Peripheral nerve stimulation - Our experiences with the wireless high-frequency system.

INTRODUCTION: Peripheral nerve stimulation using a wireless high-frequency system offers an alternative to neurostimulator implantation. This approach has the potential to reduce the treatment's risk profile and improve overall quality of life. METHODS: Between January 2021 and October 2024, 21 patients were treated at our clinic using a wireless high-frequency system for peripheral nerve stimulation (PNS). The system was implanted under local anesthesia via a Tuohy needle puncture, with ultrasound guidance. The mean surgical time was 36 min (range 16 - 60 min). All patients presented with lo

Deep Brain Stimulation-Withdrawal Syndrome in Parkinson's Disease: Risk Factors and Pathophysiological Hypotheses of a Life-Threatening Emergency.

BACKGROUND AND OBJECTIVES: Subthalamic nucleus deep brain stimulation (DBS) is the most common therapeutic surgical procedure for patients with Parkinson's disease with motor fluctuations, dyskinesia, or tremor. Routine follow-up of patients allows clinicians to anticipate replacement of the DBS battery reaching the end of its life. Patients who experience a sudden stop of the DBS battery experience a rapid worsening of symptoms unresponsive to high dose of levodopa, in a life-threatening phenomenon called "DBS-withdrawal syndrome." In the current context of the COVID-19 pandemic, in which man

Protocol for a randomized clinical trial investigating early sacral nerve stimulation as an adjunct to standard neurogenic bladder management following acute spinal cord injury.

BACKGROUND: Neurogenic bladder (NGB) dysfunction after spinal cord injury (SCI) is generally irreversible. Preliminary animal and human studies have suggested that initiation of sacral neuromodulation (SNM) immediately following SCI can prevent neurogenic detrusor overactivity and preserve bladder capacity and compliance. We designed a multicenter randomized clinical trial to evaluate the effectiveness of early SNM after acute SCI. METHODS/DESIGN: The scientific protocol comprises a multi-site, randomized, non-blinded clinical trial. Sixty acute, acquired SCI patients (30 per arm) will be ran

Invasive stimulation therapies for the treatment of refractory pain.

Invasive neurostimulation therapies may be proposed to patients with neuropathic pain refractory to conventional medical management, in order to improve pain relief, functional capacity, and quality of life. In this review, the respective mechanisms of action and efficacy of peripheral nerve stimulation (PNS), nerve root stimulation (NRS), spinal cord stimulation (SCS), deep brain stimulation (DBS), and motor cortex stimulation (MCS) are discussed. PNS appears to be useful in various refractory neuropathic pain indications (as long as there is some preservation of sensation in the painful area

Optimizing real-time phase detection in diverse rhythmic biological signals for phase-specific neurostimulation.

Objective.Closed-loop, phase-specific neurostimulation is a powerful method to modulate ongoing brain activity for clinical and research applications. Phase-specific stimulation relies on estimating the phase of an ongoing oscillation in real time and issuing a control command at a target phase. Phase detection algorithms based on the fast Fourier transform (FFT) are widely used due to their computational efficiency and robustness. However, it is unclear how algorithm performance depends on the spectral properties of the input signal and how algorithm parameters can be optimized.Approach.We ev

Evaluating Thalamic Biomarkers for Closed-Loop Deep Brain Stimulation in Essential Tremor.

Deep brain stimulation (DBS) and associated local field potential (LFP) recordings offer promising avenues for developing closed-loop stimulation systems for neurological disorders such as Essential Tremor [1]-[3]. In this study, we evaluated the potential of LFP band power and inter-lead coherence as biomarkers for intended movement detection in a patient implanted with dual, contralaterally placed leads using a Medtronic Percept™ RC neurostimulator (with BrainSense™) and SenSight electrode technology. LFPs were recorded from each lead's electrodes in the ventral intermediate (VIM) nucleus an

Continuous low-level vagus nerve stimulation reduces stellate ganglion nerve activity and paroxysmal atrial tachyarrhythmias in ambulatory canines.

BACKGROUND: We hypothesize that left-sided low-level vagus nerve stimulation (LL-VNS) can suppress sympathetic outflow and reduce atrial tachyarrhythmias in ambulatory dogs. METHODS AND RESULTS: We implanted a neurostimulator in 12 dogs to stimulate the left cervical vagus nerve and a radiotransmitter for continuous recording of left stellate ganglion nerve activity, vagal nerve activities, and ECGs. Group 1 dogs (N=6) underwent 1 week of continuous LL-VNS. Group 2 dogs (N=6) underwent intermittent rapid atrial pacing followed by active or sham LL-VNS on alternate weeks. Integrated stellate g

Rectal volume tolerability and anal pressures in patients with fecal incontinence treated with sacral nerve stimulation.

PURPOSE: Sacral nerve stimulation has proven to be a promising treatment for fecal incontinence when conventional treatment modalities have failed. There have been several hypotheses concerning the mode of action of sacral nerve stimulation, but the mechanism is still unclear. This study was designed to evaluate the results of rectal volume tolerability, rectal pressure-volume curves, and anal pressures before and six months after permanent sacral nerve stimulation and to investigate the mode of action of sacral nerve stimulation. METHODS: Twenty-nine patients with incontinence (male/female r

Anesthetic management of the diaphragmatic pacemaker placement surgery. Our experience in the Institut Guttmann.

