Combining fMRI and MEG increases the reliability of presurgical language localization: a clinical study on the difference between and congruence of both modalities.

To avoid neurological impairment during surgery near language-related eloquent brain areas, we performed presurgical functional brain mapping with functional magnetic resonance imaging (fMRI) and magnetoencephalography (MEG) in 172 patients using language tasks. For MEG localizations, we used either a moving equivalent-current dipole fit or a current-density reconstruction using a minimum variance beamformer with a spatial filter algorithm. We localized the Wernicke and Broca language areas for every patient. We integrated the results into a frameless stereotaxy system. To visualize the result

Neuropsychiatric and neurodevelopmental outcome of children at age 6 and 7 years who screened positive for language problems at 30 months.

We present a prospective study at school age of neuropsychiatric and neurodevelopmental outcome of language delay suspected at child health screening around 30 months of age. In a community sample, 25 children (21 males, 4 females) screening positive and 80 children (38 males, 42 females) screening negative for speech and language problems at age 30 months were examined in detail for language disorders at age 6 years. The screen-positive children were then followed for another year and underwent in-depth neuropsychiatric examination by assessors blind to the results of previous testing. Detail

[Quality of life in patients with inflammatory bowel diseases: translation to Portuguese language and validation of the "Inflammatory Bowel Disease Questionnaire" (IBDQ)].

BACKGROUND: Crohn's disease comprises, together with ulcerative colitis, the inflammatory bowel diseases, which because their morbidity and chronic evolution have a considerable impact on the patient's quality of life. The measurement of quality of life requires the determination of the degree of subjective well-being attributed to the lack of symptoms, to psychological state and daily activity, and is useful as an instrument of evaluation, both in clinical trials and health programs. Inflammatory Bowel Disease Questionnaire is an instrument developed by North-American investigators of McMaste

Consensus and variation between speech and language therapists in the assessment and selection of preschool children for intervention: a body of knowledge or idiosyncratic decisions?

The aim was to investigate consensus between speech and language therapists (SLT) during the initial assessment of preschool children. A multimethod design was used. The qualitative component provided SLT's own views of the process as the starting point for the study. Data were generated from semistructured interviews and focus group discussions. The interviews investigated factors considered during an initial assessment of a preschool child. The focus groups discussed the process and content of a videotaped initial assessment carried out by the author. Quantitative data was generated from six

[Combined verbal and sign language for promoting communication development in non-verbal or or pre-verbal children and adolescents with mental retardation].

Conveying alternative and augmentative approaches to communication is becoming necessary for increasing numbers of persons with mental retardation with no or poor oral skills. The preverbal and nonverbal forms of communicating, hence, have special relevance in a developmental context. A natural extension of gestic messages, it in particular is the use of signing that is apt to enhance speech and language acquisition and bring about distinct improvement in understanding and making oneself understood. Exchanging messages by sign language can reveal to the child the significance of his/her own be

A Case Demonstration of the Open Health Natural Language Processing Toolkit From the National COVID-19 Cohort Collaborative and the Researching COVID to Enhance Recovery Programs for a Natural Language Processing System for COVID-19 or Postacute Sequelae of SARS CoV-2 Infection: Algorithm Development and Validation.

BACKGROUND: A wealth of clinically relevant information is only obtainable within unstructured clinical narratives, leading to great interest in clinical natural language processing (NLP). While a multitude of approaches to NLP exist, current algorithm development approaches have limitations that can slow the development process. These limitations are exacerbated when the task is emergent, as is the case currently for NLP extraction of signs and symptoms of COVID-19 and postacute sequelae of SARS-CoV-2 infection (PASC). OBJECTIVE: This study aims to highlight the current limitations of existi

Large Language Models as Raters of Hospital Incident Reports: A Blinded Multi-Model Evaluation of Severity Under-Grading and Shared Errors.

BACKGROUND: Large language models (LLMs) are increasingly proposed to classify hospital incident reports, but evidence is dominated by single-model studies reported as aggregate concordance. The authors evaluated whether frontier LLMs reliably classify incident reports and grade harm severity, whether independent models err in correlated ways, and which failure modes matter clinically. METHODS: Five LLMs (ChatGPT, Gemini, Claude, DeepSeek, Mistral) classified 824 anonymized incident reports from free text alone, using an identical prompt through providers' standard commercial interfaces, blin

Performance of large language models in complex coronary artery disease decision-making and potential improvement strategy in a real-world cohort.

