The Erosion of a Pillar of Public Health: ACIP's Role and the Future of US Vaccine Policy Post-June 2025.

Since June 2025, the restructuring of the US Advisory Committee on Immunization Practices (ACIP)-including the abrupt replacement of all voting members and leadership-marked a significant departure from its longstanding norms of continuity and governance. Subsequent deviations from evidence-based policymaking raise concerns about the independence, transparency, and scientific rigor of vaccine policies under the new Department of Health and Human Services administration. The politicization of deliberations, selective use of questionable evidence, and disregard for established methodological sta

Do feeding practices during transfusion influence the risk of developing necrotising enterocolitis in preterm infants?

Our evidence-based review set out to answer the clinical question 'In a preterm infant (patient) with anaemia of prematurity, do feeding practices (intervention) during blood transfusion reduce the risk of developing transfusion-associated necrotising enterocolitis (outcome)'? We found limited evidence that withholding feeding during red blood cell transfusion in preterm infants may reduce the risk of development of transfusion-associated necrotising enterocolitis. As clinical equipoise seemingly exists, it seems reasonable for individual units to make their own decisions regarding whether to

Leveraging Simulation to Support Competency-Based Surgical Training.

CONTEXT: Simulation-enhanced education (SEE) has evolved from a supplementary activity into an evidence-supported cornerstone of medical education, yet in most surgical training programs it remains confined to formative use with little influence on high-stakes progression decisions. This disconnect limits SEE's contribution to patient safety, particularly at transition points where residents assume greater clinical autonomy. ISSUE: High-stakes decisions about readiness for autonomy in surgery continue to rely primarily on workplace-based assessments, which are vulnerable to bias, impression m

Joint ESPGHAN/NASPGHAN Guidelines on Childhood Eosinophilic Gastrointestinal Disorders Beyond Eosinophilic Esophagitis.

INTRODUCTION: Eosinophilic gastrointestinal disorders beyond eosinophilic esophagitis (non-EoE EGIDs) are rare chronic inflammatory disorders of the gastrointestinal (GI) tract. Diagnosis is based on clinical symptoms and histologic findings of eosinophilic inflammation after exclusion of a secondary cause or systemic disease. Currently, no guidelines exist for the evaluation of non-EoE EGIDs. Therefore, the European Society for Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) and the North American Society for Pediatric Gastroenterology, Hepatology and Nutrition (NASPGHAN) form

Five common errors to avoid in clinical practice: the Italian Association of Hospital Gastroenterologists and Endoscopists (AIGO) Choosing Wisely Campaign.

Modern medicine provides almost infinite diagnostic and therapeutic possibilities if compared to the past. As a result, patients undergo a multiplication of tests and therapies, which in turn may trigger further tests, often based on physicians' attitudes or beliefs, which are not always evidence-based. The Italian Association of Hospital Gastroenterologists and Endoscopists (AIGO) adhered to the Choosing Wisely Campaign to promote an informed, evidence-based approach to gastroenterological problems. The aim of this article is to report the five recommendations of the AIGO Choosing Wisely Camp

A personalised online parenting intervention for adolescents at high risk of affective disorders: Results from the UK PIPA randomised controlled trial.

BACKGROUND: Depression and anxiety commonly emerge during adolescence and are associated with substantial personal, social, and economic burden. Parenting practices are well-established modifiable risk and protective factors for adolescent affective disorders, yet many parents lack access to evidence-based guidance. Digital parenting interventions offer a potentially scalable approach, but evidence for their effectiveness in high-risk adolescent populations remains limited. METHODS: The PIPA Trial was a school-based, UK-wide, two-arm randomised controlled trial evaluating a personalised onlin

Screening older adults for alcohol use in orthopaedic care: Healthcare team perspectives, current practices, and barriers.

BACKGROUND: Alcohol use disorder (AUD) is rising among older adults at a concerning rate and is considered an emerging public health problem. Despite this, validated screening is infrequently implemented in orthopaedic settings despite evidence that alcohol use negatively affects fracture risk and postoperative outcomes. Limited knowledge, training, and workflow integration may contribute to inconsistent practices. The purpose of this study is to evaluate orthopaedic healthcare professionals' perceptions of older adult alcohol use, current screening behaviors, familiarity with validated tools,
