Percutaneous removal of caliceal and other "inaccessible" stones: results.

Percutaneous removal of renal stones (percutaneous nephrolithotomy) is becoming an established procedure, especially for stones lying free in the renal pelvis. However, some renal stones, particularly caliceal stones, are less accessible and, therefore, more difficult by the percutaneous route. We removed percutaneously 95 caliceal or otherwise poorly accessible renal stones from 53 patients with a variety of techniques, including percutaneous puncture above the 12th rib, double or Y percutaneous nephrostomy tracts, rigid and flexible endoscopy, and intrarenal cutting with diathermy. Intraveno

[Clinical course and therapy of carotid stenosis in old age].

From 1977 to 1981 266 patients suffering from partial carotid occlusive diseases were operated, 50% of them older than 65 years, the eldest 87 years. The prognosis is the same in both groups, the good results range higher than 80%, mortality was even lower than in younger patients. Local anesthesia is preferred, it allows an optimal observation and earliest detection of operative mistakes and neurological deficits. The incidence of contralateral involvement is 33%. 25% of male patients needed preceding contralateral EIA-operation because of a missed complete thrombosis. Furthermore the importa

[Acute experimental (endogenous) hepatic coma and the clinical syndrome of acute hepatocellular injury].

In search of the adequate experimental model for creating endogenous coma, the characteristics of which would be identical to the clinical picture of acute hepatic coma in men, the ischaemic method of experimentally originated acute endogenous hepatic coma was applied. The experiment was carried out on 12 dogs in which the terminolateral portocaval shunt with ligation of all the collateral of the arterial irrigation leading to the liver was performed, while the double ligation was performed on the hepatic artery. After the animals had emerged from the postoperative state of shock, the hepatic

[Soft framework of the human body].

At the Department of Operative Surgery and Topographic Anatomy, I. M. Sechenov First Medical Institute, Moscow, a group of workers under Academician V. V. Kovanov leadership, have created a complete theory on the soft framework of the human body, investigated morphogenesis of its components, the most important of which--fasciae--form cases for muscles, tegmens for organs and sheaths for blood vessels. Age alterations of the fascial formations stipulate a wide spread of purulent infections in young and old age. Owing to the theory on fascial nodes, which represent a flexible support, pivotal ar

Perineural versus Intravenous Dexmedetomidine for Ultrasound-guided Costoclavicular Brachial Plexus Block: A Randomized Controlled Trial.

BACKGROUND: Dexmedetomidine is a sought-after adjunct to prolong the effects of local anesthetics in regional anesthesia, administered through both perineural and intravenous (IV) routes. However, there is still an ongoing debate to ascertain the preferred route of administration. We conducted this trial because there is a lack of literature about the comparative advantages of these approaches in costoclavicular brachial plexus block (CC-BPB). METHODS: Sixty subjects were randomized to Group A (received 0.5 μg/kg dexmedetomidine plus 20 mL of 0.5% ropivacaine administered perineurally, preced

Comparative Analysis of Patient Perspectives on Surgical Centers: A Study of Carpal Tunnel Releases Performed At An Outpatient Surgery Center Versus Clinical Procedural Suite.

BACKGROUND: This study compares outcomes of carpal tunnel release (CTR) performed in a clinic procedural suite (PS) versus an ambulatory surgery center (ASC). CTR in the PS is performed using the wide-awake local anesthesia no tourniquet (WALANT) technique and does not require NPO compliance. In contrast, CTR in an ASC is performed in an operating room under either WALANT or monitored anesthesia care (MAC), the latter requiring preoperative NPO restrictions. Prior studies demonstrate that WALANT CTR performed in a PS is associated with significantly reduced in-house time, lower healthcare cost

Task-Specific Effects of mGlu2/3 Receptor Agonist LY379268 on MK-801-Induced Behavioral and Neural Dysfunctions in Rats.

NMDA receptor hypofunction can lead to behavioral and cognitive disturbances, including hyperlocomotion, and is considered a core pathophysiological mechanism underlying cognitive and negative symptoms in schizophrenia. This study examined whether treatment with the mGlu2/3 receptor agonist LY379268 (1 and 2 mg/kg) could counteract such disruptions induced by the NMDA antagonist MK-801 (0.1 mg/kg). Rats were tested under two conditions: an aversive learning task (active place avoidance on a rotating arena) and a non-aversive open field test. Additionally, local field potentials were recorded f

Conscious analgesia with oxycodone improves the efficacy and safety in patients at high risk of aspiration during gastroscopy: a randomized double-blind trial.

BACKGROUND: For the patients at high-risk of aspiration, general anesthesia may increase the risk of aspiration, while local pharyngeal anesthesia may cause extreme discomfort and even failure of gastroscopy. In this study, we tested the efficacy and safety of conscious analgesia with oxycodone in these patients. METHODS: In this prospective, randomized, double-blind, parallel-controlled trial, the patients were randomly assigned to either the oxycodone group (group O) or the 0.9% saline group (group C). Before the gastroscope insertion, group O received oxycodone (0.05 mg/kg), while group C r

Submental artery island flap for reconstruction of lateral oropharyngeal carcinoma in patients with severe anesthesiological risk factors: Functional and oncologic outcomes.

The submental arterial island flap (SMIF) is used mostly for closing defects in the oral cavity, but also has its uses for oropharyngeal reconstruction. Since the duration of surgery is shorter compared with that for free flaps, it appears to be a good option for patients with severe anesthesiological risk factors. Therefore, the present study evaluated the functional and oncological outcomes of reconstructing defects after oropharyngeal carcinoma resection with a SMIF in these patients. A total of 10 patients underwent resection of lateral oropharyngeal carcinoma and ipsilateral neck dissecti

Topographic and Morphometric Assessment of Facial Foramina in Maxilla, Mandible, and Zygomatic Bones: Anatomical and Clinical Relevance.

BACKGROUND: The anatomy of the zygomatic bone, maxilla, periorbital, and mental regions is important because important vessels and nerves pass through these regions. The zygomaticofacial nerve can be injured during facial surgeries, especially during zygomatic implant surgeries. Infraorbital and mental nerves are among the nerves blocked during local anesthesia in dentistry. OBJECTIVE: To evaluate the topographic and morphometric characteristics of the zygomaticofacial, infraorbital, and mental foramina, and to determine their anatomical variations and clinical significance in order to provid

Perioperative and postoperative analgesic strategies in live donor hepatectomy: A national survey.

Donor safety is of paramount importance in live donor hepatectomy, and acute pain is the most frequent complaint reported by donors. There are various approaches to managing perioperative and postoperative pain following live donor hepatectomy. These include the administration of opioid and nonopioid analgesics and neuraxial, regional, and local anesthesia. However, there is limited data on the practice patterns of pain management for live donor hepatectomy, particularly when comparing left and right lobe hepatectomies. A national electronic survey was administered to active living donor liver