OBJECTIVE: Previous studies have demonstrated that large language models (LLMs) can achieve performance comparable to multidisciplinary heart teams (HTs) in coronary revascularisation decision-making but limited by population bias. We aimed to assess LLMs’ performance in complex coronary artery disease (CAD) decision-making using a real-world cohort and to explore the potential improvement strategy. METHODS: Patients with stable CAD and angiographically confirmed three-vessel or left main disease randomly selected from a consecutive angiography registry study were included in this cross-secti

Automating Injury Severity Score Calculation From Free-Text Discharge Diagnoses Using Domain-Specific Retrieval-Augmented Large Language Models: Retrospective Validation Study.

BACKGROUND: Trauma registries are crucial for evaluating the prognosis of injuries and the quality of trauma care. However, the calculation of Injury Severity Score (ISS) remains a predominantly manual process, which is labor-intensive, potentially error-prone, and difficult to scale. Automating this process is a critical step toward enhancing the efficiency and quality of trauma registry data. OBJECTIVE: This study conducts a feasibility test on the use of large language models (LLMs) to automatically calculate ISS from free-text discharge diagnoses. We specifically investigate the efficacy

Assessing vocal fold movement in healthy young children and infants using laryngeal ultrasound by speech and language therapists: a feasibility study.

BACKGROUND: Vocal fold motion impairment (VFMI) is a common laryngeal disorder in infants and young children and may impact respiration, phonation and swallowing. Flexible nasendoscopy (FNE) is the current gold standard for assessment; however, its invasive nature may limit its use for routine screening. Recent studies reported good sensitivity and specificity of laryngeal ultrasound (LUS), a minimally invasive alternative compared to FNE. However, its application by Speech and Language Therapists (SLTs) in the paediatric population remains underexplored. OBJECTIVES/HYPOTHESIS: This study aim

A Mixed Longitudinal EEG Study of Sensorimotor Rhythm Modulation and Its Relationship with Language Development in Children Aged 3-10 Years.

Background: Sensorimotor (mu) rhythms reflect the functional state of sensorimotor cortical networks and are of increasing interest for understanding typical and atypical neurodevelopment. However, the developmental trajectories of mu rhythm modulation in preschool and early school-age children remain poorly characterized. Objectives: We studied age-related changes in alpha (8-13 Hz) and beta (13-30 Hz) sensorimotor rhythms in children aged 3 to 10 years using a mixed longitudinal design and investigated their relationship with language development. Methods: EEG was recorded twice (interval ~1

Digital twin-supported behavioral intention in mothers of young children to prevent childhood obesity: a large language model-based intervention study.

INTRODUCTION: Childhood obesity is a major determinant of lifelong non-communicable disease risk, yet early intervention may modify this trajectory. Digital twins and large language models may provide a scalable means of translating individualized risk prediction into understandable and motivating lifestyle guidance. METHODS: We developed a precision-preventive intervention integrating a childhood-overweight digital twin with an empathic, supervised-fine-tuned open-source nutrition-guidance large language model. The model was based on Llama 3-70B and fine-tuned using the Standard Tables of Fo

Robustness of a Large Language Model (LLM)-Based Virtual Patient for Japanese History-Taking Training Under Direct and Indirect Instructional Contamination.

Large language model (LLM)-based virtual patients are increasingly used to scale history-taking practice in undergraduate and postgraduate medical education. For clinical simulation, reliability requires not only avoidance of harmful content but also role-consistent case fidelity, dialogue continuity, and adherence to constraints on what the simulated patient should disclose. We evaluated these dimensions as an operational robustness benchmark, not as evidence of deployment readiness. We systematically stress-tested a Japanese LLM-based virtual patient under six robustness conditions: clean in

Multimodal Large Language Models for Breast Ultrasound Report Auditing: Workflow-Error Detection, False-Positive Burden, and Limits of Key-Image Interpretation.

Large language models (LLMs) are increasingly being evaluated for radiology reporting. However, the incremental value of adding report-embedded key images to report text for breast ultrasound report auditing remains unclear. This retrospective study included 818 breast ultrasound examinations with pathology- or follow-up-based reference standards. A workflow-error-enriched 300-report subset was constructed, comprising 240 reports with 329 inserted errors and 60 error-free reports. GPT-5.5 and Gemini 3.1 Pro Preview were evaluated under two input settings: report text alone and multimodal input
