Evaluation of optimal medical therapy in acute myocardial infarction patients with prior stroke.

BACKGROUND: Treatment of acute myocardial infarction (AMI) patients with prior stroke is a common clinical dilemma. Currently, the application of optimal medical therapy (OMT) and its impact on clinical outcomes are not clear in this patient population. METHODS: We retrieved 765 AMI patients with prior stroke who underwent percutaneous coronary intervention (PCI) during the index hospitalization from the international multicenter BleeMACS registry. All of the subjects were divided into two groups based on the prescription they were given prior to discharge. Baseline characteristics and proced

A machine learning framework to optimize optic nerve electrical stimulation for vision restoration.

Optic nerve electrical stimulation is a promising technique to restore vision in blind subjects. Machine learning methods can be used to select effective stimulation protocols, but they require a model of the stimulated system to generate enough training data. Here, we use a convolutional neural network (CNN) as a model of the ventral visual stream. A genetic algorithm drives the activation of the units in a layer of the CNN representing a cortical region toward a desired pattern, by refining the activation imposed at a layer representing the optic nerve. To simulate the pattern of activation

Milk replacer supplementation in early life optimizes the development of intestinal microbes in goats.

Colonization and development of the gut microbiome during early life is important in establishing a host-microbial symbiotic relationship. It contributes to maintaining health and well-being throughout the life span. To date, early longitudinal development of intestinal microflora in the ileum micro-ecology of the Yimeng black goats (YBGs) is rare. The purpose of this research was to study the effect of milk replacer with age on the ileal microbiota growth and maturation in YBGs throughout the post-weaning phase. The newborn YBGs (n = 24) were divided into two groups, i.e., milk replacer (R gr

Optimized In Silico Modeling of Drug Absorption after Gastric Bypass: The Case of Metformin.

Bariatric surgery is an effective treatment for severe obesity and related comorbidities, such as type II diabetes. Gastric bypass surgery shortens the length of the intestine, possibly leading to altered drug absorption. Metformin, a first-line treatment for type II diabetes, has permeability-dependent drug absorption, which may be sensitive to intestinal anatomic changes during bypass surgery, including Roux-en-Y gastric bypass (RYGB). Previous computer simulation data indicate increased metformin absorption after RYGB. In this study, we experimentally determined the region-dependent permeab

Animal models of Parkinson's disease: a guide to selecting the optimal model for your research.

Parkinson's disease (PD) is a complex, multisystem disorder characterised by α-synuclein (SNCA) pathology, degeneration of nigrostriatal dopaminergic neurons, multifactorial pathogenetic mechanisms and expression of a plethora of motor and non-motor symptoms. Animal models of PD have already been instructive in helping us unravel some of these aspects. However, much remains to be discovered, requiring continued interrogation by the research community. In contrast with the situation for many neurological disorders, PD benefits from of a wide range of available animal models (pharmacological, to

Optimization of ultrasonographic ureteral jet detection and normal ureteral jet morphology in dogs.

Ureteral jets are visualized with ultrasound as echogenic streams extending from the ureterovesicular junction into the urinary bladder. In clinical practice, diuretics are sometimes administered to increase visibility of ureteral jets, however this has not been well described in the veterinary literature. The purpose of this prospective, crossover study was to describe the normal morphology of canine ureteral jets, determine an optimal protocol for diuretic administration to increase visibility of ureteral jets, and confirm in vitro the effect that differences in specific gravity and velocity

An Optimal Surgical Plane for Laparoscopic Functional Total Mesorectal Excision in Rectal Cancer.

BACKGROUND: Intraoperative damage of pelvic autonomic nerves is the primary reason for postoperative sexual and urinary dysfunction after rectal cancer surgery,1,2 especially in patients with low rectal cancer. In the present study, we developed the nerve plane as a novel concept in rectal cancer surgery, which served as a landmark for better preservation of pelvic autonomic nerves in standardized total mesorectal excision (TME) surgery. The nerves never exist alone, and are always surrounded by tiny capillaries and adipose tissue, which are covered by a thin layer of membranous tissue, leadin

Optimal Allocation of Proton Therapy Slots in Combined Proton-Photon Radiation Therapy.

PURPOSE: Proton therapy is a limited resource that is not available to all patients who may benefit from it. We investigated combined proton-photon treatments, in which some fractions are delivered with protons and the remaining fractions with photons, as an approach to maximize the benefit of limited proton therapy resources at a population level. METHODS AND MATERIALS: To quantify differences in normal-tissue complication probability (NTCP) between protons and photons, we considered a cohort of 45 patients with head and neck cancer for whom intensity modulated radiation therapy and intensit

Dental Occlusion: Proposal for a Classification to Guide Occlusal Analysis and Optimize Research Protocols.

AIM: Dental occlusion often rhymes with confusion, discrediting many research protocols. The profession seems to be in "major chaos about occlusion." This may be due to the lack of a precise classification of occlusal dysfunctions. We suggest using a classification based on the separation of three occlusal functions, with the identification of precise, objective clinical diagnostic criteria. This article aims to define a precise classification of occlusal functions, in order to be able to establish a positive diagnosis of occlusal disorders. This occlusal analysis method could then be used in

Determining Optimal Mean Arterial Pressure After Cardiac Arrest: A Systematic Review.

