Genetic testing for inherited ocular disease: delivering on the promise at last?

Genetic testing is of increasing clinical utility for diagnosing inherited eye disease. Clarifying a clinical diagnosis is important for accurate estimation of prognosis, facilitating genetic counselling and management of families, and in the future will direct gene-specific therapeutic strategies. Often, precise diagnosis of genetic ophthalmic conditions is complicated by genetic heterogeneity, a difficulty that the so-called 'next-generation sequencing' technologies promise to overcome. Despite considerable counselling and ethical complexities, next-generation sequencing offers to revolution

Reducing the cost of lower limb wound management through industry partnership and staff education.

Following an audit of practice in North East London Foundation Trust (NELFT), obstacles in the management of lower limb conditions were identified. An appraisal of needs in terms of skills and theory updates for staff led to a fixed-term 'honorary contract' between the trust and a wound-care company to facilitate a rolling programme of education, to upskill staff in terms of assessment and treatment, and develop standardised care pathways. After 3 months, a repeated practice audit revealed a reduction in nurse contact hours of 1156 hours. The partnership with industry proved to be beneficial a

"My child has an earache".

Appropriate management of acute infections of the middle ear rests upon the initial assessment. Therefore, one must become familiar with the technique of examination and learn how to recognize the variations in the appearance of ear drums. In the average medical practice, antibiotics are usually warranted. Ampicillin is the drug of choice under the age of four, while oral Penicillin V is recommended over the age of four. In urban private practice, where close supervision is feasible, 50 percent of earaches can be treated successfully without antibiotics; i.e., symptomatically. It is essential t

Do regional geography and race influence management of chronic spontaneous urticaria?

Chronic spontaneous urticaria is defined as migratory evanescent pruritic blanching wheals that occur with variable frequency for 6 weeks or more, with or without accompanying angioedema. This condition affects approximately 0.1% to 1.4% of persons worldwide. Second-generation H1 antihistamines are the mainstay of management, with refractory cases often managed with an array of options, including H2 antihistamines, leukotriene receptor antagonists, glucocorticosteroids, immunosuppressive agents, and omalizumab. However, the degree of practice variation as to what treatments are prescribed is p

Ecological interactions, local people awareness and practices on rodent-borne diseases in Africa: A review.

Several anthropogenic activities exposure humans to the risk of rodent-borne diseases. These activities are but not limited to logging, clearing land for crop cultivation, and consuming rodents. Rodents are a highly diverse mammalian group and harbor many zoonotic diseases. This review focuses on dominant rodent-flea species, rodent-borne zoonotic diseases and awareness and management practices against rodent-borne diseases in Africa. Relevant academic literature spanning from 1974 to 2021 was analysed. Dominant rodent species reported in Africa included:- Mastomys natalensis and Rattus rattus

From Conventional Therapy to Precision Medicine in Inflammatory Bowel Disease: A State-of-the-Art Review.

Background/Objectives: Inflammatory bowel disease (IBD) management has evolved from conventional therapies to advanced biologics and targeted small molecules; however, clinical practice often relies on empirical treatment sequencing rather than individualized approaches. The heterogeneity of IBD phenotypes, variable treatment responses, and expanding therapeutic options necessitate a shift toward precision medicine. This review aims to synthesize current evidence on personalizing IBD therapy and provide an implementation framework for clinical practice. Methods: A narrative review was conducte

Nutrition support of children with chronic liver disease: A rapid review and consensus statements from Australasian society of parenteral and enteral nutrition (AuSPEN) Paediatric liver group.

Chronic liver disease (CLD) in children is associated with high risk of malnutrition, micronutrient deficiencies, growth failure, and poor post-transplant outcomes. Although international 2019 guidelines exist, variability in practice across Australia and New Zealand (ANZ) highlights the need for locally relevant, updated nutritional guidance. This study aimed to conduct a rapid review of recent evidence (2018-2025) on the nutritional management of paediatric CLD and develop updated consensus statements to inform a best practice nutrition pathway for ANZ. A rapid review was conducted in accord

Canadian Cardiovascular Society-Canadian Heart Failure Society Focused Clinical Practice Update of Patients With Differing Heart Failure Phenotypes.

A number of societies produce heart failure (HF) management guidelines, comprising official recommendations on the basis of recent research discoveries, but their applicability to specific situations encountered in daily practice might be difficult. In this clinical practice update we aim to provide responses to fundamental questions that face health care providers, like appropriate timing for the introduction and optimization of different classes of medication according to specific patient phenotypes, when second-line therapies and valvular interventions should be considered, and management o
