Pathogenesis, Diagnosis, and Clinical Management of Aortic Aneurysms: Current Status and Future Perspectives.

Aortic aneurysm (AA) is a life-threatening degenerative vascular disease defined as localized dilation of the aorta exceeding 1.5 times the normal diameter and isprimarily classified into abdominal aortic aneurysm (AAA) and thoracic aortic aneurysm (TAA). With an aging population and increased exposure to risk factors, the incidence of AA is rising, and rupture is associated with a mortality rate of 80-90%. Recent advances in molecular biology, imaging, and interventional techniques have expanded the understanding of AA pathogenesis beyond the traditional "degenerative disease" model to encomp

[SFBC/CNBH Working Group on the evaluation of blood glucose measurement in laboratory: evolution of blood glucose measurement and diagnostic criteria for diabetes].

The evolution of blood glucose assays reflects the constant progress in laboratory medicine toward increasing analytical precision, serving both diagnosis and patient management. While pioneering, early chemical methods had significant limitations in terms of specificity and reproducibility. Their gradual replacement by enzymatic techniques, particularly those based on glucose oxidase and later on hexokinase, represented a major advance, improving the reliability, sensitivity, and standardization of assays. These developments led to the establishment of internationally recognized reference val

Discovery of a potential novel pharmacogenomic biomarker on ANK3 gene for liafensine, a triple reuptake inhibitor for treatment-resistant depression.

Liafensine is a triple reuptake inhibitor targeting transporters for serotonin, norepinephrine, and dopamine for treatment-resistant depression (TRD). It did not exhibit efficacy in non-biomarker-selected TRD patients in two Phase 2b studies. We utilized the blood samples from the patients enrolled in these two studies and extracted genomic DNA to conduct a genome‑wide association study aiming to find a biomarker which can predict liafensine response. A single single-nucleotide polymorphism (SNP), rs12217173, at ANK3 gene was identified as strongly associated with treatment response to liafens

Comorbidity-Driven Inflammation in HFpEF: Immune Profiling and Therapeutic Targets.

PURPOSE OF REVIEW: Chronic low-grade inflammation underlies the heterogenous pathophysiology of heart failure with preserved ejection fraction (HFpEF) influenced by patient comorbidities. This review summarizes circulating inflammatory mediators in HFpEF, explores how comorbidities shape distinct immune signatures, and discusses current and emerging treatment strategies. RECENT FINDINGS: Comorbidities, including hypertension, obesity, type 2 diabetes, and chronic kidney disease, drive distinct immune profiles in HFpEF through dysregulated cytokine signaling. Circulating mediators, including I

Parathyroid Carcinoma: From Molecular Pathogenesis and Diagnostic Biomarkers to Targeted Therapeutics.

Parathyroid carcinoma (PC) is a rare endocrine malignancy characterized by aggressive clinical behavior driven primarily by parathyroid hormone (PTH) overproduction. Standard morphological assessments frequently struggle to definitively distinguish true carcinomas from atypical benign lesions, presenting significant diagnostic challenges and a risk of overdiagnosis. Recent advances emphasize the genetic and epigenetic foundations of PC tumor biology. A central oncogenic mechanism involves the CDC73 gene, where the biallelic inactivation of CDC73 and the gain of function of mutant parafibromin-

Regarding Non-Classical Presentations of Rare Hereditary Hypoparathyroidism: A Case Series of CASR, GNA11, and GATA3 Mutations in Parathyroidology.

This correspondence critically evaluates the diagnostic and therapeutic paradigms presented by Ekaterina Bibik et al. in their analysis of rare hereditary hypoparathyroidism involving CASR, GNA11, and GATA3 mutations. While the case series provides valuable genomic data, the reported decade-long diagnostic delays-particularly the misidentification of mineral disorders as primary epilepsy-unmask a profound systemic failure in clinical vigilance regarding neuroimaging findings like Fahr syndrome. We emphasize that current "conventional" management, characterized by aggressive calcium and vitamin

Cardiolipin remodelling in mitochondrial therapeutics: translational evidence chains from elamipretide to emerging strategies.

Mitochondrial therapeutics have repeatedly fallen short of disease modification, in part because inner-membrane architecture constrains both molecular access and the reversibility of bioenergetic failure. Cardiolipin (CL), the signature phospholipid of the inner mitochondrial membrane, provides a mechanistically cohesive axis that links membrane mechanics to cristae organisation, respiratory-chain supercomplex stability and redox vulnerability. This Review summarises how CL biosynthesis and remodelling shape tissue-specific lipid species and how pathological remodelling can amplify oxidative i

[Expert insights on clinical application of immunotherapy for type 1 diabetes mellitus (2026 edition)].

Type 1 diabetes mellitus (T1DM) is a chronic disease characterized by progressive autoimmune-mediated destruction of pancreatic beta cells. With the establishment of the three-stage classification system for T1DM and the advancement of early screening, disease-modifying therapies (DMT), represented by immunotherapy, are driving a shift in clinical management from "insulin replacement" to "etiological intervention". This guidance statement was jointly developed by adult and pediatric endocrinology experts and is intended for clinical endocrinologists engaged in the diagnosis and management of T

Immunoinflammatory mechanisms and emerging therapies for postherpetic neuralgia.

