Clinical considerations of microleakage.

Most dental materials permit the microleakage of bacteria and bacterial products from oral fluids to reach dentin. Thus, an understanding of the clinical consequences of microleakage demands that we analyze the permeability characteristics of dentin. The more dentin surface that is exposed during tooth preparation, the greater the potential for microleakage. Thicker dentin is less permeable than thin dentin. Dentin over pulp horns is more permeable than central dentin. Similarly, the dentin making up axial walls is more permeable than dentin forming the pulpal floor of cavities. Coronal dentin

Functional impact of pathogenic mutations in the runt homology domain of mouse Runx2 on skeletal and dental phenotypes in cleidocranial dysplasia.

Runt-related transcription factor 2 (RUNX2) is essential for skeletogenesis, and mutations in its gene cause cleidocranial dysplasia (CCD), an autosomal dominant skeletal disorder. The evolutionarily conserved 128-amino acid Runt homology domain (RHD) of human RUNX2 is essential for DNA binding and heterodimerization, and serves as a mutation hotspot associated with severe CCD phenotypes. To elucidate the functional impact of pathogenic RHD mutations in vivo, we generated 2 novel mouse lines: one carrying a missense mutation, c.695G>A, p.Arg232Gln (p.R232Q), corresponding to the human RUNX2 c.

Long non-coding RNA LncTUG1 regulates favourable compression force-induced cementocytes mineralization via PU.1/TLR4/SphK1 signalling.

Orthodontic tooth movement (OTM) is a highly coordinated biomechanical response to orthodontic forces with active remodelling of alveolar bone but minor root resorption. Such antiresorptive properties of root relate to cementocyte mineralization, the mechanisms of which remain largely unknown. This study used the microarray analysis to explore long non-coding ribonucleic acids involved in stress-induced cementocyte mineralization. Gain- and loss-of-function experiments, including Alkaline phosphatase (ALP) activity and Alizarin Red S staining, quantitative real-time polymerase chain reaction (

Dental Trauma Guide: a source of evidence-based treatment guidelines for dental trauma.

Diagnosis and treatment for traumatic dental injuries are very complex owing to the multiple trauma entities represented by six luxation types and nine fracture types affecting both the primary and the permanent dentition. When it is further considered that fracture and luxation injuries are often combined, the result is that more than 100 trauma scenarios exist, when the two dentitions are combined. Each of these trauma scenarios has a specific treatment demand and prospect for healing. With such a complexity in diagnosis and treatment, it is obvious that even experienced practitioners may ha

Lingual orthodontics and forced eruption: a means for osseous and tissue regeneration.

An hopeless upper central incisor was subjected to forced eruption in order to improve and develop the implant site by distraction of the periodontal ligament-alveolar bone complex. The orthodontic extrusion was carried out with two aesthetic approaches: lingual orthodontics and a composite resin 2-2 veneer pontic. The first part of the extrusion was realized with lingual orthodontics alone, then, when the crown had been remodelled in its greatest part and the aesthetics was no more acceptable to the patient a composite resin 2-2 veneer pontic was bonded to the vestibular side of the other thr

Tissue preservation and maintenance of optimum esthetics: a clinical report.

Today, most tooth replacement in the esthetic zone is done using implants placed in a delayed surgical protocol. Unfortunately, this delay can result in loss of both hard and soft tissue during the healing period, necessitating guided tissue regeneration techniques at the time of implant placement. Recent developments with tapered implants have facilitated predictable immediate implant placement, preserving the osseous structure surrounding the socket. Further developments with custom healing abutments can preserve the crestal soft tissues, including the papillae. This article reviews techniqu

Effects of 3 different residual root treatments after post-and-core restoration: An in vitro fracture resistance experiment and finite element analysis.

STATEMENT OF PROBLEM: Crown-lengthening procedures (CLPs) and orthodontic eruption procedures (OEPs) improve the biomechanical properties of residual root restoration. However, their use is limited by clinic time, cost, and crown-root ratio. An inner shoulder retention form (ISRF) overcomes these limits; however, whether ISRF meets the biomechanical requirements is unclear. PURPOSE: The purpose of this in vitro and finite element analysis (FEA) study was to evaluate the effects of 3 residual root treatments (ISRF, CLP, and OEP) on premolar fracture resistance and stress distribution after pos

Robot-Assisted Endodontic Retreatment: A Case Report with Clinical Considerations.

Fiber posts present significant challenges for nonsurgical endodontic retreatment, as improper removal may result in iatrogenic root perforation or even root fracture. Recently, robotic technology has attracted considerable attention in dentistry and active dental robotic (ADR) systems can perform procedures based on preset instructions, minimizing reliance on the dentist's experience. This case report describes the application of an ADR system for fiber post removal through an existing zirconia crown. A 26-year-old female was diagnosed with previously treated, symptomatic apical periodontitis

Comparative Evaluation of Fracture Resistance of Teeth Instrumented by Self adjusting File, Protaper NEXT, and Hyflex EDM: An In vitro Study.

INTRODUCTION: The primary aim of endodontic treatment is to preserve the structural integrity of teeth while enhancing their resistance to fractures. Endodontically treated teeth are more susceptible to fractures due to the removal of tooth structure and radicular dentin. This study evaluates and compares the fracture resistance of teeth instrumented using three rotary file systems: self-adjusting file (SAF), ProTaper NEXT, and Hyflex EDM. METHODOLOGY: Sixty extracted human mandibular premolars were randomly divided into four groups: Group I (control, uninstrumented), Group II (SAF), Group II

A randomized controlled trial to evaluate the incidence of postoperative pain and flare-ups in single and multiple visits root canal treatment.

BACKGROUND: Irreversible pulpitis is a painful and debilitating condition. Root canal treatment (RCT) provides prompt relief and salvages the affected tooth/teeth. It has classically been performed as a multivisit procedure. A relatively newer approach constitutes performing all the steps in one single visit. This study was designed to explore if single-visit RCT could be confidently used as an effective and preferred treatment modality for irreversible pulpitis in the Indian Armed forces. METHODS: The study compared the incidence of postoperative pain, tenderness on percussion (TOP), flare-u
