A less-invasive approach with orthodontic treatment in Beckwith-Wiedemann patients.

The Beckwith-Wiedemann syndrome (BWS) is a rare genetic disorder, linked to an alteration on the short arm of chromosome 11 that comprises multiple congenital anomalies. Macroglossia is the predominant finding, with subsequent protrusion of dentoalveolar structures, which results in a protruding mandible, anterior open bite, abnormally obtuse gonial angle and increased mandibular length. A less-invasive treatment with orthopaedic appliances in a patient with early tongue reduction is presented. This work summarizes the oral signs linked to macroglossia, and highlights the influence of macroglo

The role of the T-N tract in advanced stage tongue cancer.

BACKGROUND: We investigated the role of the soft tissue tract between the primary tumor and the neck lymph nodes, the "T-N tract," in patients with tongue squamous cell carcinoma at an advanced stage. METHODS: We performed a compartmental tongue surgery in 233 patients. Cumulative incidence of relapses and overall survival curves were compared by T-N tract involvement. Multivariate Cox proportional hazards models were used to assess the independent role of T-N tract. RESULTS: At 4 years of follow-up, patients with disease in the T-N tract experienced a significantly more distant recurrence (

Alteration of functional loads after tongue volume reduction.

OBJECTIVES: An earlier study revealed that the patterns of biomechanical loads on bones around the tongue altered significantly right after tongue volume reduction surgery. The current study was to examine whether these alterations persist or vanish over time post-surgery. DESIGN: Five sibling pairs of 12-week-old Yucatan minipigs were used. For each pair, one had surgery reducing tongue volume by about 15% (reduction) while the other had same incisions without tissue removal (sham). All animals were raised for 4 weeks after surgery. Three rosette strain gauges were placed on the bone surface

[Pronunciation and swallowing in patients with tongue deficits following resection of oral cavity tumor].

PURPOSE: Anatomical changes due to resection of oral cavity neoplasm result in respiratory, swallowing, pronunciation and mastication disorders of varying extent and intensity. The aim of this study was to assess the quality of pronunciation and efficiency of deglutition in patients operated due to oral cavity neoplasm. MATERIAL AND METHODS: 112 patients (76 males, 36 females, mean age 53 years) after oral tumor excision were examined. Pronunciation quality was evaluated audio-visually using phonemes of contemporary Polish language (according to Roclawski) which were pronounced alone, in syll

Clinical Efficacy of Neurorrhaphy in Anterolateral Thigh Flap Reconstruction for Tongue Cancer Defects: A Randomized Controlled Trial.

BACKGROUND: Oral cancer, particularly tongue cancer, can lead to significant functional impairments, including loss of sensation and speech and swallowing difficulties. The anterolateral thigh (ALT) flap is typically used for reconstructing tongue defects, but restoring sensory and motor functions remains challenging. Nerve anastomosis has been proposed to enhance functional recovery. METHODS: This randomized controlled trial included 40 patients who underwent hemiglossectomy and reconstruction with ALT flaps. Patients were assigned to two groups: the ALT group (n = 19), with no nerve anastom

Seldinger technique for nasal intubation: a case series.

Nasotracheal intubation can be both challenging and traumatic, especially in cases of atypical anatomy. We present a series of 3 such cases in which an endotracheal tube introducer (bougie) was used to facilitate successful, atraumatic, nasotracheal intubation via Seldinger technique. The technique described can guide a nasotracheal tube through narrow nasal passages, small pharyngeal spaces, and past acute laryngeal approach angles, all without transoral manipulation of the tube. The technique is easy to perform, uses a routine skill set, and can be advantageous in numerous clinical scenarios

Long-term follow-up study of radial forearm free flap reconstruction after hemiglossectomy.

