Septic Polyarthritis Caused by Neisseria meningitidis: A Rare Presentation in Japan.

Neisseria meningitidis is a Gram-negative diplococcus that rarely causes invasive diseases in Japan. We report a case of polyarticular meningococcal septic arthritis in an immunocompetent 62-year-old Japanese man. After 3 days of fever, he developed pain in the dorsum of the right hand and right shoulder. Computed tomography revealed effusions in the right wrist and acromioclavicular joint. Blood cultures grew N. meningitidis serogroup Y, sequence type 1655. A PCR assay detected the 16S rRNA gene of N. meningitidis in culture-negative wrist synovial fluid. Treatment with ceftriaxone followed b

Ukrainian Orthopedic Trauma Experience in Managing Wartime Injuries of the Upper Extremity.

Warfare injuries of the upper extremity constitute approximately 45% of all injuries to the extremities. We review a warfare treatment algorithm for injuries to the shoulder, elbow, soft tissue, and peripheral nerves of the upper extremity. A very challenging problem is reconstruction of the elbow joint after gunshot injuries, with defects of distal humerus and proximal forearm. Warfare injuries of the peripheral nerves are part of a polystructural injury, with bone fractures and soft tissue defects. They are combined nerve injury patterns with the best results when treatment occurs between 3

A case of synovitis-acne-pustulosis-hyperostosis-osteitis syndrome with right pleural effusion.

A 79-year-old man presented with fatigue and right shoulder pain. Computed tomography revealed right pleural effusion and osteosclerosis of the sternoclavicular joint. There were no signs of malignancy or infection in the pleural fluid studies. His bone scintigraphy exhibited the "bull's head sign." Despite the absence of skin lesions, he was diagnosed with synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome. Remission was achieved after treatment with non-steroidal anti-inflammatory drugs and oral prednisolone. SAPHO syndrome causes pleural effusion, even in patients without skin

Migration of Broken Kirschner Wire to the Brachial Plexus after Fixation of Fracture Dislocation of the Acromioclavicular Joint.

A complication of Kirschner (K) wire fixation is the migration of the wire. We report a patient who had undergone fixation of a right clavicle fracture associated with acromioclavicular joint (ACJ) dislocation 15 years ago. He presented with features of pain and dysaesthesia in the right ulnar nerve dermatome on the movement of the neck and shoulder. Radiographic investigations showed a broken K-wire that had migrated to the lower brachial plexus. He underwent surgical removal of the K-wire and recovered uneventfully. We summarise the literature on K-wire migration reported following clavicle

Using imaging methods to assess severe calcinosis in juvenile dermatomyositis: A case report.

Calcinosis and lipodystrophy are severe complications of juvenile dermatomyositis (JDM). Up to 20% of patients have calcinosis, and the onset of calcinosis usually occurs 1 to 3 years after that of the illness. We report a case of JDM with severe complications of calcinosis and lipodystrophy, and we assess calcinosis using a variety of imaging methods. To evaluate the patient's inflammatory state, bone scintigraphy was performed, which demonstrated increased uptake in the right scapula, in addition, multiple calcifications are present subcutaneously on the shoulder and back, and inflammatory i

Cleidocranial dysplasia with hypermobile Ehlers-Danlos syndrome: A case report.

Cleidocranial dysplasia (CCD) is a rare genetic skeletal syndrome. The most common features are open fontanelles, hypertelorism, mid-face retrusion, hypoplasia, or aplasia of the clavicles leading to excessive shoulder mobility, dental abnormalities such as supernumerary, and impacted teeth. This case report describes a 31-year-old male with CCD with several unusual symptoms like generalized joint hypermobility, skin laxity, and smooth skin, which leads to set a diagnosis of hypermobile Ehlers-Danlos syndrome (hEDS). To our knowledge, this is the first case report in the literature that descri

Management of Spinal Cord Injury-Induced Upper Extremity Spasticity.