The use of cerebral autoregulation monitoring to identify patient-specific optimal mean arterial pressure (MAPOPT) has emerged as a technique to augment cerebral oxygen delivery in post-cardiac arrest patients. Our systematic review aims to determine (a) the average MAPOPT in these patients, (b) the feasibility of identifying MAPOPT, (c) the brain tissue oxygenation levels when MAP is within proximity to the MAPOPT and (d) the relationship between neurological outcome and MAPOPT-targeted resuscitation strategies. We carried out this review in accordance with the PRISMA guidelines. We included

Optimizing the electric field strength in multiple targets for multichannel transcranial electric stimulation.

Objective. Most approaches to optimize the electric field pattern generated by multichannel transcranial electric stimulation (TES) require the definition of a preferred direction of the electric field in the target region(s). However, this requires knowledge about how the neural effects depend on the field direction, which is not always available. Thus, it can be preferential to optimize the field strength in the target(s), irrespective of the field direction. However, this results in a more complex optimization problem.Approach.We introduce and validate a novel optimization algorithm that max

Design and optimization of a hip disarticulation prosthesis using the remote center of motion mechanism.

BACKGROUND: Hip disarticulation prostheses (HDPs) are not routinely seen in clinical practice, and traditional hip prostheses rotate around an axis at the front side of the pelvic socket. OBJECTIVE: This study proposes a mechanism to restore the rotation center to the acetabulum of the amputated side and uses comparative experiments with traditional HDP to verify the validity of the novel design. METHODS: A double parallelogram design of HDP based on a remote center of motion (RCM) mechanism was presented in this paper. Optimization was achieved by a genetic algorithm with the maximal integr

A Model of Care Optimized for Marginalized Remote Population Unravels Migration Pattern in India.

BACKGROUND AND AIMS: Access to basic health needs remains a challenge for most of world's population. In this study, we developed a care model for preventive and disease-specific health care for an extremely remote and marginalized population in Arunachal Pradesh, the northeasternmost state of India. APPROACH AND RESULTS: We performed patient screenings, performed interviews, and obtained blood samples in remote villages of Arunachal Pradesh through a tablet-based data collection application, which was later synced to a cloud database for storage. Positive cases of hepatitis B virus (HBV) wer

Optimization of CRISPR/Cas System for Improving Genome Editing Efficiency in Plasmodium falciparum.

Studies of molecular mechanisms and related gene functions have long been restricted by limited genome editing technologies in malaria parasites. Recently, a simple and effective genome editing technology, the CRISPR/Cas (clustered regularly interspaced short palindromic repeats/CRISPR-associated) system, has greatly facilitated these studies in many organisms, including malaria parasites. However, due to the special genome feature of malaria parasites, the manipulation and gene editing efficacy of the CRISPR/Cas system in this pathogen need to be improved, particularly in the human malaria pa

Healthcare Warranty Policies Optimization for Chronic Diseases Based on Delay Time Concept.

Warranties for healthcare can be greatly beneficial for cost reductions and improvements in patient satisfaction. Under healthcare warranties, healthcare providers receive a lump sum payment for the entire care episode, which covers a bundle of healthcare services, including treatment decisions during initial hospitalization and subsequent readmissions, as well as disease-monitoring plans composed of periodic follow-ups. Higher treatment intensities and more radical monitoring strategies result in higher medical costs, but high treatment intensities reduce the baseline readmission rates. This

Skin-specific knockdown of hyaluronan in mice by an optimized topical 4-methylumbelliferone formulation.

Hyaluronan (HA) is abundant in the skin; while HA can be synthesized by the synthases (HAS1-3), HAS2 is the leading contributor. Dysregulation and accumulation of HA is implicated in the pathogenesis of diseases such as keloid scarring, lymphedema and metastatic melanoma. To understand how HA synthesis contributes to skin physiology, and pathologic and fibrotic disorders, we propose the development of skin-specific HA inhibition model, which tests an optimal delivery system of topical 4-methylumbelliferone (4-MU). A design-of-experiments (DOE) approach was employed to develop an optimal 4-MU s

Quest for Optimal Regression Models in SARS-CoV-2 Wastewater Based Epidemiology.

Wastewater-based epidemiology is a recognised source of information for pandemic management. In this study, we investigated the correlation between a SARS-CoV-2 signal derived from wastewater sampling and COVID-19 incidence values monitored by means of individual testing programs. The dataset used in the study is composed of timelines (duration approx. five months) of both signals at four wastewater treatment plants across Austria, two of which drain large communities and the other two drain smaller communities. Eight regression models were investigated to predict the viral incidence under var

Is There an Optimal Antiplatelet Strategy after Gastrointestinal Bleeding in Patients with Coronary Artery Disease?

Gastrointestinal bleeding after percutaneous coronary intervention (PCI) is a not too uncommon clinical situation and is associated with high morbidity and mortality. After initial treatment, a number of clinical decisions must be made weighing the risks of ischemic events and future bleeding. In particular, healthcare providers must carefully balance the effectiveness of antiplatelet therapy in the secondary prevention of coronary events, primarily future spontaneous myocardial infarction and stent thrombosis, against the risk of major, most commonly gastrointestinal bleeding. The first quest
