Angiogenesis and Resistance Mechanisms in Glioblastoma: Targeting Alternative Vascularization Pathways to Overcome Therapy Resistance.

INTRODUCTION: Glioblastoma (GBM), the most aggressive form of primary brain tumor in adults, remains a significant clinical challenge due to its high recurrence and poor prognosis. Characterized by rapid growth, invasiveness, and resistance to therapy, GBM relies on a sophisticated vascular network to sustain its progression. Angiogenesis, the process of forming new blood vessels, is central to meeting the metabolic demands of the tumor. To address this issue, there is a growing consensus on the need for multi-pronged therapeutic strategies that not only inhibit angiogenesis but also disrupt a

Decoding and Overcoming Temozolomide Resistance Through CRISPR/Cas Technologies.

Intrinsic and acquired resistance to temozolomide (TMZ), the standard chemotherapeutic agent for glioblastoma (GBM), is highly common and results in poor clinical outcomes. This review highlights the emerging dual role of Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-associated protein (Cas) technologies in addressing this challenge. First, it examines genome-wide CRISPR screens that revealed DNA damage repair networks, stress adaptations, stemness maintenance, and tumor heterogeneity as key drivers of resistance to TMZ. Second, it examines CRISPR/Cas-based strategies, inc

Divergent Roles of Canonical and Non-Canonical Mismatch Repair in Regulating Temozolomide Sensitivity in Glioblastoma.

Temozolomide (TMZ) remains the cornerstone of chemotherapeutic agent for glioblastoma (GBM), yet intrinsic and acquired resistance severely limits its clinical benefit. While O6-methylguanine-DNA methyltransferase (MGMT)-mediated repair of TMZ-induced O6-methylguanine (O6-meG) lesions has been extensively studied, the DNA mismatch repair (MMR) pathway is increasingly recognized as a key determinant of TMZ cytotoxicity. Canonical MMR, mediated by MutSα (MSH2-MSH6) and MutLα (MLH1-PMS2) complexes, recognizes O6-meG: thymine mispairs generated during replication and initiates futile repair cycles

Sustained complete remission in chemotherapy-refractory advanced lung adenocarcinoma with bevacizumab and iodine-125 seed implantation: A long-term case report.

RATIONALE: The introduction of small-molecule tyrosine kinase inhibitors and immune checkpoint inhibitors has transformed therapeutic strategies for lung adenocarcinoma, significantly improving the survival of eligible patients. However, the prognosis remains poor for those lacking actionable driver mutations and presenting with brain metastases, who typically exhibit a median survival of <6 months. PATIENT CONCERNS: After the initial resection (November 2016) and progression after adjuvant paclitaxel/cisplatin chemotherapy, the patient developed brain and pulmonary metastases. DIAGNOSES: Lu

M-STGCN: A Position-Aware Multimodal Graph Convolutional Framework for Joint Spatial Domain Identification and Gene Expression Denoising.

Spatially transcriptomics (ST) has revolutionized our ability to profile gene expression within the architectural complexity of tissue microenvironments. However, decoding spatial heterogeneity requires robust multimodal data integration that unifies gene expression, spatial positions, and histopathological images to overcome modality-specific biases. Here, we propose M-STGCN, a multimodal unsupervised framework that constructs a spatial weight matrix from spatial coordinates to simultaneously refine both gene expression profiles and image features, by which we establish position-aware feature

Intraoperative radiotherapy for brain metastases after surgical resection a systematic review and meta-analysis.

Brain metastases (BMs) are common in advanced cancers such as lung, breast, and melanoma, and carry a poor prognosis with high morbidity and local recurrence rates after surgery alone. Intraoperative radiotherapy (IORT) has emerged as a promising adjuvant treatment, delivering radiation directly to the resection cavity to reduce recurrence and minimize delays to systemic therapy. IORT also demonstrated more favorable dose homogeneity than single-fraction stereotactic radiosurgery, with fractionated IMRT achieving the highest uniformity. However, evidence regarding its efficacy and safety compa

The efficacy and safety of poly ADP-ribose polymerase (PARP) inhibitors in patients with high-grade glioma (HGG): A systematic review and meta-analysis.

