Guideline for the management of long tunnelled external ventricular drains in chronic hydrocephalus.

This article reports on the journey of a child with an inoperable hypothalamic-origin pilocytic astrocytoma causing hydrocephalus, which was refractory to treatment with shunts, and required a new approach. With multidisciplinary support, excellent nursing care and parental education, the child's hydrocephalus was managed long term in the community with bilateral long-tunnelled external ventricular drains (LTEVDs). This article describes the patient's journey and highlights the treatment protocols that were created to achieve this feat. Despite the difficulties in initially setting up these pr

Consequences of physical distancing emanating from the COVID-19 pandemic: An Australian perspective.

The sobering reality of the COVID-19 pandemic is that it has brought people together at home at a time when we want them apart in the community. This will bring both benefits and challenges. It will affect people differently based upon their age, health status, resilience, family support structures, and socio-economic background. This article will assess the impact in high income countries like Australia, where the initial wave of infection placed the elderly at the greatest risk of death whilst the protective measures of physical distancing, self-isolation, increased awareness of hygiene prac

Social vulnerability and COVID-19: A call to action for paediatric clinicians.

The COVID-19 pandemic has had dramatic effects on the lives of children globally. However, socially vulnerable children have been particularly impacted. Certain populations have increased vulnerabilities, including children and youth experiencing homelessness. Increased infection risk due to congregant living and challenges with physical distancing are contributing factors. An urgent need exists for a wholistic approach to care with unique cross-sectoral partnerships across disciplines. A recognition of the unintended consequence of the COVID-19 pandemic on this population is urgently required

What Artists and Museum Educators Can Teach Us About Combatting the Opioid Epidemic.

Art world superstar Jean-Michel Basquiat painted the electrically vibrant, sketchy skull, Untitled, before dying of a heroin overdose at age 27. The painting's imagery and its creator's substance use struggles call to mind the victims of the current opioid epidemic. Large donations from the Sackler family, patrons of numerous museums and arts institutions, have prompted questions about art world affiliation and accountability. Largely in response to protests staged by activists such as artist Nan Goldin, numerous museums have renounced Sackler funding. What more can arts organizations consider

Cultural engagement and cognitive reserve: museum attendance and dementia incidence over a 10-year period.

Theories of cognitive reserve, disuse syndrome and stress have suggested that activities that are mentally engaging, enjoyable and socially interactive could be protective against the development of dementia. Using data from the English Longitudinal Study of Ageing, this study shows that for adults aged 50 and older visiting museums every few months or more was associated with a lower incidence rate of dementia over a 10-year follow-up period compared with less-frequent visiting. This association was independent of demographics, socioeconomic status, health-related variables including sensory

From individual to herd protection with pneumococcal vaccines: the contribution of the Cuban pneumococcal conjugate vaccine implementation strategy.

A new pneumococcal conjugate vaccine is currently undergoing advanced clinical evaluation prior to its planned introduction in Cuba. The implementation of the pneumococcal vaccination strategy has been designed with consideration of the need to maximize both its direct and indirect effects. A novel approach is suggested, which addresses preschool children as the first-line target group to generate herd immunity in infants and to have an impact on transmission at the community level. The clinical evaluation pipeline is described herein, including evaluations of effectiveness, cost-effectiveness

Black sky: Exposing electricity as the Achilles' heel of resilience.

The national power grid is the most critical and the most vulnerable life-support infrastructure in modern society. Electricity serves as the essential element in sustaining every other sector of critical infrastructure including water, fuel and communications. Collective experience with responding to and recovering from widespread, long-duration power outages - known as black sky events - is nearly non-existent. Understanding these risks and weaknesses requires a collaborative planning effort with the public and private sectors to identify and protect critical nodes within the interdependent

Providing support through life's final chapter for those who made it home.

Military personnel are exposed to unique environmental hazards and psychological stressors during their service to our nation. As a result, military service personnel are at high risk not only for physical injury but for psychological trauma as well that may result in post-traumatic stress disorder, depression, substance abuse, and homelessness. These medical and psychosocial issues may hasten the development of life-limiting illnesses and may complicate the delivery of end-of-life care. Community-based hospice agencies often lack the resources and expertise to address the special needs of vet

[Filariasis in clinical practice].

Filariases are infections caused by distinct species of nematodes. These infections are transmitted through insect bites and primarily affect lymph nodes and skin. Filariases are classified as neglected diseases and affect millions, producing severe disability and social stigma. This type of infection is rarely diagnosed in travellers, as prolonged stays in endemic areas are usually required acquire infection. Infections may be asymptomatic, and clinical manifestations depend on the host immune response to the infection and the parasite burden. Diagnosis is based on the demonstration of microf

The emerging story of disability associated with lymphatic filariasis: a critical review.

