Novel hypotheses related to Temporomandibular joint derived from Ankylosing spondylitis.

Ankylosing spondylitis (AS) is a seronegative arthropathy that affects fibrocartilaginous joints leading to arthritis and eventually ankylosis. Temporomandibular joint (TMJ), a fibrocartilaginous joint is affected to a lesser extent than the other joints, very rarely precipitating TMJ ankylosis. No explanation exists for the same in the present literature. Several hypothesis have been presented to explain this phenomenon of rarity of TMJ ankylosis in AS. The presence of an intraarticular disc acts as a physical barrier to the ankylotic process. However, in cases with AS, that present with TMJ

Symphysis Resection and Soft Tissue Suspension for the Long and Prominent Chin: An Alternative to Conventional Genioplasty.

Various surgical options exist for correcting long, hyperprojected chins, including modified genioplasty techniques, pogonion burring, soft tissue rejuvenation, and custom implants. These can be performed as standalone procedures or in conjunction with orthognathic surgery. Among these, lower chin border resection combined with meticulous soft tissue suspension offers a reliable method for enhancing mandibular contour and cervicomental definition. From 2018 to 2024, 24 patients with elongated, protrusive chins underwent genioplasty with symphysis resection and soft tissue suspension. The techn

Transparotid Approach for the Treatment of Condylar Osteochondroma and Ankylosis of the Temporomandibular Joint.

Among the methods to treat temporomandibular joint (TMJ), preauricular, submandibular, retromandibular, and intraoral approaches have been described. These approaches, however, occasionally offer inadequate access, owing to the oblique course of the facial nerve, and access can be limited, especially to the medial and anterior part of the infratemporal fossa. The use of the transparotid approach can offer direct visualization of the facial nerves to prevent severe damage, and a wide work field can be achieved from the medial-anterior part of the infratemporal fossa to subcondylar region by ret

[The nasopharynx and deep spaces of the face: anatomy and applications to pathology].

Deep facial spaces are anatomically delineated by the layers of deep cervical fascia. They are located between the skull base and the hyoid bone. Each space contains specific anatomic structures, which can be responsible for specific pathologic processes. Deep facial spaces can be divided in two medial odd spaces (pharyngeal mucosal and retropharyngeal spaces), and in three lateral even spaces (retrostylian, prestylian, and masticator spaces). The dividing of these core tissues are useful for the analysis of the lesions found in this area. CT scan and MRI (with axial and coronal views) provide

Custom design of a temporomandibular joint prosthesis: A kinematic approach evaluated by finite element analysis.

STATEMENT OF PROBLEM: Designing temporomandibular joint (TMJ) prostheses that accurately replicate natural kinematic movement and stress distribution remains a challenge because standardized methods for determining the prosthetic kinematic center are lacking. Current designs often rely on empirical placements without considering individualized kinematic parameters, which can compromise functionality and longevity. PURPOSE: The purpose of this study was to establish an optimal prosthetic kinematic center location for a custom TMJ prosthesis based on invariant TMJ anatomic landmarks to improve

Unraveling the patterns and pathways of local recurrence of nasopharyngeal carcinoma: evidence for individualized clinical target volume delineation.

BACKGROUNDS: Despite publication of international guidelines, there are notable controversial points of clinical target volume (CTV) delineation in nasopharyngeal carcinoma (NPC). Recently, scholars proposed a novel way of delineation of CTV in NPC-individualization of CTV delineation based on T classification and spread patterns, which yielded excellent long-term local control with limited late toxicities. The aim of this study was to clarify the anatomic patterns and pathways of local recurrence of NPC and provide a clinical reference for the delineation of CTV. METHODS: A total of 869 pati

Piezoelectric osteoarthrectomy for management of ankylosis of the temporomandibular joint.

We describe the use of a piezoelectric osteotome for removal of bone in patients with ankylosis of the temporomandibular joint (TMJ) and its advantages over conventional techniques. We studied 35 patients with ankylosis of 62 TMJ (27 bilateral and 8 unilateral, 2 recurrent) who were treated by gap arthroplasty between 1 January 2011 and 31 December 2012. We used a preauricular, with extended temporal, incision in all cases. The ankylosis was released with a piezoelectric scalpel. There were 23 men and 12 women, mean (SD) age 16 (9) years. We noticed a substantial reduction in bleeding with the

Evaluation of condylar osseous changes using a wireless detector with proton density-weighted imaging sequences.

BACKGROUND: Cone-beam computed tomography (CBCT) is the gold standard for evaluating condylar osseous changes. However, the radiation risk and low soft-tissue resolution of CBCT make it unsuitable for evaluating soft tissue such as the articular disc and lateral pterygoid muscle. This study aimed to qualitatively and quantitatively evaluate the feasibility and advantages of using wireless detectors (WD) with proton density-weighted imaging (PDWI) sequences to image condyle changes in patients with temporomandibular disorders (TMD). METHODS: This study involved 20 patients (male =8, female =12
