[Biomechanical evaluation of posterior instrumentation for lumbar burst fracture: comparison of two internal devices].

PURPOSE OF THE STUDY: Burst fractures generally occur due to trauma to the thoracolumbar spine. Surgery is indicated for unstable fractures. Posterior instrumentation with pedicular screws is generally proposed. In certain circumstances, hooks may be preferred due to excessive risk of insertion of the pedicular screw. The purpose of this study was to compare two posterior instrumentations, one using pedicular screws on either side of the fracture each protected by hoods and a second composed of the same pedicular screws inserted under the fracture hooks above. MATERIAL AND METHODS: Twelve spi

Crystal structure of human GM2-activator protein with a novel beta-cup topology.

GM2 activator protein (GM2-AP) belongs to a small group of non- enzymatic lysosomal proteins that act as cofactors in the sequential degradation of gangliosides. It has been postulated that GM2-AP extracts single GM2 molecules from membranes and presents them in soluble form to beta-hexosaminidase A for cleavage of N-acetyl-d-galactosamine and conversion to GM3. The high affinity of GM2-AP for GM2 is based on specfic recognition of the oligosaccharide moiety as well as the ceramide lipid tail. Genetic defects in GM2-AP result in an atypical form of Tay-Sachs disease known as variant AB GM2 gan

Structural basis for triplet repeat disorders: a computational analysis.

MOTIVATION: Over a dozen major degenerative disorders, including myotonic distrophy, Huntington's disease and fragile X syndrome, result from unstable expansions of particular trinucleotides. Remarkably, only some of all the possible triplets, namely CAG/CTG, CGG/CCG and GAA/TTC, have been associated with the known pathological expansions. This raises some basic questions at the DNA level. Why do particular triplets seem to be singled out? What is the mechanism for their expansion and how does it depend on the triplet itself? Could other triplets or longer repeats be involved in other diseases

Fracture resistance of a second generation of zirconia implants after artificial ageing: An in vitro study.

OBJECTIVES: The aim of this study was to determine the bending moments of a further developed generation of a one-piece zirconia implant system, which, for example, has a reduced diameter in the neck area compared to the predecessor model. METHODS: A total of 48 one-piece ceramic implants with diameters of 3.6 mm and 4.1 mm were included. After preparation of the abutments of 50 % of the implants, 32 out of the 48 implants were subjected to artificial ageing in a chewing simulator (Force: 98 N; 1.2 or 5 million loading cycles; thermocyclicing: 5 °C / 55 °C). The implants were then subjected t

Improved co-registration of ex-vivo and in-vivo cardiovascular magnetic resonance images using heart-specific flexible 3D printed acrylic scaffold combined with non-rigid registration.

BACKGROUND: Ex-vivo cardiovascular magnetic resonance (CMR) imaging has played an important role in the validation of in-vivo CMR characterization of pathological processes. However, comparison between in-vivo and ex-vivo imaging remains challenging due to shape changes occurring between the two states, which may be non-uniform across the diseased heart. A novel two-step process to facilitate registration between ex-vivo and in-vivo CMR was developed and evaluated in a porcine model of chronic myocardial infarction (MI). METHODS: Seven weeks after ischemia-reperfusion MI, 12 swine underwent i

Inadvertent tooth movement with fixed lingual retainers.

Fixed retainers are effective in maintaining the alignment of the anterior teeth more than 90% of the time, but they can produce inadvertent tooth movement that in the most severe instances requires orthodontic retreatment managed with a periodontist. This is different from relapse into crowding when a fixed retainer is lost. These problems arise when the retainer breaks but remains bonded to some or all teeth, or when an intact retainer is distorted by function or was not passive when bonded. In both instances, torque of the affected teeth is the predominant outcome. A fixed retainer made wit

Crystal structure of biphalin sulfate: a multireceptor opioid peptide.

Biphalin is a dimeric opioid peptide, composed of two tetrapeptides connected 'tail-to-tail', that exhibits a high affinity for all three opioid receptor types (i.e. mu, delta and kappa). This study presents the X-ray crystal structure of biphalin sulfate and compares it to other opioids that interact with the same biological targets. Both halves of the molecule have a folded backbone conformation but differ significantly from one another. Residues 1-4 in biphalin, which compare well with the delta selective opioid peptide DADLE, fold into a random coil. Residues 5-8, which can be fit to the m

Statistical theory for protein combinatorial libraries. Packing interactions, backbone flexibility, and the sequence variability of a main-chain structure.

