Xylazine Complicating Opioid Ingestions in Young Children.

The authors of this report present 3 cases of synthetic opioid intoxication complicated by the concomitant presence of the additive xylazine, a nonopioid sedative analgesic and muscle relaxant used in veterinary medicine that potentiates respiratory depression associated with the opioid toxidrome. Three exposed children presented with severe signs and symptoms, 2 of whom experienced cardiac arrest, a need for continuous naloxone infusion or multiple naloxone doses, or mechanical ventilation to treat respiratory failure after their exposures. Additives were detected in urine studies only throug

Xylazine effects on opioid-induced brain hypoxia.

RATIONALE: Xylazine has emerged in recent years as an adulterant in an increasing number of opioid-positive overdose deaths in the United States. Although its exact role in opioid-induced overdose deaths is largely unknown, xylazine is known to depress vital functions and cause hypotension, bradycardia, hypothermia, and respiratory depression. OBJECTIVES: In this study, we examined the brain-specific hypothermic and hypoxic effects of xylazine and its mixtures with fentanyl and heroin in freely moving rats. RESULTS: In the temperature experiment, we found that intravenous xylazine at low, hu

Xylazine Exposures Reported to Texas Poison Centers.

BACKGROUND: Xylazine is a sedative, analgesic, anesthetic, and central muscle relaxant approved for animals but not humans. Although xylazine is an emerging drug of abuse, there are limited data on potentially adverse exposures to the drug. OBJECTIVES: The intent of this study was to describe potentially adverse xylazine exposures reported to a large poison center system. METHODS: All xylazine exposures reported to Texas poison centers between 2000 and 2014 were included. The distribution of cases by select variables was determined. RESULTS: Of 76 total cases, 93% of the patients were ≥20 y

Intravenous anaesthesia in horses after xylazine premedication.

The effects of combining large doses of xylazine (1.1 mg per kg intravenously) with ketamine, methohexitone and thiopentone were studied in four experimental ponies. Onset of anaesthesia was more delayed after ketamine than after the barbiturates. Assessment of smoothness of induction and recovery indicated that all three combinations were effective and acceptable. Injection of xylazine was followed by atrioventricular (A-V) block which could be prevented by the prior administration of atropine. Blood pressure was well maintained with all three combinations of drugs. Arterial oxygen tension de

Comparison of xylazine-ketamine-guaifenesin and xylazine-tiletamine-zolazepam protocols for constant rate infusion anesthesia in horses undergoing field castration.

BACKGROUND: The common use of "triple drip" (xylazine, ketamine, and guaifenesin; XKG) is a popular protocol for constant rate infusion (CRI) in equine practice. However, due to restrictions on ketamine and guaifenesin in many countries, tiletamine-zolazepam (TZ) could serve as an alternative. To date, there have been limited studies evaluating the use of xylazine in combination with TZ (XTZ) administered via CRI to maintain surgical anesthesia in equine field practice. AIMS/OBJECTIVES: To compare the effectiveness and safety of XTZ infusion with the classical XKG protocol for field anesthesi

Differences in Xylazine and Xylazine Test Strip Usage Among Racial and Ethnic Minoritized Populations: Findings from the Stay Safe Study.

Xylazine is increasingly detected in overdose fatalities across the US. Xylazine test strips (XTS) are provided at no cost to people who use drugs (PWUD) in New York State (NYS) to encourage informed decision-making and safer drug-use behavior. XTS were only made available publicly in 2023, so there are likely discrepancies in XTS access and availability. This study aims to explore the differences in xylazine and XTS use across racial and ethnic groups. The Stay Safe Study (SSS) is a multi-state study examining drug-checking behaviors among PWUD over a 28-day observation period. Questions abou

Neonate With Intrauterine Xylazine Exposure: A Case Report.

BACKGROUND: Fentanyl is a rising cause of morbidity and mortality in the United States, affecting neonates exposed in utero. Xylazine, an alpha-2 receptor agonist, is a non-narcotic sedative that is becoming an increasingly common adulterant. There are case reports describing the clinical manifestations of xylazine exposure and toxicity in adult populations; however, cases involving neonatal populations are not described in the literature. Limitations of toxicology analyses for neonates cause delays in the identification and treatment of affected patients. CASE SUMMARY: We present the case of

Performance of a Xylazine Test Strip in Urine Biospecimens.