Spasticity affects more than 80% of patients with spinal cord injury. Neural mechanisms and musculotendinous alterations lead to typical upper extremity features including shoulder adduction/internal rotation, forearm pronation, and elbow, wrist, and finger flexion. Long-standing spasticity may lead to soft tissue and joint contractures and further impairment of upper extremity function. Surgical management involves tendon lengthening, release, and transfer, as well as selective neurotomy, in an effort to reduce spastic muscle hypertonicity, restore balance, prevent further contracture, and im

Shoulder Acromioclavicular and Coracoclavicular Ligament Injuries: Common Problems and Solutions.

Injuries to the acromioclavicular joint and coracoclavicular ligaments are common. Many of these injuries heal with nonoperative management. However, more severe injuries may lead to continued pain and shoulder dysfunction. In these patients, surgical techniques have been described to reconstruct the function of the coracoclavicular ligaments to provide stable relationship between the clavicle and scapula. These surgeries have been fraught with high complication rates including clavicle and coracoid fractures, infection, loss of reduction and fixation, hardware migration, and osteolysis. This

[Bilateral elbow dislocation related to Essex-Lopresti injury].

Elbow dislocation is second in frequency, after the shoulder, whereas bilateral dislocation is uncommon, even less than dislocations with concurrent associated fractures. One of the least frequent associations is the Essex-Lopresti injury which consists of a fracture of the radial head affecting the distal radioulnar joint with injury to the interosseous membrane. This is a case of bilateral elbow dislocation, one of the elbows associated with the Essex-Lopresti injury. During treatment, the premature closed reduction prevails, previously making sure the elbow is stable, the premise which will

Successful closed manipulation of a pure lateral traumatic dislocation of the elbow joint using a modified Stimson's technique: a case report.

INTRODUCTION: Pure lateral elbow dislocation is rare, and a successful closed reduction is even rarer. Reduction can be hindered by swelling, soft tissue interposition or associated fractures. CASE PRESENTATION: We present a pure lateral traumatic dislocation of the elbow joint in a 40-year-old man. This was successfully manipulated and reduced in casualty using a modification of the gravity-aided 'hanging arm' technique originally described for shoulder dislocations by Stimson. CONCLUSION: We strongly recommend the use of this simple technique in these rare yet difficult injuries, in order

The inframammary extending lateral intercostal artery perforator flap for reconstruction of axillary contractures: a case report.

Release and reconstruction of axillary scar contractures can be challenging due to the specific anatomic site and contouring of the axillary region. Pliable and unscarred skin coverage of resulting defects after scar release is needed which enhances the postoperative recovery and revalidation. When traditional donor regions of fasciocutaneous flaps are involved in the scarred area, options are few. We describe the design and versatility of an inframammary extended lateral intercostal artery perforator (LICAP) flap to reconstruct an axillary defect after wide scar release and debridement. The p

[Compression of the subscapular nerve in the spino-glenoid tract by a synovial cyst].

We observed a case of synovial cyst in the shoulder joint leading to spino-glenoid symptomatology in a weight-lifter. Explorations included standard radiography, echography and magnetic resonance imaging. Differentiation between a true cyst and a pseudo-cyst was basedose presence of a synovial membrane. Mechanical contraints appear to be the most frequently suspected aetiology of spinoglenoid cysts. Echography cannot always confirm the liquid nature of the formation due to the close bone. Magnetic resonance imaging gives good tissue characterization and good topographic analysis. Gadolinium in

Unilateral hypoglossal nerve paralysis following the use of the laryngeal mask airway.

We report a unilateral hypoglossal nerve paralysis following the use of a laryngeal mask airway in a 62-year-old woman with rheumatoid arthritis undergoing a shoulder joint replacement. Cervical epidural anaesthesia was combined with general anaesthesia using nitrous oxide administered via a laryngeal mask airway with the patient in the right lateral decubitus position. The next morning, the patient was noted to have a right hypoglossal nerve palsy. Compression of the nerve between the laryngeal mask airway cuff, distended with nitrous oxide, and the hyoid bone, was considered to be the cause

Comparing Treatment Approaches for Dupuytren Contracture: Percutaneous Needle Fasciotomy With and Without Corticosteroids.