Combinatorial experiments provide new ways to probe the determinants of protein folding and to identify novel folding amino acid sequences. These types of experiments, however, are complicated both by enormous conformational complexity and by large numbers of possible sequences. Therefore, a quantitative computational theory would be helpful in designing and interpreting these types of experiment. Here, we present and apply a statistically based, computational approach for identifying the properties of sequences compatible with a given main-chain structure. Protein side-chain conformations are

A dose-ranging, parallel group, split-face, single-blind phase II study of light emitting diode-red light (LED-RL) for skin scarring prevention: study protocol for a randomized controlled trial.

BACKGROUND: Skin fibrosis is a significant global health problem that affects over 100 million people annually and has a profoundly negative impact on quality of life. Characterized by excessive fibroblast proliferation and collagen deposition, skin fibrosis underlies a wide spectrum of dermatologic conditions ranging from pathologic scars secondary to injury (e.g., burns, surgery, trauma) to immune-mediated diseases. Effective anti-scarring therapeutics remain an unmet need, underscoring the importance of developing novel approaches to treat and prevent skin fibrosis. Our in vitro data show t

Phleboviruses encapsidate their genomes by sequestering RNA bases.

Rift Valley fever and Toscana viruses are human pathogens for which no effective therapeutics exist. These and other phleboviruses have segmented negative-sense RNA genomes that are sequestered by a nucleocapsid protein (N) to form ribonucleoprotein (RNP) complexes of irregular, asymmetric structure, previously uncharacterized at high resolution. N binds nonspecifically to single-stranded RNA with nanomolar affinity. Crystal structures of Rift Valley fever virus N-RNA complexes reconstituted with defined RNAs of different length capture tetrameric, pentameric and hexameric N-RNA multimers. All

Structural insight into Parkinson's disease treatment from drug-inhibited DOPA decarboxylase.

DOPA decarboxylase (DDC) is responsible for the synthesis of the key neurotransmitters dopamine and serotonin via decarboxylation of L-3,4-dihydroxyphenylalanine (L-DOPA) and L-5-hydroxytryptophan, respectively. DDC has been implicated in a number of clinic disorders, including Parkinson's disease and hypertension. Peripheral inhibitors of DDC are currently used to treat these diseases. We present the crystal structures of ligand-free DDC and its complex with the anti-Parkinson drug carbiDOPA. The inhibitor is bound to the enzyme by forming a hydrazone linkage with the cofactor, and its catech

An inverse dynamics modeling approach to determine the restraining function of human knee ligament bundles.

During knee motion, the fiber bundles of ligaments are nonuniformly loaded in a recruitment pattern which is different for successive knee-joint positions. As a result, the restraining functions of these ligaments are variable. To analyze the relative restraint contributions of the fiber bundles in different knee-joint positions, a new method was developed. Its application was illustrated for the cruciate ligaments of one knee-joint specimen. The methods developed to estimate bundle forces comprise five steps. First, the three-dimensional motions of a knee specimen are measured for anterior-po

Donor-Site Morbidity Following Radial Forearm Free Flap Harvest for Head and Neck Reconstruction: A Systematic Review and Meta-Analysis.

Background: The radial forearm free flap (RFFF) remains one of the most reliable and widely adopted reconstructive options for head and neck defects because of its consistent vascular anatomy, long pedicle, and excellent tissue pliability. Nevertheless, donor-site morbidity continues to represent its principal drawback and may negatively affect postoperative recovery, upper-limb function, and patient satisfaction. Although numerous surgical modifications and donor-site reconstruction techniques have been proposed to reduce morbidity, the available evidence remains fragmented and heterogeneous.

Revisiting photodynamic therapy in keloid and hypertrophic scars: Mechanisms, clinical evidence, and synergistic multimodal approaches.

BACKGROUND: Keloids and hypertrophic scars are fibroproliferative disorders characterized by excessive collagen deposition, persistent inflammation, and dysregulated wound healing. Despite the availability of multiple treatment modalities, recurrence remains common and cosmetic and symptomatic outcomes are often unsatisfactory. Photodynamic therapy (PDT) has emerged as a promising adjunctive approach for modulating scar fibro-biology, and micro-needling has gained attention as a minimally invasive strategy for enhancing collagen remodeling and improving scar texture. OBJECTIVE: This review ai

High-performance CT features supporting accurate pre-operative tumor staging in colon cancer.

BACKGROUND AND AIMS: Accurate pre-treatment tumor staging is essential for treatment decision-making. Multi-slice spiral computed tomography (CT) is currently the standard method for pre-operative clinical tumor staging, but accurately applying the CT findings in tumor staging remains a challenge due to the poor pathological understanding of the CT signs. We aimed to pathologically interpret the key CT findings in order to identify reliable markers for pre-treatment staging of colon cancer. PATIENTS AND METHODS: The following CT features from 136 colon adenocarcinomas were analyzed: colon wal
