An Anti-PM/Scl-75 Antibody-positive Japanese Woman Who Developed Inflammatory Myopathy.

A 69-year-old Japanese woman presented with mild muscle weakness of the neck and symmetrical proximal parts of the upper and lower limbs. Laboratory tests, needle electromyography, and a muscle biopsy revealed inflammatory myopathy with an apparent clinical classification of polymyositis and positive findings for anti-PM/Scl-75 antibody. This antibody is rare among Japanese populations, and most Japanese patients with the antibody are not classified with the inflammatory myopathy seen in polymyositis. The muscle biopsy also showed marked necrotic and regenerative fibers. We need to collectivel

Pro-angiogenic capacities of microvesicles produced by skin wound myofibroblasts.

Wound healing is a very highly organized process where numerous cell types are tightly regulated to restore injured tissue. Myofibroblasts are cells that produce new extracellular matrix and contract wound edges. We previously reported that the human myofibroblasts isolated from normal wound (WMyos) produced microvesicles (MVs) in the presence of the serum. In this study, MVs were further characterized using a proteomic strategy and potential functions of the MVs were determined. MV proteins isolated from six WMyo populations were separated using two-dimensional differential gel electrophoresi

Rewiring autophagy to target cutaneous wound healing: investigation of molecular mechanisms to therapeutic interventions.

Rewiring of pathways regulating inflammation, metabolism, and regeneration must occur in a coordinated fashion during wound healing; however, this orchestration gets disrupted in chronic and diabetic wounds. Autophagy is a conserved lysosome-dependent pathway that controls the integration of nutrient homeostasis, adaptation to stresses, immune regulation, and cell remodelling during wound repair. Here we summarize the pivotal role of autophagy in the different crucial steps of wound healing. An open wound often attracts pathogens, leading to infection and inflammation. During this inflammation

A plug-and-play multi-organ-on-chip for studying interaction between central nervous system and peripheral monocytes following neuroinjury from radiation and Parkinson's disease.

Following neuroinjury (e.g., from radiation or Parkinson's disease), peripheral monocytes infiltrate the CNS, promoting neuroinflammation and cognitive decline. However, studying this is challenging due to a lack of suitable in vitro models. Multi-organ-on-a-chips (MOCs) address this gap with interactable in vitro models. We developed a plug-and-play multi-organ-chip (PPMOC) to mimic CNS-monocyte interactions. The PPMOC features two chambers, each with an insert for CNS or monocyte models, interconnected by a channel. The cross-cultural and chronological evolution of the kidney-bone relationship: from ancient medical insights to modern molecular endocrinology.

The kidney-bone axis constitutes a conserved bidirectional endocrine network that orchestrates skeletal remodeling and systemic mineral homeostasis. Its dysregulation underlies chronic kidney disease-mineral and bone disorder (CKD-MBD), renal osteodystrophy, secondary hyperparathyroidism, and associated vascular calcification, markedly elevating fracture and cardiovascular mortality risks. This review offers a unique cross-cultural and chronological synthesis of the kidney-bone relationship, bridging ancient medical insights with modern molecular endocrinology. Horizontally, we systematically

Engineering extracellular vesicles for multi-cargo co-delivery in silicosis and related fibrotic diseases: design strategies and translational prospects.

Silicosis remains therapeutically intractable because its three driver axes-the NLRP3 inflammasome, ROS/Nrf2 oxidative stress, and TGF-β/Smad fibrosis-interlock into a self-reinforcing network whose topological redundancy reroutes signalling around any single-target drug, the structural basis for repeated single-agent failure. Existing nanocarriers cannot meet the resulting demand for multi-cargo delivery: lipid nanoparticles risk triggering NLRP3, polymeric carriers escape endosomes too poorly to deliver nucleic acids, and inorganic particles intensify the ROS-driven fibrotic loop. Engineered

Serum Extracellular Vesicles Double-positive for CD9 and CD147 or for CD9 and CD63 Decrease After Surgical Resection in Patients With Colorectal Cancer: Potential Biomarkers of Residual Tumor.

