[A case of food-dependent exercise-induced anaphylaxis caused by various foods which contained flour for thirty years].

The patient was a 65-year-old man with chief complaints of flare, swelling, itching and loss of consciousness. He had a history of diabetes diagnosed at the age of 34 years and was receiving medication from a local doctor. He had experienced systemic flare, swelling and loss of consciousness 20 minutes after drinking beer at a party at the age of 34 years. Since that time, he had frequently experienced urticaria and loss of consciousness while taking a walk after eating various foods (all of which contained flour). In February 2001, he experienced systemic flare, swelling and loss of conscious

Neural respiratory drive measurement for COPD assessment and monitoring.

Currently there is an unmet need for more objective assessments that could determine COPD severity. Ideally such objective assessments could also anticipate COPD exacerbations in order to decrease the need for repeated hospital admissions. In this review we outline how patients' neural respiratory drive (NRD) may be determined using the electromyography of the diaphragm as an objective measurement of COPD severity. Respiratory muscle NRD is indeed less influenced by patients' voluntary effort limitation than for example when testing for exercise tolerance in which case the patients themselves

Effects of Tyrosine on Parkinson's Disease: A Randomized, Double-Blind, Placebo-Controlled Trial.

Individuals with Parkinson's disease (PD) can suffer from orthostatic hypotension (OH) resulting from reduced levels of norepinephrine (NE), which inhibits the sympathetic nervous system. Levodopa reduces NE levels even further, leading to a greater decrease in blood pressure (BP) and increased OH. Tyrosine is a nonessential amino acid that is the major precursor to NE. Reduced levels of tyrosine have been shown after administration of l-dopa. This study was a single-center, randomized, double-blind, placebo-controlled trial to test the effects of supplementing l-tyrosine on BP, plasma tyrosin

Metabolic myopathies: update 2009.

Metabolic myopathies are inborn errors of metabolism that result in impaired energy production due to defects in glycogen, lipid, mitochondrial, and possibly adenine nucleotide metabolism. Fatty acid oxidation defects (FAOD), glycogen storage disease, and mitochondrial myopathies represent the 3 main groups of disorders, and some consider myoadenylate deaminase (AMPD1 deficiency) to be a metabolic myopathy. Clinically, a variety of neuromuscular presentations are seen at different ages of life. Newborns and infants commonly present with hypotonia and multisystem involvement (liver and brain),

Dipyridamole myocardial perfusion tomography in patients with severe aortic stenosis.

UNLABELLED: Patients with aortic stenosis (AS) may have classic angina pectoris. The safety of exercise testing in adults with AS is controversial and, in fact, exercise testing in such patients is considered to be contraindicated especially in severe aortic stenosis (SAS). Furthermore, exercise testing has low specificity in uncovering coronary artery disease (CAD) in patients with AS, because the baseline ECG is frequently abnormal. We wished to assess the safety and diagnostic accuracy of dipyridamole stress myocardial perfusion tomography (DMPT) in the detection of CAD in patients with SAS

Pulmonary manifestations of the eosinophilia-myalgia syndrome associated with tryptophan ingestion.

Pulmonary manifestations are not infrequent in the L-tryptophan-induced eosinophilia-myalgia syndrome (EMS). However, previous reports have not described the results of longitudinal pulmonary function, exercise testing, high-resolution computerized tomographic (HRCT) scanning of the chest, or detailed bronchoalveolar lavage (BAL) analysis. We report six patients with EMS who had dyspnea. The diffusing capacity for carbon monoxide was decreased in five patients tested. Exercise testing with arterial blood gas sampling in three patients was consistent with pulmonary vascular or parenchymal disea

Facing up to the imperceptible perspiration. Modulatory influences by diabetic neuropathy, physical exercise and antiperspirant.

BACKGROUND: Sweating is variably altered by physical exercise, diabetic neuropathy and antiperspirants. METHODS: Skin temperature, skin surface water loss (SSWL), the Corneometer(®) average capacitance (CMAC) and skin capacitance mapping (SCM) were measured before and after moderate physical exercise in 20 healthy subjects. The effect of 5% aluminium chloride hexahydrate (ACH) in a water solution was similarly tested. The same assessments were performed in 20 diabetic patients at rest. RESULTS: Diabetic neuropathy appeared at rest as an increased (compensatory) SCM on the forearms without ob

Effects of a self-efficacy intervention on initiation of recommended exercises in patients with spondylosis.

