Effects of short-term submaximal training in humans on muscle metabolism in exercise.

Muscle metabolism, including the role of pyruvate dehydrogenase (PDH) in muscle lactate (Lac-) production, was examined during incremental exercise before and after 7 days of submaximal training on a cycle ergometer [2 h daily at 60% peak O2 uptake (VO2 max)]. Subjects were studied at rest and during continuous steady-state cycling at three stages (15 min each): 30, 65, and 75% of the pretraining VO2 max. Blood was sampled from brachial artery and femoral vein, and leg blood flow was measured by thermodilution. Biopsies of the vastus lateralis were obtained at rest and during steady-state exer

[Effects of cardiac output on PETCO2 and PaCO2 during combined inhalational-epidural anesthesia].

We investigated the effects of cardiac output on PETCO2 in anesthetized patients. We studied 8 adult patients undergoing long-lasting lower abdominal surgery. Anesthesia was maintained with epidural combined with inhalational anesthesia. The minute ventilation volume was kept constant at 10 ml.kg-1 x 10 cycles.min-1. PETCO2, PaCO2, and cardiac index, (CI) by thermodilution method were measured simultaneously. PaCO2 was corrected for body temperature for comparison with PETCO2. Approximate value of alveolar dead space to tidal volume ratio was calculated as VD/ VTalv = (PaCO2-PETCO2)/PaCO2. The

Cardiovascular and respiratory effects of detomidine in isoflurane-anaesthetised horses.

Anaesthesia was induced in horses (n = 6) with a mixture of thiopentone and guaiphenesin and maintained by mechanical ventilation with a mixture of oxygen, air and isoflurane. Inspiratory and end-expiratory concentrations of oxygen, carbon dioxide and isoflurane were measured. Electrocardiography was used to evaluate heart rate and rhythm. Mean systemic arterial pressure (SAPm), pulmonary artery pressure (PAPm), right atrial pressure (RAPm) and pulmonary capillary wedge pressure (PCWP) were measured directly. Cardiac output was determined using the thermodilution method. Microcirculation in th

Depressed contractile state and increased myocardial consumption for non-mechanical work in patients with heart failure due to old myocardial infarction.

OBJECTIVE: The aim was to characterise the impaired myocardial mechanoenergetics in patients with heart failure due to old myocardial infarction by using the oxygen consumption versus left ventricular pressure--volume area (PVA) relationship. METHODS: The VO2-PVA relationship was assessed in 15 patients: group N consisted of seven patients with vasospastic angina pectoris whose left ventricular ejection fractions were normal [72 (SD 8)%], and group F consisted of eight patients with old myocardial infarction whose left ventricular ejection fractions were reduced [45(11)%]. Left ventricular vo

Captopril in congestive heart failure.

1 Vasodilator therapy is used widely for the treatment of congestive heart failure irrespective of its origin. The acute and long-term effects of captopril were studied in 50 patients with congestive heart failure due to valvular incompetence, hypertension, cardiomyopathy, and ischaemic heart disease. Haemodynamic studies were performed in all patients. In those with valvular incompetence, regurgitation fractions were measured cineangiographically. In patients with ischaemic heart disease coronary sinus blood flow was assessed by a thermodilution technique. During clinical follow-up patients w

Pulmonary blood volume and pulmonary extravascular water volume in men.

In order to estimate the pulmonary blood volume between the pulmonary artery trunk and the left atrium (PBV PAT-LA), simultaneously with the pulmonary extravascular water volume (PEWV) in the chronically stable cardiac patients, we employed the double indicator dilution method using heat as a diffusible indicator and indocyanine green as a non-diffusible indicator. The PEWV was obtained as the extravascular lung thermal volume estimated in the aortic root (LTVAo). In the group of hemodynamically normal patients, in spite of ischemic heart disease (G-N, n = 22), the mean pulmonary artery wedge

What is the role of invasive monitoring procedures in the management of the critically ill?

In summary, therefore, to effectively apply invasive methods and beneficially use data derived from these procedures, it is essential that physicians have an understanding of the pathophysiology of ischemic heart disease and power failure, and of other situations in cardiology including the management of the complex problems of the critically ill from a variety of causes. There should be an experience and proficiency in the techniques from the optimal application and use of invasive methods including direct arterial puncture and recording of arterial blood pressure, and the use of the Swan-Gan

Etorphine induces pathophysiology in immobilized white rhinoceros through sympathomimesis that is attenuated by butorphanol.

White rhinoceros are a sentinel species for important ecosystems in southern Africa. Their conservation requires active management of their population, which, in turn, requires immobilization of individuals with an ultra-potent opioid such as etorphine. Unfortunately, when immobilized with etorphine, they develop severe hypoxaemia that may contribute to morbidity and mortality. We hypothesized that (i) etorphine causes sympathetic upregulation that is responsible for physiological complications that produce hypoxaemia and (ii) butorphanol, a partial μ opioid agonist, mitigates sympathetic upre

Comparison of Invasive Versus Non-Invasive Pulse Contour-Based Cardiac Output Measurements at Rest and During Exercise in Pulmonary Hypertension.

