Sub-Tenon's anesthesia in vitreoretinal surgery: a needleless technique.

PURPOSE: To evaluate the safety and efficacy of "needleless" regional anesthesia via the sub-Tenon's route without adjunct facial nerve or subconjunctival block for vitreoretinal surgery. METHODS: Forty-eight consecutive patients were included and studied prospectively. The number, time, and volume of anesthetic solutions given were recorded. Any complications such as severe intraocular pressure rise or orbital hemorrhage were noted. Within 24 hours after surgery, the patients were asked to grade their level of pain during the procedure using a standard 10-point visual analog scale. The use o

Visual results of lens-sparing vitreoretinal surgery in infants.

Lens removal is routinely performed when vitreoretinal surgery is done in infant eyes. We performed vitreous surgery in nine infants (10 eyes) without removing the crystalline lens. Surgery was done to correct vitreoretinal pathology involving the macula. Indications for surgery included: 1) tractional retinal detachment secondary to retinopathy of prematurity, familial exudative vitreoretinopathy, or congenital retinal folds; and, 2) nonclearing preretinal hemorrhage secondary to shaken-baby syndrome. Surgical objectives were achieved in all cases. Postoperatively, 8 of 10 eyes showed no evid

Current and future applications of intraocular bioadhesives in vitreoretinal surgery.

Achieving retinal adhesion and tissue healing in rhegmatogenous retinal detachment repair has been explored using numerous methods since the foundational principles established by Jules Gonin. Current approaches rely on delayed chorioretinal adhesion through thermal injury and inflammatory-mediated scarring. Intraocular bioadhesives (IOBs) offer an alternative strategy, providing efficient sealing of retinal breaks, and abrogating reliance on tamponade agents and their associated morbidities. Despite this, no IOBs are currently licensed for vitreoretinal surgery with limited clinical data to s

A Review of Robotic and OCT-Aided Systems for Vitreoretinal Surgery.

The introduction of the intraocular vitrectomy instrument by Machemer et al. has led to remarkable advancements in vitreoretinal surgery enabling the limitations of human physiologic capabilities to be reached. To overcome the barriers of perception, tremor, and dexterity, robotic technologies have been investigated with current advancements nearing the feasibility for clinical use. There are four categories of robotic systems that have emerged through the research: (1) handheld instruments with intrinsic robotic assistance, (2) hand-on-hand robotic systems, (3) teleoperated robotic systems, a

The role of vitreoretinal surgery in the treatment of posttraumatic macular hole.

PURPOSE: To determine if vitreoretinal surgery is successful in closing traumatic macular holes and subsequently improving visual acuity. Blunt trauma may result in a macular hole when it causes traumatic separation of the vitreous from the retina, contusion necrosis, or subfoveal hemorrhage. Like idiopathic macular holes, traumatic macular holes are surrounded by a ring of subretinal fluid and result in severely diminished visual acuity. METHODS: Fourteen eyes with full-thickness posttraumatic macular holes were treated. The patients ages ranged from 15 years to 36 years (mean, 22 years). Pr

[Disorders of the visual system following vitreoretinal and macular surgery].

The visual system is highly complex. Monocular or binocular disorders can develop, for example during tractive alterations of the macula, such as in epimacular membranes, and result in symptoms, such as double vision, aniseikonia or metamorphopsia. These symptoms are also observed following vitreomacular or vitreoretinal surgery, especially after treatment of large retinal detachment involving the macular area using pars plana vitrectomy or scleral buckle surgery but also as a result of vitreomacular interventions, such as membrane peeling in epimacular gliosis. Some patients have preoperative
