Supraspinatus tears: propagation and strain alteration.

It was hypothesized that there would be an alteration in strain when macroscopically normal supraspinatus tendons were subjected to three patterns of surgically created tear. The propagation of joint-side partial-thickness tears was also examined. Cadaveric shoulders were tested on a purpose-built rig with static loading from 20 to 200 N and during glenohumeral abduction from 0 degrees to 120 degrees with a 100-N tensile load. Differential variable reluctance transducers were used to calculate strain. Six-millimeter-wide midsubstance full-thickness tears (n = 2) caused an increase in bursal-si

Kinematic effects of two therapeutic interventions for glenohumeral joint instability: A preliminary feasibility trial.

PURPOSE: Multidirectional instability (MDI) of the glenohumeral joint involves excessive humeral translations with symptoms such as pain or apprehension. As MDI is typically managed through therapeutic exercise, this feasibility study examined the practicality of comparing targeted stabilization and general-strengthening interventions on glenohumeral kinematics and patient-reported outcomes. SCOPE: Twenty competitive swimmers with MDI were randomized to a targeted rehabilitation group (MDI-EX) or a general shoulder exercise group (GSE). Participants completed 16 exercise sessions over eight w

Editorial Commentary: Biomechanical Data and Early Clinical Results Demonstrate an Emerging Strategy for Combining Superior Capsule Reconstruction and Lower Trapezius Transfer in the Management of Massive Irreparable Posterosuperior Rotator Cuff Tear.

Numerous biomechanical studies demonstrate that superior capsule reconstruction (SCR) improves glenohumeral superior translation and subacromial contact pressures in cadaveric models of massive irreparable rotator cuff tears (MIRCT) at time zero. These findings provide a logical basis for clinical observations of significant improvements in pain, range of motion, and patient-reported outcome measures at short-term follow-up. However, the efficacy of SCR at longer-term follow up is controversial, and a recent survey of AANA members demonstrates that there is decreasing use of SCR for MIRCTs as

Comparison of glenohumeral joint kinematics between swimmers clinically classified with multidirectional instability and asymptomatic controls.

The clinical classification of glenohumeral joint instability is characterized by presumed increased humeral translations in conjunction with symptoms of instability. Prior research reports inconsistent kinematic differences in glenohumeral kinematics between individuals clinically classified with multidirectional instability and asymptomatic controls. Differing clinical classifications and motion tracking methods likely contribute to this gap. This analysis aimed to compare three-dimensional (3D) glenohumeral joint kinematics during active arm raising between individuals clinically classified

Subscapularis atrophy and function after arthroscopic Trillat procedure.

INTRODUCTION: Several studies have reported a strength deficit in internal rotation (IR) following a Latarjet procedure, which can persist for months or even years. The arthroscopic Trillat procedure does not require splitting the subscapularis muscle, potentially making it less damaging. HYPOTHESIS: The arthroscopic Trillat procedure does not cause any atrophy or strength deficit in the subscapularis muscle. METHODS: This was a single center, retrospective study of patients treated between 2013 and 2021. Included were patients who had chronic anterior shoulder instability with an indication

Comparison of two coracoid process transfer techniques on stress shielding using three-dimensional finite-element model.

BACKGROUND: We created patient-based 3D finite-element (FE) models that simulate the congruent-arc Latarjet (CAL) and traditional Latarjet (TL) procedures and then compared their stress distribution patterns with different arm positions and glenoid defects. METHODS: The computed tomography data of 10 adult patients (9 men and 1 woman, ages: 18-50 years) were used to develop the 3D FE glenohumeral joint models. Twenty-five and 35% bony defects were created on the anterior glenoid rim, and the coracoid process was transferred flush with the glenoid by the traditional and congruent-arc technique

Machine Learning and Statistical Prediction of Pitching Arm Kinetics.

BACKGROUND: Over the past decade, research has attempted to elucidate the cause of throwing-related injuries in the baseball athlete. However, when considering the entire kinetic chain, full body mechanics, and pitching cycle sequencing, there are hundreds of variables that could influence throwing arm health, and there is a lack of quality investigations evaluating the relationship and influence of multiple variables on arm stress. PURPOSE: To identify which variables have the most influence on elbow valgus torque and shoulder distraction force using a statistical model and a machine learnin

Biomechanical evaluation of suture buttons versus cortical screws in the Latarjet-Bristow procedure: a fresh-frozen cadavers study.

INTRODUCTION: A commonly used method of fixation of the transferred coracoid in the traditional Latarjet-Bristow procedure (open or arthroscopic) is by two bicortical screws. Although mechanically effective, screw fixation is also a major source of hardware and neurologic complications. This study aimed to compare the biomechanical performances of traditional metal screws and endobuttons as fixators of the Latarjet-Bristow procedure. MATERIALS AND METHODS: Nine fresh-frozen cadaveric human scapulae with the conjoined tendon attached to the coracoid process were used for the Latarjet-Bristow p

Treatment of Cervical Spondylotic Amyotrophy With Nerve Transfers.

