Biophysical testing in premature rupture of the membranes.

The management of pregnancies complicated by premature rupture of the membranes (PROM) should include some type of surveillance for the development of infectious complications. This is a necessity because neonatal sepsis is not heralded by maternal chorioamnionitis in the majority of cases. Fetal biophysical assessment holds promise as a method to assess fetal well-being and predict the development of infectious complications. The frequent use of fetal biophysical profiles in patients with PROM helps to distinguish healthy fetuses from those infected or at high risk of infection. This distinct

Spontaneous Twin Pregnancy: A Challenging and Exceptional Scenario in a Patient on Maintenance Hemodialysis in Sub-Saharan Africa.

Chronic hemodialysis is associated with reduced fertility. Hence, pregnancy remains rare, challenging, and deleterious when unplanned, especially in low-resource countries. Contraception and births are very important in these settings. Though the main modes of contraception have been proposed in the chronic kidney disease (CKD) population, contraception still remains challenging in patients on maintenance hemodialysis. Most doctors, however, overlook contraception because of the low fertility, high rate of amenorrhea, and low libido. Furthermore, patients are less receptive to contraceptive co

Effects of Levothyroxine Treatment on Fertility and Pregnancy Outcomes in Subclinical Hypothyroidism: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Background: Subclinical hypothyroidism, defined by elevated thyrotropin (TSH) and normal free thyroxine levels, is associated with adverse pregnancy outcomes, including preterm birth, pre-eclampsia, and small for gestational age. Despite the uncertainty regarding the effectiveness of levothyroxine (LT4) treatment on pregnancy outcomes in subclinical hypothyroidism, LT4 is widely administered with a pre-treatment threshold TSH level of 2.5 mU/L. The aim of this study is to investigate the efficacy of periconceptional LT4 treatment for subclinical hypothyroidism, including TSH levels >2.5 mU/L,

Association between interpregnancy interval and adverse birth outcomes in women with a previous stillbirth: an international cohort study.

BACKGROUND: WHO recommends that women wait at least 2 years after a livebirth and at least 6 months after a miscarriage or induced abortion before conceiving again, to reduce the risk of adverse birth outcomes in the subsequent pregnancy. No recommendation exists for the optimal interval after a stillbirth. We investigated the association between interpregnancy interval after stillbirth and birth outcomes in the subsequent pregnancy. METHODS: In this international cohort study, we used data from birth records from Finland (1987-2016), Norway (1980-2015), and Western Australia (1980-2015). Con

Sympathetic and T helper (Th)2 bias may ameliorate uterine fibroids, independent of sex steroids.

We propose that inadequate sympathetic bias and Th2 bias in the uterine environment contributes to the formation of fibroids, independent of the sex steroid status. We also propose that fibroids represent a modern maladaptation that partly results from decreasing exposure to seminal fluid, which contains catecholalmines, transforming growth factor beta1 (TGFbeta1), aldosterone, prostaglandins, and other factors that shift the uterine environment to sympathetic and T helper (Th)2 bias. Lower risk of fibroids is associated with pre-menarche, post-menopause, pregnancy, exposure to contraceptives,

Effect of Adherence to the Mediterranean Diet on Maternal Iron Related Biochemical Parameters during Pregnancy and Gestational Weight Gain.

Gestation is a crucial life stage for both women and offspring, and outcomes are affected by many environmental factors, including diet. The Mediterranean dietary pattern (MD) is considered a healthy eating pattern that can provide the nutritional requirements of pregnancy. Meanwhile, iron deficiency anemia is one of the most frequent complications related to pregnancy. This study aimed to evaluate how the level of adherence to the MD influences maternal gestational weight gain and specific iron-related maternal biochemical parameters during the pregnancy. Accordingly, an observational, popula

Adrenocortical carcinoma with tumor thrombus extension into the right atrium.

OBJECTIVE: Infertile women requiring ovarian stimulation and assisted reproduction techniques (ART) are faced with difficult issues. The fear that using hormones could increase their risk of cancer is the most significant. One of the main challenges for assessing cancer risk after ART is the difficulty to separate it from the underlying condition of infertility per se. The delay or the inability to achieve a pregnancy is an important risk factor for breast, endometrial and ovarian cancer. We analyzed the current literature on the topic. MATERIALS AND METHODS: The published literature in Medli

Measuring 24-hour Movement Profiles During Pregnancy: Protocol for the 24MOVE Prospective Cohort Study.