BACKGROUND/AIM: Extracellular vesicles (EVs), including exosomes, are abundant in body fluids, and EV surface proteins can be profiled as potential minimally invasive biomarkers. CD147 (EMMPRIN/Basigin) is a tumor-associated surface glycoprotein implicated in cancer progression. CD9 and CD63 are tetraspanin membrane proteins widely used as EV markers. The present study aimed to evaluate the clinical significance of serum EV subsets double-positive for CD9 and CD63, or for CD9 and CD147 in patients with colorectal cancer (CRC) before and after tumor resection. MATERIALS AND METHODS: Sixteen pa

Sequential- vs . density gradient- centrifugation for the isolation of mitochondria-containing extracellular vesicles.

A subset of extracellular vehicles ( EVs ) with particle diameters >200 nm, large vesicles ( lEVs ) contain mitochondria that increase recipient cell bioenergetics. To date, sequential centrifugation ( SC ) is the most reported protocol to separate lEVs from the smaller EVs (<200 nm)/exosomes. We have previously demonstrated that lEVs derived from brain endothelial cells ( BECs ) using the standard SC method transferred their innate mitochondria to recipient BECs, increased recipient BEC bioenergetics, reduced brain infarct volume, and improved behavioral outcomes in a mouse model of transient

Targeted drug delivery systems for pancreatic cancer therapy: advances, challenges, and future perspectives.

The review summarizes recent advances, challenges, and future perspectives in targeted drug delivery systems (DDSs) for pancreatic cancer (PC) therapy. Given the dismal prognosis of PC treatment is hindered by a dense desmoplastic stroma, profound hypoxia, limited T-cell infiltration, and abundant immunosuppressive myeloid populations, together forming physical and immunological barriers to effective therapy. Targeted DDSs based on organic, inorganic, and biological platforms (e.g., liposomes/lipid nanoparticles, polymeric nanoparticles, carrier-free drug self-assembly systems, hybrid inorgani

Convergence of multiplexed immunosensors, nanotechnology, and AI for early pancreatic cancer diagnosis.

Pancreatic cancer (PC) remains one of the deadliest malignancies, largely because of its asymptomatic onset, rapid progression, and absence of sensitive, specific tools for early detection at the population level. Conventional biomarkers such as carbohydrate antigen 19-9 (CA19-9) and standard imaging modalities offer limited performance in identifying pre-malignant or early-stage disease, underscoring the urgent need for next-generation diagnostic strategies. This narrative review examines the emerging convergence of multiplexed immunosensors, nanotechnology-enabled signal amplification, and a

Advancements in CRISPR/Cas Technologies for Sensitive Cancer Detection: Mechanisms, Platforms, and Clinical Translation Roadmap.

Early cancer detection is critical for improving patient outcomes, yet current diagnostic approaches often fail to identify malignancies when tumor-specific biomarkers are relatively scarce. Emerging CRISPR/Cas-based technologies have revolutionized the ultra-sensitive detection of cancer biomarkers in liquid biopsies, overcoming the inherent limitations of traditional diagnostic approaches such as tissue biopsies and imaging, which frequently fail to detect early-stage malignancies with sufficient sensitivity. This review explores recent advances in CRISPR/Cas diagnostics (CRISPR/Cas-Dx) that

Harnessing Chlorella vulgaris-derived extracellular vesicles for potentiated cancer immunotherapy.

Plant-derived extracellular vesicles (EVs) have emerged as promising drug delivery vehicle due to their inherent biocompatibility, high stability and large-scale production. Chlorella vulgaris (C. vulgaris) has been increasing application in the food industry and biomedical fields. Nevertheless, the biogenesis and immunomodulatory functions of EVs from Chlorella species remain poorly understood. Herein, we found that the EVs in C. vulgaris originated from "vacuolar structures", a process analogous to the biogenesis of exosomes which originates from endosomes in mammalian cells. Therefore, we f

Investigation of sterile hydrogels as topical vehicles for APOSECTM, a stressed peripheral blood mononuclear cell secretome for the treatment of poorly healing wounds.

