[Dislocation fractures of the forearm. Galeazzi, Monteggia, and Essex-Lopresti injuries].

Combined forearm fractures are identified according to their location as Galeazzi, Monteggia, or Essex-Lopresti injuries. The feature common to these three forms is the combination of a forearm fracture with instability of the distal or proximal radioulnar joint. Appropriate management of the injury at an early stage is indispensable to achieve good functional results. Galeazzi fractures should initially be treated by open reduction and correct anatomy restored by plate osteosynthesis. Fixation of the distal radioulnar joint with Kirschner wires should be performed in cases of persistent dislo

Analysis of the Correlation Between Postoperative MRI Findings, Patient-Reported Outcome Measures, and Residual Pain After Arthroscopic TFCC Repair-A Pilot Study.

Background: Triangular fibrocartilage complex (TFCC) tears are a common source of ulnar-sided wrist pain. Surgery has to be performed in case of instability, pain, or if non-operative treatment fails. Overall, the results are very good. However, some patients still suffer from pain after surgery. Post-operative MR imaging can reveal potential pathologies but it needs to be assessed whether depicted changes are normal or whether these findings have a clinical significance. Therefore, the purpose of this study was to evaluate postoperative MR imaging and the function of the patients' wrists in o

The Wind Off Test: A Novel Clinical Test for TFCC Tears.

BACKGROUND: Distal radioulnar joint (DRUJ) instability is a common yet underdiagnosed cause of ulnar-sided wrist pain, often associated with triangular fibrocartilage complex (TFCC) injuries. Clinical assessment of DRUJ instability is challenging due to the limited reliability of existing tests. This study introduces the wind off test (WOT) as a novel clinical test for detecting TFCC tears and evaluates its diagnostic accuracy compared to arthroscopic findings. MATERIALS AND METHODS: A retrospective audit was conducted on patients who underwent diagnostic wrist arthroscopy over 12 months. The

Contribution of arthroscopy to the management of intra-articular distal radius fractures: Knowledge update based on a systematic 10-year literature review.

The objective of this study was to review current data on the contribution of wrist arthroscopy to the management of intra-articular distal radius fractures (IADRFs). The literature published over the last 10 years was reviewed by searching databases with the key indexing terms 'distal radius fracture', 'articular', and 'wrist arthroscopy'. The information in the 21 articles thus retrieved was used to answer the following questions: (i) Can arthroscopy improve the reduction of IADRFs? (ii) How does arthroscopy contribute to the diagnosis and treatment of concomitant ligament injuries? and (iii

Normal sonographic anatomy of the wrist and hand.

The advent of ultra-high-frequency sonographic transducers has significantly enhanced our ability to image superficial structures. As a result, sonography now can be used to assess injuries of the tendons in the wrist and hand. A clear understanding of normal sonographic anatomy is required to prevent misdiagnosis and ensure optimal patient care. The anatomy of the wrist and hand is best described by considering the extensor and flexor surfaces separately. The carpal extensor retinaculum divides the dorsal extensor tendons into six separate synovial compartments, which are demarcated by the po

Soft-tissue stabilizers of the distal radioulnar joint: an in vitro kinematic study.

PURPOSE: Distal radioulnar joint (DRUJ) stability is dependent on osseous anatomy, soft-tissue stabilizers, and muscle activity. The relative importance of DRUJ soft-tissue stabilizers remains controversial and has not been examined in the more physiologic setting of simulated muscle loading in the intact specimen. The purpose of this study was to examine the role of static stabilizers on the kinematics of the DRUJ during active simulated motion. METHODS: Twelve cadaveric upper extremities underwent computer-controlled, simulated, active forearm rotation. Joint kinematics were measured in the

[The value of wrist joint arthroscopy in injuries].

UNLABELLED: Wrist arthroscopy clearly shows lesions of carpal bone cartilages, the triangular fibrocartilage (TFC), and of carpal ligaments. Between October, 1993, and August, 1994, 37 out of 47 wrist arthroscopies were performed in the consequence of trauma. The indication was on one hand a joint check for planning further surgery, on the other hand a profound suspicion of a severe intraarticular damage, which was raised in a standardized clinical examination with localizing any points of tenderness, testing for carpal instability, and provoking carpal subluxations and pathological clicks.