Cervical spondylotic amyotrophy is characterized by severe, proximal upper extremity weakness including an inability to abduct the shoulder and flex the elbow. Treatment using both medical and surgical decompression approaches has produced variable results. This paper reports the use of nerve transfers (spinal accessory to suprascapular, flexor carpi ulnaris fascicle of ulnar to biceps motor branch, radial nerve branch to triceps to axillary) to restore shoulder and elbow function in a case of unilateral cervical spondylotic amyotrophy involving C5 and C6 myotomes. Evidence of regeneration was

One-stage surgical treatment for concomitant rotator cuff tears with shoulder stiffness has comparable results with isolated rotator cuff tears: a systematic review.

BACKGROUND: Addressing preoperative shoulder stiffness before rotator cuff repair (RCR) is advocated, but the effectiveness of this approach is debatable. We hypothesized that 1-stage treatment of concomitant rotator cuff tear (RCT) with shoulder stiffness has comparable results with isolated RCT. METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, the databases including MEDLINE, Embase, Cochrane Library, and Scopus were searched using the keywords of "shoulder stiffness" OR "adhesive capsulitis" OR "frozen shoulder" AND "rotator cuf

Effect of humeral stem design on humeral position and range of motion in reverse shoulder arthroplasty.

PURPOSE: The impacts of humeral offset and stem design after reverse shoulder arthroplasty (RSA) have not been well-studied, particularly with regard to newer stems which have a lower humeral inclination. The purpose of this study was to analyze the effect of different humeral stem designs on range of motion and humeral position following RSA. METHODS: Using a three-dimensional computer model of RSA, a traditional inlay Grammont stem was compared to a short curved onlay stem with different inclinations (155°, 145°, 135°) and offset (lateralised vs medialised). Humeral offset, the acromiohumer

Validation of the pectoralis minor length test: A novel approach.

INTRODUCTION: Pectoralis minor (PM) shortness is believed to promote faulty shoulder mechanics including reduced scapular posterior tilt. A pectoralis minor length (PML) test that measures the acromion-table distance with and without manual pressure on the coracoid process is supposed to examine the passive mechanical properties of the PM. A threshold for "shortening" has been set at 2.6 cm, but data regarding its validity are lacking. We hypothesized that, under conditions of good reliability, an evaluation of the effect of PM tenotomy, could adequately investigate the construct validity of t

Simultaneous Postural Adjustments (SPA) scrutinized using the Lissajous method.

The goal of this research was to study the postural adjustments that occur during the course of a voluntary movement (Simultaneous Postural Adjustments: SPA). A pointing task performed at maximal velocity was considered and upper limb kinematics and body kinetics were recorded. A 2-DOF model was elaborated that distinguishes between the body segments that are mobilized in order to perform the pointing movement. These segments are the right upper limb (termed the “focal” component) and the rest of the body (termed the “postural” component). This model allowed for the calculation of both sub-sys

Anterior glenohumeral laxity is associated with posterior shoulder tightness among professional baseball pitchers.

BACKGROUND: Baseball pitchers often have increased anterior glenohumeral (GH) laxity and posterior shoulder tightness due to large forces produced during the throwing motion. As such, many clinicians have hypothesized that a relationship exists between these common characteristics. HYPOTHESIS: A negative relationship will exist between anterior GH laxity and the summation of GH adduction range of motion (ROM) and GH internal rotation ROM asymmetry. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Anterior GH laxity, GH rotation ROM, and GH horizontal adduction ROM were me

The rationale for an arthroscopic approach to shoulder stabilization.

The gold standard of treatment for glenohumeral instability has traditionally been viewed as open shoulder stabilization. With the increased awareness of complex instability patterns and the ability to preoperatively detect concomitant pathology with advanced imaging modalities, an evidence-based shift to an all-arthroscopic approach to shoulder stabilization surgery is occurring. Current data suggest that patients who meet eligibility criteria for arthroscopic stabilization (those without significant bony lesions or significant deformity) can expect equivalent rates of recurrence, better func

[Medium-term results of arthroscopic repair of relapsing anteroinferior glenohumeral instability].

OBJECTIVES: Arthroscopic repair of Bankart lesion has become the treatment of choice of anterior shoulder instability. Our objective is to analyze the medium-term results of arthroscopic Bankart repair. MATERIAL AND METHODS: Between January 1999 and November 2007, 86 shoulders of 85 patients with diagnosis of relapsing shoulder dislocation were treated arthroscopically. After a mean 62-month follow-up (minimum 24 months) the results obtained were retrospectively assessed according the Rowe and Constant functional scales. A statistical analysis was done of the relation between functional resul
