BACKGROUND: 25 years after the first successful arterial switch operation (ASO) for complete transposition of the great arteries (TGA), it is possible to evaluate the mid-term results after this procedure.
PATIENTS AND METHODS: From March 1986 through December 1993, 188 patients with TGA underwent ASO (primary in 181 patients and secondary after previous banding of the pulmonary artery in 7 patients). Of the 181 patients with primary ASO, 152 had simple TGA or TGA with a small ventricular septal defect, 90 patients had TGA with a large ventricular septal defect, and 10 had a Taussig-Bing comp

Right atrial thrombus is an uncommon yet potentially life-threatening complication associated with central venous catheterization. We report an unusual case of catheter-associated right atrial thrombosis (CRAT) in a patient receiving maintenance hemodialysis, who presented with recurrent thrombus on the fifth day following surgical thrombectomy. This report summarizes the clinical presentation, diagnostic modalities, and management strategies for CRAT. Our intention is to improve clinicians' awareness of this complication and provide practical insights into optimal management in hemodialysis p

Pediatric intraluminal pulmonary vein stenosis (PVS) is a challenging condition where the diagnosis is confirmed via cardiac catheterization. The patient population is complex with inherent hemodynamic vulnerabilities, and the high rate of restenosis necessitates recurrent, technically demanding transcatheter interventions. Furthermore, decisions made during each procedure can have a lasting impact on future care. To address these challenges, we present a guideline for transcatheter interventions for PVS, informed by both published literature and the Authors' clinical experience.

Lacunar strokes are the hallmark of cerebral small vessel disease. There are several well-established mechanisms for the pathogenesis of lacunar stroke, but the cardioembolic mechanism is not well-established. Three cases of acute ischemic stroke following elective cardiac and cerebral catheterization are reported. These cases had typical lacunar-looking infarcts on neuroimaging despite strong evidence of an embolic source with temporal correlation. Awareness of such findings and pathogenesis may help investigational workup and management of these patients.

Pericardial disease is a rare complication after renal transplantation. We present a patient who developed high-output cardiac failure from a large arteriovenous (AV) fistula with recurrent pericardial effusion resulting in a constrictive hemodynamic pattern that was revealed during cardiac catheterization. Pericardiectomy was considered for recurrent effusive pericarditis, but per cardiac surgery recommendations, closure of the AV fistula dramatically cured the patient's heart failure, and no recurrence of pericardial effusion was seen during follow-up almost a year later.

A 44-year-old man with history of D-transposition of the great arteries status post Mustard repair with pulmonary baffle obstruction was referred for further management. Cardiac computed tomography demonstrated calcific stenosis of the pulmonary venous baffle (PVB) outflow and right heart catheterization demonstrated elevated superior vena cava, pulmonary artery, and pulmonary capillary wedge pressures. A course of treatment is described demonstrating the challenges and potential complications of hybrid PVB stenting in a patient with challenging anatomy.

Catheter associated urinary tract infections (CAUTIs) are common hospital-acquired infections and remain a significant medical and financial challenge to the healthcare system. Despite this risk, incontinent women may require prolonged catheterization to accurately monitor urine output and prevent skin breakdown. The PureWick Female External Urinary Catheter is a promising non-invasive urine collection system for use in incontinent women that may help reduce CAUTI rates, maintain skin integrity, accurately quantify urine output, and avoid extra healthcare costs.

Atrial septal aneurysm (ASA) is an uncommon but wellrecognized cardiac abnormality. This educational text reviews the case of a 54-year-old female presenting an ASA related to a small ostium secundum atrial septal defect. The considerable signs and symptoms, interestingly, have not been justified by the clinical and hemodynamic investigations. So, we opted for a better imaging investigation with cardiac catheterization and transesophageal echocardiography. The surgical process was earlier indicated and performed with aid of cardiopulmonary bypass.

Intracavitary right coronary artery (RCA) is a very rare anomaly occurring in less than 0.1%1 and up to 0.4% reported by coronary computed tomography angiography (CTA).2 Literature is scarce, however its recognition is crucial, especially prior to surgical or interventional procedures such as coronary artery bypass graft (CABG) surgery, right heart catheterization or device implantation: Hereby the RCA is at risk of injury with fatal bleeding, or suturing a CABG anastomosis may be difficult or impossible.

