Underlying mechanisms of tinnitus: review and clinical implications.

BACKGROUND: The study of tinnitus mechanisms has increased tenfold in the last decade. The common denominator for all of these studies is the goal of elucidating the underlying neural mechanisms of tinnitus with the ultimate purpose of finding a cure. While these basic science findings may not be immediately applicable to the clinician who works directly with patients to assist them in managing their reactions to tinnitus, a clear understanding of these findings is needed to develop the most effective procedures for alleviating tinnitus. PURPOSE: The goal of this review is to provide audiolog

Temporal-specific roles of fragile X mental retardation protein in the development of the hindbrain auditory circuit.

Fragile X mental retardation protein (FMRP) is an RNA-binding protein abundant in the nervous system. Functional loss of FMRP leads to sensory dysfunction and severe intellectual disabilities. In the auditory system, FMRP deficiency alters neuronal function and synaptic connectivity and results in perturbed processing of sound information. Nevertheless, roles of FMRP in embryonic development of the auditory hindbrain have not been identified. Here, we developed high-specificity approaches to genetically track and manipulate throughout development of the Atoh1+ neuronal cell type, which is high

Valproate-induced reversible sensorineural hearing loss: a case report with serial audiometry and pharmacokinetic modelling during a valproate rechallenge.

Hearing loss has been reported with valproic acid (VPA) use. However, this is the first case of VPA-induced hearing loss that was tested and confirmed with a VPA rechallenge, supported by serial audiometry and pharmacokinetic modelling. A 39-year-old truck driver with temporal lobe epilepsy was treated with VPA at 400 mg, twice daily, and developed hearing loss after each dose, but recovered within three hours. Hearing loss fully resolved after VPA discontinuation. Audiometry performed five hours after VPA rechallenge showed significant improvement in hearing thresholds. Pharmacokinetic modell

Somatic mtDNA mutations cause progressive hearing loss in the mouse.

Mitochondrial dysfunction has been implicated in the commonly occurring age-associated hearing loss (presbyacusis). We have previously generated mtDNA mutator mice with increased levels of somatic mtDNA point mutations causing phenotypes consistent with premature ageing. We have now utilized these mice to investigate whether elevated levels of somatic mtDNA mutations affect the auditory system. The mtDNA mutator mice develop a progressive impairment of hearing (ABR thresholds). Quantitative assessment of hair cell loss in the cochlea did not show any significant difference between the mutator

Noncanonical short-latency auditory pathway directly activates deep cortical layers.

Auditory processing in the cerebral cortex is considered to begin with thalamocortical inputs to layer 4 (L4) of the primary auditory cortex (A1). In this canonical model, A1 L4 inputs initiate a hierarchical cascade that propagates to higher-order cortices for slower integration of complex sounds. Here, we identify parallel ascending pathways in mice that bypass A1 and directly reach the secondary auditory cortex (A2), alongside the canonical hierarchical route. We found that layer 6 (L6) of both A1 and A2 receive short-latency (<10 ms) sound inputs via higher-order thalamic nuclei. Additiona

Bassoon-disruption slows vesicle replenishment and induces homeostatic plasticity at a CNS synapse.

Endbulb of Held terminals of auditory nerve fibers (ANF) transmit auditory information at hundreds per second to bushy cells (BCs) in the anteroventral cochlear nucleus (AVCN). Here, we studied the structure and function of endbulb synapses in mice that lack the presynaptic scaffold bassoon and exhibit reduced ANF input into the AVCN. Endbulb terminals and active zones were normal in number and vesicle complement. Postsynaptic densities, quantal size and vesicular release probability were increased while vesicle replenishment and the standing pool of readily releasable vesicles were reduced. T

Short-term pathophysiologic changes and histopathologic findings of the auditory pathway after closed head injury, using a rabbit model.

Hearing impairment is a well-known consequence of closed head injury (CHI). The aim of this study was to elucidate the pathogenesis of CHI-induced hearing loss, using a rabbit model. Twelve New Zealand white rabbits were divided into two groups of 6. In the first group, CHI was induced mechanically, whereas the rabbits of the second group served as controls. Baseline distortion product otoacoustic emissions (DPOAEs), contralateral suppression (CS) of the DPOAEs and auditory brainstem response (ABR) were obtained. The same measurements were performed in the first group after CHI. Three hours la

The management of cochlear nerve deficiency.