This review examines the immunoinflammatory mechanisms underlying postherpetic neuralgia (PHN) and emerging mechanism-based therapies. PHN is increasingly recognized as a neuroimmune disorder driven by varicella-zoster virus reactivation, peripheral nerve injury, cytokine-mediated inflammation, oxidative stress, glial activation, and central sensitization. Although conventional treatments, including antidepressants, anticonvulsants, topical agents, opioids, and neuromodulation, provide symptomatic relief, they rarely modify the underlying disease process. A key scientific gap is the lack of co

Creutzfeldt-Jakob Disease in the Era of Diagnostic Triumph and Therapeutic Defeat: A Perspective from the Bedside.

Creutzfeldt-Jakob disease (CJD) exemplifies a cruel paradox of modern neurology: we can now diagnose it with near-perfect accuracy within hours, yet we cannot alter its invariably fatal course. The advent of cerebrospinal fluid (CSF) and skin real-time quaking-induced conversion (RT-QuIC), coupled with high-specificity brain MRI, has enabled robust ante-mortem diagnostic support for CJD, which was previously only definitively established post-mortem. Yet no disease-modifying therapy exists. Drawing on a molecularly supported case of sporadic CJD, this Perspective argues that the widening gap b

The CCT6A paradigm: biomarker potential and therapeutic challenges in oncology and beyond.

Chaperonin Containing TCP1 Subunit 6A (CCT6A), a structurally conserved core subunit of the eukaryotic CCT/TRiC complex, orchestrates ATP-dependent protein folding, cytoskeletal dynamics, and stress-responsive adaptation, reflecting its indispensable role in cellular homeostasis. Emerging evidence highlights CCT6A as a multifaceted oncogenic driver across diverse malignancies (such as colorectal cancer and hepatocellular carcinoma), through metabolic reprogramming, stabilization of oncoproteins, and immunosuppression. Its overexpression correlates with aggressive phenotypes, therapeutic resist

ncRNAs as Key Regulators in Gastric Cancer: From Molecular Subtyping to Therapeutic Targets.

Gastric cancer (GC) poses a major global health challenge, underscoring the need for advanced diagnostic and therapeutic approaches. Non-coding RNAs (ncRNAs), including microRNAs (miRNAs), long non-coding RNAs (lncRNAs), and circular RNAs (circRNAs), have emerged as pivotal regulators in GC, with their dysregulated expression driving key processes such as tumorigenesis, metastasis, immune evasion, and chemoresistance. The functional diversity of ncRNAs across different GC molecular subtypes highlights their potential as biomarkers for improved subtype classification and patient stratification.

Deconvoluting clonal and cellular architecture in IDH-mutant acute myeloid leukemia.

Isocitrate dehydrogenase 1/2 (IDH) mutations are early initiating events in acute myeloid leukemia (AML). The complex clonal architecture and cellular heterogeneity in IDH-mutant AML underlies the heterogeneous clinical presentation and outcomes. Integrating single-cell genotyping and transcriptomics, we demonstrate a stem-like and inflammatory phenotype of IDH-mutant AML and identify clone-specific programs associated with NPM1, NRAS, and SRSF2 co-mutations. Furthermore, these clones had distinct responses to treatment with combination IDH inhibitors and chemotherapy, including elimination, r

When Brain Models Aren't Universal: Benchmarking of Ethnic Bias in MRI-Based Cognitive Prediction Across Modalities.

Predictive neuroimaging models promise precision medicine but risk exacerbating health inequities if they perform unevenly across ethnic/racial groups. Using the Adolescent Brain Cognitive Development data, we benchmarked ethnic/racial bias in models predicting cognitive functioning from 81 MRI phenotypes across four training strategies. Models trained on one ethnicity performed best within that group. Models trained on participants sampled without regard to ethnicity, a common practice, performed better on White participants, likely because the ABCD sample was predominantly White. Training on

Sex-specific Associations of Gene Expression with Alzheimer's Disease Neuropathology and Ante-mortem Cognitive Performance.

The biological mechanisms underlying women's increased Alzheimer's disease (AD) prevalence remain undefined. Previous case/control studies have identified sex-biased molecular pathways, but sex-specific relationships between gene expression and AD endophenotypes, particularly sex chromosomes, are underexplored. With bulk transcriptomic data across 3 brain regions from 767 decedents, we investigated sex-specific associations between gene expression and post-mortem β-amyloid and tau as well as antemortem longitudinal cognition. Of 23,118 significant gene associations, 10% were significant in one

Navigating chronic pancreatitis pain: a pathophysiological and therapeutic overview.

Pain management in chronic pancreatitis (CP) patients remains a major challenge, largely due to complex and refractory pain. Such pain detrimentally impacts patients by reducing quality of life, limiting daily activities, increasing psychological distress, necessitating frequent hospitalizations, and contributing to opioid dependence and socioeconomic burden. This review delineates the multifaceted nature of CP-related pain, highlighting the roles of neurogenic inflammation, maladaptive neuroplasticity, and disrupted pain modulation pathways. Current management strategies are multidisciplinary