Spontaneous coronary artery dissection (SCAD) is characterized by tear of the inner layer in the coronary artery, creating a false lumen between the inner and central layer. Its infrequent incidence often leads to delay in diagnosis posing challenges in management. There are currently no guidelines for the treatment of this condition. We describe an adult patient who presented with multiple episodes of ventricular fibrillation, in whom cardiac catheterization showed SCAD, treated by off-pump coronary artery bypass.

Urethral condyloma is a therapeutic challenge. This article reports the case of a patient with spinal cord injury with the incidental finding of a massive spread of urethral condyloma. After removal of the condyloma with biopsy forceps neither recurrence of the condyloma nor a urethral stricture occurred. In patients where intermittent catheterization is performed, condyloma may be dispersed into the bladder, therefore, prompt endoscopic removal is crucial. As condyloma frequently recurs even after complete removal regular controls are mandatory.

A 17-year-old girl with neurofibromatosis type 1 presented with unstable angina. Cardiac catheterization revealed an aneurysm with thrombus in the left anterior descending coronary artery. She was discharged on medical treatment but returned 2 months later with severe chest pains. Angiography revealed an increase in the size of the aneurysm in the left anterior descending coronary artery, with thrombus and dissection. The patient underwent coronary artery bypass surgery. Follow-up after 1 year revealed no problems.

Subclavian artery dissection is a rare complication of cardiac catheterization. The usual reason for entering this vessel is to access the origin of the left internal mammary artery (LIMA) when this has previously been used, or is being considered, as a bypass graft. We report an instance of such dissection occurring long after bypass surgery which had included a LIMA graft. Successful correction was achieved by angioplasty and stent placement, using a retrograde left brachial approach.

A two and a half year old girl who had undergone the Yasui procedure as a neonate for ventricular septal defect, subaortic stenosis, and interrupted aortic arch underwent follow-up catheterization 2 years postoperatively. It showed that the neo-left ventricular tract reconstructed by Damus-Kaye-Stansel anastomosis had occluded due to closure of the ventricular septal defect and residual subaortic stenosis at the original left ventricular outflow tract. The patient therefore underwent takedown of the Yasui procedure.

Anomalous origin of the left main coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly that usually presents in childhood. Ninety percent of the patients with ALCAPA die within the first year of life without surgical intervention. In adults, ALCAPA is associated with left ventricular dysfunction, mitral regurgitation, and sudden death. The present report describes the case of an adult patient who presented with an abnormal stress test and ALCAPA was diagnosed during cardiac catheterization.

There are several possible pathophysiological links between the development of pulmonary hypertension and myelofibrosis with myeloid metaplasia. We report a woman with myelofibrosis and myeloid metaplasia who presented with dyspnea and massive, painful splenomegaly. Right heart catheterization evidenced pulmonary hypertension. Her management consisted of splenic irradiation associated to sildenafil. Dyspnea in patients with myelofibrosis and myeloid metaplasia can be secondary to pulmonary hypertension and conversely the differential diagnosis of pulmonary hypertension should include a myelopr

Pediatric neuropathic bladder dysfunction can cause irreversible renal damage and urinary incontinence. Etiologically, it is usually the consequence of a congenital neural tube defect. The majority of affected children can be successfully managed with the standard medical treatment of clean intermittent catheterization and anticholinergic (typically oxybutynin) medication. A subset of patients experience severe side effects or insufficient suppression of detrusor overactivity on oral oxybutynin. Intravesical instillations offer an effective alternative; this Review considers their indications,

Before investing in a human patient simulator, we designed a preliminary study that examined student responses to a laboratory exercise that used lower-fidelity simulation. Our purpose was to compare beginning-level, baccalaureate nursing students' self-reported assessment in the domains of confidence, ability, stress, and critical thinking before and after they participated in the simulation. Results showed statistically significant improvement in all domains for skills in urinary catheterization, intravenous and nasogastric medication administration, and sterile dressing change.

Percutaneous bypass catheters are routinely used for veno-venous bypass (VVBP) during orthotopic liver transplantation (OLT). The recognized risks include bleeding, injury of vascular and nerve structures and lymphatic leakage. We describe a case where there were difficulties during catheterization and the patient suffered a cardiac arrest on commencing VVBP. Post-mortem examination revealed the bypass catheter tip in the pleural space and a large right haemothorax. Possible mechanisms of vascular perforation and preventative measures are discussed.