INTRODUCTION: Percutaneous needle fasciotomy (PNF) is a standard treatment for Dupuytren contracture. In some instances, clinicians administer intraoperative corticosteroids (ICS) and/or postoperative corticosteroids (PCS) alongside PNF to improve outcomes. However, few studies have systematically investigated the efficacy of ICS and/or PCS as adjuvant therapies to PNF. The purpose of this study was to review clinical outcomes between PNF alone and PNF with corticosteroid use in the treatment of Dupuytren contracture. METHODS: A systematic review was done in February 2024 using SCOPUS and OVI

Increased Glenohumeral Joint Loads Due to a Supraspinatus Tear Can Be Reversed With Rotator Cuff Repair: A Biomechanical Investigation.

PURPOSE: To evaluate the effect of an isolated full-thickness supraspinatus (SSP) tear on glenohumeral kinematics and contact mechanics, as well as to quantify improvement following rotator cuff repair (RCR). METHODS: Ten fresh-frozen cadaveric shoulders (mean age: 63.1 ± 4.6 years) were tested using a dynamic shoulder simulator. A pressure-mapping sensor was placed between the humeral head and the glenoid. Each specimen underwent the following three conditions: 1) native, 2) isolated full-thickness SSP tear, and 3) RCR. Maximum abduction angle (MAA) and superior humeral head migration (SHM)

History of shoulder instability and subsequent injury during four years of follow-up: a survival analysis.

BACKGROUND: Little is known about the risk factors for glenohumeral joint instability. We hypothesized that a prior history of instability would be a significant risk factor for subsequent injury. METHODS: We conducted a prospective cohort study over a four-year period within a high-risk group of young athletes to address the research hypothesis. Subjects were freshmen entering the U.S. Military Academy in June of 2006. Part of the baseline assessment included documenting a prior history of glenohumeral instability on entry into the study. All subjects were followed for subsequent glenohumera

Combined Clavicular Hook Plate and Coracoid Screw Fixation for Coracoid Process Fractures Associated with Acromioclavicular Joint Dislocation.

Background and Objectives: Coracoid process (CP) fractures combined with acromioclavicular (AC) joint dislocation are extremely rare, and evidence guiding optimal surgical management remains limited. This retrospective, single-center case series study evaluated clinical and radiologic outcomes after simultaneous fixation of both lesions using a clavicular hook plate and a coracoid screw. Materials and Methods: We retrospectively reviewed 15 consecutive patients with Ogawa type I CP fractures combined with AC joint dislocation who underwent clavicular hook plate and coracoid screw fixation betw

Extension-Block Pinning for Unstable Dorsal Proximal Interphalangeal Joint Fracture-Dislocations: A Simple, Percutaneous Technique With Reproducible Outcomes.

BACKGROUND: Proximal interphalangeal (PIP) joint fracture-dislocations can be technically challenging injuries to treat, and no technique has proven to be superior nor lead to predictably good outcomes. We describe our experience of treating unstable dorsal fracture-dislocations of the PIP joint with extension-block pinning (EBP) at our institution over a 22-year period. METHODS: In all, 23 patients with 24 unstable dorsal fracture-dislocations of the PIP joint treated with EBP between January 1998 and October 2020 were identified. All patients underwent closed reduction of the PIP joint and

A computer-vision method to estimate joint angles and L5/S1 moments during lifting tasks through a single camera.

Weight lifting is a risk factor of work-related low-back musculoskeletal disorders (MSD). From the ergonomics perspective, it is important to measure workers' body motion during a lifting task and estimate low-back joint moments to ensure the low-back biomechanical loadings are within the failure tolerance. With the recent development of advanced deep neural networks, an increasing number of computer vision algorithms have been presented to estimate 3D human poses through videos. In this study, we first performed a 3D pose estimation of lifting tasks using a single RGB camera and VideoPose3D,
