Case Report: Imaging Features of Gallbladder Sessile Polyp Confirmed by Contrast-Enhanced Ultrasonography and Dynamic Computed Tomography in a Dog With Asymptomatic Chronic Cholecystitis.

A 6-year-old dog was presented for health screening. It never suffered from any disease. Ultrasonography confirmed mass-like thickening with irregular margins protruding toward the gallbladder (GB) lumen. On contrast-enhanced ultrasonography (CEUS) and dynamic computed tomography (CT), contrast enhancement of the corresponding structures was confirmed in arterial phase. After cholecystectomy, cauliflower-like sessile polyps were identified. Histopathological examination revealed chronic lymphoplasmacytic cholecystitis. Bile culture revealed Escherichia coli growth. Our novel findings suggest t

Incidental Gallbladder Cancer on Cholecystectomy: Strategy for Re-resection of Presumed Benign Diseases from a Retrospective Multicenter Study by the Yokohama Clinical Oncology Group.

BACKGROUND/AIM: Current expert consensus recommends re-resection for incidental gallbladder cancer (IGBC) of pT1b-3. This study examined whether this consensus was reasonably applicable to patients with IGBC in one Japanese region. PATIENTS AND METHODS: This was a multicenter, retrospective analysis of cholecystectomies for presumed benign diseases between January 2000 and December 2009. RESULTS: IGBC was diagnosed in 70 (1.0%) out of 6,775 patients undergoing cholecystectomy. Five-year disease-specific cumulative survival was 100% in 19 patients with pT1a, 80.0% in five with pT1b, 49.5% in

Diffuse large B-cell lymphoma of the gallbladder arised 8 years after malignant lymphoma of the right testis: A case report and literature review.

INTRODUCTION: Gallbladder involvement in lymphoma is extremely rare, and only 68 cases have been reported in the English literature so far. We experienced a case of diffuse large B-cell lymphoma (DLBCL) of the gallbladder arising 8 years after DLBCL of the right testis. PRESENTATION OF CASE: A 68-year-old man underwent orchiectomy for malignant lymphoma of the right testis pathologically diagnosed as DLBCL 8 years ago. Systemic surveillance incidentally revealed a gallbladder tumour, and elective resection of the gallbladder bed of the liver was performed under a preoperative diagnosis of gal

An unusual complication of pyloric ring obstruction caused by flange of lumen apposing metal stent in endoscopic ultrasound-guided gallbladder drainage: A case report.

INTRODUCTION: Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) is an alternative treatment option for patients with acute cholecystitis, especially for those who are unsuitable for cholecystectomy. Recently used luminal apposing metal stents (LAMS) in EUS-GBD has several advantages over standard metal stents. However, there is no current guideline on where to locate the LAMS when transgastric approach is required. This study reports a case of gastric outlet obstruction (GOO) by placing LAMS too close to the pyloric ring. PATIENT CONCERNS: A 79-year-old female patient was referred t

The optimal number of lymph nodes to evaluate among patients undergoing surgery for gallbladder cancer: Correlating the number of nodes removed with survival in 6531 patients.

BACKGROUND: The aim of the current study was to identify the minimum number and the optimal range of lymph nodes (LNs) to be examined among patients with gallbladder cancer (GBC). METHODS: Between January 1, 2004, and December 31, 2015, patients with a diagnosis of GBC were identified in the National Cancer Database. A machine-based learning approach was used to identify the minimum number and range of LNs to evaluate relative to long-term outcomes. RESULTS: Among 6531 patients with GBC, median number of LNs evaluated was 2 (IQR:1-5); only 21.1% (n = 1376) of patients had 6 or more LNs evalu

Validation of the 8th Edition of the American Joint Committee on Cancer Staging System for Gallbladder Cancer and Implications for the Follow-up of Patients without Node Dissection.

PURPOSE: The 8th edition of gallbladder cancer staging in the American Joint Committee on Cancer (AJCC) staging system changed the T and N categories. MATERIALS AND METHODS: In order to validate the new staging system, a total of 348 surgically resected gallbladder cancers were grouped based on the 8th edition of the T and N categories and compared with patients' survival. RESULTS: Significant differences were noted between T1b-T2a (p=0.003) and T2b-T3 (p < 0.001) tumors, but not between Tis-T1a, T1a-T1b, and T2a-T2b tumors. However, significant survival differences were observed both by the

A novel gallbladder umbrella stent (the Shai™ Stent) for prevention of stone migration and impaction: results on feasibility and short-term safety in a porcine model.

