Primary mitochondrial disease as a rare cause of unclear breathlessness and distinctive performance degradation - a case report.

BACKGROUND: Primary muscular disorders (metabolic myopathies, including mitochondrial disorders) are a rare cause of dyspnea. We report a case of dyspnea caused by a mitochondrial disorder with a pattern of clinical findings that can be classified in the known pathologies of mitochondrial deletion syndrome. CASE PRESENTATION: The patient presented to us at 29 years of age, having had tachycardia, dyspnea, and functional impairment since childhood. She had been diagnosed with bronchial asthma and mild left ventricular hypertrophy and treated accordingly, but her symptoms had worsened. After mo

An unusual concomitance of acute heart failure: prolactinoma in a patient with left ventricular dysfunction-a case report.

BACKGROUND: Heart failure concomitant with prolactinoma is extremely rare. CASE SUMMARY: We present the case of a 29-year-old man who had acute decompensated heart failure concomitant with visual loss in his right eye. Transthoracic echocardiography indicated severely decreased left ventricular (LV) function. A massive tumour on the sella turcica was detected by brain computed tomography. The findings of the laboratory tests showed hyperprolactinaemia with hypopituitarism, and the antigen test for coronavirus disease 2019 was positive as an incidental finding. Medication for heart failure and

Compound and heterozygous mutations of DSG2 identified by Whole Exome Sequencing in arrhythmogenic right ventricular cardiomyopathy/dysplasia with ventricular tachycardia.

BACKGROUNDS: This study was designed to identify the pathogenic mutations in two Chinese families of arrhythmogenic right ventricular cardiomyopathy/dysplasia (ARVC/D) using the Whole Exome Sequencing (WES). METHODS AND RESULTS: The proband 1 (Family 1, II:1) and proband 2 (Family 2, II:1) underwent the WES of DNA from peripheral blood. The genes susceptible to arrhythmias and cardiomyopathies were analyzed and both the probands carried the same exonic mutation of DSG2 p.F531C (NM_001943, exon 11: c.T1592G). The proband 1 also carried the splicing mutation of DSG2 (NM_001943: exon 4:c.217-1G>

Force-frequency relationship and early relaxation kinetics are preserved upon sarcoplasmic blockade in human myocardium.

In this study, we investigated the quantitative and qualitative role of the sarcoplasmic reticulum (SR) in the regulation of the force-frequency relationship (FFR). We blocked the function of SR with cyclopiazonic acid (CPA) and ryanodine and measured twitch kinetics and developed force at various stimulation frequencies in nonfailing and failing intact human right ventricular trabeculae. We found that developed forces are only slightly reduced upon SR blockade, while the positive FFR in nonfailing trabeculae and negative FFR in failing trabeculae were both preserved. The contraction kinetics

Sleep Disordered Breathing: Hypertension and Cardiac Structure and Function.

Obstructive sleep apnea (OSA) is a common form of sleep disordered breathing and has a relatively high prevalence in the general population. The frequency and severity of OSA is associated with age, male sex, and obesity, and OSA has been linked to cardiovascular complications and death. Importantly, OSA has a strong association with both prevalent and incidental hypertension and has a particularly high prevalence in patients with resistant hypertension. In these patients, CPAP and other OSA-directed treatments have been proposed as therapy to help control blood pressure (BP), especially in pa

Stress echocardiography in paediatric cardiology.

Stress echocardiography is a well-established technique in adult cardiology and is mainly used for assessing regional myocardial function in patients with known or suspected coronary artery disease (CAD). Apart from detecting ischaemia, stress echocardiography has found its place in the assessment of the haemodynamic significance of valve disease, particularly in selected patients with aortic stenosis and mitral regurgitation. Also in the paediatric population, stress imaging is most commonly used for the detection of ischaemia in patients with CADs such as post heart transplantation, Kawasaki

[Fontan procedure in patients with hypoplastic left heart syndrome].

In the recent years, the Fontan completion rate for hypoplastic left heart syndrome (HLHS) is improved because of the outcome of stage-1 palliation improved. However, there are still problems to be solved to achieve better hemodynamics after Fontan operation in patients with HLHS. The 2 major commandments other than classic 10 commandments are A:"good ventricular function" and B:"low pulmonary vascular resistance". To obtain these 2, the following problems should be rectified. They include residual arch obstruction, impaired right ventricule (RV) function, tricuspid regurgitation, elevated pul

Late-phase depolarization and repolarization abnormalities in english bulldogs with phenotypic expression of arrhythmogenic cardiomyopathy.

In humans with arrhythmogenic cardiomyopathy (ACM) late-phase depolarization and repolarization abnormalities, such as epsilon (ɛ) waves and T wave inversion (TWI), are commonly observed in right precordial leads (V1-V3). This myocardial disorder has also been described in Boxer and English bulldogs (EBs), but data on electrocardiographic alterations in these breeds are lacking. The objective of this study was to describe electrocardiographic abnormalities in EBs with ACM, comparing QRS complexes and T waves with ≤ 5-year-old EBs without phenotypic expression of the disease. This is a retrospe

Postoperative supraventricular tachycardia and polymorphic ventricular tachycardia due to a novel SCN5A variant: a case report of a rare comorbidity that is difficult to diagnose.

