Ligamentoplasty using the ulnotriquetral ligament with wafer procedure for chronic triangular fibrocartilage complex foveal tear: Short-tem outcome.

BACKGROUND: We developed a new ligamentoplasty technique using the ulnotriquetral ligament (UTL) via a palmar approach for cases with dorsal instability of the ulna due to a foveal tear of the triangular fibrocartilage complex (TFCC). The study aimed to evaluate the clinical outcomes of this procedure. METHODS: We retrospectively analyzed 19 cases with foveal tears of TFCC who underwent the ligamentoplasty using UTL. We harvested the UTL from the triquetrum as a pedicle graft, avoiding damage of the connection between the base of the UTL and palmar radioulnar ligament (PRUL) as possible. The

Pneumothorax detection in chest radiographs: optimizing artificial intelligence system for accuracy and confounding bias reduction using in-image annotations in algorithm training.

OBJECTIVES: Diagnostic accuracy of artificial intelligence (AI) pneumothorax (PTX) detection in chest radiographs (CXR) is limited by the noisy annotation quality of public training data and confounding thoracic tubes (TT). We hypothesize that in-image annotations of the dehiscent visceral pleura for algorithm training boosts algorithm's performance and suppresses confounders. METHODS: Our single-center evaluation cohort of 3062 supine CXRs includes 760 PTX-positive cases with radiological annotations of PTX size and inserted TTs. Three step-by-step improved algorithms (differing in algorithm

Percutaneous Screw Fixation of Lateral Condylar Humeral Fractures.

Lateral condylar humeral fractures are the second most common elbow injury in children and commonly occur between the ages of 5 and 10 years. There are several systems for classification of this fracture, including those of Milch (fracture line location) and Jakob et al. (displacement). Although nonoperative management is indicated for nondisplaced or minimally displaced fractures (<2 mm), operative fixation is indicated for greater displacement. Traditionally, open reduction and internal fixation has been the method used to ensure adequate reduction. However, closed reduction and internal fix

Diagnostic approach to diplopia.

PURPOSE OF REVIEW: This article offers a pragmatic roadmap to the practicing neurologist on how to approach the patient with double vision. Strategies of history taking and examination techniques are reviewed, followed by a broad overview of the causes of diplopia organized by neuroanatomic localization. Diplopia may be the first symptom of serious vision or life-threatening neurologic disease, and its correct localization and diagnosis are therefore essential. The systems responsible for ocular movement and alignment in the vertical and horizontal plane include complex supranuclear circuitry,

Hypofractionated radiation therapy for early stage breast cancer: outcomes, toxicities, and cost analysis.

A French prospective randomized trial comparing whole breast radiotherapy with 45 Gy in 25 fractions versus 23 Gy in four fractions demonstrated equivalent 5-year local control and survival. On the basis of this data, we offer the hypofractionated regimen to women who refuse to undergo standard radiotherapy. We report our outcomes and a cost analysis. Between 2000 and 2012, 84 patients participated in this IRB-approved study and underwent whole breast radiation to 23 Gy in four fractions. Local control and survival were analyzed using the Kaplan-Meier method. Acute toxicities and overall long-

Collapsibility of the upper airway during anesthesia with isoflurane.

BACKGROUND: The unprotected upper airway tends to obstruct during general anesthesia, yet its mechanical properties have not been studied in detail during this condition. METHODS: To study its collapsibility, pressure-flow relationships of the upper airway were obtained at three levels of anesthesia (end-tidal isoflurane = 1.2%, 0.8%, and 0.4%) in 16 subjects while supine and spontaneously breathing on nasal continuous positive airway pressure. At each level of anesthesia, mask pressure was transiently reduced from a pressure sufficient to abolish inspiratory flow limitation (11.8 +/- 2.7 cm

Determinants of cardiorespiratory fitness after successfully reperfused ST-elevation myocardial infarction.