The assessment process is critical in deciding whether a profoundly deaf child with cochlear nerve deficiency (CND) will be suitable for a cochlear or auditory brainstem implant (ABI). Magnetic resonance imaging (MRI) using submillimetric T2 weighted gradient echo or turbo spin echo sequences is mandatory for all profoundly deaf children to diagnose CND. Evidence of audition on behavioural or electrophysiological tests following both auditory and electrical stimulation sometimes allows identification of significant auditory tissue not visible on MRI. In particular electric auditory brainstem r

[Cochlear implant management of young children].

INTRODUCTION: Since 1988, more than 450 children have received cochlear implants at the Department of Otolaryngology of the Medizinische Hochschule Hannover. Among them are 38 children who underwent surgery before the age of two. Due to increasing experience with this technique, the mean age at implantation has decreased over time so that most children nowadays receive implants between the ages of two and five. In terms of the critical periods of both development of the auditory system and the acquisition of language, it is advantageous for even younger children to receive implants soon after

Distribution of bismuth in the brain after intraperitoneal dosing of bismuth subnitrate in mice: implications for routes of entry of xenobiotic metals into the brain.

Bismuth (Bi) can produce neurotoxic effects in both humans and animals under certain dosing conditions, but little else is known about the effects of Bi in the brain. In the present study we determined the distribution of Bi in the brains of adult female Swiss-Webster mice 4, 7, 14, 21 and 28 days after a single 2500 mg/kg i.p. injection of Bi subnitrate (BSN), which establishes a depot of absorbable Bi and produces morphological signs of neurotoxicity. Sections of brains were processed by autometallographic (AMG) procedures that produced silver grains at the site of Bi localization (AMGBi). V

Development of vestibular afferent projections into the hindbrain and their central targets.

In contrast to most other sensory systems, hardly anything is known about the neuroanatomical development of central projections of primary vestibular neurons and how their second order target neurons develop. Recent data suggest that afferent projections may develop not unlike other sensory systems, forming first the overall projection by molecular means followed by an as yet unspecified phase of activity mediated refinement. The latter aspect has not been tested critically and most molecules that guide the initial projection are unknown. The molecular and topological origin of the vestibular

Effect of copper-deficient diet on metabolism in rat auditory structures.

Copper is a trace element known to be critical for normal brain function, and abnormal copper metabolism has been associated with some disorders involving the auditory system. We examined effects of copper deficiency on metabolism in major structures of the auditory system. Homogenates of cochlea, cochlear nucleus and inferior colliculus of rats, as well as whole brain, were assayed for activities of enzymes of oxidative and glycolytic energy metabolism--malate and lactate dehydrogenase, enzymes of acetylcholine metabolism--choline acetyltransferase and acetylcholinesterase, and concentrations

Quantification of the stapedial reflex reveals delayed responses in autism.

Autism is a developmental disorder characterized, in part, by sensory abnormalities. It is well established that most if not all patients with autism have problems with auditory processing, ranging from deafness to hyperacusis, and physiological testing of auditory function (i.e. auditory brain stem responses) implicates brain stem dysfunction in autism. Additionally, previous research from this lab has revealed significantly fewer auditory brain stem neurons in autistic subjects as young as 2 years of age. These observations have led us to hypothesize that objective, noninvasive measures of a

Synaptic transmission at the endbulb of Held deteriorates during age-related hearing loss.

KEY POINTS: Synaptic transmission at the endbulb of Held was assessed by whole-cell patch clamp recordings from auditory neurons in mature (2-4 months) and aged (20-26 months) mice. Synaptic transmission is degraded in aged mice, which may contribute to the decline in neural processing of the central auditory system during age-related hearing loss. The changes in synaptic transmission in aged mice can be partially rescued by improving calcium buffering, or decreasing action potential-evoked calcium influx. These experiments suggest potential mechanisms, such as regulating intraterminal calcium

The influence of evoking stimulus level on the neural suppression of transient evoked otoacoustic emissions.

This study concerns the suppression of transient evoked otoacoustic emissions (TEOAEs) by contralateral noise. The suppression is interpreted as neurally induced changes in cochlear mechanics. The magnitude of TEOAE suppression is explored in response to a single level of contralateral noise, in 20 normal subjects, and as a function of TEOAE evoking stimulus power in 6 subjects. TEOAE were found to be relatively more susceptible to contralateral suppression when the TEOAE evoking stimulus was low. This suggests that saturation of the TEOAE generator by the evoking stimulus reduces the suscepti
