Different treatment modalities for underexpanded stents-Effectiveness and outcomes: A practical guide.

Underexpanded stents are a dreaded complication of percutaneous coronary intervention (PCI) and are a major predictor of stent thrombosis and in-stent restenosis. Lesion preparation and plaque modification before stent implantation are essential to prevent stent underexpansion and optimize the technical outcomes of PCI. Once stent underexpansion occurs, the treatment options are limited. New devices for the treatment of complex calcified lesions have emerged in the past few years, and experience has accumulated with their use in the treatment of underexpanded stents. This review aims to provid

Use of cystourethroscopy to remove an indwelling double-J ureteral stent 6 years following simultaneous radical sigmoid colon cancer and partial bladder resection: A case report.

Ureteral stents are widely used to ensure good urinary drainage and to relieve obstruction, pain and infection during urologic procedures. However, long-term indwelling ureteral stents can cause various complications, such as encrustation, hematuria and infection. Here, the case of an 88-year-old man who had undergone simultaneous radical resection of sigmoid colon cancer and partial resection of the bladder 6 years prior is presented. The patient complained of urinary frequency and urgency, dysuria and intermittent fever. A kidney ureter bladder X-ray examination revealed the presence of an e

Endoscopic management of submandibular sialolithiasis.

BACKGROUND: Transoral removal and sialoadenectomy are the two main modalities of treatment for submandibular stones. However, missed ductal stones are not uncommon, and there is a risk of lingual or hypoglossal nerve injury. We attempted to avoid these complications by using an endoscopic technique. METHODS: The case notes of the patients who had undergone endoscopic removal of submandibular stones were studied retrospectively. The procedure was performed under general anesthesia. The submandibular orifice was incised by carbon dioxide laser, and a 3.1-mm rigid scope was inserted under direct

Endoscopic Treatment of Intrauterine Device Migration into the Bladder with Stone Formation.

Background: An intrauterine device is commonly used for contraception globally. Although intrauterine device placement is an effective and safe method of contraception, migration into the bladder with stone formation is a rare and serious complication. The management approaches for an intrauterine device embedded in the bladder include endoscopic procedures and open surgical removal. In this study, we report the case of a patient with recurrent urinary tract infection associated with intrauterine device migration and urolithiasis, who successfully underwent endoscopic treatment combined with l

Thirty-day outcomes of Direct In-Scope Suction (DISS) in real-world practice using magneto, MOSES, and Thulium fiber laser technologies for renal and ureteral stones: an EAU endourology study.

Direct in-scope suction (DISS) ureteroscopy integrates aspiration through the ureteroscope working channel, but the interaction between suction and contemporary laser platforms is incompletely defined. We prospectively compared MOSES high-power holmium: YAG, thulium fiber laser (TFL), and Magneto holmium: YAG during DISS-assisted flexible ureteroscopy across 16 centres in 14 countries. Adults treated between May and December 2025 underwent baseline and 30-day non-contrast computed tomography (NCCT). The primary efficacy endpoint was zero-residual-fragment stone-free status (ZRF-SFS; Grade A) o

Bladder Stone Incarceration in Posterior Urethral Valve Leading to Urinary Retention: A Case Report.

INTRODUCTION: Posterior urethral valve (PUV) and vesical calculus are individually among the most common causes of obstructive lower urinary symptoms in children. Intermittent urinary retention can be caused by a combination of PUVs and bladder stones. There are very few reports of an association between PUVs and bladder calculus. CASE PRESENTATION: We present a case of intermittent urinary retention resulting from a stone obstructing the PUV. The patient experienced intermittent urinary retention for over a month. A computed tomography scan revealed a bladder stone in the posterior urethra.

Experience with ureteroscopy in children.

OBJECTIVE: To present our experience of ureteroscopic procedures in children using miniaturized instrumentation. PATIENTS AND METHODS: Fourteen children aged between 13 months and 14 years underwent 20 ureteroscopic procedures. Semi-rigid 7.2 F and flexible 9.5 F ureteroscopes were used in a retrograde and an antegrade fashion. Eighteen ureteroscopies were performed retrogradely and two antegradely. Of the 20 ureteroscopic procedures, 18 were for stone disease, one for haematuria of unknown origin and one for removal of a migrated stent. The average size of the stone was 12.9 x 6.6 mm. RESUL

An Innovative Technique of Transurethral Seminal Vesiculoscopy with Ultrasonic Lithotripter for Severe, Persistent Hematospermia.

BACKGROUND AND PURPOSE: Transurethral seminal vesiculoscopy (TSV) provides an efficient approach to diagnose and treat hematospermia, but still needs further improvement in manipulation and corresponding instruments. In this study, we develop an innovative technique with ultrasonic lithotripter (EMS) to treat severe, persistent hematospermia. PATIENTS AND METHODS: Data of patients who underwent TSV with or without ultrasonic lithotripter between May 2012 and December 2015 was reviewed. For the innovative procedure, a 3.3F ultrasonic lithotripter was introduced through the working channel of a

Automated irrigation systems in mitigating laser-induced temperature during simulated ureteroscopy.

OBJECTIVE: To compare the temperature changes with laser activation when providing irrigation with a pressure bag vs a fluid management system (FMS). MATERIALS AND METHODS: Benchtop experiments were performed with a LithoVue™ Elite (Boston Scientific Corp., Marlborough, MA, USA) flexible ureteroscope placed through an 11/13-F ureteric access sheath into a benchtop pelvicalyceal kidney model. Room temperature saline irrigation, via the scope, was provided either with a pressure bag or FMS. Both systems were pressurised to 200 mmHg and left for 10 min. After 10 min, a holmium:yttrium-aluminium-

Ablation efficiency and laser safety of a novel superpulsed thulium fiber laser: a in vitro study.

