Flexible bronchoscopic findings and the relationship to repeated extubation failure in critical children.

BACKGROUND: Extubation failure (EF) in acute pediatric cases causes high morbidity and prolonged hospitalization, some of which might encounter EF repeatedly. This study aims to investigate flexible bronchoscopic findings of airway problems associated with repeated EF (REF) in children. METHODS: We retrospectively reviewed the medical records of intubated children from 2005 to 2013 and enrolled those with EF (reintubated within 48 h after extubation) and receiving flexible bronchoscopy (FB) examinations. We divided all subjects into two groups, the REF group (reintubated within 48 h after FB

The Easytube for airway management: a systematic review of clinical and simulation studies.

STUDY OBJECTIVE: Endotracheal intubation is considered the criterion-standard technique for securing the airway. Supraglottic airway devices (SADs) represent a major advance in airway management and are recommended by the guidelines in difficult situations such as Advanced Life Support and "cannot ventilate-cannot intubate" scenarios. The Easytube (EzT) is an SAD introduced a decade ago but not included yet in the above guidelines. DESIGN: Systematic review of MEDLINE and EMBASE according to PRISMA guidelines available up to January 12, 2016. SETTING: We collected experimental and clinical e

Effect of just-in-time simulation training on provider performance and patient outcomes for clinical procedures: a systematic review.

BACKGROUND: Providing simulation training directly before an actual clinical procedure-or 'just-in-time' (JiT)-is resource intensive, but could improve both provider performance and patient outcomes. OBJECTIVES: To assess the effects of JiT simulation training versus no JiT training on provider performance and patient complications following clinical procedures on patients. STUDY SELECTION: We searched MEDLINE, Cochrane Library, CINAHL, PsycINFO, ERIC, ClinicalTrials.gov, simulation journals indexes and references of included studies during October 2014 for randomised trials, non-randomised

Use of Microcuff(®) endotracheal tubes in paediatric laparoscopic surgeries.

BACKGROUND AND AIMS: Traditionally, uncuffed endotracheal tubes have been used in children. Cuffed tubes may be useful in special situations like laparoscopy. Microcuff(®) endotracheal tube is a specifically designed cuffed endotracheal tube for the paediatric airway. We studied the appropriateness of Microcuff(®) tube size selection, efficacy of ventilation, and complications, in children undergoing laparoscopy. METHODS: In a prospective, observational study, 100 children undergoing elective laparoscopy were intubated with Microcuff(®) tube as per recommended size. We studied appropriateness

Results of noninvasive ventilation in very old patients.

BACKGROUND: Noninvasive ventilation (NIV) is frequently used for the management of acute respiratory failure (ARF) in very old patients (≥ 80 years), often in the context of a do-not-intubate order (DNI). We aimed to determine its efficacy and long-term outcome. METHODS: Prospective cohort of all patients admitted to the medical ICU of a tertiary hospital during a 2-year period and managed using NIV. Characteristics of patients, context of NIV, and treatment intensity were compared for very old and younger patients. Six-month survival and functional status were assessed in very old patients.

Quantitative assessment of pelvic ileal reservoir emptying with a semisolid radionuclide enema. A correlation with clinical outcome.

Fecal stasis in an ileal reservoir may lead to overgrowth of bacteria and changes in mucosal morphology that are of future concern. Moreover, improper evacuation may lead to increased stool frequency and reduced continence. The aims of this study were: to compare the functional results of two types of ileal reservoirs constructed with an ileoanal anastomosis, to compare their emptying to a normal rectum as assessed by a radionuclide enema, and to correlate functional results with emptying. The reservoirs were made of two (J) or three (S) limbs of terminal ileum. Thirty-three patients (16 with

[Myasthenia gravis: perioperative complications of 1002 transsternal thymectomies - historical overwiev of 34 years long practice at one institution].

Introduction: Thymectomy became an important part of the treatment of myasthenia gravis, since Alfred Blalock reported about his first surgery 80 years ago. Despite of several different surgical techniques already accepted abroad, sternal approach was the almost exclusive exposure for thymectomy in Hungary till 2006. In this publication, we analyze the direct surgical consequences and complications of this method. Methods: At the Surgical Department of Budai MÁV Hospital, 1002 transsternal thymectomies were performed during 34 years on patients suffering from myasthenia gravis. Surgeries were

Noninvasive ventilation in the event of acute respiratory failure in patients with idiopathic pulmonary fibrosis.

BACKGROUND: Some patients with idiopathic pulmonary fibrosis (IPF) develop severe acute respiratory failure (ARF) requiring admission to an intensive care unit (ICU) and ventilatory support. A limited number of observational studies have reported that noninvasive ventilation (NIV) can be an effective treatment to support breathing and to prevent use of invasive mechanical ventilation in these patients. This study aimed to retrospectively investigate the clinical status and outcomes in IPF patients receiving NIV for ARF and to identify those clinical and laboratory characteristics, which could

Community-Level Disparities in the Inhalation Injury Patient Population.

Low-income neighborhoods face higher fire risk due to overcrowding, substandard housing, and limited access to safety devices, contributing to greater burn severity among affected residents. Inhalation injury, a frequent consequence, often necessitates early intubation and transfer to burn centers. Given the high degree of coordination required, this study examines how management strategies and outcomes vary across levels of social vulnerability and urbanicity, 2 community-level proxies for access to care. We retrospectively reviewed inhalation injuries diagnosed by bronchoscopy at a single AB

Transcranial Doppler findings in patients with COVID-19: a prospective observational study.

BACKGROUND: Transcranial Doppler (TCD) is a diagnostic tool to detect real-time spontaneous microembolic signals (MESs) in intracranial circulation. Critically-ill coronavirus disease 2019 (COVID-19) patients may present an increased likelihood of MESs due to systemic inflammation and hypercoagulability. OBJECTIVE: To assess the presence of MESs and other TCD parameters, including cerebral blood flow velocity (CBFV), pulsatility, and resistance indices, and to calculate the estimated cerebral perfusion pressure (eCPP) in patients with and without COVID-19. METHODS: The TCD monitoring was per