OBJECTIVE: Herein, we evaluate the performance of xylazine test strips (XTS) in urine samples. XTS is used for community drug checking (powders and liquids) but lacks regulatory approval for human specimen testing. METHODS: We obtained n=85 human urine specimens from a toxicology laboratory in Philadelphia, originally submitted for qualitative mass spectrometry (MS) expanded drug analysis. Residual urine was tested for xylazine using XTS (BTNX Inc.), and results were then compared against the MS method. Synthetic urine spiked with xylazine standards was used to determine the XTS cutoff. An ex

Qualitative and quantitative determination of xylazine in oral fluid.

Xylazine has emerged in recent years as a dangerous adulterant in illicit fentanyl use, and methods for the detection of xylazine in toxicology panels are still lagging. We developed methods for the screening and quantitation of xylazine in oral fluid (OF), a popular testing medium due to its ease of collection and reflection of presence in blood for many classes of drugs. Enzyme-linked immunosorbent assays were employed for the rapid screening of xylazine directly from the collection device buffer with a cutoff of 1 ng/mL. Solid-phase extraction coupled with liquid chromatography-tandem mass

Intrathecal adenosine enhances the antinociception of Xylazine in goats.

BACKGROUND: The role of adenosine (AD) in neuromodulation of nociceptive signaling at the level of the spinal cord has been established in both preclinical and clinical models. Recently, the signaling pathway that involves adenosine 5-monophosphate activated protein kinase has been reported to mediate the antinociceptive effects of xylazine (XYL). The objective of this study was to investigate the antinociceptive, cardiorespiratory and hematological effects of intrathecal administration of combined XYL-AD in goats as compared to XYL alone. Six clinically healthy adult goats weighing 25 ± 2 kg

Sedative and cardiopulmonary effects of buprenorphine and xylazine in horses.

This study investigated the sedative, cardiopulmonary, and gastrointestinal effects produced by buprenorphine and xylazine given in combination to horses. Six healthy adult horses underwent 4 randomized treatments, with an interval of 1 wk between treatments. A control group was given a saline solution intravenously (IV) and the experimental groups received buprenorphine [10 μg/kg bodyweight (BW)] in combination with 1 of 3 different doses of xylazine: 0.25 mg/kg BW (BX25), 0.50 mg/kg BW (BX50), or 0.75 mg/kg BW (BX75), all of them by IV. Cardiopulmonary parameters were evaluated for 120 min a

A clinical comparison of CT1341, ketamine, and xylazine in cats.

The new anesthetic agent CT1341 was compared to ketamine and xylazine in experimental and clinical situations with cats. Anesthesia with CT1341 was characterized by fast recovery times, good muscle relaxation, eyelid closure, occasional vomition, defecation, or urination during induction and recovery, generally smooth induction, and occasional hyperreflexive recoveries with much sneezing. The agent was moderately hypotensive, but caused little change in heart rate, central venous pressure, arterial pH, or blood gases.

Pharmacologic Reversal of Xylazine-Induced Unconsciousness in Rats.

BACKGROUND: Xylazine is an alpha-2 adrenergic agonist approved for veterinary use as a sedative and analgesic for animals. Unfortunately, xylazine has recently become a common adulterant of street drugs in the United States with xylazine-related overdoses and deaths increasing each year. Although the alpha-2 adrenergic antagonist, atipamezole, is an efficacious reversal agent for xylazine that is approved for use in animals, it is not approved for humans. In this study, we aimed to test alternative reversal agents for xylazine, and compare them with atipamezole in a rat model of xylazine-induc

Presumptive Tests for Xylazine-A Computer Vision Approach.

Abuse of xylazine is an immediate global public health concern. We report the distinct and measurable colour changes when xylazine is exposed to the Mandelin, Marquis and Mecke presumptive test reagents. The colour changes observed with xylazine are distinct from those of drugs that give colour changes from the same presumptive tests. To overcome the subjective limitations of determining spot test results by-eye, we applied image and video analyses to quantify the distinctive features of presumptive tests with xylazine and thus differentiate it from other illicit substances tested under the sa

Implications of xylazine exposure in pregnancy: a narrative review.

The rise in pregnancy-related overdose deaths has been driven by the use of high-potency illicitly-manufactured synthetic opioids including fentanyl. Xylazine, a veterinary sedative, is increasingly noted as a common adulterant in the United States illicit opioid supply. Exposure to the xylazine-fentanyl combination has been associated with severe harms including sedation, necrotic wounds, and symptoms of xylazine withdrawal. Due to limited data that directly addresses the risks of xylazine exposure during human pregnancy, we conducted a narrative review to summarize the available evidence abo
