Adult survivors of childhood trauma: Complex trauma, complex needs.

BACKGROUND: Childhood trauma is common and can have profound consequences throughout a person's life. Adult survivors of childhood abuse pose a number of challenges for general practitioners (GPs). The diagnosis of their medical and psychiatric illnesses is complex; the therapeutic relationship can be both delicate and critical to recovery; and the treatments are varied, often expensive and frequently inaccessible. OBJECTIVE: The aim of this article is to provide a conceptual framework for the assessment and management of an adult survivor of childhood adversity. DISCUSSION: GPs can be effec

[Healthcare in One's Own Home or Outside: A Comparison of the Health Status of Family Caregivers].

The GEDA-Survey 2012 is a representative, nationwide survey conducted by means of computer-assisted telephone interviews (CATI) between March 2012 and March 2013 by the Robert Koch Institute with 19 294 completed interviews. A total of 1 219 persons tending to the needs of a care-dependent person responded to questions about sex, age and health status. Using the statistics software "SPSS", the GEDA-data were evaluated descriptively and finally visualized with "Microsoft Office". The survey results reveal that 47,2% of family caregivers from this cohort are between 45 and 64 years old. This coh

[Pathological gambling and personality disorders: a pilot-study with the MCMI-II].

In this paper, the most frequent personality disorders related to pathological gambling are described. A sample of 50 pathological gamblers, who were assessed with the MCMI-II before treatment, and of 50 normative subjects from general population with the same demographic features (age, sex and socioeconomic level) was selected. According to the results, the 40% of clinical sample (versus the 14% of normative sample) showed at least one personality disorder. The most prevalent one was the Narcissistic (32%), followed by the Antisocial and Passive-Aggressive (16% each one of them). Furthermore,

Double dissociation between representational personal and extrapersonal neglect.

BACKGROUND: Although perceptual and representational neglect are frequently associated, the demonstration of a double dissociation between both neglect forms suggests that both rely on different central mechanisms. In addition, perceptual neglect can be selectively observed within personal space or extrapersonal space. However, it is not known whether the latter dissociation also exists in representational neglect. METHODS: The authors investigated this question in two brain-damaged patients with anatomically different lesions sites, using neuropsychological tests specifically designed to ass

Defense styles and Personality dimensions of research subjects with anxiety and depressive disorders.

We used the Bond Defense Style and Cloninger Tridimensional Personality questionnaires to assess defense styles and personality dimensions in subjects with anxiety and depressive disorders. When measured against a comparison group, maladaptive defense style scores were significantly higher in those with major depression, panic disorder, generalized anxiety disorder, and social phobia, and higher at a trend level in the subjects with obsessive-compulsive disorder and mixed anxiety and depression. However, there were no differences in adaptive defense style scores between the subjects and the co

Psychotic symptoms in the elderly: a prospective study in a population sample.

BACKGROUND: Most of what is known about psychotic symptoms in older persons is based on hospital series of severe disorders. METHOD: In the course of a longitudinal community-based survey of older persons, the presence of psychotic symptoms was assessed on two occasions 3-4 years apart. RESULTS: The point prevalence was 5.7%. Risk factors were cognitive impairment, living alone, being male, having had limited education, social isolation, poor health and depressive symptoms. Of the 65 persons who were symptomatic at the start, only six of those still alive had one or more psychotic symptoms a

[The influence of the type and level of aggression on homicidal behaviour].

A comparative analysis carried out on the type and level of aggressiveness in 105 perpetrators of homicide shows that in those assessed there is a lack of homogeneity in the personality structure, the developmental factors which determine the more stable mechanism of behavior, the course and the level of socialization, and the depth of psychopathological disturbance. Aggression understood as a permanent readiness to react to external stimuli and situations by using open and direct aggression characterizes the perpetrator with a disturbed socialization process. A deep level of hostility of the

Evidence of nonsusceptibility to diethylcarbamazine in Wuchereria bancrofti.

This study assessed Wuchereria bancrofti-infected patients who received diethylcarbamazine (DEC) to determined if drug levels were comparable between individuals who continued to harbor circulating microfilariae and those whose blood became clear of parasites, between those with high and low microfilaria counts, and between infected and noninfected persons. Haitian volunteers undergoing treatment for W. bancrofti and two nonendemic controls were enrolled. DEC levels in serum and urine samples were determined using a gas chromatographic method. No correlation was found between pre- and posttrea

Personality in torticollis: a controlled study.

The Eysenck Personality Questionnaire, the trait scales of the Leyton Obsessional Inventory, the Spielberger State Trait Anxiety Inventory, and a questionnaire assessing marital status and harmony were completed by 100 torticollis patients and a control group of 49 cervical spondylosis sufferers. Information regarding employment status, and events preceding onset of their complaint was also obtained. The two groups did not differ in terms of any of the personality dimensions evaluated, or in their self-reports of events prior to onset of their illness. The groups differed significantly with re

Symptoms of anxiety and depression in informal caregivers of persons with dementia: a latent profile analysis and computer-simulated network analysis.

