Actinomyces meyeri brain abscess following dental extraction.

We describe the rare occurrence of an Actinomyces meyeri cerebral abscess in a 55-year-old woman following a dental extraction. This patient presented with a 2-day history of hemisensory loss, hyper-reflexia and retro-orbital headache, 7 days following a dental extraction for apical peridonitis. Neuroimaging showed a large left parietal abscess with surrounding empyema. The patient underwent craniotomy and drainage of the abscess. A. meyeri was cultured. Actinomycosis is a rare cause of cerebral abscess. The A. meyeri subtype is particularly rare, accounting for less than 1% of specimens. This

Staged double-layer closure of palatal defects in 6 dogs.

OBJECTIVE: To report a staged approach that includes selective dental extractions before definitive double-layer hard palate defect closure in dogs. STUDY DESIGN: Retrospective case series. ANIMALS: Dogs (n = 6) with a palatal defect. METHODS: Dogs had selective maxillary teeth extractions 4-8 weeks before definitive hard palate defect repair by double-layer local full-thickness mucosal flaps. RESULTS: All palatal defects were considered complex. Complete hard palate closure was achieved after initial attempt in 3 dogs; 2 dogs had revision surgery before complete closure, and in 1 dog, clo

Management of rapidly progressing periopical pathologies: a case report.

Dentists often treat swellings from odontogenic periapical pathologies. One management option involves immediate treatment with antibiotics, followed by surgical intervention. We report a clinical case in which an 8-year-old patient sought care for such a lesion and received pharmacological therapy alone. The lesion expanded into multiple facial spaces approximating the ocular organ and other vital cranial structures. Eventual treatment of the lesion required a CT-scan, followed by an intubated general anesthetic, incision and drainage, extraction of the involved dentition and an overnight hos

Harlequin ichthyosis: a case report.

Harlequin Ichthyosis (HI) is a very rare subtype of a group of hereditary disorders that have in common the accumulation and shedding of significant amounts of hyperkeratotic epidermis. HI has a very specific clinical presentation that includes eclabium, fish mouth appearance, mouth breathing and xerostomia. The addition of a very high caloric demand to the clinical features place these affected individuals at extremely high risk for developing rampant dental decay. This case report discusses a two-year-old boy with HI that presents with severe early childhood caries. The specific accommodatio

"De mandibulae dysarthria"--thinking outside the box.

BACKGROUND: Urgent decisions in the Emergency Department allow for only a short history and physical examination. OBJECTIVES: To highlight the risks associated with a strict application of protocols, especially in the emergency setting. CASE REPORT: An unusual case of acute dysarthria is presented. CONCLUSION: Even in the emergency setting, thorough history-taking and physical examination remain fundamental, and it is necessary to "think outside the box."

Correlation of osteoradionecrosis and dental events with dosimetric parameters in intensity-modulated radiation therapy for head-and-neck cancer.

PURPOSE: Osteoradionecrosis (ORN) is a known complication of radiation therapy to the head and neck. However, the incidence of this complication with intensity-modulated radiation therapy (IMRT) and dental sequelae with this technique have not been fully elucidated. METHODS AND MATERIALS: From December 2000 to July 2007, 168 patients from our institution have been previously reported for IMRT of the oral cavity, nasopharynx, larynx/hypopharynx, sinus, and oropharynx. All patients underwent pretreatment dental evaluation, including panoramic radiographs, an aggressive fluoride regimen, and a m

[Third molar and mandibular canal: selection of radiographic techniques].

Injury of the inferior alveolar nerve during extraction of mandibular third molar is a well recognized, but uncommon, serious complication. It has a serious impact on oral function and its psychological impact on the patient is great. To prevent this complication, several studies have been conducted to identify the high-risk radiological signs associated with intimate anatomic relationships between third molar and infra-alveolar nerve and thereby establish a unified process of radiological exploration. In this paper, the authors present a review of the literature of the recent studies involvin

Treatment modalities of neurosensory deficit after lower third molar surgery: a systematic review.

PURPOSE: To conduct a systematic review to answer the clinical question, "What are the available treatment modalities and their outcomes of neurosensory deficit after lower third molar surgery?" MATERIALS AND METHODS: A systematic search, including a computer search of several databases with specific keywords, a reference search, and a manual search of 3 key maxillofacial journals were performed. Relevant articles were then evaluated and those that fulfilled the 6 predetermined criteria were chosen to enter the final review. The various treatment modalities and their outcomes of neurosensory

Esthetic effect of orthodontic appliances on a smiling face with and without a missing maxillary first premolar.

INTRODUCTION: In this study, we evaluated the effects of orthodontic appliances and an extracted maxillary first premolar on the esthetic evaluation of a smiling face according to the perceptions of orthodontists and laypeople. METHODS: Standardized oblique photographs of a young woman with a pleasing smile were taken. The photos were edited by using digital imaging software to produce 10 combinations of presence or absence of orthodontic appliances with either esthetic or metal brackets, rubber bands of different colors, and presence or absence of the maxillary first premolar. Two groups of

Mixed infection involving Actinomyces, Aggregatibacter, and Fusobacterium species presenting as perispinal tumor.

A representative case in which a polymicrobial infection involving Fusobacterium nucleatum, Actinomyces israelii and Aggregatibacter (formerly Actinobacillus) actinomycetemcomitans was initially diagnosed as malignancy in an edentulous patient. Additional history obtained after the nature of the syndrome was elucidated revealed that he had had his two remaining teeth extracted four months prior to this episode.

Orthodontists' and surgeons' opinions on the role of third molars as a cause of dental crowding.

INTRODUCTION: Despite a substantial body of literature refuting an association between third molar eruption and crowding of the anterior dentition, the issue continues to be controversial. METHODS: A survey was developed to evaluate and compare the current opinions of orthodontists (n = 393) and oral and maxillofacial surgeons (n = 458) regarding the link between third molars and the development of anterior crowding. RESULTS: A smaller percentage of orthodontists than surgeons believed that maxillary (P <.0001) and mandibular (P <.0001) third molars produce anterior forces during eruption. S

Subperiosteal abscess of the orbit: an unusual complication of the third molar surgery.

Few procedures in oral surgery show severe complications with the potential to result in life-threatening problems. Subperiosteal orbital abscess is an extremely rare but transcendent complication arising spontaneously or after dental surgery. This report describes a case of subperiosteal abscess of the orbit in a 57-year-old man that occurred following the uneventful extraction of the left maxillary third molar. In the emergency department, proptosis and extraocular muscle dysfunction were marked but no decrease in visual acuity was observed. Echography, computed tomography scan, and magnetic

Bisphosphonate-associated osteonecrosis of the jaw: conclusions based on an analysis of case series.

ONJ appears to be associated with BPs; however, the pathophysiology, incidence, and co-morbidities require further investigation. The major risk factors identified to date appear to be cancer (or chemotherapy for cancer) and dental procedures or oral trauma. A clear definition of ONJ is critical to understanding this disease entity. Although recommendations regarding the prevention and management of ONJ exist, clinical studies are needed to establish more definitive guidelines for the management of ONJ. The use of intensive hyperbaric oxygen therapy may be beneficial to patients with ONJ.
