Economic and Environmental Considerations During Low Fresh Gas Flow Volatile Agent Administration After Change to a Nonreactive Carbon Dioxide Absorbent.

BACKGROUND: Reducing fresh gas flow (FGF) during general anesthesia reduces costs by decreasing the consumption of volatile anesthetics and attenuates their contribution to greenhouse gas pollution of the environment. The sevoflurane FGF recommendations in the Food and Drug Administration package insert relate to concern over potential toxicity from accumulation in the breathing circuit of compound A, a by-product of the reaction of the volatile agent with legacy carbon dioxide absorbents containing strong alkali such as sodium or potassium hydroxide. Newer, nonreactive absorbents do not produ

Automated hemodynamic management system for the prevention of hypotension and cardiac output reduction during general anesthesia: preclinical experimental validation.

PURPOSE: Intraoperative hypotension and cardiac output (CO) reduction adversely affect outcomes and require cause-specific treatment against vasodilation and/or hemorrhage. However, tight hemodynamic management increases anesthesiologists' workloads. We aimed to develop an automated hemodynamic management system that maintains arterial pressure (AP) and CO according to the causes, and to experimentally validate the system in anesthetized dogs. METHODS: The system automatically titrates norepinephrine (NE) and fluid to control systemic vascular resistance and stressed blood volume, respectivel

Surgical management of myopic traction maculopathy; expert perspectives from the myopia society.

Myopic traction maculopathy (MTM) occurs in approximately 9-34 % of highly myopic eyes and requires surgical treatment to prevent irreversible vision loss due to progression to macular hole (MH) or retinal detachment (RD). While treatment of MTM, which presents with a variety of morphological patterns, shares the common goal of managing the causative tractional tissues on the retina, it is essential to tailor the surgical plan according to the locations of each lesion. Here, we classify MTM into four core lesions: retinoschisis, foveal RD, lamellar MH (LMH), and full-thickness MH (FTMH), and p

Anesthetic Management of Multiple Endocrine Neoplasia Type 2A (MEN2A) Syndrome With Contralateral Recurrent Pheochromocytoma: A Case Report.

This is a classic and complete representation of the multiple endocrine neoplasia type 2A (MEN2A) syndrome, involving primary hyperparathyroidism, medullary thyroid carcinoma, and pheochromocytomas. This case is particularly noteworthy for the recurrent, contralateral presentation of the pheochromocytoma, which emerged years after the initial surgical management. Recurrent and contralateral pheochromocytomas present unique anesthetic, surgical, and postoperative problems due to excess catecholamines and severe, potentially life-threatening hemodynamic instability in the perioperative phase. We

Transcranial Doppler Monitoring of the Wolf Procedure Versus Conventional Ablation to Treat Atrial Fibrillation.

BACKGROUND AND PURPOSE: Transfemoral radiofrequency ablation (tfRFA) is the most performed procedure to address atrial fibrillation (AF); however, tfRFA produces thousands of microemboli that can reach major cerebral arteries and cause silent ischemic lesions. We measured the microembolization rate during an alternative procedure, the Wolf Procedure (formerly Mini-Maze), to address AF and compared our findings with the literature. METHODS: We used transcranial Doppler monitoring on 10 patients with AF (seven permanent, three paroxysmal) who underwent the Wolf Procedure at Houston Methodist Ho

Visual marker-based augmented reality for intraoperative ultrasound in minimally invasive surgery.

BACKGROUND: Intraoperative ultrasonography (IOUS) is essential in minimally invasive hepatobiliary-pancreatic surgery but requires surgeons to alternate attention between laparoscopic and ultrasound monitors, disrupting workflow. We developed Flag AR, an augmented reality system that superimposes real-time ultrasound images onto laparoscopic displays using visual marker tracking. METHODS: This prospective observational study included 14 patients undergoing minimally invasive hepatobiliary-pancreatic procedures between March and April 2025. Three experienced surgeons performed target identific

Quantitative analysis of tumor-derived methylated RUNX3 sequences in the serum of gastric cancer patients.

PURPOSE AND EXPERIMENTAL DESIGN: Using real-time quantitative methylation-specific PCR (RTQ-MSP), methylated RUNX3 sequences were quantified and the fractional concentrations of circulating tumor DNA in serum were determined, along with peripheral blood cells collected preoperatively, intraoperatively and postoperatively from 65 patients with gastric cancer. RESULTS: RTQ-MSP was sufficiently sensitive to detect RUNX3 methylation. Quantitative MSP data were expressed in terms of the methylation index, which was defined as the relative amount of methylated RUNX3 sequences divided by the concent

Low baseline Tc-MEP amplitude is associated with false-positive alerts in spine surgery: a ROC-based analysis across spinal pathologies.

This study aimed to determine whether absolute baseline transcranial motor evoked potential (Tc-MEP) amplitude is associated with false-positive (FP) alerts during intraoperative neurophysiological monitoring and to estimate pathology-specific cutoff values. We retrospectively reviewed 307 high-risk spine surgery cases monitored with Tc-MEPs from 2018 to 2020. For waveform-level analysis, 1,748 muscle waveforms from 287 FP/true-negative cases were analyzed. Alerts were defined as final amplitude ≤ 30% of baseline, and FP alerts were defined as alerts without new postoperative motor deficit. Re

PICC migration and perforation of internal jugular vein after neck dissection.

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

Present Electrically Evoked Compound Action Potentials and Lack of Auditory Sensation Through a Cochlear Implant After Vestibular Schwannoma Resection.

OBJECTIVE: Auditory nerve monitoring through electrically evoked compound action potentials (eCAP) can be used to assess cochlear neural integrity during vestibular schwannoma (VS) resection. Herein, we report on 5 cochlear implant (CI) patients who underwent VS resection with present eCAP responses both intraoperatively and postoperatively. Patients either did not perceive any sound through their CI or rapidly adapted to electrical stimulation, suggesting that the presence of eCAP does not guarantee auditory nerve function in patients with VS resection. PATIENTS: Five patients who underwent

Pronounced Mitral Annular Disjunction Is Associated With Increased Postoperative Palpitations After Mitral Valve Surgery for Barlow's Disease.

OBJECTIVES: Pronounced mitral annular disjunction (pMAD ≥8 mm) in myxomatous mitral valve disease has been associated with ventricular arrhythmias in selected cohorts, whereas minor degrees of mural separation are frequently observed in structurally normal hearts. Its clinical relevance after mitral valve surgery remains unclear. We investigated whether pMAD is associated with postoperative symptom burden and rhythm outcomes. METHODSV: In this retrospective cohort with prospective follow-up, 246 adults undergoing mitral valve surgery for Barlow's disease were classified intraoperatively as pM
