Matched Pair Analysis of Rotational Stability With 1- Versus 2-Screw Antegrade Fixation in Distal Metaphyseal Ulnar Shortening Osteotomy Constructs.

BACKGROUND: The purpose of this study is to evaluate the rotational stability of 1-screw versus 2-screw antegrade fixation in distal metaphyseal ulnar shortening osteotomies (DMUSOs) using matched cadaveric forearm specimens. In addition, we aim to assess the mode of failure for each construct, specifically determining whether failure occurs due to hardware failure or peri-implant fractures. METHODS: Sixteen fresh-frozen cadaveric forearm specimens, organized into 8 matched pairs, were used. Ulnas were harvested, transected at the midshaft, and all soft tissue attachments were removed. Rotational order-disorder and spin crossover behaviour in a neutral iron(II) complex based on asymmetrically substituted large planar ionogenic ligand.

Octahedrally coordinated spin crossover (SCO) FeII complexes represent an important class of switchable molecular materials. This study presents the synthesis and characterisation of a novel complex, [FeII(ppt-2Fph)2]0·2MeOH, where ppt-2Fph is a new asymmetric ionogenic tridentate planar ligand 2-(5-(2-fluorophenyl)-4H-1,2,4-triazol-3-yl)-6-(1H-pyrazol-1-yl)pyridine. The complex exhibits a hysteretic thermally induced SCO transition at 285 K on cooling and at 293 K on heating, as well as light induced excited spin state trapping (LIESST) at lower temperatures with a relaxation T(LIESST) temper

Effect of esketamine on postoperative quality of recovery in patients undergoing arthroscopic rotator cuff repair: protocol for a prospective, randomized, double-blind study.

BACKGROUND: The utilization of shoulder arthroscopic surgery has progressively increased as a prevalent surgical approach for the treatment and repair of rotator cuff injury in clinical practice. However, early postoperative pain and neurological dysfunction remain significant factors that impact patients' postoperative recovery. Esketamine, an NMDA receptor antagonist, effectively reduces opioid consumption, attenuates postoperative inflammatory reactivity, and has also demonstrated neuroprotective effects. The aim of this study is to evaluate the quality of early recovery after anesthesia ma

Using Rotational Thromboelastometry (ROTEM) to Evaluate Coagulation Status After Intrapleural Recombinant Tissue Plasminogen Activator (rtPA) Administration in Regional Anesthesia.

We report a case of a 45-year-old male who underwent thoracotomy for empyema and received multiple doses of intrapleural recombinant tissue plasminogen activator (rtPA). Given the recent administration of rtPA, the acute pain service performed a rotational thromboelastometry (ROTEM) test to assess coagulation before proceeding with regional anesthesia. The ROTEM results indicated normal to hypercoagulable clotting parameters, with a normal extrinsic thromboelastometry (EXTEM) lysis index at 30 minutes, suggesting no systemic effects of rtPA. Based on these findings, a single-shot paravertebral

Tissue-engineered Bicipital Autologous Tendon Patch Enhances Massive Rotator Cuff Defect Repair in a Rabbit Infraspinatus Tendon Defect Model.

BACKGROUND: Massive rotator cuff defects represent an important source of shoulder pain and functional debilitation, substantially diminishing patients' quality of life. The primary treatment of massive rotator cuff defects includes complete or partial repair and patch augmentation. However, because of the tendon's limited regenerative ability, the tendon retear risk after rotator cuff defect repair is still high. Thus, a new therapy is needed to promote tendon regeneration for repair of massive rotator cuff defects. QUESTIONS/PURPOSES: Using an in vitro analysis, we first asked: (1) What is

Dry needling as an adjunct treatment to multimodal rehabilitation protocol following rotator cuff repair surgery: a preliminary, randomized sham-controlled trial.

BACKGROUND: Rotator cuff repair (RCR) is one of the most prevalent procedures to manage rotator cuff tears (RCT). Postoperative shoulder pain is a common complication following RCR and may be aggravated by activation of myofascial trigger points (MTrP) associated with the injury to the soft tissues surrounding the surgical incision. This study aimed to describe a preliminary, randomized, sham-controlled trial to evaluate the effectiveness of implementing 4 sessions of myofascial trigger point dry needling (MTrP-DN) as a muscle treatment approach along with 10 sessions of multimodal rehabilitat

Single-session mechanical thrombectomy for iliofemoral deep vein thrombosis using a dual mechanism of action device combining basket and rotational thrombectomy.

OBJECTIVE: Interventional treatments for acute iliofemoral deep vein thrombosis (DVT) remain controversial after publication of the Acute Venous Thrombosis: Thrombus Removal with Adjunctive Catheter-Directed Thrombolysis (ATTRACT) study. Interventions have been shown to reduce post-thrombotic syndrome severity and improve quality of life in DVT patients, but have been accompanied by risk of major bleeding from thrombolytics. We describe thrombus removal using a novel combined basket-rotational thrombectomy device that minimizes the need for thrombolytics or repeat procedures. METHODS: The aim

Rotator cuff training with upper extremity blood flow restriction produces favorable adaptations in division IA collegiate pitchers: a randomized trial.

BACKGROUND: Recent evidence indicates that combined upper extremity blood flow restriction (BFR, applied distally to the shoulder) and low-load resistance exercise (LIX) augments clinically meaningful responses in shoulder region tissues proximal to the occlusion site. The purpose of this investigation was to determine the efficacy of BFR-LIX for the shoulder when added to standard offseason training in Division IA collegiate baseball pitchers. We hypothesized that BFR-LIX would augment training-induced increases in shoulder-region lean mass, rotator cuff strength, and endurance. As secondary