Previous studies on postoperative long-term results in patients who underwent reconstructive free flap transfer following hemiglossectomy had some issues, including the heterogeneity of the patient population and the observation period. The present study aimed to evaluate changes of reconstructed tongues in patients who underwent radial forearm free flap (RFFF) after hemiglossectomy with long-term follow-up. We enrolled 23 patients who underwent RFFF after hemiglossectomy with a postoperative follow-up of 5 years or more. Postoperative status (eating, speech, sensation function) was assessed b

Tongue squamous cell carcinoma in young nonsmoking and nondrinking patients: 3 clinical cases of orthodontic interest.

Oral squamous cell carcinoma traditionally affects older men who smoke and drink. A change in this profile has been reported because of an increased incidence in young nonsmoking and nondrinking patients. The purpose of this article was to describe a series of young nonsmoking and nondrinking patients diagnosed with tongue squamous cell carcinoma who had recently received orthodontic treatment or evaluation. Details regarding diagnosis, treatment, follow-up, and disease evolution are presented, with a review of the pertinent literature. Orthodontists often treat young adults, who have frequent

Functional effects of a free jejunum flap used for reconstruction in the oropharyngeal region.

Postoperative articulatory function was assessed in 5 patients who had had radical operations for cancers of the oropharyngeal region with surgical reconstruction using free jejunum flaps. A patient with total resection of the soft palate had the lowest scores of 39.9%. 4 patients with less resection than total removal of the soft palate had good scores (86.0%), and the articulatory dysfunction was limited only in plosive sounds produced in the velar region. A comparative study with glossectomized patients revealed that those who had had resection of the oropharynx achieved excellent articulat

Pattern of invasion and margin assessment in patients with oral tongue cancer.

BACKGROUND: Involvement of resection margins and the pattern of tumor invasion are reported to be important predictors of local recurrence and survival in surgically treated patients. In this study we have retrospectively assessed the significance of these two prognostic factors in a relatively homogeneous patient population. Patients and Methods This study was confined to 150 previously untreated patients who had surgery for squamous carcinoma of the oral tongue between 1987 and 1993. There were 82 men and 68 women who ranged in age from 25 to 89 years (median 60 years). Glossectomy was peror

Assessment of Chronological Volume Changes in Radial Forearm Free Flaps for Tongue Cancer.

OBJECTIVE: The radial forearm free flap (RFFF) is a commonly used free flap for tongue cancer. Postoperative changes in the volume of free flap are known to influence the functional restoration of the resected tongue. This study aimed to estimate chronological volume changes in RFFFs for tongue cancer and to determine the clinical factors affecting these changes. METHODS: Clinical data for 19 patients who underwent RFFF reconstruction for tongue cancer between May 2004 and September 2013 were retrospectively reviewed. The flap volume was measured 3, 6, 12, 24, and 36 months after surgery usin

Squamous cell carcinoma of oral tongue in young patients - A 10 years tertiary care experience.

INTRODUCTION: Appendiceal mucocele is a relatively rare condition characterized by progressive dilation of the appendix caused by intraluminal accumulation of mucoid substance. Its incidence is 0.07 - 0,63% of all appendectomies performed. CASE REPORT: We report the case of a 70-year-old man who came to our observation with gravative pain in right lower abdominal region. A computed tomography abdominal scan revealed a cystic/tubular structure like an appendicular mass with wall enhancement but without calcifications suggestive of a mucocele. Into peritoneal cavity we found profuse mucinous ma

Prognostic significance of invasion depth in oral tongue squamous cell carcinoma.

PURPOSE: Staging for tongue carcinoma does not consider its depth of invasion. We aim to determine the prognostic significance of invasion depth in tongue cancer. MATERIALS AND METHODS: Retrospective review of patients with tongue carcinoma who underwent curative surgery between 2002 and 2005; 67 patients were analyzed. Tumors were divided into 2 groups: group A = invasion depth <4 mm; group B = invasion depth ≥4 mm. Recurrence and survival rates were calculated for each group and compared. RESULTS: The local recurrence rate in group B was significantly higher (10 vs. 29.7%, p = 0.048). Grou
