An Alternative to Traditional Bedside Teaching During COVID-19: High-Fidelity Simulation-Based Study.

BACKGROUND: Bedside teaching is integral to medical education and has been highlighted to improve clinical and communication skills, as well as clinical reasoning. Despite the significant advantages of bedside teaching, its usage within medical education has been declining, and COVID-19 has added additional challenges. The pandemic has resulted in a significant reduction in opportunities to deliver bedside teaching due to risk of viral exposure, patients declining student interactions, and ward closures. Educators have therefore been required to be innovative in their teaching methods, leading

COVID-19: Zinc and Angiotensin-Converting Enzyme 2 (ACE2) Deficiencies as Determinants of Risk and Severity of Disease: A Narrative Review.

A growing body of evidence supports the premise that deficiencies of zinc and angiotensin-converting enzyme 2 (ACE2, a zinc enzyme) determine severity of coronavirus disease 2019 (COVID-19). ACE2 is part of the renin-angiotensin system (RAS) and acts as a feedback control system moderating blood pressure, keeping blood pressure within normal limits. For a virus to infect a person, the virus has to get inside the person's cells. The virus that causes COVID-19 uses ACE2 to get into the cell. Think of this like an invader from outer space attacking your car by getting in through your cruise contr

An Interactive Smartphone App, Nenne Navi, for Improving Children's Sleep: Pilot Usability Study.

BACKGROUND: Healthy sleep is important not only for physical health but also for brain development in children. Several reports have revealed that Japanese adults and children have later bedtimes and shorter sleep durations compared with those in other countries, possibly because of Japanese culture and lifestyles. Therefore, an intervention tool that is suitable to the Japanese sociocultural environment is urgently needed to improve children's sleep problems in their early years. OBJECTIVE: To provide appropriate sleep health literacy to caregivers and change their parenting behavior, we dev

Using systems thinking and the Intervention Level Framework to analyse public health planning for complex problems: Otitis media in Aboriginal and Torres Strait Islander children.

BACKGROUND: Middle ear disease (otitis media) is endemic among Aboriginal and Torres Strait Islander children in Australia and represents an important cause of hearing loss. The disease is the result of a mix of biological, environmental and host risk factors that interact in complex, non-linear ways along a dynamic continuum. As such, it is generally recognised that a holistic, systems approach is required to reverse the high rates of otitis media in Aboriginal and Torres Strait Islander children. The objective of this paper is to examine the alignment between efforts designed to address otit

Endoscopy Simulators.

Endoscopic simulators allow trainees and practicing endoscopists to learn new skills in a safe environment, at their own pace. Four types of simulators are described: mechanical, ex vivo, in vivo, and virtual reality simulators. Mechanical simulators are the most commonly used type as they are simple to prepare and least expensive. Ex vivo and in vivo simulators use animal organs and live animals, respectively. They have better tissue feedback but are more difficult to prepare as they involve animals. Every simulator needs a curriculum to be more effective. The simulation-based mastery learnin

Categorization is 'baked' into the brain.

Categorization, the grouping of objects, living organisms, actions or events into equivalence clusters, is fundamental to adaptive behaviour. Traditionally, it is assumed that categorization begins with feature detection and ends with assigning representations stored in memory. Here we review converging evidence from neuroanatomy, electrophysiology, brain imaging and cognitive science to suggest an alternative view: categorization is not the end stage of perception but occurs throughout signal processing, from the very beginning. It is a core computational strategy of the brain, implemented th

Shape analysis of evoked action potentials recorded on the splenic nerve.

Stimulation of the splenic nerve represents a new way of treatment for inflammatory diseases and in particular rheumatoid arthritis. The implementation of this treatment requires studies on the functioning of the nerve and its response to stimulation in order to find the most suitable parameters allowing appropriate biological feedback while maintaining the condition of the nerve and optimizing the energy of the stimulation with a view to maximizing the lifespan of the future implantable device. In this study, we searched how to analyze the evoked nervous response to characterize the influence

L3MBTL3 is induced by HIF-1α and fine tunes the HIF-1α degradation under hypoxia in vitro.

HIF-1α plays a crucial part in hypoxia response by transcriptionally upregulating genes to adapt the hypoxic condition. HIF-1α is under severe cellular control as its exceptional activation is always associated with tumorigenesis and tumor progression. Here, we report L3MBTL3 serves as a novel negative regulator of HIF-1α. It is upregulated during hypoxia and acts as a transcriptional target of HIF-1α. In the nuclei, L3MBTL3 makes an interaction with HIF-1α and promotes its ubiquitination and degradation. These findings indicate L3MBTL3 forms a negative feedback loop with HIF-1α in vitro to da

Molecular and cellular regulation of psoriatic inflammation.

This review highlights the molecular and cellular mechanisms underlying psoriatic inflammation with an emphasis on recent developments which may impact on treatment approaches for this chronic disease. We consider both the skin and the musculoskeletal compartment and how different manifestations of psoriatic inflammation are linked. This review brings a focus to the importance of inflammatory feedback loops that exist in the initiation and chronic stages of the condition, and how close interaction between the epidermis and both innate and adaptive immune compartments drives psoriatic inflammat

Electroencephalographic Correlates of Obsessive-Compulsive Disorder.

This chapter reviews EEG research in Obsessive-Compulsive Disorder (OCD), focusing on Event-Related Potentials (ERPs) such as the Contingent Negative Variation, N2, Error-Related Negativity, the feedback Error-Related Negativity and the Readiness Potential and their neural bases. The functional significance, utility and correlation of these ERPs with OCD symptoms will be discussed, alongside novel theories for integrating the research findings. I will consider hypotheses including goal-directed behaviour, overreliance on habits, dissociations between action and knowledge, and excessive intoler

Choice (-history) correlations in sensory cortex: cause or consequence?

One challenge in neuroscience, as in other areas of science, is to make inferences about the underlying causal structure from correlational data. Here, we discuss this challenge in the context of choice correlations in sensory neurons, that is, trial-by-trial correlations, unexplained by the stimulus, between the activity of sensory neurons and an animal's perceptual choice. Do these choice-correlations reflect feedforward, feedback signalling, both, or neither? We highlight recent results of correlational and causal examinations of choice and choice-history signals in sensory, and in part sen

Editorial: Moving Away From Focussing on Gastric Pathophysiology in Functional Dyspepsia: New Insights and Therapeutic Implications.

Functional dyspepsia (FD) is a prevalent syndrome that affects up to one in six Americans, although misdiagnosis as gastroesophageal reflux disease may be common. Early satiety, postprandial fullness, and epigastric pain or burning are the cardinal symptoms. Traditionally, FD has been blamed on gastric dysfunction and much research effort has focussed on unlocking the gastric pathophysiology. New research suggests gastric dysfunction has only a minor role in the pathogenesis in many cases, and the focus has begun to shift to evaluation of duodenal feedback and subtle inflammation including duo
