Cell-based and extracellular vesicle-based MSC therapies for acute radiation syndrome affecting organ systems.

Exposure to ionizing radiation can induce harmful biological effects on the human body, particularly in cases of high-dose γ-irradiation affecting the gastrointestinal tract, bone marrow, skin and lung. Such exposures lead to lethal outcomes as individuals experience a breakdown in their immune system's ability to defend against pathogens, predisposing them to sepsis-induced multiple organ failures. Mesenchymal stromal/stem cells (MSCs) possess diverse biological characteristics, including immunomodulation, anti-inflammation and tissue regeneration. Off-the-shelf culture-expanded human bone ma

Impact of spinal injury on mortality in patients with major trauma: A propensity-matched cohort study.

Major trauma is defined as a significant injury or injury that has the potential to be life-threatening and is quantitatively identified as an injury severity score (ISS) >15. Spinal injuries are common in patients with major trauma; however, because spinal injury is not independently included in the ISS calculation, the impact of spinal injury on mortality in patients with major trauma has not been fully elucidated. The purpose of this study is to identify the association between spinal injury and mortality in patients with major trauma. From January 1, 2016, to December 31, 2020, retrospecti

Laboratory Mice - A Driving Force in Immunopathology and Immunotherapy Studies of Human Multiple Myeloma.

Mouse models of human cancer provide an important research tool for elucidating the natural history of neoplastic growth and developing new treatment and prevention approaches. This is particularly true for multiple myeloma (MM), a common and largely incurable neoplasm of post-germinal center, immunoglobulin-producing B lymphocytes, called plasma cells, that reside in the hematopoietic bone marrow (BM) and cause osteolytic lesions and kidney failure among other forms of end-organ damage. The most widely used mouse models used to aid drug and immunotherapy development rely on in vivo propagatio

Reuse of liver allograft from a brain-dead recipient: A case report.

We report our first case of deceased-donor liver transplantation (LT) using a reuse liver graft after the first LT. The recipient was a 38-year-old female with fulminant hepatic failure from toxic hepatitis. She had a history of herb intake and her liver function deteriorated progressively. She was enrolled as the Korean Network for Organ Sharing (KONOS) status 1 and the model for end-stage liver disease score was 34. The donor was a 42-year-old male patient who fell into brain death after LT for alcoholic liver cirrhosis. Donation of multiple organs including the transplanted liver graft was

Costimulatory molecules as targets for the induction of transplantation tolerance.

Transplantation is the only cure for end-stage organ failure. Transplanted tissues are usually recognized by the immune system as foreign and are rapidly rejected in the absence of immunosuppression. Transplanted organs between genetically distinct individuals are termed allografts and their acute rejection is orchestrated by the activation of allospecific T cells. To prevent acute allograft rejection, current therapies suppress all T cells irrespective of their specificities and must be taken life-long, leaving patients with decreased defenses against infectious agents and cancers. The goal i

Effect of acute upper gastrointestinal bleeding manifestations at admission on the in-hospital outcomes of liver cirrhosis: hematemesis versus melena without hematemesis.

OBJECTIVES: Patients with acute upper gastrointestinal bleeding (AUGIB) often manifest as hematemesis and melena. Theoretically, hematemesis will carry worse outcomes of AUGIB. However, there is little real-world evidence. We aimed to compare the outcomes of hematemesis versus no hematemesis as a clinical manifestation of AUGIB at admission in cirrhotic patients. METHODS: All cirrhotic patients with AUGIB who were consecutively admitted to our hospital from January 2010 to June 2014 were considered in this retrospective study. Patients were divided into hematemesis with or without melena and

Model for end-stage liver disease score predicts adverse events related to ventricular assist device therapy.

BACKGROUND: The Model for End-stage Liver Disease (MELD) score is a marker of multisystem organ dysfunction. It has been used to predict outcomes in patients undergoing hepatic and nonhepatic interventions. End-stage heart disease exhibits a varying degree of multiorgan dysfunction, which impacts the adverse events related to ventricular assist device (VAD) therapy. Our aim for the present study was to investigate the value of MELD score in predicting adverse events related with VAD therapy. METHODS: Data were collected on demographics, clinical characteristics, MELD score; Interagency Regist

Bile Acids as Mitochondrial Toxins: Emerging Insights in Fontan and Heart Failure with Preserved Ejection Fraction.

Elevated bile acids, particularly hydrophobic secondary species, impair mitochondrial structure and function across multiple organs. In cardiomyocytes, pathologically elevated circulating bile acids impair mitochondrial biogenesis, diminish ATP generation, disrupt calcium homeostasis, and compromise myocardial energetics. These alterations promote oxidative stress and activate mitochondrial apoptotic pathways, culminating in systolic dysfunction, diastolic impairment, and heightened arrhythmogenic risk. Parallel mitochondrial injury in liver and skeletal muscle contributes to liver fibrosis an

[Non-clinical safety assessments using human cells for pediatric drug development].

Pediatric drug development is an urgent need to provide appropriate therapies for pediatric patients. Because children are undergoing continuous growth and development of their body and organs, pharmacological responses to the same drugs may differ from those observed in adults. Clinical trials in children are essential to evaluate efficacy and safety in pediatric populations; however, pediatric drug development faces multiple challenges, including the limited number of eligible patients. To improve the safety and efficiency of clinical trials, it is important to confirm the safety and efficac

Neorectal Perforation and Necrotizing Soft Tissue Infection of the Lower Limb Following Pelvic Reirradiation for Recurrent Rectal Cancer: A Case Report.

