site complication rate, the predictive factors for these complications, and patient comfort levels after coronary procedures with traditional compression or angioseal haemostasis. METHODS: Data were collected from a cardia

ESR Essentials: arterial vascular access and closure devices-practice recommendations by the Cardiovascular and Interventional Radiological Society of Europe.

Vascular access is the initial, very important, step of endovascular procedures. Various access sites include the common femoral artery, brachial artery, radial artery, popliteal artery, and distal tibial vessels (pedal arteries). Successful arterial access requires advanced knowledge of anatomy, as well as proper training and experience. Today, vascular access should be obtained using real-time, ultrasound guidance to reduce access time, patient discomfort, and puncture-related complications including dissection, arteriovenous communication, and bleeding. Nevertheless, high-level evidence to