An intervention designed to enhance preaction self-efficacy beliefs (i.e., beliefs about ability to initiate behavior despite anticipated barriers during the initiation period) was tested in patients with spondylosis in relation to initiation of exercises recommended by a consultant in orthopedic rehabilitation. Sixty patients (age 28-83 years; 44% men) with spondylosis who had not previously performed exercises recommended for degenerative spine diseases were randomly assigned to a control (education session) or intervention group. Three weeks later, intervention patients performed recommende

[Ergospirometric findings in respiration- and QT-controlled pacemakers].

This study used ergospirometry with quantification of gas exchange parameters to analyze the beneficial effects of two sensors for rate responsive pacing: QT-interval and minute ventilation (MV). Bicycle exercise testing was performed with modulating VVIR pacing modes in random sequence. Rate responsiveness effected a mean increase of exercise tolerance of 31.1% in the QT group and 22.6% in the MV group. With both systems maximal oxygen uptake increased by 29.4% (QT) and 28.3% (MV). Oxygen consumption at the anaerobic threshold was improved by 28.3% (QT) and 32.1% (MV). The increase of cardiop

[Left ventricular hemodynamics at rest and during exercise in patients with idiopathic mitral valve prolapse syndrome (author's transl)].

Idiopathic Mitral Valve Prolapse has been regarded by some authors as a primary cardiomyopathy. To determine, if left ventricular hemodynamics at rest and during exercise are impaired, 20 young symptomatic patients with idiopathic mitral valve prolapse, proven by echo- and phonocardiography, without coronary artery disease or severe mitral regurgitation, underwent supine bicycle exercise stress testing. Pulmonary artery pressure, central venous oxygen saturation and cardiac index as well as heart rate were measured with a Swan-Ganz thermodilution catheter and the results compared with those of

The clinical significance of exercise-induced ST-segment elevation.

The significance of exercise-induced ST-segment elevation remains unsettled. We reviewed the treadmill tests of 840 consecutive patients and exercise-induced ST-segment elevation was noted in 29 (3.5%). Only eight of these (28%) stopped because of angina. Anterior myocardial infarction (AMI) was found on the resting electrocardiogram in 25 (85%). Angiographic studies performed on 21 showed critical lesions of the left anterior descending (LAD) in 19 (90%) and left ventricular aneurysm in 18 (86%). When all the patiens who had AMI or critical LAD obstruction during the study period were reviewe

The effect of sertraline and voluntary exercise during pregnancy on litter characteristics and postpartum affective behaviour in rat dams.

INTRODUCTION: Sertraline is the frontline pharmacotherapy for the treatment of depression and anxiety during pregnancy. However, there is little evidence regarding the effects of sertraline on maternal behaviour or the maternal brain. Furthermore, the efficacy of non-pharmacological approaches to treatment in pregnancy, such as exercise, are unclear. Therefore, the aim of this study was to examine the effects of sertraline and exercise during pregnancy on maternal postpartum depressive-like, anxiety-like and associated behaviours, as well as litter characteristics, in a rat model of depression

Effectiveness of the Specific Mobility Exercises on Pain Intensity and Quality of Life Among Stoma Patients: A Quasi-experimental Study.

BACKGROUND: An intestinal stoma is a surgically created artificial opening in the abdominal wall that helps the large or small intestine end to divert the faecal matter for stoma patients with an underlying condition of inflammatory bowel disease and colorectal cancer. When a stoma is formed following surgery, one of the difficulties stoma patients confront has been identified as prolonged immobilization, which can eventually result in muscle inactivity that results due to their illness. Patients with stoma often experience an increase in pain and a decrease in quality of life. Patients can be

Differing effects of airway obstruction on physical work capacity and ventilation in men and women with COPD.

BACKGROUND: Although the natural history of COPD is thought to be well known, studies assessing differences in the onset and course of the disease by gender are surprisingly lacking. This study is a cross-sectional analysis using progressive cycle ergometry exercise testing to assess male and female patients at specific levels of airway obstruction to see if they differ in their exercise capacity and decline in functional capacity. METHODS: The study group included 417 patients with COPD, 55 to 85 years of age, who were compared with 29 controls of similar age; all patients had COPD (FEV1/FVC

Sodium bicarbonate ingestion does not alter the slow component of oxygen uptake kinetics in professional cyclists.

We examined the effects of pre-exercise sodium bicarbonate (NaHCO3) ingestion on the slow component of oxygen uptake (VO2) kinetics in seven professional road cyclists during intense exercise. One hour after ingesting either a placebo or NaHCO3 (0.3 g x kg body mass(-1)), each cyclist (age, 25 +/- 2 years; VO2max, 74.7 +/- 5.9 ml x kg(-1) x min(-1); mean +/- s) performed two bouts of 6 min duration at an intensity of 90% VO2max interspersed by 8 min of active recovery. Gas exchange and blood data (pH, blood lactate concentration and [HCO3-]) were collected during the tests. In both bouts, the
