Rib-muscle pedicle flap for the repair of congenital tracheal stenosis.

Congenital tracheal stenosis (CTS) is an uncommon congenital anomaly that presents early in life with symptoms of biphasic stridor. Most cases require surgical correction. Techniques have included dilation, resection of the involved segment, and tracheoplasty. Today pericardium and costal cartilage are the most frequently used materials for tracheoplasty, but patients still often encounter problems with the graft, with the procedure, or with late complications. This preliminary study was undertaken to determine the feasibility of a rib-intercostal muscle pedicle flap for the treatment of CTS.

Pharyngeal bot flies (Oestridae) from sympatric wild cervids in southern Spain.

The prevalence of 2 species of pharyngeal bot flies parasitizing sympatric wild ungulates from southern Spain is given. Pharyngomyia picta was found in all red deer and fallow deer whereas Cephenemyia auribarbis was found in 59% of fallow deer and 81% of red deer examined. The mean number of P. picta larvae was 3-6 times more than that of C. auribarbis larvae. These data suggest that red deer are more heavily parasitized than fallow deer and that they are predominantly infested with P. picta larvae.

Consecutive 17 tracheobronchial injury caused by blunt chest trauma were reviewed. 14 patients were injured by traffic accidents, 2 by fall from the high, and one by accident during play in the house. 16 were male and one was female. Patient's age range from 4 to 60 years (average 25). Site of tracheobronchial injuries were scattered and there were not found risky area. Several problem to rescue tracheobronchial injuries are discussed. To maintain the ventilation in the patient of carinal injury, it is supposed that jet ventilation may be a possible method. For the infant victims, it is diffic

Laryngo-tracheo-oesophageal cleft: a plea for early diagnosis.

Congenital laryngeal clefts are rare. This paper reports on the management of 3 patients with Type 2 laryngo-tracheo-oesophageal cleft. One patient died following tracheopexy after successful cleft closure. Prognosis is related to the presence of other major anomalies, the early diagnosis of the lesion and institution of appropriate respiratory and nutritional care prior to correction of the defect. A greater awareness of the condition combined with aggressive diagnostic endoscopy should result in early diagnosis and improved survival.

[Experimental study of patch reconstruction of tracheal defects with Marlex mesh].

Animal experiments of the patch reconstruction of tracheal defects using Marlex mesh were performed on 41 mongrel and beagle dogs. They were consisted of four groups. In group I, cervical tracheal defects were reconstructed with simple Marlex mesh patches. In group II, cervical tracheal defects were reconstructed with Marlex mesh patches which were reinforced by three polyacetal-resin half rings. In group III, thoracic tracheal defects were reconstructed with Marlex mesh patches which were reinforced by half rings, and were covered with omental pedicle flaps. In group IV, a pedicled rectus abd

Bronchoalveolar lavage and tracheal wash to determine lung involvement in a cat with cryptococcosis.

A one-year-old 3-kg female domestic shorthair cat was diagnosed as having cryptococcosis. Although the cat had no clinical signs referable to the pulmonary system and results of thoracic radiography were normal, organisms were recovered by use of tracheal wash and bronchoalveolar lavage. In cases of fungal infections, the latter 2 methods can be more reliable than radiography and evaluation of clinical signs for determining pulmonary involvement.

[A case of thyroid carcinoma required the incision of the thyroid cartilage for inserting the silicone T tube after extensive tracheal resection and reconstruction].

A 57-year-old female with thyroid carcinoma, who had developed tracheal stenosis, underwent extensive tracheal resection and reconstruction. After the tracheal sleeve resection 5.2 cm in length, primary tracheal reconstruction was performed. Although complication did not occur at the anastomotic site, the patient had dyspnea due to cord dysfunction by bilateral recurrent nerve paralysis. After 20 days transnasal intubation we reoperated to perform a tracheostomy under neck incision. But the reconstructed trachea was too short to pull out from the mediastinum. In order to insert the silicone T

Application of pulsed and continuous wave 1.32 and 1.06 microns wavelengths of the Nd:YAG laser in the canine tracheobronchial tree: a comparative study.

Previous investigations have shown good clinical potential for the use of the 1.32 microns wavelength Nd:YAG laser because its soft tissue absorption is better than that of the 1.06 microns wavelength Nd:YAG laser. The 1.32 microns wavelength Nd:YAG laser has an absorption coefficient in water that is 10 times higher than the 1.06 microns wavelength Nd:YAG laser. A comparative in vivo study of laser soft tissue effects was performed by using the 1.32 microns wavelength and the 1.06 microns wavelength Nd:YAG lasers in a pulsed wave (PW) mode and continuous wave (CW) mode using a non-contact end

Pulmonary vascular sling: report of seven cases and review of the literature.

A total of 130 cases of pulmonary vascular sling, including seven new cases in our collection, were studied. The sex distribution was 60% male and 40% female. The age at presentation was in the first year of life in approximately 90% of cases. Barium-esophagraphy showing anterior indentation was diagnostic in most of the cases. Bronchoscopy and tracheobronchography were useful in detecting associated tracheobronchial anomalies preoperatively. The analysis of 68 autopsied cases revealed associated tracheobronchial anomalies in 40% of the cases. Anomalies of the tracheobronchial tree took three

Tracheoesophageal injury secondary to disc battery ingestion.