PURPOSE: To assess the ablation efficiency of the Superpulsed Thulium Fiber Laser (SP-TFL) and investigate the thermal effects of SP-TFL. METHODS: A SP-TFLwas employed to evaluate ablation efficiency. Fresh ex-vivo pig kidneys and ureters were utilized to evaluate the renal pelvis and ureter temperature changes, different irrigation rates(0, 15, 38mL/min) and a long pulse width were used. RESULTS: The research indicated that as laser output power increased, ablation rates significantly increased. Ablation rates(mg/min) were higher and the energy per ablated mass(J/mg) was lower at lower freq

Study on automatic recognition of stone composition in intraoperative endoscopic images-a single center study.

OBJECTIVES: Urinary stone composition critically influences treatment selection and recurrence prevention, yet current intraoperative assessment remains imprecise. This study aims to achieve intraoperative prediction of stone composition by applying a deep convolutional neural network (CNN) to routinely captured endoscopic images. METHODS: We retrospectively studied endoscopic images from stone-breaking surgeries in Beijing Tsinghua Changgung Hospital during 2022-12-2024-12. Images were captured before and after laser lithotripsy. Based on postoperative infrared spectroscopy, stones were divi

Effectiveness of ureteroscopy among the youngest patients: One centre's experience in an endemic region in Turkey.

BACKGROUND: In recent years, the incidence of urinary stone disease reportedly has been increasing. The use of shockwave lithotripsy has seen low success rates, the inefficacy of a single session, and the need for general anaesthesia in children; additionally, children are exposed to radiation. These suboptimal treatment conditions have all led to ureteroscopy (URS) becoming the treatment method of choice for paediatric ureter stones. The aim of this study is to examine the effectiveness of 4.5-Fr URS when used on children younger than 1 year of age. PATIENTS AND METHODS: The operation result

[Management of difficult cases of biliary lithiasis].

UNLABELLED: To assess that insertion of a biliary endoprosthesis is a safe and effective treatment in patients with retained common bile duct stones (CBDS). BACKGROUND: Before 1974, surgical exploration including cholecystectomy with choledochotomy was the only effective treatment for CBDS. However, morbi-mortality increases rapidly in the elderly and is over 10% in patients over 65, particularly if other serious medical conditions are present. Endoscopic sphincterectomy is now an established procedure, with a success rate of about 90%, complications are relatively rare, and mortality is abou

Reducing kidney motion: optimizing anesthesia and combining respiratory support for retrograde intrarenal surgery: a pilot study.

BACKGROUND: One of the greatest challenges presented with RIRS is the potential for movement of the stone within the operative field associated with diaphragm and chest respiratory excursions due to mechanical ventilation. To overcome this challenge, we propose in this pilot study a new general anesthesia technique combining high frequency jet ventilation (HFJV) with small volume mechanical ventilation (SVMV). Data regarding safety, feasibility and surgeons' impression was assessed. METHODS: Patients undergoing RIRS for kidney stones from November 2017 to May 2018 were prospectively recruited

Delayed presentation of retained renal shrapnel: a case report and literature review.

Retained renal foreign bodies (FBs) following blast or firearm injury are rare and usually present early; delayed presentation is a challenging diagnostic scenario. The present report describes a case of retained renal shrapnel presenting with obstructive complications 18 years after the initial injury. Imaging revealed a 24 mm calculus encrusting a retained metallic FB at the left renal hilum, with severe hydronephrosis and reduced differential renal function (27.4%). Following ureteroscopic stenting, percutaneous nephrolithotomy was performed. The surrounding calculus was fragmented, but the

Resolution of urethral obstruction using temporary urethral stents in two female cats.

CASE SUMMARY: The management of urethral obstructions is well documented in male cats but is less established for females. These cases describe two female cats that presented with non-dissolvable urocystoliths. Urocystoliths were removed by laser lithotripsy and basket retrieval. Following urolith removal, urethral obstruction occurred in both cats. Both cats were successfully managed using temporary urethral stents in lieu of indwelling urethral catheters permitting outpatient, spontaneous recovery of the urethra. RELEVANCE AND NOVEL INFORMATION: Use of temporary urethral stents has not been

[A Case of Urethral Calculi Treated by Transurethral Surgery Due to Mesh Exposure in the Urethra Following a Tension-Free Vaginal Tape Procedure].

A 66-year-old female patient visited another hospital complaining of frequent urination. Computed tomography scan revealed a 3 cm urethral stone, and she was referred to our hospital for surgery. The patient had a history of tension-free vaginal tape (TVT) surgery for stress urinary incontinence approximately 15 years ago. When transurethral lithotripsy was initiated, the lower end of the stone was found near the external urethral meatus, and an attempt to push it into the bladder was unsuccessful. When fragmentation of the stone was performed using a Ho-YAG laser, exposure of the mesh, presum

Coronary Intravascular Lithotripsy in Clinical Practice.

The presence of coronary artery calcification is associated with lower procedural success rates and worse long-term clinical outcomes. Atherectomy devices, such as rotational atherectomy (RA), orbital atherectomy (OA), and excimer laser coronary atherectomy (ELCA), have been used as preparation devices for severely calcified lesions. Intravascular lithotripsy (IVL) has recently been used to safely and selectively disrupt calcified coronary lesions. It produces acoustic shockwaves, which interact with the coronary calcium to create multiplanar fractures. These calcium fractures increase vessel