BACKGROUND: Modern treatment of ST-elevation myocardial infarction (STEMI), emphasizing prompt reperfusion, has improved clinical outcomes by preserving cardiac function. Despite this, cardiorespiratory fitness remains impaired. The purpose of this study was to assess determinants of cardiorespiratory fitness in patients with reperfused STEMI and preserved cardiac function. METHODS: We prospectively collected data from two patient cohorts with reperfused STEMI who underwent cardiopulmonary exercise testing on either a semi-supine cycle ergometer (Cohort 1) or treadmill (Cohort 2). Peak oxygen

A Lip Fixation Device Facilitates Labial Salivary Gland Biopsy for Sjögren's Disease.

The protocol aims to standardize and simplify labial salivary gland biopsy (LSGB) in suspected Sjögren's disease (SjD) using a lip fixation device, enabling safe, efficient, and operator-independent procedures with reduced technical difficulty, especially for junior rheumatologists. We developed a step-by-step protocol encompassing preoperative assessment, antisepsis, local anesthesia, device-assisted lip stabilization, precise vertical incision (5-7 mm), blunt dissection, gland harvesting (minimum 4-6 glands), and standardized closure. The device consists of two clamp arms, an oval pressure r

Emergency Department Accuracy of Point-of-Care Ultrasound in Identifying Clinically Significant Pneumothorax in High-Severity Trauma Patients.

BACKGROUND: Thoracic point-of-care ultrasound (POCUS) is an improved modality for detecting pneumothorax (PTX) with high accuracy compared with supine chest x-ray (CXR) study. However, recent research has questioned the sensitivity of POCUS for diagnosis of PTX in trauma patients. OBJECTIVE: The authors determined the accuracy of emergency physician (EP) POCUS in identifying clinically significant PTX in high-severity trauma patients based on the red criteria of the 2021 National Expert Panel on Field Triage. METHODS: The authors conducted a retrospective review of high-severity trauma patie

A novel soft tissue-integrated kinematic solver for skeletal motion: Validation and applications.

BACKGROUND AND OBJECTIVE: Kinematic solvers for human motion analysis, relying on oversimplified joint definitions, face inherent limitations in capturing the true spectrum of skeletal motion. Recent advancements incorporated soft tissue constraints to derive more realistic joint kinematics. However, these methods require marker data input and are computationally expensive, limiting their application to specific joints. This paper proposes a novel kinematic solver that addresses this gap by explicitly accounting for soft tissues, while allowing for accurate and computational efficient modeling

Impact of contralateral pelvic drop and femoral adduction on the femoral head acetabular coverage: A study on the reproducibility of a new radiographic measurement method.

PURPOSE: Traditional radiographic measurements for acetabular dysplasia and femoroacetabular impingement syndrome (FAIS) are typically done in static positions, overlooking dynamic behaviours. This study investigated the reproducibility of a new radiographic method that incorporates pelvic and femoral motion during running. METHODS: This cross-sectional retrospective study included 10 patients (5 males/5 females; Mean 42.4, SD: 3.0 years) with symptomatic unilateral FAIS. Participants underwent three-dimensional running analysis and standard supine anteroposterior (AP) pelvis radiographs. Usi

Anti-nociceptive properties of cardiopulmonary baroreceptors in patients with chronic back pain.

INTRODUCTION: Reduced pain perception following a persistent noxious stimulus during a study session (short-term habituation) is believed to be partially mediated by descending inhibitory mechanisms, although these mechanisms have not been fully elucidated. We examined the hypothesis that cardiopulmonary baroreceptor would significantly increase short-term habituation in chronic back pain (CBP) patients. METHODS: A short-term habituation protocol was utilized that involved 1-sec pulses (×10) at 105% heat pain threshold on the anterior forearm at 0.5 Hz. Cardiopulmonary baroreceptor unloading

Sequelae of an acute Essex Lopresti lesion: a case report.

INTRODUCTION: the forearm is considered a joint in itself where the ulna and radius interact. It is composed of the proximal radioulnar joint (PRUJ), the interosseous membrane (IOM), with the central band as the main component, and the distal radioulnar joint (DRUJ), which includes the triangular fibrocartilage complex (TFCC). Essex-Lopresti lesion (ELL) is a complex injury caused by axial forearm loading, leading to longitudinal radioulnar dissociation and stability loss due to IOM rupture, PRUJ injury with radial head involvement, and DRUJ injury with TFCC involvement. Early diagnosis is cru