BACKGROUND: Loss-of-function mutations of human cardiac sodium channel gene SCN5A induce a wide range of arrhythmic disorders. Mutation carriers with co-existing conditions such as congenital heart diseases and histories of cardiac surgeries, could develop complex arrhythmic events that are difficult to diagnose. CASE PRESENTATION: A 41-year-old Japanese male with a history of a surgical closure of an ASD presented impairment of consciousness by wide QRS tachycardia. Because the patient's baseline ECG in sinus rhythm showed similar QRS axis with right bundle brunch block morphology, we suspec

Brugada Syndrome: New Insights From Cardiac Magnetic Resonance and Electroanatomical Imaging.

BACKGROUND: Brugada syndrome (BrS) is considered a purely electrical disease with variable electrical substrates. Variable rates of mechanical abnormalities have been also reported. Whether exists a link between electrical and mechanical abnormalities has never been previously explored. This investigational physiopathological study aimed to determine the relationship between the substrate size/location, as exposed by ajmaline provocation, and the severity of mechanical abnormalities, as assessed by cardiac magnetic resonance in patients with BrS. METHODS: Twenty-four consecutive high-risk pat

Prevalence and predictive factors for clinical outcomes of isolated functional tricuspid regurgitation.

BACKGROUND: A substantial number of patients have functional tricuspid regurgitation (TR). Isolated functional TR has been undertreated and may be a next target for transcatheter intervention. However, the prevalence, patient characteristics, and predictive factors for prognosis remain unclear. METHODS: From patients in our echocardiographic database (N = 64,242), we extracted those with severe TR and examined prognosis according to etiologies of TR. Thereafter, we focused on two types of isolated functional TR; progressive TR after left-sided valve surgery (postoperative TR) and TR associate

Atrial fibrillation cryoablation complicated by diffuse coronary artery vasospasm and cardiac arrest: a case report.

BACKGROUND: Pulmonary vein isolation (PVI) is recommended for rhythm control in selected patients with symptomatic atrial fibrillation (AF). Coronary artery spasm (CAS) is a rare but recognized complication of AF ablation and may result in life-threatening arrhythmias. CASE SUMMARY: A 47-year-old man underwent cryoballoon PVI for symptomatic paroxysmal AF. Transient inferior ST-segment elevation occurred following left-sided PVI and resolved spontaneously. Shortly after completion of the procedure, recurrent ST-segment elevation developed, progressed to malignant ventricular arrhythmia and ca

Machine-learning approach on echocardiography to improve the detection of transthyretin amyloid cardiomyopathy: GRAAL algorithm.

AIMS: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an increasingly recognized cause of heart failure, yet detection remains challenging due to its echocardiographic similarities with age- and hypertension-related cardiac remodelling. METHODS AND RESULTS: We retrospectively included 260 patients (76.5 ± 12.9 years old, 59.6% male) referred for suspected ATTR-CM. A supervised machine-learning diagnosis algorithm differentiating patients with (n = 111) and without (n = 149) ATTR-CM based on echocardiographic data, and subsequently validated in an external multicentre cohort of 454 patients

Modeling Aleatoric Uncertainty in Cardiac MRI Segmentation: Probabilistic Detection and Contour Regression.

Accurate segmentation of cardiac MRI is essential for assessment of cardiac function through biomarkers such as the left and right ventricular ejection fraction (LVEF, RVEF). Although AI methods have achieved high average segmentation accuracy, the precision of biomarkers for individual patients-quantified by estimation variance, remains critical for reliable diagnosis. Calibrated biomarkers, whose uncertainty accurately reflects the true variability, are highly desirable. However, existing evaluations predominantly focus on population-level segmentation accuracy, leaving biomarker-level uncer

Total 3d endoscopic approach to complete repair of Ebstein's anomaly: a case series.

BACKGROUND: Ebstein anomaly is a rare congenital malformation of the tricuspid valve. Although robotic-assisted minimally invasive repair has shown promising results, its high cost limits accessibility in developing countries. We report our initial experience with a totally endoscopic cone repair using three-dimensional (3D) visualization and standard thoracoscopic instruments, performed without robotic assistance, for the treatment of Ebstein anomaly. METHODS: This retrospective case series included five adult patients who underwent total 3D endoscopic tricuspid valve cone reconstruction at E

Left Bundle Branch Area Pacing in Transthyretin Cardiac Amyloidosis: A Narrative Review.

Background/Objectives: Transthyretin cardiomyopathy (ATTR-CM) is frequently associated with conduction disease requiring pacing. Conventional right ventricular pacing may worsen cardiac function, whereas left bundle branch area pacing (LBBAP) aims to preserve physiological activation. Evidence for LBBAP in ATTR-CM remains limited. Methods: A structured narrative review of PubMed and Google Scholar was performed through November 2025 using predefined terms related to LBBAP and ATTR-CM. Peer-reviewed articles, case reports, case series, and relevant abstracts were included. Studies exclusively o