BACKGROUND AND AIMS: Cholelithiasis is the most common biliary tract disorder. Surgery is the treatment of choice for symptomatic gallstones. Aims of this study were to investigate the feasibility and short-term safety of a new endoscopic procedure with a specially designed Nitinol gallbladder stent for blockage of gallstone migration, the Shai™ Stent. The Shai stent is designed to enable free bile flow, which will be supposed to prevent recurrent attacks. METHODS: The Shai™ Stent was inserted into the gallbladder during a standard ERCP procedure using a conventional metal stent delivery syst

NT5E and FcGBP as key regulators of TGF-1-induced epithelial-mesenchymal transition (EMT) are associated with tumor progression and survival of patients with gallbladder cancer.

Epithelial-mesenchymal transitions (EMTs) are essential manifestations of epithelial cell plasticity during tumor progression. Transforming growth factor-β(TGF-β) modulates epithelial plasticity in tumor physiological contexts by inducing EMT, which is associated with the altered expression of genes. In the present study, we used DNA micro-array analysis to search for differentially expressed genes in the TGF-β1 induced gallbladder carcinoma cell line (GBC-SD cells), as compared with normal GBC-SD cells. We identified 225 differentially expressed genes, including 144 that were over-expressed a

Pathologic staging of pancreatic, ampullary, biliary, and gallbladder cancers: pitfalls and practical limitations of the current AJCC/UICC TNM staging system and opportunities for improvement.

Tumors of the ampulla-pancreatobiliary tract are encountered increasingly; however, their staging can be highly challenging due to lack of familiarity. In this review article, the various issues encountered in staging of these tumors at the pathologic level are evaluated and possible solutions for daily practice as well as potential improvements for future staging protocols are discussed. While N-stage parameters have now been well established (the number of lymph nodes required in pancreatoduodenectomies is 12), the T-staging has several issues: for the pancreas, the discovery of small cancer

Intracholecystic papillary-tubular neoplasms (ICPN) of the gallbladder (neoplastic polyps, adenomas, and papillary neoplasms that are ≥1.0 cm): clinicopathologic and immunohistochemical analysis of 123 cases.

The literature on the clinicopathologic characteristics of tumoral intraepithelial neoplasms (neoplastic polyps) of the gallbladder (GB) is fairly limited, due in part to the variability in definition and terminology. Most reported adenomas (pyloric gland type and others) were microscopic and thus regarded as clinically inconsequential, whereas papillary in situ carcinomas have been largely considered a type of invasive adenocarcinoma under the heading of "papillary adenocarcinomas." In this study, 123 GB cases that have a well-defined exophytic preinvasive neoplasm measuring ≥1 cm were analyz

From the archives of the AFIP. Benign tumors and tumorlike lesions of the gallbladder and extrahepatic bile ducts: radiologic-pathologic correlation. Armed Forces Institute of Pathology.

A diverse spectrum of benign tumors and tumorlike lesions arises from the gallbladder and bile ducts, and despite their diversity, these lesions share common embryologic origins and histologic characteristics. Although these lesions are relatively uncommon, their importance lies in their ability to mimic malignant lesions in these locations. Benign neoplasms are derived from the epithelial and nonepithelial structures that compose the normal gallbladder and bile ducts. The epithelium gives rise to adenomas, cystadenomas, and the unusual condition of biliary papillomatosis. Granular cell tumors

[Prospective study of functional changes in the gallbladder and formation of gallstones induced by long-term treatment of somatostatin analogue SMS201-995 in patients of active acromegaly].

We reports the result about the changes of function of gallbladder in 17 acromegalic patients after treatment with long-acting somatostatin analogue SMS201-995 for 3-36 months by ultrasonogram. The number of cases developed bile sludges, gallstones, chronic and acute cholecystitis was 14, 8, 10 and 1 respectively during therapy. The contractibility of gallbladder after fatty meal was inhibited in all of the patients during the course of therapy. A bolus injection of SMS201-995 with a dosage of 100 micrograms for acromegalic patients resulted in marked suppression of the contractibility of the

Global burden landscape (1990-2021) of gallbladder and biliary tract cancer attributable to high body mass index in older adults and a projection to 2050.

