Nutritional, metabolic and genetic considerations to optimise regenerative medicine outcome for knee osteoarthritis.

Knee osteoarthritis (KOA) is a multifactorial degenerative disorder of joints, affecting the world's population over the age of 65 and with a higher prevalence in females. KOA is responsible for many age associated joint problems such as stiffness and pain. Conventional methods for managing KOA such as nonsteroidal anti-inflammatory drugs (NSAID) may not improve pain or alter the disease progression and may have adverse side effects. Non-pharmacological management of OA is fundamental to management of functional limitations and provides effective symptom relief but has not shown that disease p

Age, Resilience, and Hope Predict Daily Functioning After a Flood.

We examined associations among resilience characteristics, risk factors, and self-reported functional impairment using archival data from persons exposed to the 2016 flooding in Baton Rouge, LA. Three flood exposure groups were compared: non-flooded (controls), single disaster (flooded in 2016), and double disaster (flooded in 2005 and again in 2016). The Barkley Functional Impairment Scale (BFIS), a self-report measure of daily functioning, was the primary outcome measure. The single disaster group reported more problems in daily functioning than did the non-flooded controls. Correlations rev

A Rare Case of Epstein-Barr Virus Hepatitis and Rash in an Adolescent.

Infection from Epstein-Barr virus (EBV) manifests with a diverse spectrum of presentations. One common manifestation is hepatobiliary involvement with hepatitis, which in most cases is mild and self-limited. Severe or fulminant hepatitis can potentially occur, though this is rare, particularly in immunocompetent pediatric populations. We present a previously healthy female adolescent with severe abdominal pain and jaundice, but no lymph node or tonsillar swelling. The workup revealed a marked elevation in liver enzymes, bilirubin, and mesenteric adenitis. A positive polymerase chain reaction (

Unusual Penetrating Orbital and Globe Injury With a Veterinary Needle.

We present a rare case of a penetrating injury to the globe and orbit by a veterinary needle. The patient underwent globe exploration and retinal detachment repair under general anesthesia after receiving medical treatment for orbital cellulitis. The case was managed by a multidisciplinary team consisting of a pediatric ophthalmologist, an oculoplastic surgeon, and a retina surgeon. The orbital cellulitis resolved gradually, after which the site of globe penetration was found to be self-sealed, and the retinal detachment was repaired. B-scan ultrasonography and magnetic resonance imaging (MRI)

Recurrent Aseptic Meningitis From Herpes Simplex Virus-2: Mollaret's Meningitis in a 30-Year-Old Female.

Aseptic meningitis is most commonly caused by herpes simplex virus (HSV), most often viral subtype 2. While typical meningeal symptoms include headache, photophobia/phonophobia, and nuchal rigidity, these are often much less severe than in bacterial meningitis. Rarely, patients may develop recurrent episodes of aseptic meningitis, sometimes with years between each presentation. A minimum of three episodes with at least one documented viral identification is classified as Mollaret meningitis. First described by Mollaret in 1945, the condition is self-limiting and often requires no intervention

Sequential transcatheter aortic valve implantation due to valve dislodgement - a Portico valve implanted over a CoreValve bioprosthesis.

Transcatheter aortic valve implantation (TAVI) has become an important treatment in high surgical risk patients with severe aortic stenosis (AS), whose complications need to be managed promptly. The authors report the case of an 86-year-old woman presenting with severe symptomatic AS, rejected for surgery due to advanced age and comorbidities. The patient underwent a first TAVI, with implantation of a Medtronic CoreValve®, which became dislodged and migrated to the ascending aorta. Due to the previous balloon valvuloplasty, the patient's AS became moderate, and her symptoms improved. After sev

Facilitating aging in place: A qualitative study of practical problems preventing people with dementia from living at home.

Although the majority of people with dementia wish to age in place, they are particularly susceptible to nursing home admission. Nurses can play an important role in detecting practical problems people with dementia and their informal caregivers are facing and in advising them on various ways to manage these problems at home. Six focus group interviews (n = 43) with formal and informal caregivers and experts in the field of assistive technology were conducted to gain insight into the most important practical problems preventing people with dementia from living at home. Problems within three do

Operative treatment with a laparotomy for anorectal problems arising from a self-inserted foreign body.

An anorectal foreign body can cause serious complications such as incontinence, rectal perforation, peritonitis, or pelvic abscess, so it should be managed immediately. We experienced two cases of operative treatment for a self-inserted anorectal foreign body. In one, the foreign body could not be removed as it was completely impacted in the anal canal. We failed to remove it through the anus. A laparotomy and removal of the foreign body was performed by using an incision on the rectum. Primary colsure and a sigmoid loop colostomy were done. A colostomy take-down was done after three months. T