BACKGROUND: Physical activity (PA) during the waking period and sleep during pregnancy may mitigate the increased risk for adverse pregnancy outcomes posed by gestational diabetes mellitus (GDM) and excessive gestational weight gain (GWG) in pregnant individuals with pre-pregnancy overweight and obesity. Recent studies have conceptualized PA, sedentary behavior, and sleep as part of a 24-hour movement framework; however, there is a gap in the knowledge about the relationship between 24-hour movement and pregnancy outcomes. OBJECTIVE: This paper describes the study protocol for the 24MOVE stud

Maternal cytokine and cellular adhesion molecules profile in pregnant women with and without congenital heart disease.

INTRODUCTION: Despite corrective surgery, patients with congenital heart disease (CHD) have residual regions of disturbed oscillatory blood flow which can induce upregulation of proinflammatory cytokines and adhesion molecules. Data on cytokine and cellular adhesion molecule (CAM) profiles in low risk pregnancy and pregnancy complicated by CHD are limited. The objective of this work was to study the profile of IL-6, IL-10, TNFα, ghrelin, GROα, and ICAM/VCAM in pregnancy in women with and without CHD and test the hypothesis that the circulating levels of these are correlated with obstetric outc

Development, validation and clinical utility of a risk prediction model for adverse pregnancy outcomes in women with gestational diabetes: The PeRSonal GDM model.

BACKGROUND: The ability to calculate the absolute risk of adverse pregnancy outcomes for an individual woman with gestational diabetes mellitus (GDM) would allow preventative and therapeutic interventions to be delivered to women at high-risk, sparing women at low-risk from unnecessary care. We aimed to develop, validate and evaluate the clinical utility of a prediction model for adverse pregnancy outcomes in women with GDM. METHODS: A prediction model development and validation study was conducted on data from a observational cohort. Participants included all women with GDM from three metrop

[Pregnancy, contraception and HRT and venous thromboembolism].

Endogeneous and exogeneous hormonal factors may favor venous thromboembolism (VTE) in the woman. Pregnancy and post-partum, combined contraception and hormonal replacement therapy (HRT) of menopause are associated with an increased risk of VTE. Even when the relative risk is statistically significant, the absolute risk remains generally low unless additional factors are present. The occurrence of a thrombosis depends on the conditions associated to these risk factors: multiparity, caesarean section, type of contraception or HRT, and also on risk factors associated to the woman such as age, obe

Budget Impact Model of a Novel cfRNA Test-and-Treat Strategy for Preeclampsia in Advanced Maternal Age Pregnancies Compared with Current Guidelines.

BACKGROUND: Preeclampsia (PE) affects 8% of United States (US) pregnancies, driving substantial maternal and neonatal morbidity, costs, and mortality. Current United States Preventive Services Task Force (USPSTF) risk-based stratification has limited specificity, which may reduce the ability to effectively target preventive interventions and optimize PE prevention. OBJECTIVES: We evaluated the budget impact of adopting a cell-free RNA (cfRNA) PE blood test compared with USPSTF screening in advanced maternal age (AMA; maternal age ≥ 35 years at delivery) pregnancies without pre-existing high-r

Pravastatin plus L-arginine prevents adverse pregnancy outcomes in women with uteroplacental vascular dysfunction.

BACKGROUND: Uteroplacental vascular dysfunction, characterized by diminished uterine artery (UtA) blood flow in the second trimester is a clinically useful predictor of the further development of preeclampsia, fetal growth restriction and stillbirth. Efforts to develop effective treatments to protect pregnancies with abnormal UtA Dopplers would be of significant clinical benefit for mothers and their fetuses. OBJECTIVE: The aim of this pilot non randomized control study was to use pravastatin +L-arginine to improve uteroplacental haemodynamics and prevent adverse maternal and neonatal outcome

Autoimmune diseases and adverse pregnancy outcomes: an umbrella review.

BACKGROUND: There is a high prevalence of autoimmune conditions in women specially in the reproductive years; thus, the association with adverse pregnancy outcomes has been widely studied. However, few autoimmune conditions/adverse outcomes have been studied more than others, and this umbrella review aims to consolidate existing knowledge in this area with the aim to provide new knowledge and also identify gaps in this research area. METHODS: Medline, Embase, and Cochrane databases were searched from inception to December 2023. Screening, data extraction, and quality appraisal (AMSTAR 2) were

Ectopic pregnancy secondary to in vitro fertilisation-embryo transfer: pathogenic mechanisms and management strategies.

BACKGROUND: Ectopic pregnancy (EP) is the leading cause of maternal morbidity and mortality during the first trimester and the incidence increases dramatically with in vitro fertilisation and embryo transfer (IVF-ET). The co-existence of an EP with a viable intrauterine pregnancy (IUP) is known as heterotopic pregnancy (HP) affecting about 1% of patients during assisted conception. EP/HP can cause significant morbidity and occasional mortality and represent diagnostic and therapeutic challenges, particularly during fertility treatment. Many risk factors related to IVF-ET techniques and the cau
