[Radiologic diagnosis of the gallbladder and bile ducts - part 2 : Acute and chronic cholecystitis, primary sclerosing cholangitis (PSC), benign and malignant masses of the biliary system].

Upper abdominal pain, icterus and cholestasis are the symptoms leading to evaluation of the biliary tract. Together with its complications biliary stone disease is the main reason for inflammation of the biliary system. A distinction is made between acute and chronic variants. In chronic bile duct inflammation primary sclerosing cholangitis (PSC), primary biliary cirrhosis (PBC) and more recently IgG4-associated sclerosing cholangitis are of particular importance. Besides benign and tumor-like-lesions, malignant entities as gallbladder carcinoma and cholangiocarcinoma (CCC) in its three locati

Prognostic significance of long non-coding RNA MALAT1 for predicting the recurrence and metastasis of gallbladder cancer and its effect on cell proliferation, migration, invasion, and apoptosis.

Источник пометил публикацию как отозванную

The objective of this study is to explore the role of MALAT1 as a molecular indicator in predicting the recurrence, metastasis, and prognosis of gallbladder cancer (GBC) and its effect on the proliferation, invasion, migration, and apoptosis of GBC cells in vitro. GBC tissues and adjacent normal tissues were collected from 102 patients. MALAT1 short hairpin RNA (shRNA) plasmids were first constructed to transfect the GBC-SD cells. Reverse transcription quantitative polymerase chain reaction (RT-qPCR) was applied to detect MALAT1 expression. CCK-8 assay, flow cytometry, and Transwell assay were

Emergency cholecystectomy and hepatic arterial repair in a patient presenting with haemobilia and massive gastrointestinal haemorrhage due to a spontaneous cystic artery gallbladder fistula masquerading as a pseudoaneurysm.

BACKGROUND: Haemobilia usually occurs secondary to accidental or iatrogenic hepatobiliary trauma. It can occasionally present with cataclysmal upper gastrointestinal haemorrhage posing as a life threatening emergency. Haemobilia can very rarely be a complication of acute cholecystitis. Here we report a case of haemobilia manifesting as massive gastrointestinal haemorrhage in a patient without any prior history of biliary surgery or intervention and present a brief review of literature. CASE PRESENTATION: A 22 year old male admitted with history suggestive of acute cholecystitis subsequently d

Feasibility and safety of endoscopic ultrasound-guided gallbladder drainage using an electrocautery-enhanced lumen-apposing metal stent for inoperable acute cholecystitis with Niemeier type II perforation.

BACKGROUND AND AIMS: Gallbladder perforation has been considered a contraindication to EUS-guided gallbladder drainage (EUS-GBD). We evaluated the feasibility of EUS-GBD using an electrocautery-enhanced lumen-apposing metal stent (EC-LAMS) in inoperable acute cholecystitis (AC) complicated by Niemeier type II perforation. METHODS: We evaluated 19 consecutive high-risk surgical patients with moderate-to-severe AC and radiologic evidence of Niemeier type II perforation. The primary outcome was technical success. Secondary outcomes included clinical success and adverse events (AEs). RESULTS: Af

Pancreatic Colloid Carcinoma Arising From Intraductal Papillary Mucinous Neoplasm in the Setting of Gallbladder Agenesis, Ansa Pancreatica, and Santorinicele: Follow-Up of a Previously Reported Case.

This report details the long-term follow-up of a patient previously reported from our centre with recurrent acute pancreatitis and episodic jaundice in the setting of three rare anatomic variants: gallbladder agenesis, ansa pancreatica, and a santorinicele. The index report described progressive main pancreatic duct dilatation and endoscopic findings consistent with intraductal papillary mucinous neoplasm (IPMN) despite non-diagnostic cross-sectional imaging. We now present the subsequent clinical course, including elective pancreaticoduodenectomy, definitive histopathology, and surveillance o

Development and validation of a user-friendly prediction tool for preoperative T-Staging in gallbladder Cancer: A multicenter study using contrast-enhanced CT-Based fusion models.

INTRODUCTION: Accurate T-staging of gallbladder cancer (GBC) is critical for surgical planning; however, existing imaging techniques have limited identification accuracy. This study aimed to develop a robust model and prediction tool to address these limitations. MATERIALS AND METHODS: A retrospective cohort of 189 GBC patients from two institutions between January 2014 and December 2023 were analyzed. Patients were randomly assigned to internal training (ITC, n = 111), internal validation (IVC, n = 48), and temporal validation (TVC, n = 30) cohorts. Radiomics (Rad) and deep learning (DL) fea

Use of a bipolar vessel-sealing device for cholecystectomy in dogs with gallbladder mucocele has similar complication rates to traditional cystic duct ligation methods (2018-2024).

OBJECTIVE: To compare complication rates in dogs undergoing open cholecystectomy for gallbladder mucocele with the use of a bipolar vessel-sealing device (BVSD) for cystic duct (CD) ligation versus traditional ligation methods. METHODS: Medical records from dogs undergoing cholecystectomy for gallbladder mucocele at a privately owned referral institution between 2018 and 2024 were reviewed. Complications during the perioperative (following surgery to hospital discharge) and short-term (time of discharge to 2 months following surgery) periods were categorized as mild, moderate, and severe. Clinical impact of percutaneous transhepatic gallbladder drainage followed by laparoscopic cholecystectomy in patients with moderate to severe acute cholecystitis: A propensity score-matched case-control study.