PURPOSE: While poly ADP-ribose polymerase (PARP) inhibitors represent a promising treatment strategy in early phase trials of patients with high-grade glioma (HGG), the clinical outcomes exhibit variability, mainly due to drug-specific pharmacokinetics and insufficient biological stratification involving MGMT and IDH status. We aimed to investigate the safety and efficacy of PARP inhibitors in grade 3 and 4 gliomas. METHODS: A systematic review and data synthesis were conducted. Records were extracted from PubMed, Scopus, Web of Science, and Embase from inception to May 24th, 2025, and includ

Synchronous diagnosis of multicentric glioma with distinct isocitrate dehydrogenase molecular profiles: a case report.

This case report characterizes the molecular pathology of two synchronous IDH mutant gliomas in a 28-year-old female patient. The patient exhibited symptoms of dizziness, retro-orbital pain, headache, and numbness with paresthesia in her right arm. MRI imaging revealed two distinct non-enhancing T2/FLAIR hyperintense lesions in the left frontal and parietal lobes. Histopathologic and molecular analyses, including whole exome sequencing, were performed on the resected specimens from each location. The left parietal tumor was diagnosed as a grade 4 astrocytoma with an IDH1 R132H mutation, while

Beyond Oligodendroglioma: An Integrated Diagnostic Approach to CNS Tumors with Oligodendroglioma-like Morphology, with a Focus on Morphological Pitfalls, Immunoprofiles, and Molecular Signatures.

Oligodendroglioma-like morphology, classically recognized by round nuclei, optically clear cytoplasm, and perinuclear halos, is one of the most familiar patterns in neuropathology but also one of the most diagnostically misleading. Although historically associated with oligodendroglioma, this phenotype is now recognized across a wide spectrum of neoplastic and non-neoplastic central nervous system lesions, including adult-type diffuse gliomas, ependymal tumors, pediatric-type low-grade gliomas, glioneuronal and neurocytic tumors, metastatic clear-cell neoplasms, demyelinating disease, and suba

Prevalence and clinicopathological characteristics of breast cancer patients with brain metastases in Ghana: A single-center cross-sectional study.

Breast cancer remains a leading cause of cancer-related mortality among women globally. While advances in early diagnosis and systemic therapies have improved survival, they have also increased the likelihood of brain metastases over time, particularly in low-resource settings where limited diagnostic and treatment capacity exacerbates the burden of late-stage disease. The study aimed to determine the prevalence and describe the clinicopathological characteristics of breast cancer patients diagnosed with brain metastasis in a limited-resource healthcare setting. This research was a single-inst

A Rare Case of Occult Small-cell Lung Cancer with Initial Manifestations of Brain Metastases.

We report a rare case of small-cell lung cancer (SCLC) detected 14 months after the initial manifestation of brain metastases. A 71-year-old man presented with neurological symptoms that led to the resection of brain neuroendocrine carcinoma (NEC). Despite extensive imaging, no primary tumor was detected during the initial visit. Fourteen months later, a lung nodule appeared in the emphysematous bullae in the left upper lobe, which was confirmed as SCLC. Chemotherapy plus durvalumab was administered to control the tumor. This case highlights the diagnostic challenges of occult SCLC and emphasi

Opioid withdrawal syndrome induced by naldemedine administration in a cancer patient without brain metastasis.

OBJECTIVES: Naldemedine is a peripherally acting μ-opioid receptor antagonist used to treat opioid-induced constipation. As this drug does not cross the blood-brain barrier, it is believed that patients without brain metastases do not experience opioid withdrawal symptoms. METHODS: Here, we experienced a case in which a cancer patient without brain metastasis presented with anxiety and restlessness that was severe enough to interfere with daily life. The patient was diagnosed with naldemedine-induced opioid withdrawal syndrome. RESULTS: The patient was a 66-year-old male with liver cancer me

Exosomes: Their role in the diagnosis, progression, metastasis, and treatment of glioblastoma.

Exosomes are crucial for the growth and spread of glioblastomas, an aggressive form of brain cancer. These tiny vesicles play a crucial role in the activation of signaling pathways and intercellular communication. They can also transfer a variety of biomolecules such as proteins, lipids and nucleic acids from donor to recipient cells. Exosomes can influence the immune response by regulating the activity of immune cells, and they are crucial for the growth and metastasis of glioblastoma cells. In addition, exosomes contribute to drug resistance during treatment, which is a major obstacle in the