BACKGROUND: Most informal caregivers of persons with dementia suffer from symptoms of anxiety and depression, but there is still a lack of clear optimal intervention targets for this group. This study aims to explore the interrelationships between symptoms of anxiety and depression among informal caregivers of persons with dementia, identifying key symptoms to provide a scientific basis for developing precise mental health prevention and intervention strategies. METHODS: This study utilized a cross-sectional design. The Generalized Anxiety Disorder-7 and the Patient Health Questionnaire-9 were

The waiting room: Unmet sexual health service needs among men and gender-diverse individuals having sex with men in England, findings from an online, cross-sectional community survey in 2024.

BackgroundSexual health service (SHS) delivery in England shifted substantially with rapid expansion of online services during the COVID-19 pandemic. While digital services may improve reach, limited data exist on unmet need for in-person SHS in England, especially among men and gender-diverse individuals who have sex with men, key populations disproportionately affected by sexual health inequalities.MethodsWe analysed data from "Reducing inequalities in Sexual Health" (RiiSH) 2024 (Nov/Dec 2024), an online survey of UK-resident men and gender-diverse individuals having sex with men. We assess

Personality variability during development predicts alcohol use before and during the COVID-19 pandemic.

Changes in personality during adolescence may shape future alcohol use, especially in times of societal crisis. The COVID-19 pandemic disrupted social norms and stress regulation, making it a natural experiment to examine how early-life personality trajectories predict later substance use. Using longitudinal data from the European IMAGEN cohort (N = 968), we investigated whether intra-individual personality changes during adolescence predicted alcohol use at three timepoints: pre-pandemic, immediately after COVID-19 onset (T1), and several months into the pandemic (T2). Generalized Additive Mi

Trends in epilepsy mortality of three incidence cohorts across 2006-2023 in Sweden: a matched register-based study.

BACKGROUND: Idiopathic epilepsy alone accounts for a large health burden as shown in the Global Burden of Diseases Study. In countries with ageing populations, secondary epilepsies are now even more common. Altered clinical practice and reduced use of older enzyme-inducing drugs may be beneficial, but understanding of trends in the prognosis of all epilepsy is needed. Our objective was to determine and study trends in mortality in persons with epilepsy in Sweden through 2006-2023. METHODS: We performed a matched cohort study by cross-referencing National Patient, Drug, and Cause of Death Regi

The COVID-19 pandemic and its effects on follow-up of patients with early breast cancer: A patient survey.

PURPOSE: Despite limited evidence supporting its effectiveness, most guidelines recommend long-term, routinely scheduled in-person surveillance of patients with early breast cancer (EBC). The COVID-19 pandemic led to increased use of virtual care. This survey evaluated patient perspectives on follow-up care. METHODS: Patients with EBC undergoing surveillance were surveyed about follow-up protocols, perceptions, and interest in clinical trials assessing different follow-up strategies. RESULTS: Of 402 approached patients 270 completed the survey (response rate 67%). Median age 62.5 years (rang

Examining longitudinal associations between interpersonal outcomes and general psychopathology factors across preadolescence using random intercept cross-lagged panel model.

BACKGROUND: Interpersonal outcomes and mental health problems are closely associated. However, their reciprocal influence has not been directly examined while considering the temporal stability of these constructs, as well as shared and unique variance associated with internalising, externalising and attention problems. Using random intercept cross-lagged panel models (RI-CLPM), we tested the hypotheses that negative bidirectional associations at the between-person and negative cross-lagged effects at the within-person level would emerge between interpersonal outcomes (friendship quality and p

Personality traits and physical activity in middle-aged and older adults: a rapid review of evidence and interventions.

BACKGROUND AND OBJECTIVES: Promoting physical activity (PA) is essential for health in middle-aged and older adults, yet global activity levels remain low. While personality is a recognized determinant of PA in the general adult population, personality-PA associations and personality-informed interventions in middle-aged and older adults remain under-explored. This rapid review aimed to synthesize evidence on Five-Factor Model personality traits and PA outcomes in adults aged ≥40 years and to explore trait-tailored interventions for this group. RESEARCH DESIGN AND METHODS: Following PRISMA gu

Supervised In-Person versus Home-Based Exercise in Peripheral Artery Disease: A Pilot Comparative Study.

BACKGROUND: Peripheral artery occlusive disease (PAD) causes intermittent claudication and functional impairment. Exercise is a primary therapeutic strategy, yet comparisons between supervised in-person and remote home-based programs are limited. This pilot study compared both modalities on physical function and quality of life in individuals with PAD. METHODS: Fifteen adults under 65 years with PAD were assigned to an in-person group (n = 8) or a home-based group (n = 7). Both completed an 8-week aerobic and resistance-training program (3 60-min sessions/week). Vascular function (PVR 2CP), b

Personalized Nutrition for Hypertension: Current Evidence and Future Directions.

Hypertension is a leading risk factor for cardiovascular disease, with lifestyle interventions, particularly diet, playing a pivotal role in its management. Recent advancements in personalized nutrition that tailor dietary recommendations based on an individual's unique biology (eg, genetics, microbiome) and lifestyle factors can potentially improve blood pressure control. Indeed, emerging evidence suggests that personalized nutrition can result in more effective and sustained blood pressure reductions than general dietary guidelines. This review explores the current evidence supporting person