Percutaneous transhepatic gallbladder drainage (PTGBD) is frequently performed for moderate to severe acute cholecystitis (AC) when early laparoscopic cholecystectomy (LC) is not feasible. While PTGBD relieves symptoms, its impact on surgical outcomes is uncertain. We retrospectively analyzed 505 patients with grade II or III AC undergoing LC (2012-2022). After propensity score matching, 122 received PTGBD and 122 underwent direct LC. PTGBD was associated with longer time to surgery (10.1 vs. 3.6 days), higher ICU admission (44.3 % vs. 22.1 %), and prolonged hospital stay (18.2 vs. 11.3 days

Research Priorities in Percutaneous Image- and Endoscopy-Guided Interventions for Biliary and Gallbladder Diseases: Proceedings from the Society of Interventional Radiology Foundation Multidisciplinary Research Consensus Panel.

Recent technological advancements, including the introduction of disposable endoscopes, have enhanced the role of interventional radiology (IR) in the management of biliary and gallbladder diseases. There are unanswered questions in this growing field. The Society of Interventional Radiology Foundation convened a virtual research consensus panel consisting of a multidisciplinary group of experts to develop a prioritized research agenda regarding percutaneous image- and endoscopy-guided procedures for biliary and gallbladder diseases. The panelists discussed current data, opportunities for IR,

Preoperative risk score to predict subtotal cholecystectomy after gallbladder drainage for acute cholecystitis: Secondary analysis of data from a multi-institutional retrospective study (CSGO-HBP-017B).

INTRODUCTION: Subtotal cholecystectomy (STC) has become recognized as a "bailout procedure" to prevent bile duct injury in patients undergoing laparoscopic cholecystectomy (LC). Predictors of conversion to STC have not been identified because LC difficulty varies based on pericholecystic inflammation. We analyzed data from patients enrolled in a previously performed multi-institutional retrospective study of the optimal timing of LC after gallbladder drainage for acute cholecystitis (AC). These patients presumably had a considerable degree of pericholecystic inflammation. METHODS: In total, 3

Comparison of Postoperative Port-Site Pain After Gallbladder Retrieval From Epigastric Versus Umbilical Port in Patients of Laparoscopic Cholecystectomy for Symptomatic Cholelithiasis: A Randomized Controlled Trial.

Introduction Gallbladder (GB) retrieval is an important cause of postoperative pain (POP) after laparoscopic cholecystectomy (LC). Retrieval is through the epigastric or umbilical port based on the surgeon's preference. There is limited evidence to support the superiority of one port over the other in terms of POP. This study was done to compare POP between epigastric and umbilical ports after GB retrieval in LC for symptomatic cholelithiasis.  Material and methods All patients who underwent elective LC for symptomatic cholelithiasis were randomized for GB retrieval either through the umbilica

In vivo 1 H MRS of human gallbladder bile in understanding the pathophysiology of primary sclerosing cholangitis (PSC): Immune-mediated disease versus bile acid-induced injury.

Primary sclerosing cholangitis (PSC) has been considered to be either an "autoimmune disease" or a "bile acid-induced injury." In vitro MRS studies on PSC patients have limitations due to the contamination of bile with contrast agent (commonly administered during endoscopic retrograde cholangiopancreatography) and/or the use of patient cohorts with other diseases as controls. The objective of this study was to quantify biliary metabolites using in vivo 1 H MRS and gain insight into the pathogenesis of PSC. Biliary metabolites in 10 PSC patients and 14 healthy controls were quantified in vivo u

[A Case of Locally Advanced Gallbladder Cancer That Developed Early Multiple Liver Metastasis after Curative Resection, and Achieved Long-TermPartial Remission Using S-1 and Gemcitabine].

A 76-year-old woman was referred to our hospital because of an abdominal tumor in September 2009. An irregularly shaped large tumor was detected in the right subcostal abdominal cavity on computed tomography, and was diagnosed as advanced gallbladder cancer without distant metastasis following further examination. We then performed a laparotomy. The tumor had invaded directly into the descending portion of the duodenum and transverse colon. We performed a curative resection of the tumor macroscopically. Pathological findings were moderately differentiated tubular adenocarcinoma derived from ga

Aberrant left main bile duct draining directly into the cystic duct or gallbladder: an unreported anatomical variation and cause of bile duct injury during laparoscopic cholecystectomy.

Despite recent advances, iatrogenic bile duct injury remains one of the most common complications of laparoscopic cholecystectomy. Aberrant biliary tract anatomy is one of the major risk factors for iatrogenic bile duct injury. In this case report, for the first time, we report a case of aberrant left main bile duct draining directly into the cystic duct or gallbladder that presented with bile duct injury after laparoscopic cholecystectomy. We hope that the diagnostic and management approach used in this case will help physicians to identify and manage their patients should they face such a ra

Pain persists in many patients five years after removal of the gallbladder: observations from two randomized controlled trials of symptomatic, noncomplicated gallstone disease and acute cholecystitis.

After removal of the gallbladder, pain may persist in some patients. To study this condition, 124 patients from two randomized trials, including those with symptomatic noncomplicated gallbladder stones (n = 90) and acute cholecystitis (n = 34), were interviewed, while 139 patients (90%) excluded from both trials responded to a questionnaire 5 years after the operation. Thirty-four patients (27%) of those randomized had pain; 23 (18%) had diffuse, steady pain; and 11 (9%) had pain attacks resembling their preoperative symptoms. A significant dominance of diffuse pain occurred in women (P = 0.02
