Machine learning model for predicting recurrence following intensity-modulated radiation therapy in nasopharyngeal carcinoma.

BACKGROUND: Nasopharyngeal carcinoma (NPC) exhibits unique histopathological characteristics compared to other head and neck cancers. The prognosis of NPC patients after intensity-modulated radiation therapy (IMRT) has not been fully studied, and there remains a high risk of recurrence. This study aims to construct a reliable model for predicting post-treatment recurrence by integrating high-accuracy machine learning (ML) models. METHODS: A total of 859 NPC patients who underwent IMRT treatment at the First Affiliated Hospital of Zhejiang University School of Medicine from January 2013 to Dec

Appropriateness of Prophylactic Surgery for Sigmoid Volvulus in View of Recurrence and Nutritional Status.

Background  Sigmoid volvulus causes colonic obstruction and ischemia. Urgent surgery is required for colonic necrosis or perforation, whereas prophylactic surgery is recommended after endoscopic treatment in nonemergency cases. However, many patients with sigmoid volvulus only undergo endoscopic treatment, highlighting a gap between guideline recommendations and clinical practice. We aimed to elucidate the actual management and outcomes of sigmoid volvulus in hospitalized patients to reconsider the disadvantages of delaying prophylactic surgery. Methods  We retrospectively recruited patients w

Effect of Mitral Isthmus Ablation on Recurrence Rates in High-Burden Paroxysmal Atrial Fibrillation.

BACKGROUND Patients with high-burden paroxysmal atrial fibrillation (AF) have a higher rate of AF recurrence after catheter ablation than do those with a lower AF burden. The blockade rate of mitral isthmus (MI) ablation can contribute to the maintenance of sinus rhythm and be high based on the ethanol infusion of the vein of Marshall. MATERIAL AND METHODS Patients with high-burden paroxysmal AF who underwent PVI alone or PVI with additional MI ablation were assigned to the "2C" and "2C1L" groups, respectively, based on 2: 1 propensity score matching. The 12-month freedom from AF or atrial tac

Aberrant expression of Bcl2L12 as a potential biomarker for predicting recurrence in nasal polyp.

OBJECTIVE: Bcl2L12 plays an important role in the inflammatory response, but its role in chronic rhinitis with nasal polyps (CRSwNP) is unknown. We aimed to evaluate the relationship of Bcl2L12 with eosinophilic inflammation in CRSwNP and its value in predicting postoperative recurrence. METHODS: We included 80 patients with CRSwNP treated with Functional Endoscopic Sinus Surgery (FESS), and 40 Health Control (HC) subjects. The expression levels of Bcl2L12 in tissues were examined by RT-PCR, Western Blotting (WB) and Immunohistochemistry (IHC). Correlation analysis was performed to explore th

Cytomegalovirus Reactivation Is Associated With Lower Rates of Hepatocellular Carcinoma Recurrence After Liver Transplantation.

In patients with hepatocellular carcinoma (HCC), undergoing liver transplantation (LT), cytomegalovirus reactivation (CMVr) may modulate the immune system to prevent tumor recurrence. In this multicenter retrospective study (2010-2015) involving 15 institutions, we assessed the effect of early CMVr in tumor recurrence rates among 771-LT HCC patients with tacrolimus-based immunosuppression (88% men, mean age 58 years). CMV prophylaxis was implemented for 19.7% of patients, while the rest were managed with preemptive therapy. The Milan criteria were met by 88% of patients. Microvascular invasion

[Recurrence pattern of pathological complete response after neoadjuvant chemoradiotherapy in locally advanced rectal cancer].

Patients with locally advanced rectal cancer who undergo neoadjuvant chemoradiotherapy may achieve pathological complete response (pCR). The incidence of recurrence is low among patients with pCR, there is still a lack of consensus on postoperative treatment and follow-up strategy. This review summarizes the recurrence patterns of patients with pCR, including distant metastasis rate, characteristics of distant metastasis time and location, local recurrence rate, and local recurrence time. The aim is to provide reference for the postoperative treatment and follow-up strategy of patients with pC

Recurrence-free survival curve for pulmonary metastasectomy in colorectal cancer plateaus at 3 years.