BACKGROUND: The diaphragm pacemaker (DP) has proven its utility in replacing mechanical ventilation in patients with chronic spinal cord injury (SCI) and amyotrophic lateral sclerosis (ALS). The DP improves patient quality of life and reduces morbidity and mortality and their associated health care costs. The anesthetic management of these patients and the particularities of the surgical procedure are challenging. The aim of our study is to analyze anesthetic management and intraoperative complications in patients undergoing DP placement in our hospital. METHODS: We performed a chart review o

Robot-assisted versus manual navigated stereoelectroencephalography in adult medically-refractory epilepsy patients.

OBJECTIVE: Stereoelectroencephalography (SEEG) has experienced a recent growth in adoption for epileptogenic zone (EZ) localization. Advances in robotics have the potential to improve the efficiency and safety of this intracranial seizure monitoring method. We present our institutional experience employing robot-assisted SEEG and compare its operative efficiency, seizure reduction outcomes, and direct hospital costs with SEEG performed without robotic assistance using navigated stereotaxy. METHODS: We retrospectively identified 50 consecutive adult SEEG cases at our institution in this IRB-ap

miRNA-187-3p-Mediated Regulation of the KCNK10/TREK-2 Potassium Channel in a Rat Epilepsy Model.

Regulatory RNAs play a key role in the regulation of protein expression patterns in neurological diseases. Here we studied the regulation of miRNAs in a chronic rat model of temporal lobe epilepsy. The analysis was focused on a putative link with pharmacoresponsiveness as well as the functional implications of the regulation of a selected miRNA. The findings did not reveal a difference in hippocampal miRNA expression between phenobarbital responders and nonresponders. However, when comparing rats following status epilepticus with control rats we identified 13 differentially expressed miRNAs wi

Effects of the fatty acid amide hydrolase inhibitor URB597 on pain-stimulated and pain-depressed behavior in rats.

Cannabinoid receptor (CBR) agonists produce antinociception in conventional preclinical assays of pain-stimulated behavior but are not effective in preclinical assays of pain-depressed behavior. Fatty acid amide hydrolase (FAAH) inhibitors increase physiological levels of the endocannabinoid anandamide, which may confer improved efficacy and safety relative to direct CBR agonists. To further evaluate FAAH inhibitors as candidate analgesics, this study assessed the effects of the FAAH inhibitor URB597 in assays of acute pain-stimulated and pain-depressed behavior in male Sprague-Dawley rats. Long-term outcomes of responsive neurostimulation in dominant-hemisphere Rasmussen encephalitis.

Rasmussen encephalitis (RE) is a rare, immune-mediated disorder characterized by drug-resistant focal epilepsy, progressive unilateral hemispheric dysfunction, and cognitive and motor decline, with epilepsia partialis continua (EPC) as a particularly disabling feature. Hemispheric disconnection can control seizures in up to 80% of cases but carries high risk of severe deficits, particularly in dominant-hemisphere involvement. Responsive neurostimulation (RNS) is FDA-approved for medically intractable focal epilepsy, but experience in RE-related EPC remains limited. We describe a 13-year-old ri

Effects of carotid baroreceptor stimulation on aortic remodeling in obese rats.

BACKGROUND AND AIM: Our previous study found carotid baroreceptor stimulation (CBS) reduces body weight and white adipose tissue (WAT) weight, restores abnormal secretion of adipocytokines and inflammation factors, decreases systolic blood pressure (SBP) by inhibiting activation of sympathetic nervous system (SNS) and renin-angiotensin system (RAS) in obese rats. In this study, we explore effects of CBS on aortic remodeling in obese rats. METHODS AND RESULTS: Rats were fed high-fat diet (HFD) for 16 weeks to induce obesity and underwent either CBS device implantation and stimulation or sham o

Balancing stability and cleavability of magnetoelectric nanovesicles for wireless and noninvasive deep brain stimulation in epilepsy therapy.

Polymeric vesicles hold considerable promise as noninvasive delivery platforms for magnetoelectric nanoparticles (MENPs). This strategy enables wireless and noninvasive neural stimulation and offers effective therapeutic avenues for epilepsy, representing a translatable bench-to-bedside pathway for deep-brain magnetoelectric stimulation. However, polymer vesicles have not been widely explored for delivering MENPs in neurostimulation. A key challenge is balancing the structural stability of magnetoelectric vesicles during delivery with their rapid, on-demand cleavage at the target site, which i

Safety of Intracranial Neuromodulation in Drug-Resistant Pediatric and Adult Epilepsy.

BACKGROUND AND OBJECTIVES: Intracranial neuromodulation is an established therapy for drug-resistant epilepsy (DRE) in adults. Improving seizure outcomes and expanding its application to pediatric patients remain priorities. Off-label use of 2-lead and 4-lead devices targeting multiple network nodes, combined with varied stimulation parameters, has emerged in pursuit of better outcomes. Although early findings are promising, the safety of these approaches remains underexplored. This study evaluates adverse event (AE) rates associated with different intracranial neuromodulation devices from a s