APOSECTM, a complex mixture of secreted proteins, lipids, and exosomes from stressed peripheral blood monocytes, is currently in clinical trials for the treatment of chronic wounds. When applied to open wounds, 1 mL reconstituted APOSECTM lyophilisate is syringe-mixed with 3 g sterile hydrogel prior to administration. This study investigates the pharmaceutical performance of this novel administration system. A gel formulation (APOgel) was developed for terminal sterilisation in pre-filled syringes with post-sterilisation viscosity (∼325-350  Pa·s at 1  s-1) comparable to a commercial benchmark

Extracellular Vesicle-Mediated Macrophage Polarization in Sepsis-Induced Acute Lung Injury: Molecular Mechanisms and Therapeutic Opportunities.

Sepsis-induced acute lung injury (SI-ALI) is a severe complication of sepsis characterized by dysregulated inflammatory responses and impaired immune homeostasis. Growing evidence indicates that extracellular vesicles (EVs), particularly exosomes, are important mediators of intercellular communication. Despite the heterogeneity of infectious sources underlying sepsis, EVs can regulate macrophage polarization and functional reprogramming by transferring diverse bioactive cargo. Consequently, EVs are involved in the pathophysiological progression of SI-ALI arising from sepsis of different etiolo

Bionic intelligent responsive barbed microneedles synergistically remodel the microenvironment for the treatment of refractory pigmentation disorders (melasma).

Clinically refractory melasma remains difficult to treat because it is driven by a multidimensional pathological microenvironment that includes melanocyte hyperactivity, aberrant vascular growth, oxidative stress and chronic inflammation. Single-pathway therapies therefore often produce incomplete or transient responses. To remodel this microenvironment more broadly, we developed a bioinspired, stimulus-responsive co-delivery microneedle platform (TXA/Exo@HAMA-PBA MNs). Inspired by the backward-curved barbs on the tick hypostome, the microneedles were engineered with outward-facing barbs to im

Nanomedicine targeting neuroinflammatory pathways in Alzheimer's disease: a new frontier in inflammopharmacology.

Alzheimer's disease (AD) is a multifactorial neurodegenerative illness characterized by progressive cognitive impairment, synaptic compromise, and relentless neuroinflammation. Increasing evidence suggests that neuroinflammatory cascades orchestrated by microglial activation, astrocytic malfunction, cytokine hyperproduction, and inflammasome signalling are at the core of AD pathogenesis. Conventional anti-amyloid and cholinergic treatments are only symptomatic and neglect the inherent neuroimmune dysregulation. Nanomedicine is a revolutionary frontier in inflammopharmacology, which enables the

Tongue cancer microbial biomarkers: landscape of saliva, cancer tissue, and para-carcinoma tissue.

BACKGROUND: Oral cancer poses a significant global health challenge due to delayed intervention and a lack of effective early screening methods. About two-thirds of patients are diagnosed with advanced-stage at initial consultation. Tumor-associated bacteria are implicated in tissue carcinogenesis and have potential as diagnostic biomarkers. METHODS: In this pilot study, 24 patients with tongue squamous cell carcinoma were enrolled. Saliva samples were collected from all participants before treatment. Cancer and para-carcinoma tissues were surgically obtained from 20 patients. Bacterial exoso

Pathological mechanisms of hepatic ischemia-reperfusion injury and stem cell-based therapeutic strategies: Mechanistic insights and translational perspectives.

Hepatic ischemia-reperfusion injury (HIRI) remains a major clinical challenge in liver surgery, transplantation, and trauma, contributing to graft dysfunction and increased morbidity. This review summarizes the complex pathophysiology of HIRI, including metabolic derangements during ischemia, bursts of oxidative stress on reperfusion, and sterile inflammation driven by reactive oxygen species (ROS) and immune-cell activation. It also examines regulated cell death pathways-apoptosis, pyroptosis, necroptosis, and ferroptosis-that interact to create self-reinforcing cycles and further amplify tis