With electronic miniaturization, disc batteries have become increasingly popular; hence, they are ingested more frequently by children. Their small size and benign appearance belie their caustic nature. This case report illustrates the principles of management, and is the eighth report of major complications from disc battery impaction in the esophagus. We believe our patient is the youngest child reported, to date, to have suffered this injury. The pathophysiology and surgical management are discussed, and a protocol for the management of suspected disc battery ingestion is presented.

Urgent thoracotomy for pulmonary or tracheobronchial injury.

Three hundred eighty-eight of 7,283 (5.3%) admitted trauma patients underwent urgent thoracotomy. In 61 patients (15.7%), pulmonary or tracheobronchial injury prompted thoracotomy (11, blunt; 50, penetrating). Pulmonary hemorrhage necessitated thoracotomy in 54 patients (88.5%); tracheobronchial injury in five patients (8.2%). The mortality was 27.9%. Nine patients (14.8%) underwent pneumonectomy: eight died of intractable hemorrhagic shock during thoracotomy despite rapid control of pulmonary hemorrhage: one died of sepsis. Eleven patients (18.0%) underwent lobectomy: six (54.5%) died of conc

Plasma atropine concentrations via intravenous, endotracheal, and intraosseous administration.

To date, there have been limited studies on the pharmacokinetics of intravenous atropine and no pharmacokinetic studies on the endotracheal or intraosseous administration of atropine. This study examines the time to peak plasma concentration of atropine following intravenous, endotracheal, and intraosseous administration in anesthetized monkeys using a triple crossover design. Plasma atropine was assayed by a radioreceptor method. The time to peak plasma concentration of atropine was shortest with intravenous administration; and longest with endotracheal administration. The mean plasma concent

Acute airway compromise due to tracheal mucocele.

Two cases of patients with acute airway compromise due to benign tracheal mucocele are presented. In each, the diagnosis was made by flexible fiberoptic bronchoscopy. Each patient developed the acute episode at 12 months of age following an upper airway infection. The mucoceles were "unroofed" and drained through rigid bronchoscopes. Both patients had been intubated following premature birth; trauma to the tracheal mucosa is proposed as the etiologic factor in the development of these mucoceles.

Hyporesponsiveness to aerosolized but not to infused methacholine in cigarette-smoking dogs.

Seven beagles were exposed via a tracheostomy to smoke from 10 cigarettes/day, 5 days/wk for 10 months; 2 other dogs were exposed for 6 months only. Four dogs served as sham-exposed control animals. Mucus was collected 2 times/wk prior to and during the exposure period by resting a cytology brush on the lower trachea for 2 to 5 min. At least once prior to and as often as 3 times during exposure, transpulmonary pressure and flow were monitored under anesthesia to determine resistance RL. Two airway responses to methacholine were determined: the infusion response delta Ri, the increment in RL 4

Congenital stenosis of the esophagus due to tracheobronchial remnants: a missed diagnosis.

Lower esophageal strictures in infants in more than 90% of cases are related to reflux esophagitis, while the remainder are congenital or induced by injuries such as penetrating trauma or corrosion by caustic agents. Among congenital disorders cartilaginous remnants in the esophagus are very rare, but it is a possibility that should always be considered, especially if signs of esophagitis are not seen on esophagoscopy. The first aim, when evaluating the vomiting or regurgitating infant, is to establish whether this is due to an organic disorder. When stricture of the esophagus becomes evident

The vascular pedicle and the vena azygos. Part III: In trauma--the "vanishing" azygos.

The chest radiographs of 158 patients with widened vascular pedicles were examined. The vascular pedicle was measured and the presence or absence of the right paratracheal stripe and the vena azygos on the radiographs was noted. Increasing the systemic blood volume (or assuming the supine position) causes the vascular pedicle of the heart to widen predominantly to the right, and the vena azygos to enlarge simultaneously. In contrast, extravascular bleeding from aortic damage causes the pedicle to widen predominantly to the left of the midline; at the same time the paratracheal stripe and azygo

Repair of postintubational lesions of the cartilaginous nose in infants--sometimes a surgical problem.

Among the complications of prolonged nasotracheal intubation in infants the possibility of pressure necrosis of the cartilaginous nose due to inadequate fixation or improper size of the tube is rarely mentioned. Three cases with postintubational alar defects and another child with a severe stenosis of the nasal entrance are reported. In the latter case a multi-staged reconstructive rhinoplasty had to be done in order to restore nasal breathing. While describing our surgical procedures the multiple difficulties and hazards of reconstructive rhinoplasty in early childhood are discussed.

Serial dilation compared to elective laryngo-tracheoplasty in the treatment of acquired subglottic stenosis in children.

Acquired incomplete subglottic stenosis in children has historically been managed by serial bouginage of the strictured region sometimes accompanied by injection or systemic administration of corticosteroid medications. The forceful stretching of the scar whilst awaiting growth of the region to compensate for scar contracture, however, dooms that child to a prolonged period of waiting for an unpredictable result with a tracheotomy in place. Recently, laryngo-tracheoplasty has been undertaken in younger age groups and in a review of 30 children, with very similar types of stenosis, it was noted