Globally, 40 million people live with the chronic effects of lymphatic filariasis (LF), making it the second leading cause of disability in the world. Despite this, there is limited research into the experiences of people living with the disease. This review summarises the research on the experiences of people living with LF disability. The review highlights the widespread social stigma and oppressive psychological issues that face most people living with LF-related disability. Physical manifestations of LF make daily activities and participation in community life difficult. The findings confi

The potential of local farming on the Navajo Nation to improve fruit and vegetable intake: barriers and opportunities.

American Indian populations have low produce intake compared to other ethnic groups and higher rates of diet-related chronic diseases. Programs linking farmers to their community (Farm-to-Table) are an innovative way to alter the food environment. We interviewed Navajo farmers (n = 20), storeowners (n = 7), and non-governmental organization representatives (n = 4) to better understand local farming practices and the potential of a Farm-to-Table program to increase produce intake. Barriers to participation in a Farm-to-Table program included lack of water, insufficient help, and exotic species.

A policy change strategy for head lice management.

The purpose of this project was to formulate an effective change strategy for head lice management in a group of five separate school districts within one county. Despite a desire to use evidence to support their practice, school nurses often encounter educational system barriers that prevent independent management of health conditions. The use of collaborative community relationships, identification of underlying hierarchy structures in school policy development, and targeted system education were the catalysts for changing beliefs and ultimately policy within these school districts. The focu

The vulnerable adult in police custody: lessons learned from the case of R v Paul James Aspinall.

This case review highlights some of the problems that vulnerable adults, especially those with a mental disorder as defined in section 1 (2) of the Mental Health Act 1983, face in police custody. These service users already experience some form of discrimination within the wider society in terms of access to community services. The role of the appropriate adult in providing support to the person in custody is fundamental, as enshrined in the Police and Criminal Evidence Act 1984 Code of Practice. It is one of the important safeguards of the legislation often referred to as a 'gateway safeguard

Management of side effects of novel therapies for multiple myeloma: consensus statements developed by the International Myeloma Foundation's Nurse Leadership Board.

Nurses play an essential role in managing the care of patients with multiple myeloma, who require education and support to receive and adhere to optimal therapy. The International Myeloma Foundation created a Nurse Leadership Board comprised of oncology nurses from leading cancer centers and community practices. An assessment survey identified the need for specific recommendations for managing key side effects of novel antimyeloma agents. Myelosuppression, thromboembolic events, peripheral neuropathy, steroid toxicities, and gastrointestinal side effects were selected for the first consensus s

What lies beneath? Assessment of leg ulcers during acute hospital admission.

Chronic leg ulceration is a common condition often noted in patients during an acute hospital admission. We present the case of a patient in whom thorough examination and investigation of an incidentally noted ulcer revealed a serious, previously unexpected diagnosis of disseminated Merkel cell carcinoma. This article illustrates how important it is that medical staff are aware of the different patterns of an ulcer disease and are alert to atypical appearances. Acute admission, regardless of cause, represents an opportunity for full examination of all ulcers with a view to further investigatio

Motivators for change and barriers to help-seeking in Australian problem gamblers.

Although prevalence studies consistently indicate that many thousands of Australians experience gambling-related problems, only a relatively small proportion of these people seek professional help. This study examines the principal motivations for, and impediments to, help-seeking in a sample of 77 problem gamblers recruited from agencies and the general community. The results indicated that profession help-seeking is predominantly crisis-driven rather than being motived by a gradual recognition of problematic behaviour. Shame, denial and social factors were identified as the most significant

Seven new cases of familial isolated bladder exstrophy and epispadias complex (BEEC) and review of the literature.

Clustering of the bladder exstrophy and epispadias complex (BEEC) has been described in 29 families. To explore the genetic contribution we located new families through the German and Austrian Bladder Exstrophy Support Group and the Association for the Bladder Exstrophy Community (predominantly US and Canada). We report six new families with two occurrences of BEEC, one family where the proband is the product of a consanguineous union, and four discordant twin pairs. In conjunction with the published clinical and epidemiological reports this collection suggests that there is a significant gene

The grieving mother: care in the community?

The assumption is sometimes made that the mother's own home is the most appropriate place for the grieving mother to begin to mourn her loss. For this reason she may be transferred with undue haste. We must ask why this assumption is made and whether the mother participates in the decision to return home. Other's research suggests that the community may not be able to provide the support which the mother needs and may even make excessive demands of her. The midwife's visits to the mother's home provide an important opportunity to care for the mother, although this opportunity may not always be

The capacity of dogs to serve as reservoirs for gastrointestinal disease in children.

A total of 40 Canine faecal specimens were collected from 10 different halting sites in the Dublin area. Specimens were taken from the site surface. The sites were determined by laboratory confirmed gastroenteritis infections reported in children. Each stool specimen was primarily examined for Giardia and Cryptosporidium but was also analysed for the presence of bacterial enteropathogens. The occurrence of canine zoonoses was not established, however the 40% prevalence of Giardia coupled with the 30% prevalence of bacterial enteropathogens on these sites support the supposition that dogs are a