BACKGROUND: Obesity and population aging significantly influence the disease burden of gallbladder and biliary tract cancers (GBTCs). The burden of GBTC attributable to a high body mass index (BMI) among older adults was determined using data from the Global Burden of Disease (GBD) Study 2021. METHODS: GBTC trends in deaths, disability-adjusted life years (DALYs), and age-standardized ratios (ASRs) were examined for different populations, geographic regions, and socio-demographic index (SDI) levels. Stratified clustering by estimated annual percentage change (EAPC) was used to assess patterns

Pre-admission Barthel Index and serum albumin are associated with discharge to the pre-admission residence after percutaneous transhepatic gallbladder drainage for acute cholecystitis.

PURPOSE: Percutaneous transhepatic gallbladder drainage (PTGBD) is often performed for acute cholecystitis in medically fragile patients, but the factors associated with discharge to the pre-admission residence remain unclear. Thus, we examined the determinants of discharge to the pre-admission residence after PTGBD. METHODS: The subjects of this retrospective study were 73 patients who underwent PTGBD for acute cholecystitis between March 2018 and March 2025. The primary outcome was final discharge to the pre-admission residence, defined as discharge without escalation of residential care ne

Global, regional, and national burden of gallbladder and biliary tract cancer, 1990 to 2021 and predictions to 2045: an analysis of the Global Burden of Disease study 2021.

BACKGROUND: Identifying past, present, and future temporal trends in gallbladder and biliary tract cancer (GBTC) can increase public awareness and promote changes in prevention and treatment strategies. METHODS: The incidence and death rates of GBTC between 1990 and 2021 were extracted from the Global Burden of Disease study 2021 and assessed according to country, region, year, age, and sex. Time trends were measured using the average annual percentage change (AAPC) and projections of the burden of disease for 2022 to 2045 were made using the Bayesian age-period-cohort model. RESULTS: In 202

Trends and cross-country inequalities in the global, regional, and national burden of gallbladder and biliary tract cancer from 1990 to 2021, along with the predictions for 2035.

BACKGROUND: The global burden of gallbladder and biliary tract cancer (GBTC) is on the rise. METHODS: The incidence, mortality, and disability-adjusted life years (DALYs) of GBTC from 1990 to 2021 were obtained from the Global Burden of Diseases Study (GBD) 2021. The Bayesian Age-Period-Cohort (BAPC) model was also employed to project disease trends for the next 15 years. RESULTS: In 2021, new GBTC cases observed globally amounted to 216,768, with 171,961 mortality and 3732,121 DALYs. From 1990-2021, a notable surge was recorded in the incidence of GBTC by 101 %, mortality by 74.26 %, and DA

Clinical outcomes of Endoscopic ultrasound--guided gallbladder drainage in patients with acute cholecystitis with ≥1 year of follow-up: a systematic review and meta-analysis.

BACKGROUND AND AIMS: EUS-guided gallbladder drainage (EUS-GBD) has emerged as a reliable management option in high-risk surgical patients with acute cholecystitis. Its technical success has been well described in the literature. Guidelines now support the use of EUS-GBD over percutaneous transhepatic gallbladder drainage in this patient population. In this context, we aimed to assess the clinical success of individuals treated with EUS-GBD for acute cholecystitis, with a follow-up period exceeding 1 year. METHODS: Multiple databases were searched for studies looking at EUS-GBD for acute chole

Morphological changes in the biliary mucosa in pediatric patients with congenital biliary dilatation are more influenced by the duration of amylase exposure than by amylase levels in the gallbladder.

PURPOSE: We investigated the relationship between bile amylase (AMY) levels and biliary epithelial changes in pancreaticobiliary maljunction (PBM), a congenital anomaly characterized by pancreaticobiliary reflux due to duct fusion outside the duodenal wall. METHODS: We enrolled 43 children with congenital biliary dilatation (CBD) of Todani types Ia, Ic, and IVa who underwent surgery at the Hokkaido Medical Center for Child Health and Rehabilitation between November 2007 and June 2023.