Pulmonary metastasectomy in colorectal cancer (CRC) has encouraging results; however, specific criteria for lung resection and the timing of resection remain undetermined. Therefore, in this study, we aimed to examine the long-term prognosis and 10-year survival rates and analyze poor prognostic factors in patients who underwent resection of pulmonary metastases from CRC. This retrospective, single-institution study included 156 patients with controlled primary CRC and metastases confined to the lungs or liver who underwent pulmonary metastasectomy between 2005 and 2022. Statistical analyses w

Increasing monocytes after lung cancer surgery triggers the outgrowth of distant metastases, causing recurrence.

Patients with lung cancer have a high incidence of tumor recurrence even after curative surgical resection. Some reports indicated that immunosuppressive cells induced by surgical stress could contribute to tumor recurrence after surgery; however, the underlying mechanisms are not fully understood. In this study, we found that increased postoperative blood monocytes served as a risk factor for tumor recurrence in 192 patients with non-small cell lung cancer (NSCLC). We established the lung cancer recurrent mouse model after tumor resection and showed that the surgical stress immediately increa

Spheno-Orbital Meningiomas: A Systematic Review of Treatment Modalities, Adjuvant Therapies, and Recurrence Risk.

Spheno-orbital meningiomas (SOMs) are rare tumors that involve the sphenoid wing and orbit, leading to symptoms such as proptosis and vision loss. Their proximity to critical neurovascular structures complicates surgical resection, making management challenging. A systematic review of 22 paper series involving 1042 patients was conducted using PubMed and Scopus. Studies focused on SOM diagnosis, surgical techniques, recurrence rates, and the role of adjuvant therapies were analyzed. Proptosis and visual impairment were the most common symptoms. Gross total resection (GTR) reduced recurrence, b

Clinical Predictors of Early and Late Endoscopic Recurrence Following Ileocolonic Resection in Crohn's Disease.

BACKGROUND AND AIMS: Multiple factors are suggested to place Crohn's disease patients at risk of recurrence after ileocolic resection with conflicting associations. We aimed to identify clinical predictors of recurrence at first [early] and further [late] postoperative colonoscopy. METHODS: Crohn's disease patients undergoing ileocolic resection were prospectively recruited at six North American centres. Clinical data were collected and endoscopic recurrence was defined as Rutgeerts score ≥i2. A multivariable model was fitted to analyse variables independently associated with recurrence. RES

Outcomes and Endpoints of Postoperative Recurrence in Crohn's Disease: Systematic Review and Consensus Conference.

BACKGROUND: Outcomes after ileocolonic resection in Crohn's disease [CD] are heterogeneous, and a clear definition of postoperative recurrence remains to be determined. Our Endpoints Working Group of the International Organization for the study of Inflammatory Bowel Disease [IOIBD] aimed to standardise postoperative outcomes, to discuss which endpoints should be used for postoperative clinical trials, and to define those which could be used in trials or registries. METHODS: Based on a systematic review of the literature, recommendations and statements were drafted and sent to all IOIBD member

Recurrence of Hepatocellular Carcinoma after Liver Transplantation: Clinical Patterns and Hierarchy of Salvage Treatments.

INTRODUCTION: The multiparametric nature of recurrence of hepatocellular carcinoma (HCC) after liver transplantation (LT) still leads to uncertainty with its practical management. This study aims to characterize the main posttransplant recurrence patterns of HCC and to explore the therapeutic modalities targeting recurrence. METHODS: Consecutive patients who underwent LT for HCC at a single tertiary center were analyzed. The time from first recurrence to death was investigated for each site of presentation. The impact of each recurrence-targeted treatment on survival was studied. RESULTS: Of

Evaluating Treatment Modalities for Reducing Recurrence in Central Giant Cell Granuloma: A Narrative Review.

Treating central giant cell granuloma (CGCG) is challenging due to high recurrence rates and variable therapy responses. This study examines the efficacy of various treatments in reducing CGCG recurrence. A literature review explored outcomes of surgical excision, curettage, intralesional corticosteroid injection, and adjuvant therapy, considering factors like lesion location, size, and histological features. Aggressive surgical techniques such as en bloc resection were found to potentially lower recurrence rates compared to conservative approaches. However, treatment should be tailored to ind