Neorectal perforation is a rare but serious complication following treatment for rectal cancer, and may lead to catastrophic sequelae if diagnosis and management are delayed. We report the case of a 54-year-old woman with locally recurrent rectal cancer who developed neorectal perforation and necrotizing soft tissue infection of the lower limb following chemoradiotherapy. Initial symptoms were attributed to sciatic nerve inflammation, and surgical consultation was delayed. The patient subsequently developed septic shock and was found to have extraperitoneal perforation extending through the gr

Whole-genome sequence of a female Loa loa adult worm from Cameroon.

OBJECTIVE: Although loiasis affects more than 20 million people and that its associated clinical complications can cause multiple organ failures, this disease remains understudied. Consequently, the elimination of loiasis is not on the agenda of public health services. Defining an effective strategy to tackle this disease is challenging and requires a better understanding of the parasite's biology, including its genetic aspects. Loa loa adult worms provide large amounts of biological tissues for genetic sequencing, but are very difficult to collect. Therefore, limited genomic data are availabl

Case Report: A rare case of complex Behçet's disease complicated with acute tubular necrosis and IgA nephropathy, coexists with myelodysplastic syndrome, trisomy 8 and intestinal involvement.

Behçet's disease (BD) is a rare systemic disease in which small-vessel vasculitis impacts multiple bodily organs. It is typically marked by recurrent oral and genital ulcers, uveitis, and cutaneous lesions. However, peripheral vessels, cardiovascular structures, central nervous system, gastrointestinal tract, joints, lungs, or kidneys may be affected as well. Renal involvement, although uncommon, may manifest as proteinuria, hematuria, and varying degrees of renal insufficiency. Herein, we describe a 35-year-old man with longstanding BD, myelodysplastic syndrome (MDS), and trisomy 8. He presen

Estimating Time Window to Administer Anti-Cytokine Therapeutic Drugs to COVID-19 Patients.

The severe COVID-19 infection often leads to "Cytokine Release Syndrome (CRS)", which is a serious adverse medical condition causing multiple organ failures. Anti-cytokine therapy has shown promising results for the treatment of the CRS. As part of the anti-cytokine therapy, the immuno-suppressants or anti-inflammatory drugs are infused to block the release of cytokine molecules. However, determining the time window to infuse the required dose of drugs is challenging due to the complex processes involving the release of inflammatory markers, such as IL-6 and C-reactive protein (CRP) molecules.

Cyclic nucleotide phosphodiesterases as therapeutic targets in cardiac hypertrophy and heart failure.

Cyclic nucleotide phosphodiesterases (PDEs) modulate the neurohormonal regulation of cardiac function by degrading cAMP and cGMP. In cardiomyocytes, multiple PDE isozymes with different enzymatic properties and subcellular localization regulate local pools of cyclic nucleotides and specific functions. This organization is heavily perturbed during cardiac hypertrophy and heart failure (HF), which can contribute to disease progression. Clinically, PDE inhibition has been considered a promising approach to compensate for the catecholamine desensitization that accompanies HF. Although PDE3 inhibit

Understanding the causes and consequences of measles virus persistence.

Measles is an acute systemic viral disease with initial amplification of infection in lymphoid tissue and subsequent spread over 10-14 days to multiple organs. Failure of the innate response to control initial measles virus (MeV) replication is associated with the ability of MeV to inhibit the induction of type I interferon and interferon-stimulated antiviral genes. Rather, the innate response is characterized by the expression of proteins regulated by nuclear factor kappa B and the inflammasome. With eventual development of the adaptive response, the rash appears with immune cell infiltration

Heart transplantation in adults with congenital heart disease.

With the advances in congenital cardiac surgery and postoperative care, an increasing number of children with complex congenital heart disease now reach adulthood. There are already more adults than children living with a congenital heart defect, including patients with complex congenital heart defects. Among these adults with congenital heart disease, a significant number will develop ventricular dysfunction over time. Heart failure accounts for 26-42% of deaths in adults with congenital heart defects. Heart transplantation, or heart-lung transplantation in Eisenmenger syndrome, then becomes

Significance of Proliferation Markers and Prognostic Factors in Egyptian Patients with Multiple Myeloma.

BACKGROUND: Multiple myeloma (MM) is influenced by genetic and micro-environmental changes. Malignant plasma cells produce an abnormal monoclonal immunoglobulin, as well as cytokines, such as IL-10 and IL-6 which stimulate cells of the bone marrow microenvironment (BMM) and cause dysfunction and failure of many organs. B cell activating factor (BAFF), IL6 and IL10 are known to influence the growth and survival of malignant clones. AIM: The objectives of the present study were to investigate the circulating levels of BAFF , IL-10 and IL-6, correlate them with well-known parameters of disease a

CYLD mediates ciliogenesis in multiple organs by deubiquitinating Cep70 and inactivating HDAC6.

Cilia are hair-like organelles extending from the cell surface with important sensory and motility functions. Ciliary defects can result in a wide range of human diseases known as ciliopathies. However, the molecular mechanisms controlling ciliogenesis remain poorly defined. Here we show that cylindromatosis (CYLD), a tumor suppressor protein harboring deubiquitinase activity, plays a critical role in the assembly of both primary and motile cilia in multiple organs. CYLD knockout mice exhibit polydactyly and various ciliary defects, such as failure in basal body anchorage and disorganization o