Background/Objectives: Measuring cardiac output (CO) is essential for diagnosis and therapeutic monitoring in pulmonary hypertension (PH). CO assessment based on thermodilution (TD) or Direct Fick (DF) during standard right heart catheterization (RHC) is impractical for regular follow-up. We evaluated the accuracy and agreement of non-invasive Modelflow (MF)-based CO assessment compared with TD and DF during rest and exercise RHC in PH. Methods: This post hoc analysis from a crossover RCT included 24 PH patients (7 females, 59 ± 14 years; mean pulmonary artery pressure 37 ± 11 mmHg) who underw

Changes in myocardial blood flow and microvascular resistance in patients with coronary artery disease undergoing transcatheter aortic valve implantation.

BACKGROUND: Severe aortic stenosis (AS) causes a pathophysiological cascade, which impairs myocardial blood flow. This effect is exacerbated in the presence of coronary disease (CAD). Treatment with transcatheter aortic valve implantation (TAVI) may promote reversal of these pathophysiological conditions. METHODS: We performed multimodality assessment of cardiac physiology in patients with AS and concurrent CAD requiring percutaneous coronary intervention, prior to and 6 months after undergoing TAVI. Techniques include: coronary angiography and bolus thermodilution-derived indices of microvas

Comparison of cardiac output estimates obtained from the Antares oscillometric pulse wave analysis algorithm and from Doppler transthoracic echocardiography.

BACKGROUND: In cardiology, cardiac output (CO) is an important parameter for assessing cardiac function. While invasive thermodilution procedures are the gold standard for CO assessment, transthoracic Doppler echocardiography (TTE) has become the established method for routine CO assessment in daily clinical practice. However, a demand persists for non-invasive approaches, including oscillometric pulse wave analysis (PWA), to enhance the accuracy of CO estimation, reduce complications associated with invasive procedures, and facilitate its application in non-intensive care settings. Here, we a

Cardiorespiratory effects of intramuscular alfaxalone combined with low-dose medetomidine and butorphanol in dogs anesthetized with sevoflurane.

BACKGROUND: The intramuscular (IM) administration of 7.5-10 mg/kg of alfaxalone produces anesthetic effects that enable endotracheal intubation with mild cardiorespiratory depression in dogs. However, the effects of IM co-administration of medetomidine, butorphanol, and alfaxalone on cardiorespiratory function under inhalation anesthesia have not been studied. AIM: To assess the cardiorespiratory function following the IM co-administration of 5 μg/kg of medetomidine, 0.3 mg/kg of butorphanol, and 2.5 mg/kg of alfaxalone (MBA) in dogs anesthetized with sevoflurane. METHODS: Seven intact healt

Concurrently Low Coronary Flow Reserve and Low Index of Microvascular Resistance Are Associated With Elevated Resting Coronary Flow in Patients With Chest Pain and Nonobstructive Coronary Arteries.

BACKGROUND: Coronary microvascular function can be distinctly quantified using the coronary flow reserve (CFR) and index of microvascular resistance (IMR). Patients with low CFR can present with low or high IMR, although the prevalence and clinical characteristics of these patient groups remain unclear. METHODS: One hundred ninety-nine patients underwent coronary microvascular assessments using coronary thermodilution techniques. A pressure-temperature sensor-tipped guidewire measured proximal and distal coronary pressure, whereas the inverse of the mean transit time to room temperature salin

Central Venous Waveform Analysis and Cardiac Output in a Porcine Model of Endotoxemic Hypotension and Resuscitation.

BACKGROUND: Cardiac output (CO) is a valuable proxy for perfusion, and governs volume responsiveness during resuscitation from distributive shock. The underappreciated venous system has nuanced physiology that confers valuable hemodynamic information. In this investigation, deconvolution of the central venous waveform by the fast Fourier transformation (FFT) algorithm is performed to assess its ability to constitute a CO surrogate in a porcine model of endotoxemia-induced distributive hypotension and resuscitation. STUDY DESIGN: Ten pigs were anesthetized, catheterized, and intubated. A lipop

EFFECTS OF A SUPPLEMENTAL ETORPHINE DOSE ON PULMONARY ARTERY PRESSURE AND CARDIAC OUTPUT IN IMMOBILIZED, BOMA-HABITUATED WHITE RHINOCEROS ( CERATOTHERIUM SIMUM): A PRELIMINARY STUDY.

The effects of etorphine on the pulmonary vascular system of white rhinoceros ( Ceratotherium simum) have not been described and could play a role in the severe hypoxemia that develops after immobilization with etorphine-based drug combinations. Characterization of these effects requires measurement of pulmonary vascular pressures and cardiac output (CO). To refine a technique for pulmonary arterial catheterization, five boma-habituated white rhinoceros (three females and two males weighing 1,012-1,572 kg) were immobilized by remote injection with etorphine plus azaperone followed by butorphan
